Trata-se de uma dor no lado do dedo mindinho, na parte de trás do pulso — o lado onde a pulseira do relógio fica, caso você o use na altura do pulso. Geralmente, a dor é mais intensa quando você vira a palma da mão para cima e dobra o pulso em direção ao dedo mindinho; exatamente o que acontece numa rebatida de tênis, num swing de golfe ou ao torcer um pano para secar.

Existem duas variantes desse problema, e muitas pessoas apresentam um pouco das duas.

Tendão irritado. A dor é uma sensação de desconforto ao longo do tendão; às vezes há inchaço palpável, e dói ao tentar mover o pulso contra resistência. Geralmente, esse desconforto se acumula ao longo de semanas devido ao uso repetido, em vez de surgir de repente.

Tendão que sai do seu sulco. Nesse caso, ocorre um estalo, clique ou ruído característico ao girar o antebraço — algo que muitas vezes a própria pessoa consegue reproduzir. Geralmente, esse sintoma aparece após um incidente específico, como a raquete atingindo o chão ou uma rebatida forte. Pode ser assustador, dando a impressão de que algo está “cedendo”.

O que realmente está acontecendo

O tendão envolvido é o extensor do carpo ulnar, geralmente abreviado como ECU. Ele se estende do músculo no antebraço até o lado do dedo mindinho do pulso, situando-se num sulco raso na extremidade da ulna — o osso do antebraço desse lado. Um pequeno invólucro de tecido, um “túnel” próprio, mantém o tendão nesse sulco.

Esse sulco é realmente raso, com profundidade média de apenas cerca de 1,5 milímetro; por isso, o invólucro é responsável pela maior parte da função de manter o tendão no lugar. Se esse invólucro se romper ou esticar — geralmente devido a uma torção brusca com a palma da mão voltada para cima e o pulso dobrado — o tendão pode sair do sulco e voltar para lá. Caso contrário, se o tendão e seu revestimento ficarem irritados e espessados por cargas repetidas, seu deslizamento torna-se doloroso; isso corresponde à versão tendinopática da condição.

É importante saber o seguinte, pois isso altera a interpretação dos exames: certo grau de movimento do tendão é perfeitamente normal. Quando pesquisadores examinaram 755 pessoas sem nenhum sintoma no pulso, cerca de 1 em cada 15 apresentava o ECU “estalando”, e quase metade dessas pessoas tinha o tendão visivelmente saindo do seu sulco em exames de ultrassom — sem dor nem qualquer problema. O simples fato de o tendão estalar não é prova de que algo esteja errado.

O que podemos fazer a respeito

No Mater Private Hospital Rockhampton, o Dr. Kieran Hirpara baseia o diagnóstico principalmente no exame clínico, e não em exames de imagem; vale a pena explicar o motivo.

Os exames de imagem precisam ser interpretados com cuidado nesse caso. Em um estudo com mais de 4.000 ressonâncias magnéticas do punho, alterações no tendão ECU foram observadas em cerca de 1 a cada 8 exames, inclusive em pessoas que fizeram o exame por outros motivos. Se considerarmos um aspecto anormal do ECU na ressonância e perguntarmos com que frequência essa pessoa realmente tem problemas no tendão, a resposta é apenas cerca de 6% das vezes. A ressonância magnética é útil para excluir outras causas de dor nessa região do punho — como uma ruptura de cartilagem, por exemplo —, mas a presença de alterações no tendão, por si só, não constitui um diagnóstico definitivo. A ultrassonografia costuma ser mais útil, pois permite observar o movimento do tendão em tempo real enquanto o paciente reproduz o movimento que causa o estalo.

A maioria das pessoas melhora sem cirurgia. O tratamento geralmente começa com a modificação das atividades que provocam o problema, seguida de um período de imobilização com talas ou gesso. No caso de um tendão que “escapa” de seu sulco, a posição do punho é fundamental: geralmente mantém-se o antebraço virado com a palma para baixo e o punho ligeiramente flexionado para trás, por cerca de quatro a seis semanas, pois essa é a posição oposta àquela que permite o deslocamento do tendão.

