Seu uso no fenômeno de Raynaud resistente ao tratamento médico tem apresentado resultados favoráveis, servindo como alternativa não invasiva à cirurgia, com eficácia comprovada em estudos de pequena escala [2].

O mecanismo de ação merece ser compreendido, pois é contra-intuitivo. A toxina botulínica é conhecida por paralisar músculos; a suposição óbvia seria que ela relaxaria diretamente o músculo presente na parede arterial. Contudo, a compreensão atual é diferente: ela interfere na sinalização nervosa simpática responsável pela constrição dos vasos, além de parecer atuar também nas vias da dor. A distinção que determina tudo o mais

A questão de maior importância é saber se o fenômeno é primário, ocorrendo por si só, ou secundário a uma condição subjacente, geralmente uma doença autoimune do tecido conjuntivo, como esclerodermia ou lúpus.

Essa diferença não é meramente acadêmica. O Raynaud primário é um incômodo, porém com prognóstico favorável e sem danos teciduais. Já o Raynaud secundário pode evoluir para úlceras e perda de tecido nas pontas dos dedos; além disso, a doença subjacente precisa ser identificada e tratada.

Os sinais que despertam preocupação são o início dos sintomas após cerca de 30 anos de idade, assimetria entre as mãos, úlceras ou cicatrizes puntiformes nas pontas dos dedos, bem como capilares anormais na prega ungueal.
