A bursa olecraniana é um pequeno saco cheio de líquido e bastante lubrificante, localizado exatamente sobre a ponta óssea do cotovelo. Por que remover a bursa?

Na nossa clínica, o Dr. Kieran Hirpara orienta os pacientes por meio de uma avaliação cuidadosa para confirmar o diagnóstico e explorar primeiro as opções não cirúrgicas. Geralmente recomendamos a cirurgia somente quando os tratamentos conservadores não proporcionam alívio suficiente para problemas de longa data.

O tratamento de primeira linha para uma bursa inchada é quase sempre não cirúrgico: repouso do cotovelo, evitar apoiar o braço nele, uso de medicamentos anti-inflamatórios e, às vezes, drenagem do líquido com uma agulha (aspiração), ocasionalmente seguida de injeção de esteroides. Uma revisão das evidências científicas demonstrou que o manejo não cirúrgico apresenta maior taxa de resolução do problema e menos complicações do que a cirurgia imediata; por isso optamos por essas abordagens primeiro.

Consideramos a remoção da bursa (chamada “bursectomia”) apenas nos seguintes casos:

  • A bursite é crônica ou reaparece constantemente, mesmo após repouso, mudanças nas atividades, aspiração e/ou injeção de esteroides ao longo de vários meses, e realmente está causando desconforto.
  • Existe infecção recorrente ou já estabelecida (bursa séptica) que não cede ao tratamento com antibióticos e drenagem. Mesmo nesses casos, os cirurgiões geralmente tentam primeiro a drenagem e os antibióticos; estudos indicam que a cirurgia não garante melhores resultados a longo prazo no tratamento de infecções, sendo reservada para casos que não melhoram, quando há pus espesso impossível de drenar com agulha ou um abscesso evidente [1].
  • Um esporão ósseo na ponta do cotovelo irrita repetidamente a bursa; nessa situação, o esporão pode ser removido durante a mesma cirurgia.

Em resumo: essa operação destina-se à minoria das bursas que simplesmente não respondem a outros tratamentos. Se o inchaço é recente, indolor e não está associado a infecção, é muito improvável que a cirurgia seja a solução adequada.

O que envolve a operação

Geralmente, trata-se de um procedimento ambulatorial, portanto você poderá ir para casa no mesmo dia; porém, ocasionalmente os pacientes permanecem internados por uma noite. A operação é realizada de forma aberta: faz-se uma pequena incisão na ponta do cotovelo, através da qual toda a bursa inflamada é cuidadosamente removida, e a pele é então suturada. Caso haja um esporão ósseo sob a bursa, ele pode ser alisado durante o mesmo procedimento.

O tipo de anestesia geralmente é anestesia geral (você fica completamente inconsciente) ou bloqueio regional (o braço fica insensível enquanto você permanece acordado ou levemente sedado), às vezes ambos combinados. O anestesista discutirá com você qual a melhor opção no dia da cirurgia.

É uma operação relativamente rápida; porém, a habilidade necessária não reside tanto na remoção em si, mas sim no manejo cuidadoso da pele delicada e no fechamento adequado da incisão. Conforme descrito abaixo, é justamente a pele que costuma apresentar problemas.

A parte honesta: cicatrização e recorrência

Esta é a seção que merece ser lida duas vezes, pois é o principal motivo de nossa cautela quanto a esta cirurgia.

A pele na ponta do cotovelo é fina, móvel e possui suprimento sanguíneo relativamente precário. Além disso, essa região sofre constante pressão sempre que apoiamos o braço em uma mesa, cadeira ou porta de carro. Essa combinação faz com que problemas de cicatrização e a recorrência da bursa sejam os dois problemas mais prováveis de ocorrer, mais do que na maioria das cirurgias desse porte. Não é que a cirurgia seja difícil; é que o cotovelo é um local impiedoso para se fazer uma incisão.

O que as evidências científicas realmente dizem? É importante ser honesto: os estudos publicados são, em sua maioria, séries de casos pequenas, portanto os números variam [1]. Uma revisão abrangente de pacientes submetidos à bursectomia mostrou que cerca de um em cada nove precisou de uma cirurgia de revisão; pessoas com artrite reumatoide, diabetes ou bursite nos dois cotovelos apresentaram risco maior de necessitar de novas intervenções. Especialmente os pacientes com artrite reumatoide tinham maior probabilidade de precisar de um retalho cutâneo para garantir a cicatrização. Os estudos relatam taxa significativa de complicações como cicatrização retardada, abertura da ferida, acúmulo de líquido (seroma) e infecção [2, 3]. O ponto principal não é um percentual exato, mas o padrão: esta cirurgia tem chance acima do normal de gerar cicatrização lenta ou problemática, além de possibilidade real de retorno do inchaço.

