A interpretação prática: se um estudo apresentar um valor muito baixo, pergunte se os pesquisadores realmente buscaram esses casos. Sintomas que desaparecem são suficientemente comuns para serem considerados esperados, e não complicações; já os sintomas persistentes são, de fato, pouco frequentes.

A anestesia consciente não é automaticamente mais segura

Existe uma crença amplamente difundida, tanto entre pacientes quanto em outros grupos, de que evitar a anestesia geral seria mais seguro, especialmente em idosos e no que diz respeito à confusão mental pós-operatória.

O teste mais direto dessa hipótese vem da cirurgia de fratura de fêmur, onde a questão foi estudada em larga escala, dado que os pacientes são idosos e frágeis. Ao analisar 3.736 pacientes, constatou-se que a anestesia regional não reduziu significativamente o delírio pós-operatório nem a mortalidade em comparação com a anestesia geral [2].

Quando os bloqueios anestésicos realmente se justificam

Em relação à artroscopia do ombro, uma revisão com 1.382 pacientes constatou que o bloqueio do nervo supraescapular não foi mais eficaz que o bloqueio interescalênico no controle da dor, porém apresentou um perfil de efeitos colaterais mais favorável [4]. Os bloqueios interescalênicos costumam deixar o braço temporariamente fraco e podem afetar o nervo diafragmático, o que é relevante em pacientes com função respiratória já comprometida. Nesses casos, optar por outro tipo de bloqueio não é uma solução de compromisso, mas sim a melhor escolha específica para cada procedimento cirúrgico.

Por que o jejum é feito dessa maneira

O jejum existe por um único motivo: uma via aérea sob efeito de anestesia não consegue se proteger sozinha; assim, o conteúdo do estômago pode descer para os pulmões. Esse evento é raro, porém grave, e a regra do jejum é uma medida preventiva de baixo custo contra ele.

O erro comum é tratar a orientação “nada por via oral” como uma espécie de competição. Jejuar por mais tempo não aumenta a segurança; além disso, chegar desidratado faz a pessoa se sentir pior, dificulta a inserção de cateteres e não traz nenhum benefício para o estômago. Líquidos claros saem do estômago mais rapidamente do que alimentos; por isso, alguns hospitais permitem seu consumo mais próximo da cirurgia.
