Những triệu chứng bạn đang gặp phải

Viêm gân có lắng đọng canxi nghĩa là có một lượng nhỏ canxi tích tụ bên trong một trong các gân của chóp xoay, tức là nhóm gân giữ và vận động khớp vai của bạn. Cơn đau thường nằm ở mặt bên hoặc phía trên vai, ngay trên gân cơ trên gai, là gân hay bị ảnh hưởng nhất. Động tác nâng cánh tay sang bên hoặc đưa tay qua đầu thường làm cơn đau nặng hơn, và việc với lên kệ cao hay phơi quần áo có thể trở nên khó khăn.

Cơn đau thường bùng lên vào ban đêm, nhất là khi bạn nằm đè lên bên vai đó, và có thể khiến bạn thức giấc. Cơn đau cũng có thể xuất hiện sau khi hoạt động hoặc ngay khi mới ngủ dậy vào buổi sáng. Một số người thấy cơn đau đến từng đợt, xen kẽ với những khoảng thời gian dịu hơn, và có thể kéo dài nhiều tháng. Khoảng thời gian khó chịu kéo dài đó có thể khiến bạn phải nghỉ làm và làm giảm chất lượng cuộc sống của bạn.

Bệnh này phổ biến nhất ở phụ nữ trong độ tuổi từ 30 đến 60, tuy nhiên những người khác cũng có thể mắc. Bản thân mảng lắng đọng thường dài từ 0,5 đến 1,5 cm, và phần lớn là đặc chứ không xốp như bông. Ở khoảng 9% người bệnh có nhiều hơn một gân bị ảnh hưởng, điều này có thể làm cơn đau lan ra quanh vai.

Một điều bạn nên biết: mảng lắng đọng canxi thấy trên phim chụp không loại trừ khả năng có rách ở cùng gân đó. Điều gì đang thực sự xảy ra

Chóp xoay là một nhóm gồm bốn gân giữ và vận động khớp vai của bạn. Các gân này trượt qua một khoảng hẹp bên dưới một vòm xương nhỏ ở phía trên vai, với một túi đệm mỏng chứa dịch nằm giữa chúng. Hãy hình dung gân như một sợi dây chạy qua một chiếc ròng rọc chật hẹp.

Trong viêm gân có lắng đọng canxi, một phần của sợi dây đó biến đổi. Các tế bào bên trong gân dần dần chuyển thành những tế bào giống tế bào sụn, và canxi bắt đầu lắng đọng vào vùng mô đã biến đổi. Quá trình này diễn ra theo từng giai đoạn: đầu tiên gân biến đổi, sau đó canxi hình thành và nằm lại ở đó, và cuối cùng cơ thể phân hủy mảng lắng đọng rồi hấp thu nó. Đây là một quá trình chủ động, không đơn thuần là sự hao mòn do sử dụng, và nó khác với sự tích tụ canxi mà bạn có thể thấy ở một gân đã lão hóa, bị mòn ở những nơi khác.

Giai đoạn bệnh rất quan trọng vì mỗi giai đoạn gây ra cảm giác khác nhau. Trong lúc canxi chỉ nằm yên trong gân, bạn có thể đau ít hoặc không đau. Khi cơ thể bắt đầu hấp thu canxi, cơn đau thường trở nên dữ dội và đột ngột, và có thể nặng nhất vào ban đêm. Bản thân mảng lắng đọng cũng có thể gây rắc rối do kích thước của nó: khi nó phồng lên trong khoảng hẹp, nó có thể bị kẹt vào vòm xương phía trên, cản trở cánh tay cử động trơn tru. Đó là lý do việc nâng cánh tay sang bên có thể có cảm giác cứng hoặc bị chặn lại.

Mảng lắng đọng thường nằm nhiều nhất ở gân cơ trên gai, cách chỗ gân bám vào đầu trên xương cánh tay khoảng 1,5 đến 2 cm. Vị trí đó giải thích vì sao cơn đau nằm ở chỗ đó, và vì sao động tác với tay qua đầu là động tác khó nhất.

