A posição da tala é um fator crucial. A tala de movimento relativo ou tala em “jugo” mantém o dedo lesionado ligeiramente mais estendido que os dedos vizinhos; isso alivia a tensão sobre a banda em processo de cicatrização, ao mesmo tempo em que permite o uso da mão. Trata-se de uma forma de imobilização relativamente bem tolerada, o que explica por que a adesão ao tratamento costuma ser boa.

O momento do início do tratamento é fundamental, pois se trata de uma lesão de tecidos moles que cicatriza por meio da formação de tecido fibroso. Quando a imobilização é feita precocemente, a banda cicatriza num comprimento adequado para manter o tendão centralizado. Caso contrário, após meses de imobilização inadequada, a banda cicatriza alongada; o tendão continua a sofrer subluxação, e a única opção restante passa a ser a reconstrução cirúrgica, e não a cicatrização espontânea.

Por que a classificação está sendo revisada em vez de estabelecida

Os estudos recentes têm se concentrado em aperfeiçoar a forma de classificar essas lesões; propõe-se uma modificação no sistema de classificação mais utilizado para orientar o tratamento e padronizar a descrição [2]. A lesão da banda sagital é uma das três lesões fechadas do mecanismo extensor, classificadas conforme o local da ruptura: lesão no dedo em formato de martelo na ponta do dedo, lesão de tipo “boutonnière” na articulação média e lesão da banda sagital na articulação do dedo [3]. Cada uma exige uma posição específica para imobilização com talas; além disso, um dedo inchado logo após a lesão pode apresentar sintomas ambíguos.