A injeção de corticosteroide na bainha do tendão é um bom próximo passo no tratamento da versão irritativa do problema. No melhor estudo disponível, 13 pacientes com diagnóstico confirmado obtiveram algum alívio; 10 deles tiveram alívio completo, e nenhum precisou recorrer à cirurgia — a maioria continuou sem sintomas quatro a cinco anos depois. Um bom indicador precoce: se a primeira injeção resolve o problema por completo, esse alívio tende a persistir. Caso o efeito seja apenas parcial, geralmente permanece alguma dor residual.

Limitamos as injeções a, no máximo, três aplicações; somos ainda mais cautelosos em atletas que praticam arremessos ou jogos com raquetes, pois há uma preocupação — ainda não comprovada numericamente, mas relatada na literatura — de que injeções repetidas de corticosteroide nesse tendão possam, eventualmente, levar à sua ruptura.

A cirurgia é indicada apenas para pacientes cuja instabilidade do tendão continua a causar dor mesmo após um tratamento adequado com gesso, geralmente após cerca de dois meses. A operação visa reconstruir ou reforçar a bainha que mantém o tendão em seu sulco. Existem diversas técnicas, e todas apresentam bons resultados.

O que esperar

A recuperação sem cirurgia depende do grau de irritação do tendão. Atletas com um problema leve geralmente retornam às atividades em duas a três semanas; casos mais graves levam de seis a oito semanas; já um deslocamento completo tratado com gesso pode exigir de cinco a seis meses para que o paciente volte a praticar esportes exigentes como o tênis.

Após a cirurgia, deve-se esperar cerca de quatro a seis semanas com o antebraço imobilizado em gesso ou tala, com a palma da mão voltada para baixo. Em seguida, ocorre uma recuperação gradual da mobilidade; o fortalecimento começa por volta dos dois meses, e os exercícios específicos para o esporte são iniciados entre três e quatro meses. Estudos publicados indicam que os pacientes retornam às atividades anteriores em aproximadamente dois meses e meio a três meses e meio.

Dois pontos importantes sobre a cirurgia: primeiro, praticamente todos os estudos publicados afirmam que o fenômeno de “clicar” não volta a ocorrer; porém, um estudo que fez exames de imagem posteriormente constatou que, em cerca de metade dos pacientes, o tendão ainda permanecia fora de seu sulco, mesmo que esses pacientes estivessem satisfeitos e apresentassem bons resultados. Em outras palavras, a cirurgia alivia os sintomas de forma confiável, mas nem sempre restaura a anatomia ao estado original. Segundo, os ganhos em força de preensão e amplitude de movimento são pequenos; a cirurgia é indicada para aliviar a dor e o fenômeno de “clicar”, não para tornar o punho mais forte.

As complicações são raras, mas possíveis: dormência ou sensibilidade local decorrente da irritação de um pequeno nervo na parte dorsal do punho, rigidez e, muito raramente, uma síndrome de dor persistente.

Se o “clicar” do tendão não causa dor, provavelmente não há necessidade de correção. Pessoas com esse fenômeno indolor geralmente conseguem manter um alto nível de desempenho apenas com fitas adesivas e acompanhamento médico.

Quando procurar ajuda médica

Consulte seu médico de família se a dor no lado do punho próximo ao dedo mindinho não melhorar após algumas semanas de redução da atividade que a provoca, ou se ela estiver impedindo a prática de esportes ou o desempenho do trabalho.

Solicite uma avaliação por um especialista em cirurgia da mão se o punho “estalar” ou “clicar” dolorosamente ao girar o antebraço, se a dor surgiu após uma lesão por torção específica, ou se você já tentou repouso, uso de talas e injeções sem obter resultados duradouros.

Procure atendimento imediato se o punho ficar quente, vermelho e inchado; se houver formigamento ou fraqueza na mão; ou se você sentiu uma perda súbita de força, acompanhada de sensação de “ceder” — esse último sintoma pode indicar uma ruptura do próprio tendão.

Os problemas relacionados ao extensor carpi ulnaris merecem uma leitura mais aprofundada, pois ambos os sinais geralmente usados para confirmar o diagnóstico — um sinal anormal na ressonância magnética e o estalido do tendão ao girar o punho — são comuns em pessoas que não apresentam nenhum sintoma.