Thêm một điều nữa bạn nên biết: nhiều người mang các mảng lắng đọng này mà hoàn toàn không đau, và chỉ khoảng một phần ba số mảng lắng đọng từng gây ra triệu chứng. Vì vậy, canxi trên phim chụp của bạn là một phát hiện có thật, nhưng việc nó có phải là nguyên nhân gây đau hay không còn tùy thuộc vào giai đoạn của nó và cách cơ thể bạn phản ứng với nó.

Vật lý trị liệu nhằm làm dịu cơn đau và xây dựng lại khả năng cử động cánh tay trơn tru, có kiểm soát. Hãy dành đủ thời gian cho phương pháp này trước khi đánh giá nó, vì bệnh này có thể kéo dài nhiều tháng. Thuốc kháng viêm là phương pháp điều trị đầu tay thường dùng để giảm đau. Nếu thuốc không đủ hiệu quả, chúng tôi có thể thực hiện các thủ thuật nhắm trực tiếp vào mảng lắng đọng. Một lựa chọn là liệu pháp sóng xung kích, trong đó sóng âm được hướng vào khối canxi từ bên ngoài cơ thể để phá vỡ mảng lắng đọng. Đây là phương pháp không xâm lấn, nghĩa là không có gì đâm xuyên qua da, và được thực hiện ở một cường độ và số xung nhất định để làm bong tách khối canxi. Một lựa chọn khác là chọc kim dưới hướng dẫn siêu âm, trong đó kim được đưa vào mảng lắng đọng nhờ siêu âm, một phương pháp chụp cho thấy hình ảnh trực tiếp của gân, để phá vỡ và hút canxi ra. Cả hai phương pháp đều có thể giảm đau, phục hồi chức năng và làm sạch canxi. Thủ thuật chọc kim có thể được lặp lại nếu triệu chứng vẫn còn, với khoảng cách ít nhất sáu tuần giữa các lần thực hiện.

Phẫu thuật được cân nhắc khi các phương pháp điều trị này không hiệu quả. Phẫu thuật bao gồm việc loại bỏ mảng lắng đọng canxi khỏi gân, và sửa chữa cả gân nếu gân bị ảnh hưởng nghiêm trọng. Nếu cơn đau của bạn không thuyên giảm sau khi đã thực sự thử các lựa chọn nêu trên, chúng tôi sẽ cùng bạn trao đổi xem phẫu thuật có hợp lý với bạn hay không và cùng nhau quyết định.

Những điều có thể xảy ra

Trong hầu hết các trường hợp, triển vọng của viêm gân có lắng đọng canxi là tốt. Nhiều người mang các mảng lắng đọng này mà không hề hay biết, và chỉ khoảng một phần ba trong số đó từng gây đau. Khi mảng lắng đọng gây ra triệu chứng, nó có nhiều khả năng sẽ khỏi hoàn toàn về lâu dài. Cơ thể bạn có thể tự hấp thu canxi, và quá trình đó đã bắt đầu diễn ra ngay từ khi mảng lắng đọng hình thành.

Tuy vậy, con đường đi đến đó không phải lúc nào cũng nhanh. Các triệu chứng có thể kéo dài nhiều tháng, và khoảng thời gian khó chịu đó có thể khiến bạn phải nghỉ làm và ảnh hưởng thực sự đến chất lượng cuộc sống. Cơn đau thường đến từng đợt, xen kẽ với những khoảng thời gian dịu hơn, và có thể dữ dội, đột ngột khi cơ thể bắt đầu phân hủy mảng lắng đọng. Một số mảng lắng đọng thuyên giảm trong vòng vài tháng, số khác mất nhiều thời gian hơn, và không có cách nào đáng tin cậy để dự đoán chính xác mảng lắng đọng của bạn sẽ diễn tiến ra sao.