Fazemos várias coisas para aumentar suas chances de sucesso, e sua participação é tão importante quanto a nossa:

  • Manuseio delicado e fechamento cuidadoso durante a cirurgia; quando adequado, evitamos fazer incisões bem na ponta do cotovelo.
  • Após a cirurgia, aplicamos curativo de compressão firme para evitar acúmulo de líquido; às vezes usamos também uma tala para manter o cotovelo imóvel enquanto a pele cicatriza.
  • O mais importante: não apoiar o cotovelo. A pressão sobre um cotovelo em processo de cicatrização é o principal fator que leva tanto à abertura da ferida quanto à recorrência da bursa. Evitar apoiar o cotovelo por várias semanas realmente influencia o resultado final.

Nada disso tem o objetivo de assustar você. O propósito é garantir que, caso decidamos realizar a cirurgia, você esteja plenamente ciente da situação e compreenda que as instruções de recuperação não são meros detalhes opcionais, mas sim o que assegura o sucesso do tratamento.

Sua recuperação

Cada pessoa cicatriza em seu próprio ritmo; as instruções específicas do seu cirurgião têm prioridade absoluta, mas segue abaixo um panorama geral.

A primeira ou segunda semana. Você terá um curativo de compressão firme e acolchoado sobre o cotovelo, muitas vezes acompanhado de uma tala. Mantenha o curativo limpo e seco, eleve o braço sempre que possível para reduzir o inchaço e tome analgésicos simples conforme orientação médica. A coisa mais importante que você pode fazer nessas primeiras semanas é manter o cotovelo imóvel e sem nenhuma pressão na parte posterior: não apoie seu peso nele, nem se incline sobre mesas ou braços de cadeira. Movimentos suaves dos dedos, punho e ombro geralmente são permitidos e ajudam a prevenir rigidez.

Pontos e curativos. O envoltório de compressão costuma ser mantido nos primeiros dias; muitos cirurgiões pedem que você continue usando uma faixa compressiva leve (tipo “ACE”) no cotovelo por algumas semanas para evitar novo acúmulo de líquido. Os pontos são normalmente retirados na consulta, entre 7 e 10 dias após a cirurgia, quando a pele já teve tempo de fechar. Nessa visita, examinaremos cuidadosamente a ferida, pois o cotovelo pode demorar mais para cicatrizar.

Voltando a se movimentar. Assim que a ferida estiver estável e os pontos retirados, você retomará gradualmente o uso do braço. Em geral, as pessoas podem retomar a maioria das atividades normais e o pleno uso do cotovelo entre três e seis semanas; o inchaço costuma desaparecer por completo por volta da sexta semana. O cirurgião pode impor restrições temporárias ao levantamento de pesos enquanto a pele amadurece.

Dirigir é possível quando você consegue mover o cotovelo sem dificuldade, não está tomando analgésicos fortes e consegue controlar o veículo e fazer uma frenagem de emergência com segurança. Para muitas pessoas, isso ocorre após algumas semanas; porém, depende do braço afetado e do ritmo de sua recuperação, portanto consulte-nos antes.

O trabalho depende inteiramente da sua profissão. Atividades de escritório costumam ser viáveis em uma ou duas semanas (tomando cuidado para não apoiar o cotovelo). Já trabalhos que exigem levantamento de pesos, ajoelhar-se com o cotovelo apoiado ou qualquer esforço que cause pressão ou tensão no cotovelo demandam mais tempo (várias semanas), às vezes com retorno gradual. Quando procurar ajuda

Um pouco de inchaço, hematomas e desconforto são normais após esta cirurgia. No entanto, entre em contato imediatamente com seu cirurgião, médico de família ou clínica de atendimento fora do horário se notar quaisquer sinais de que a ferida não está cicatrizando bem ou está se infectando, tais como:

  • Vermelhidão que se espalha, calor crescente ou dor intensificada ao redor da ferida após os primeiros dias.
  • A ferida se abrindo, vazando líquido ou pus, ou apresentando mau odor.
  • Febre, sensação geral de mal-estar ou calafrios.
  • O cotovelo voltar a inchar significativamente: isso pode indicar acúmulo de líquido novamente.

Além disso, como em qualquer cirurgia, a formação de coágulos sanguíneos é um risco raro, porém grave. Dirija-se imediatamente ao pronto-socorro (ou ligue para os serviços de emergência) caso desenvolva uma panturrilha dolorida, inchada e quente, ou, com maior urgência, se sentir falta de ar súbita, dor no peito ou tosse com sangue. Esses podem ser sinais de coágulo na perna ou nos pulmões e exigem avaliação imediata.
