O tendão do tríceps se descolou, parcial ou totalmente, da sua inserção no olécrano. Uma ruptura total significa que o músculo não está mais firmemente ligado ao osso; assim, a força que ele gera não consegue mais ser transmitida para esticar o cotovelo. Já numa ruptura parcial, algumas fibras permanecem ligadas, mantendo parte da força muscular.

Essas rupturas quase sempre ocorrem quando o tendão é submetido a carga enquanto o músculo está contraindo intensamente: por exemplo, o braço é forçado a dobrar-se exatamente no momento em que se tenta esticá-lo ou sustentá-lo. Alguns fatores aumentam a probabilidade de ocorrência: o problema é muito mais comum em homens, especialmente naqueles que levantam pesos; o uso de esteroides anabolizantes enfraquece os tendões e é um fator de risco reconhecido; injeções de esteroides ao redor do cotovelo, episódios recorrentes de bursite do olécrano e algumas condições médicas, como doença renal crônica, também tornam o tendão mais frágil.   • Rupturas completas, rupturas parciais que causam fraqueza no braço, e rupturas em pessoas ativas ou com alta demanda física são, geralmente, mais bem tratadas com cirurgia, para reatar o tendão ao osso. Isso é feito passando pontos resistentes por pequenos túneis ósseos ou fixando o tendão novamente em seu local original com implantes especiais. Reparar a lesão precocemente, antes que o tendão cicatrize e encurte, garante o melhor resultado.

O que esperar

Quando um tendão do tríceps rompido é reparado em tempo hábil, os resultados geralmente são muito bons: a maioria dos pacientes recupera uma extensão do cotovelo forte e confiável, voltando ao trabalho e às atividades esportivas. A maior revisão sistemática sobre tratamentos cirúrgicos abrange 560 pacientes [1]. Para comparação, as informações sobre lesões no ombro e no joelho apresentadas neste site baseiam-se em populações agregadas de dezenas de milhares de pacientes. Neste caso, toda a literatura disponível abrange apenas algumas centenas de pacientes com diversos tipos de lesões. Isso não significa que os tratamentos sejam ineficazes. Significa, porém, que afirmações categóricas sobre pequenas diferenças entre eles carecem de respaldo científico e devem ser recebidas com ceticismo sempre que forem apresentadas.

Os benefícios da reparação estão bem documentados

Onde as evidências são mais consistentes é na questão principal. Em 318 pacientes – sendo mais de 90% militares em serviço ativo, atletas e trabalhadores da população geral – todos retornaram ao esporte ou às suas atividades profissionais após a reparação do tríceps distal [2].

Essa população é relevante para a interpretação dos resultados. São pessoas que exercem atividades físicas exigentes; seu retorno ao trabalho ou ao esporte pode ser verificado objetivamente, e não apenas relatado por eles mesmos. Por isso, esse número é mais informativo do que um valor semelhante obtido em uma população geral.

Uma comparação técnica de fato revelou diferenças

Apesar da constatação geral de que as técnicas não apresentam diferenças significativas, uma comparação específica de fato evidenciou uma distinção. Em 56 pacientes, os resultados relatados pelos próprios pacientes foram favoráveis tanto para o reparo com âncora de sutura quanto para o reparo por túnel transósseo; contudo, o reparo com âncora de sutura apresentou resultados significativamente melhores nos testes de força isocinética, nas taxas de complicações e nas taxas de reincisão [3].

O tamanho da amostra é pequeno, portanto esse resultado deve ser interpretado como um indício, e não como uma conclusão definitiva – exatamente o que a revisão mais abrangente destaca sobre toda essa literatura.

Por que o diagnóstico precoce é importante neste caso

O tríceps é o único músculo responsável por estender o cotovelo contra resistência; portanto, uma ruptura completa tem importância funcional muito maior do que as rupturas parciais. As rupturas completas são mais bem tratadas por meio de reparo cirúrgico, especialmente quando realizado de forma imediata [4].

Duas características tornam essa lesão fácil de ser ignorada: geralmente ainda há alguma capacidade de extensão graças aos demais pontos de fixação, fazendo com que o braço não pareça completamente inútil; além disso, o inchaço e os hematomas na parte posterior do cotovelo podem ser atribuídos à própria queda. O teste prático consiste na extensão resistida à gravidade; a incapacidade de manter o braço estendido ao receber uma força descendente — e não apenas a sensação de dor — é o sinal clínico relevante.