Hầu hết mọi người đều tiến triển tốt mà không cần phẫu thuật. Điều trị bảo tồn, nghĩa là điều trị không phẫu thuật, là lựa chọn đầu tiên thường dùng, và mang lại cải thiện đáng kể cho hầu hết mọi người: 72% số người được điều trị theo cách này cho biết đạt kết quả tốt hoặc rất tốt, bất kể kích thước hay vị trí của mảng lắng đọng. Liệu pháp sóng xung kích và chọc kim dưới hướng dẫn siêu âm đều giúp giảm đau và làm sạch canxi ở nhiều người, và cả hai đều có độ an toàn cao. Chọc kim thường hiệu quả nhất khi triệu chứng xuất hiện chưa lâu và mảng lắng đọng nhỏ hơn. Nếu một trong các thủ thuật này không phù hợp với mảng lắng đọng cụ thể của bạn, chúng tôi sẽ cùng bạn trao đổi về các lựa chọn khác.

Phẫu thuật được dành cho những người có cơn đau không thuyên giảm sau khi đã thực sự thử các phương pháp điều trị này. Phẫu thuật loại bỏ mảng lắng đọng canxi khỏi gân, và mang lại kết quả tốt ở 88,5% những người có cơn đau không đáp ứng với các phương pháp điều trị khác. Sự hồi phục sau phẫu thuật diễn ra từ từ: chức năng vai cải thiện chậm trong sáu tháng đầu, và phần lớn sự cải thiện về mức độ dễ chịu và vận động được tích lũy dần trong khoảng thời gian đó chứ không đến ngay lập tức.

Khi nào nên gặp bác sĩ

Hầu hết các mảng lắng đọng canxi đều tự thuyên giảm, và nhiều mảng hoàn toàn không gây rắc rối gì. Nhưng bệnh này có thể kéo dài nhiều tháng, và có thể khiến bạn mất thời gian làm việc và làm giảm chất lượng cuộc sống. Hãy đến gặp bác sĩ đa khoa hoặc yêu cầu được khám bởi bác sĩ chuyên khoa nếu cơn đau vai của bạn không thuyên giảm, nặng dần lên qua nhiều tuần, khiến bạn thức giấc vào ban đêm, hoặc khiến bạn không thể làm việc hay sử dụng cánh tay. Hãy nói cả với bác sĩ nếu bạn cảm thấy cơn đau bị chặn lại hoặc bị kẹt khi nâng cánh tay sang bên, vì một mảng lắng đọng bị phồng lên có thể chặn gân cử động bên dưới vòm xương phía trên nó. Thêm một điều nữa nên nêu ra: mảng lắng đọng canxi trên phim chụp không loại trừ khả năng có rách ở cùng gân đó, vì vậy chỉ riêng kết quả phim chụp không đủ để xác định chẩn đoán. Việc tìm hiểu về tình trạng viêm gân có lắng đọng canxi là rất đáng giá, vì nó có đặc điểm khác biệt so với hầu hết các bệnh lý vai: hiện tượng lắng đọng này thường gặp ở những người hoàn toàn không có triệu chứng đau; cơ thể thường tự hòa tan chúng mà không cần can thiệp; và phương pháp điều trị nhằm loại bỏ canxi lại không phải là cách giúp giảm đau nhanh nhất.

Sự xuất hiện của các mảng lắng đọng canxi là hiện tượng phổ biến; không nhất thiết là dấu hiệu của bệnh

Các mảng lắng đọng canxi được phát hiện qua chụp chiếu có thể dễ dàng được coi là nguyên nhân gây ra triệu chứng, vì chúng có thể nhìn thấy được và cơn đau là có thật. Tuy nhiên, các số liệu về tỷ lệ mắc bệnh lại cho thấy điều ngược lại. Trong số 1.219 người trưởng thành, các mảng lắng đọng này xuất hiện ở 7,8% những người không có triệu chứng và 42,5% những người mắc hội chứng đau vùng dưới mỏm vai [1]. Trong một nghiên cứu khác với 302 trường hợp, các mảng lắng đọng này khá phổ biến trong dân số nói chung; tuy nhiên chỉ khoảng một phần ba trong số đó gây đau [2].