Um exame de ressonância magnética anormal não significa tanto quanto parece

O tendão ECU percorre um sulco na parte posterior da ulna, no lado do dedo mindinho do punho. Quando alguém sente dor nessa região e o exame mostra alterações de sinal nesse tendão, a conclusão parece óbvia.

No entanto, os dados contradizem isso. Ao analisar 4.301 exames de ressonância magnética de punho, verificou-se que as alterações de sinal no tendão ECU são comuns e frequentemente assintomáticas; além disso, tais alterações raramente estão associadas a tendinopatia sintomática, resultando em um baixo valor preditivo positivo [1].

O “baixo valor preditivo positivo” é o ponto crucial. Isso significa que, entre todos os punhos que apresentam esse achado, apenas uma minoria o tem como causa da dor. O exame por si só não pode estabelecer o diagnóstico; ele precisa ser corroborado pela presença de sensibilidade exatamente naquela região e pela reprodução da dor ao realizar movimentos resistidos na direção correta.

O fenômeno de “clicar” é comum em pessoas saudáveis

O segundo sinal confirmatório é o fato de o tendão “clicar” visivelmente ou audivelmente ao sair de seu sulco durante a rotação do antebraço. Em um estudo realizado com 755 indivíduos assintomáticos, observou-se que a ocorrência desse fenômeno é tão comum que não pode ser considerada, por si só, um marcador confiável de patologia [2].

Na prática, se o paciente percebe esse “clique” e também sente dor exatamente naquele local do punho, isso é significativo. Já o “clique” detectado casualmente durante o exame, em um punho onde a dor se manifesta em outro ponto, provavelmente representa uma variação normal.

Considerando também os resultados da ressonância magnética, o padrão observado é consistente: esse tendão gera resultados atípicos em punhos aparentemente saudáveis. Nesse caso, o diagnóstico depende mais da correlação entre os diversos achados do que de qualquer um deles isoladamente.

Isso é importante porque o punho ulnar possui diversas possíveis fontes de dor.

O tendão ECU está localizado bem ao lado do TFCC, da articulação radioulnar distal e da articulação pisotriquetral; todas essas estruturas podem causar dor na mesma pequena região. É fácil atribuir essa dor a um tendão ECU com aparência anormal; porém, considerando os dados acima, isso costuma ser incorreto. Esse é o principal motivo para se ter cautela antes de considerá-lo o culpado pela dor.

O tratamento conservador é o pilar do tratamento e é eficaz

Quando o diagnóstico está correto, o tratamento não é nada espetacular. Uma combinação de medidas conservadoras e reabilitação pode deixar os pacientes livres de sintomas e permitir que retornem às atividades desejadas [3].

Duas características específicas desse tendão moldam a reabilitação. Como ele é submetido a carga durante a rotação do antebraço combinada com desvio ulnar — posição que ocorre em um swing de golfe, em um golpe com raquete ou ao levantar uma panela pesada —, a modificação das atividades nesse caso significa alterar a direção da carga, em vez de simplesmente repousar. Além disso, como o tendão fica inserido em uma bainha que pode ser esticada ou rompida, permitindo sua subluxação, a imobilização costuma ser feita na posição de pronação com o punho estendido; isso mantém o tendão em seu sulco enquanto a bainha cicatriza.

b. Where Your right to use the Licensed Material has terminated under Section 6(a), it reinstates:

1. automatically as of the date the violation is cured, provided it is cured within 30 days of Your discovery of the violation; or

2. upon express reinstatement by the Licensor.

For the avoidance of doubt, this Section 6(b) does not affect any right the Licensor may have to seek remedies for Your violations of this Public License.

c. For the avoidance of doubt, the Licensor may also offer the Licensed Material under separate terms or conditions or stop distributing the Licensed Material at any time; however, doing so will not terminate this Public License.

d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.

d. Nothing in this Public License constitutes or may be interpreted as a limitation upon, or waiver of, any privileges and immunities that apply to the Licensor or You, including from the legal processes of any jurisdiction or authority.
