Những triệu chứng bạn đang gặp phải

Chấn thương này hầu như luôn bắt đầu từ việc ngã trực tiếp vào vùng vai: ngã khỏi xe đạp, va chạm mạnh trong thể thao, hoặc đơn giản là vấp ngã trên mặt phẳng cứng. Ngay lập tức, vùng đỉnh vai sẽ bị đau; trong vòng một hoặc hai ngày sau, thường xuất hiện một khối sưng đau hoặc chỗ lồi ngay tại chỗ tiếp giáp giữa xương đòn và vai.

Vùng này rất đau khi chạm vào hoặc khi nâng tay lên; việc vươn tay sang ngang người hoặc nâng tay lên trên đầu cũng gây cảm giác khó chịu. Nếu ấn vào khối sưng đó, bạn sẽ cảm nhận được độ đàn hồi, giống như đang ấn xuống đầu mút xương đòn rồi cảm nhận nó nảy trở lại, tương tự như khi nhấn phím piano. Nhiều người có phản xạ ôm tay lại vì việc để tay thõng xuống sẽ gây căng tức ở khớp vai. Đây chính là hiện tượng “trật khớp vai” mà mọi người thường nhắc đến; nó không giống với tình trạng vai bị trật hoàn toàn, tức là đầu xương vai tách ra khỏi ổ khớp.

Điều gì thực sự đang xảy ra

Ở phần trên cùng của vai, có một khớp nhỏ nơi đầu ngoài của xương đòn (clavicle) tiếp giáp với mỏm xương của xương bả vai gọi là mỏm acromion. Đây chính là khớp acromioclavicular (AC). Khớp này được giữ vững nhờ các dây chằng – vừa là những dây chằng nằm quanh khớp, vừa là một nhóm dây chằng mạnh nằm phía dưới một chút (gọi là các dây chằng coracoclavicular, hay CC) đóng vai trò như những sợi dây cố định xương đòn xuống dưới.

Khi bị ngã vào vùng vai, xương bả vai bị đẩy xuống dưới trong khi xương đòn vẫn giữ nguyên vị trí, khiến các dây chằng này bị căng hoặc rách. Nếu chỉ có các dây chằng quanh khớp bị giãn, xương đòn vẫn giữ được vị trí tương đối ổn định. Nhưng nếu những sợi dây chằng mạnh hơn cũng bị rách, xương đòn sẽ không còn bị giữ chặt nữa và bị đẩy lên trên, tạo thành chỗ lồi và độ chênh lệch rõ rệt trên bề mặt vai.

Các bác sĩ phẫu thuật phân loại những chấn thương này dựa trên mức độ tổn thương, sử dụng thang phân loại Rockwood từ I đến VI. Các mức I và II là những trường hợp giãn dây chằng với mức di lệch rất ít hoặc không có; mức III nằm ở giữa, có chỗ lồi rõ rệt nhưng vai vẫn vận động tương đối bình thường; còn các mức IV đến VI là những chấn thương nặng, khi xương đòn bị lệch khỏi vị trí rất nhiều. Cần lưu ý rằng đây là tình trạng khác biệt so với khớp AC bị thoái hóa, viêm khớp (do quá trình hao mòn theo thời gian) và so với gãy xương đòn (cả hai tình trạng này chúng tôi sẽ đề cập riêng).

Những gì chúng tôi có thể làm

Tại phòng khám của chúng tôi, bác sĩ Kieran Hirpara điều trị chấn thương này bằng cách xác nhận chẩn đoán thông qua việc đánh giá kỹ lưỡng và chụp hình ảnh.

Tin vui là hầu hết các chấn thương khớp AC không cần phải phẫu thuật.

Các chấn thương mức độ nhẹ (độ I, II và phần lớn độ III) được điều trị khá đơn giản. Việc đeo áo treo cánh tay để giảm khó chịu trong vài tuần sẽ giúp giảm áp lực lên khớp và giúp vùng bị tổn thương hồi phục. Khi cơn đau ban đầu giảm bớt, vật lý trị liệu sẽ giúp phục hồi vận động và tái tạo các cơ quanh xương bả vai và vai. Hầu hết bệnh nhân đều lấy lại được chức năng vận động tốt, thậm chí là hoàn toàn, rồi có thể trở lại làm việc và tham gia các hoạt động thể thao. Điểm cần lưu ý duy nhất là về mặt thẩm mỹ: ngay cả sau khi lành hẳn, chỗ lồi ở phía trên vai vẫn thường tồn tại vĩnh viễn; nó có vẻ khác so với bên vai còn lại, nhưng thường không ảnh hưởng đến khả năng vận động của vai.

Phẫu thuật chỉ được chỉ định cho những trường hợp thực sự cần thiết. Đó là các chấn thương mức độ nặng (độ IV, V và VI), nơi xương đòn bị lệch vị trí nghiêm trọng; cùng một nhóm nhỏ các chấn thương mức độ nhẹ vẫn gây đau đớn, yếu cơ hoặc không ổn định dù đã được điều trị phục hồi đúng cách – đặc biệt là ở những người làm công việc nặng nhọc, phải vận động quá mức hoặc các vận động viên chuyên nghiệp. Ca phẫu thuật sẽ khâu nối lại các dây chằng bị rách để kéo xương đòn về đúng vị trí và giữ cố định trong thời gian lành thương. Những điều có thể xảy ra

Đối với đa số bệnh nhân bị chấn thương mức độ nhẹ, diễn biến bệnh thường khả quan. Cơn đau dữ dội sẽ giảm dần trong vài tuần, và nhờ vật lý trị liệu, vai sẽ dần phục hồi sức mạnh cũng như khả năng vận động. Hầu hết bệnh nhân có thể trở lại các hoạt động và môn thể thao thường ngày; họ chấp nhận rằng vết sưng có thể là dấu vết vĩnh viễn của chấn thương mà không ảnh hưởng đến chức năng của vai. Ngay cả một số trường hợp chấn thương mức độ III, dù trông có vẻ nghiêm trọng lúc đầu, cũng có thể hồi phục tốt mà không cần phẫu thuật.