Như vậy, việc có các mảng lắng đọng canxi làm tăng đáng kể khả năng chúng là nguyên nhân gây bệnh; tuy nhiên cứ khoảng 13 người không có triệu chứng thì có 1 người lại có các mảng lắng đọng này. Những yếu tố có thể khiến các mảng lắng đọng này trở nên gây đau bao gồm kích thước và vị trí của chúng: khả năng gây triệu chứng cao nhất là ở những phụ nữ từ 30 đến 60 tuổi bị đau vùng dưới mỏm vai và có mảng lắng đọng dài hơn 1,5 cm [1]; đồng thời cường độ đau còn phụ thuộc vào vị trí của mảng lắng đọng so với cơ trên gai và việc có nhiều hơn một gân bị ảnh hưởng hay không [2].

Thông thường, các vết vôi hóa sẽ tự tiêu đi, và điều này quyết định mọi thứ

Quá trình tự nhiên là sự hấp thụ trở lại của các vết vôi hóa; đó là lý do tại sao nhiều phương pháp điều trị dường như đều có hiệu quả. Ví dụ rõ ràng nhất là từ một nghiên cứu ngẫu nhiên, trong đó mọi bệnh nhân đều được chích kim và rửa sạch vùng tổn thương, sau đó được phân ngẫu nhiên vào nhóm tiêm steroid hoặc nhóm tiêm dung dịch muối sinh lý: sau 12 tháng, các vết vôi hóa đã biến mất ở 83% bệnh nhân trong nhóm tiêm dung dịch muối và 74% bệnh nhân trong nhóm tiêm steroid [3].

Hãy đọc kỹ điều này, vì nó chứa một kết quả trái với trực giác thông thường. Corticosteroid giúp giảm đau trong 6 tuần và cải thiện chức năng vận động trong 3 tháng, nhưng không có tác động đáng kể đến việc các vết vôi hóa có biến mất hay không [3]. Việc giảm đau và sự tiêu biến của các vết vôi hóa là hai quá trình riêng biệt. Một phương pháp điều trị có thể mang lại hiệu quả ở một trong hai khía cạnh mà không ảnh hưởng đến khía cạnh còn lại; đồng thời, những gì hiện lên trên hình ảnh chụp không phải là yếu tố trực tiếp gây ra các triệu chứng từng tuần.

Đó là lý do tại sao các phương pháp điều trị không phẫu thuật đều cho kết quả tương tự

Dù khối tụ máu có thể tự tiêu biến, các phương pháp điều trị này chủ yếu nhằm giúp bệnh nhân chịu đựng được thời gian chờ đợi thay vì chữa khỏi hoàn toàn. Điều này được thể hiện rõ qua các so sánh. Trên 257 bệnh nhân, vật lý trị liệu, tiêm corticosteroid và thủ thuật barbotage dưới hướng dẫn siêu âm đều mang lại tỷ lệ tránh được phẫu thuật tương đương nhau [4]. Trên 239 bệnh nhân, cả ba phương pháp này đều khá hiệu quả; riêng vật lý trị liệu đơn thuần lại có tỷ lệ thất bại cao nhất [5].

Trong nhóm các phương pháp can thiệp, liệu pháp sóng xung kích năng lượng cao là phương pháp ít xâm lấn được nghiên cứu kỹ lưỡng nhất; nó được chứng minh là an toàn và hiệu quả trong ngắn hạn và trung hạn. Ngược lại, thủ thuật chọc kim dưới hướng dẫn siêu âm không được chứng minh là vượt trội hơn so với việc tiêm thuốc dưới hướng dẫn siêu âm vào vùng dưới mỏm vai [6]. Khi tổng hợp dữ liệu từ 1.258 bệnh nhân, cả liệu pháp sóng xung kích, chọc kim và nội soi khớp đều cho kết quả lâm sàng tốt [7]; thủ thuật barbotage trên 908 bệnh nhân cũng được chứng minh là an toàn và có tỷ lệ thành công cao, nhưng chưa từng được so sánh trực tiếp với các phương pháp chính khác [8].

