Acalmando a situação. Os primeiros passos são simples: modifique temporariamente as atividades que sobrecarregam o lado ulnar do punho (especialmente o aperto forte, torções e suporte de peso), use uma tala para punho para repousar a articulação e utilize medicamentos anti-inflamatórios para aliviar a dor e o inchaço. Às vezes, uma injeção de esteroides na articulação ajuda a interromper o ciclo de inflamação.

Confirmando o diagnóstico. Paralelamente ao tratamento, geralmente realizamos raios-X para medir com precisão o alinhamento dos dois ossos (às vezes com uma projeção especial de “preensão” que acentua a colisão entre eles); também solicitamos frequentemente uma ressonância magnética para avaliar a cartilagem, o TFCC e o osso subjacente. Ocasionalmente, uma avaliação por artroscopia é utilizada para confirmar o diagnóstico e tratar a condição ao mesmo tempo.

Caso persista. Quando o tratamento não cirúrgico adequado não traz melhora, o objetivo da cirurgia é aliviar a pressão no lado ulnar do punho. A opção mais consolidada é a osteotomia de encurtamento da ulna, na qual o cirurgião remove uma pequena porção da ulna para ajustar seu comprimento, fixando-a com uma placa até a cicatrização. Em casos selecionados, uma alternativa menos invasiva é o procedimento artroscópico conhecido como “wafer”, que consiste em desbastar uma pequena porção da extremidade da ulna por meio de cirurgia minimamente invasiva. Ambos os métodos visam impedir que o osso friccione contra a articulação do punho.

O que esperar

Além disso, no caso do problema mais comum, o procedimento cirúrgico mais recente e menos invasivo apresenta índices de complicações melhores do que o método tradicionalmente utilizado.

Por que essa região do punho é difícil de avaliar

Em um raio de poucos centímetros encontram-se o TFCC, a articulação radioulnar distal, o tendão do extensor carpi ulnaris e sua bainha, a articulação pisotriquetral, o ligamento lunotriquetral, além do ponto onde a ulna se une aos ossos do carpo. Cada um desses elementos pode causar dor no mesmo local; muitos deles apresentam alterações visíveis em exames de imagem em pessoas assintomáticas, e mais de um pode estar envolvido simultaneamente.

Por isso, a avaliação nessa região depende mais da localização precisa dos sintomas e de testes de provocação do que em outras partes do punho. Também é por isso que um exame de imagem que indique a presença de várias estruturas anormais é um resultado comum, porém pouco útil. É igualmente por esse motivo que causas menos comuns, como a impactação midcarpal, são ocasionalmente descritas como fatores subestimados [1].

Impactação ulnar: o problema mecânico e sua solução mecânica

A causa estrutural mais comum é a impactação ulnar, na qual a ulna fica ligeiramente mais longa em relação ao rádio; por isso, os ossos do carpo são pressionados contra sua extremidade durante a ação de agarrar ou quando o antebraço é rotacionado com a palma voltada para baixo. Com o tempo, isso causa desgaste do complexo fibrocartilaginoso triangular do punho (TFCC) e da cartilagem dos ossos adjacentes.

O tratamento consiste em encurtar a ulna, seja através do corte e encurtamento do seu eixo, seja pela remoção de uma pequena porção óssea de sua extremidade, procedimento este que pode ser realizado por artroscopia. Reunindo dados de 956 pacientes, as técnicas artroscópica e aberta de remoção da porção óssea podem ser alternativas viáveis à osteotomia de encurtamento ulnar, mais utilizada atualmente, embora, na época, não tivesse sido comprovada sua superioridade clínica [2].

Dados mais recentes foram além. Ao comparar o procedimento artroscópico de remoção da porção óssea com a osteotomia de encurtamento ulnar em 311 pacientes, ambos se mostraram eficazes; contudo, o grupo submetido ao procedimento wafer apresentou menos complicações e menor taxa de reoperação, levando os autores a considerá-lo uma alternativa potencialmente superior [3].

Por que a osteotomia acumula complicações

O motivo não é que a cirurgia seja mal executada. O encurtamento do eixo ósseo exige o corte do osso, a fixação com uma placa durante a cicatrização e a aceitação do risco de que o osso não consolide. Uma revisão sistemática envolvendo 1.423 pacientes analisou especificamente os casos de não união óssea; não foi encontrada nenhuma diferença entre osteotomias transversais e oblíquas, levando à conclusão de que a escolha entre elas não deve ser baseada no risco de não união [4].

O procedimento “wafer” evita completamente esse tipo de problema, pois remove osso da extremidade do osso em vez de dividir o eixo ósseo. Não há fratura a ser unida, e geralmente não é necessário remover nenhuma placa posteriormente; é aí que reside a diferença nas taxas de reoperação.

A contrapartida é que o procedimento “wafer” remove apenas uma quantidade limitada de osso e exige que a articulação radioulnar distal esteja íntegra; portanto, o encurtamento do eixo ósseo continua sendo necessário nos casos em que é preciso maior correção ou quando essa articulação também está comprometida.

A consequência prática para o paciente

Como várias estruturas podem ser a causa do problema, um tratamento direcionado à estrutura errada não surtirá efeito, independentemente da qualidade com que for executado. É justamente nessa situação que a injeção diagnóstica, os exames de imagem direcionados e, ocasionalmente, a artroscopia têm seu papel, antes da tomada de decisão. É também nesse contexto que se deve chegar a um diagnóstico específico — e não apenas a uma descrição genérica como “dor no lado ulnar do punho” — antes de planejar qualquer cirurgia.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.
