A fibromialgia é uma dor generalizada e crônica: uma dor latejante, ardente ou incômoda que pode se deslocar pelo corpo inteiro, sendo sentida praticamente em toda parte, e não apenas numa única articulação. O que ocorre é na forma como o sistema nervoso processa a dor. Na fibromialgia, o sistema responsável por transmitir os sinais de dor fica hiperativo: o “volume” dos sinais é ajustado para um nível mais alto do que o normal. Sensações comuns são amplificadas até se tornarem dor, e a dor real parece ainda mais intensa. Os médicos chamam isso de sensibilização central. Trata-se de uma alteração física real na forma como os nervos e o cérebro lidam com os sinais: não é algo “só na sua cabeça”, nem sinal de fraqueza. A fibromialgia é comum, é real e é reconhecida como condição médica.

Ela também costuma estar associada a outras condições (como síndrome do intestino irritável, dores de cabeça, humor deprimido ou outras dores crônicas), e pode intensificar a dor após uma cirurgia. O que podemos fazer a respeito

  • Movimento suave e gradual. A atividade física é o tratamento mais comprovado para a fibromialgia. O segredo é começar muito devagar e aumentar o esforço aos poucos: caminhadas, alongamentos ou exercícios aquáticos são ótimos pontos de partida. Exagerar logo no início pode piorar a dor; por isso, a paciência e a constância são fundamentais.
  • Sono de qualidade. Como o sono de má qualidade agrava a dor e a sensação de “neblina mental”, manter uma rotina regular e bons hábitos de sono fazem grande diferença.
  • Paceamento. Distribuir as atividades ao longo do dia e da semana (em vez de fazer tudo num dia bom e ficar exausto depois) ajuda a manter o equilíbrio.
  • Controle do estresse e abordagens psicológicas. O estresse intensifica os sintomas. Técnicas como a TCC (terapia cognitivo-comportamental), relaxamento e atenção plena ajudam a acalmar um sistema nervoso hiperativo.

Os medicamentos podem ajudar algumas pessoas, mas os mais úteis não são analgésicos comuns: eles acalmam o sistema nervoso em vez de bloquear os sinais de dor. A fibromialgia merece uma leitura mais aprofundada, pois a ideia mais útil a respeito dela é também a menos intuitiva: apesar de se manifestar como um problema muscular e articular, as evidências apontam para causas completamente distintas dos músculos e articulações.

O indício que nada tem a ver com articulações

Se a fibromialgia fosse uma doença dos músculos ou do tecido conjuntivo, deveria limitar-se a esses tecidos. Contudo, não é o caso.

Num estudo com 44.494 participantes, as pessoas com fibromialgia ou outras dores musculoesqueléticas apresentaram maior probabilidade de relatar perda auditiva. A interpretação dos autores é o ponto crucial: esse achado está em conformidade com a hipótese de que a fibromialgia está relacionada a uma desregulação geral do sistema nervoso central; o mesmo pode valer para outras dores musculoesqueléticas generalizadas [1].

A audição não possui qualquer relação mecânica com as dores nos ombros. Quando uma condição caracterizada por dor musculoesquelética também está associada ao modo como o ouvido e o cérebro processam os sinais sensoriais, o fator comum é justamente esse processamento, e não o tecido em si. Este é o modelo da sensibilização central, observado aqui de forma inesperada: o “controle de volume” dos estímulos sensoriais fica aumentado, e a dor é a consequência mais evidente, embora não a única.

Por que os exames são normais, e por que isso não significa que a condição não existe

Os pacientes ouvem frequentemente que todos os exames deram resultados normais, e interpretam isso com descrença. A pesquisa explica por que os exames são normais, sem sugerir que a dor não esteja presente.

Uma meta-análise sobre as citocinas — moléculas de sinalização inflamatória que seriam alteradas em casos de artrite inflamatória — reuniu 1.255 participantes e concluiu que o papel fisiopatológico dessas citocinas na fibromialgia ainda é incerto, sendo necessários estudos maiores e mais rigorosos [2]. A investigação sobre o papel do microbioma intestinal chegou a uma conclusão semelhante: trata-se de uma possível ligação dentro do recém-descoberto eixo intestino–sistema musculoesquelético, porém ainda faltam pesquisas de qualidade nesse campo [3].

Portanto, não existe nenhum exame de sangue capaz de detectar algo anormal, pois a alteração está no processamento dos sinais nervosos, e não em danos teciduais ou inflamação. Resultados normais em exames inflamatórios e em exames de imagem são exatamente o que se espera, e eles servem para excluir outras condições, não para descartar a fibromialgia.

O que, na verdade, demonstrou ser útil

O tratamento baseado em movimento possui os dados de apoio mais robustos; a forma específica do exercício é menos importante do que o simples fato de realizá-lo. Uma meta-análise envolvendo 936 pacientes constatou que os exercícios tradicionais chineses — tai chi, qigong e outras práticas de baixa intensidade que combinam movimento e respiração — resultaram em melhoria significativa na dor, na qualidade do sono e nos sintomas de ansiedade e depressão [4].

Observe o que esse conjunto de estudos mede: os benefícios abrangem dor, sono e humor simultaneamente, o que é exatamente o que se esperaria caso o alvo do tratamento fosse o sistema nervoso sensibilizado, e não uma estrutura lesionada. O sono não é um aspecto secundário aqui; o sono ruim reduz o limiar de tolerância à dor, e ambos os fatores se reforçam mutuamente.

A sobreposição com a menopausa

Os sintomas da fibromialgia geralmente começam por volta da menopausa, tendem a se tornar mais graves posteriormente e podem piorar após a histerectomia, com ou sem remoção dos ovários [5]. As listas de sintomas apresentam grande sobreposição com a síndrome musculoesquelética da menopausa.

A conclusão da revisão é um alerta para não se confundirem essas duas condições: a fibromialgia também ocorre em mulheres não menopausadas e em homens; portanto, os hormônios sexuais não explicam todo o mecanismo patogênico. O que isso significa quando se discute a cirurgia

É aqui que o modelo de sensibilização central ganha aplicação prática. Uma operação corrige um problema estrutural. Contudo, se parte da sua dor for gerada por alterações no processamento central, essa parte permanecerá após a cirurgia, pois ela não foi abordada pelo procedimento.

Isso não é um argumento contra a realização da cirurgia quando existe um problema estrutural real a ser corrigido. É, sim, um argumento para se definir com precisão, previamente, qual parcela da dor a operação deverá eliminar, bem como para tratar a fibromialgia de forma independente, em vez de esperar que a cirurgia resolva tudo. c. For the avoidance of doubt, the Licensor may also offer the Licensed Material under separate terms or conditions or stop distributing the Licensed Material at any time; however, doing so will not terminate this Public License.

d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.