Những lợi ích cụ thể mà phẫu thuật mang lại

Phẫu thuật cũng có những ưu điểm nhất định; mức độ hiệu quả của nó đáng được nêu rõ thay vì chỉ mô tả sơ lược. Dựa trên dữ liệu từ 2.352 bệnh nhân tham gia các thử nghiệm ngẫu nhiên, phương pháp điều trị phẫu thuật giúp cải thiện đáng kể các chỉ số chức năng và giảm đau tương đương so với các phương pháp điều trị không phẫu thuật, đặc biệt là thủ thuật chọc kim phá vỡ khối vôi hóa dưới hướng dẫn siêu âm; cả hai phương pháp đều mang lại hiệu quả lâm sàng đáng kể [9]. Giữa các kỹ thuật phẫu thuật khác nhau thì không có sự khác biệt đáng kể; việc chỉ loại bỏ khối tích tụ cũng cho kết quả tương tự như việc loại bỏ khối này kết hợp với thủ thuật giảm áp dưới mỏm vai [10].

Điểm quan trọng cần lưu ý là sự khác biệt giữa mục tiêu cải thiện chức năng và giảm đau. Nếu bệnh nhân chủ yếu than phiền về cơn đau, các bằng chứng hiện có không cho thấy phẫu thuật là lựa chọn ưu việt hơn. Ngược lại, nếu tình trạng cứng khớp và suy giảm chức năng kéo dài, phẫu thuật sẽ là phương pháp thích hợp hơn.

Mối liên hệ thường bị bỏ qua

Viêm gân có lắng đọng canxi không xuất hiện một cách đều đặn ở mọi bệnh nhân. Trong số 102 bệnh nhân, những người mắc kèm các rối loạn nội tiết, chủ yếu là bệnh tuyến giáp và tiểu đường, thường xuất hiện triệu chứng ở độ tuổi trẻ hơn, diễn tiến bệnh kéo dài hơn đáng kể, và cần phẫu thuật nhiều hơn [11].

Nếu thời gian mắc bệnh của bạn bất thường (kéo dài hoặc xuất hiện sớm), bạn nên trao đổi với bác sĩ đa khoa về điều này. Việc điều trị các rối loạn nội tiết không giúp chữa khỏi hoàn toàn chứng viêm vai, nhưng nó giúp xác định được mốc thời gian phục hồi hợp lý. Việc được thông báo rằng bệnh sẽ tự khỏi thật khó chấp nhận khi bạn đã phải chịu đau đớn suốt hai năm; mối liên hệ với các rối loạn nội tiết chính là một lý do giải thích tại sao những lời khuyên thông thường không áp dụng cho mọi trường hợp.

Anatomical Location & Structures

Pathogenesis & Staging

Clinical Presentation & Pain Mechanisms

Investigations

Plain Radiography

  • Standardized plain films are almost always sufficient to garner the information needed for shoulder imaging [47].
  • The purpose of imaging the shoulder is to help establish the diagnosis, determine the severity of the pathoanatomy, assist in surgical planning, and enable the surgeon to illustrate the condition of the shoulder to the patient [47].
  • At least two X-ray views should be obtained for shoulder imaging: an anteroposterior in the plane of the glenoid and an axillary projection with the arm in abduction [87].
  • The axillary view is taken with the arm in the functional position of elevation in the plane of the scapula and oriented so that both the spinoglenoid notch and the scapular neck are visible [47].
  • The axillary view demonstrates glenohumeral relationships in the functional position of elevation, which is referred to as the "truth view" [47].
  • CT scans may offer a few degrees of increased precision in the measurement of glenoid version, but this precision does not improve the quality of the surgery or the clinical outcome [47].
  • The temptation to "overimage" should be resisted, obtaining only the scans or reconstructions that are necessary for the care of the patient [47].
  • Calcific tendinitis can be observed on plain radiograph [94].

Ultrasonography

  • Ultrasound is a simple and accurate test for identifying rotator cuff tears and calcific tendinitis [87].
  • Ultrasound can be useful in guiding injections or barbotage (aspirating calcific deposits in the rotator cuff) [87].
  • Ultrasonography is a low-cost alternative to MRI and arthrography for evaluating both skeletal and soft-tissue structures of the shoulder [94].