Những chấn thương cần thời gian hồi phục lâu hơn là các trường hợp mức độ nặng, hoặc các chấn thương mức độ nhẹ nhưng không có dấu hiệu cải thiện. Trong những trường hợp này, câu hỏi đặt ra là việc phẫu thuật tái tạo có giúp vai trở nên ổn định hơn, mạnh mẽ hơn và ít đau hơn để đáp ứng các hoạt động mà bạn thực hiện hay không; câu trả lời phụ thuộc nhiều vào công việc, môn thể thao bạn tham gia và tình trạng vai trong vài tuần đầu sau chấn thương. Có nhiều bằng chứng cho thấy việc đưa ra quyết định này nên được thực hiện một cách thận trọng, cho vai cơ hội hồi phục tự nhiên trước khi quyết định phẫu thuật; đồng thời không nên để tình trạng chấn thương mức độ nặng, không ổn định kéo dài vô thời hạn.

Khi nào nên đi khám bác sĩ

  • Ngã đập vào đỉnh vai gây đau đớn, kèm theo một khối sưng đau hoặc chỗ lồi lên ở vùng đó; trường hợp này cần được khám để xác định mức độ chấn thương và loại trừ khả năng gãy xương đòn.
  • Một khối u lồi rõ rệt khiến cánh tay có cảm giác bị kéo xuống: đây là dấu hiệu của chấn thương nghiêm trọng hơn cần được đánh giá kỹ lưỡng.
  • Đau hoặc yếu cơ không thuyên giảm sau vài tuần đeo nẹp và tập vật lý trị liệu; đặc biệt nếu bạn làm việc nặng, phải vận động trên cao hoặc tham gia các môn thể thao.
  • Da bị căng ra trên vùng khối u, thay đổi màu sắc da ở chỗ đó, hoặc cảm giác xương đang đẩy mạnh vào da: cần đi khám càng sớm càng tốt.

Advanced reading: the deeper science (optional)

Phần này đi sâu hơn mức cần thiết để bạn tự đưa ra các quyết định điều trị. Chấn thương khớp AC đáng để đọc thêm vì có một sự mâu thuẫn nằm ở trung tâm của mọi quyết định liên quan đến nó: phẫu thuật có thể khắc phục chính xác những gì được thấy trên phim X-quang, nhưng lại không làm thay đổi chức năng của vai sau phẫu thuật.

Phẫu thuật chỉ khắc phục hình ảnh X-quang, chứ không thay đổi kết quả lâm sàng

Các trường hợp trật khớp nặng trông rất đáng ngại trên phim X-quang: xương đòn nhô rõ rệt ra khỏi mỏm vai; vì vậy người ta cho rằng việc đưa xương về vị trí cũ chắc chắn sẽ tốt hơn. Tuy nhiên, trong nghiên cứu gồm 954 bệnh nhân bị trật khớp cấp độ nặng, không có sự khác biệt lâm sàng nào về các chỉ số đánh giá chức năng giữa nhóm điều trị bằng phẫu thuật và nhóm điều trị không phẫu thuật. Những điểm khác biệt lại xuất hiện theo cả hai hướng: nhóm điều trị không phẫu thuật trở lại làm việc sớm hơn, nhưng chấp nhận kết quả thẩm mỹ kém hơn; trong khi phẫu thuật giúp đạt được mức chỉnh trật khớp tốt hơn trên phim X-quang [1].

Khi chỉ xét riêng các ca thuộc loại Rockwood III – mức độ mà tranh luận về chỉ định phẫu thuật vẫn còn gay gắt – một nghiên cứu năm 2025 trên 397 bệnh nhân cũng không phát hiện sự khác biệt đáng kể nào về chức năng khớp được bệnh nhân tự đánh giá hay theo các tiêu chí khách quan; phẫu thuật giúp cải thiện mức độ chỉnh trật khớp nhưng lại làm tăng nguy cơ biến chứng phẫu thuật [2]. Một nghiên cứu trước đó trên 646 trường hợp cũng đi đến kết luận tương tự: không có sự khác biệt đáng kể về nguy cơ viêm khớp sau này hay tình trạng đau dai dẳng; tỷ lệ tái trật khớp ở nhóm phẫu thuật là 14% [3].

Ba nguồn bằng chứng độc lập, nhưng cùng một kết luận. Tình trạng biến dạng khớp là có thật và sẽ tồn tại vĩnh viễn nếu không phẫu thuật; tuy nhiên việc chỉnh sửa nó cũng không làm thay đổi đáng kể khả năng vận động của vai.

Quyết định phẫu thuật liên quan đến chỗ lồi ra trên xương, chứ không phải chức năng vận động

Việc xác định lại vấn đề theo cách này mới là giá trị thực tiễn của các bằng chứng y khoa. Nếu câu hỏi là “Phẫu thuật có giúp vai tôi vận động tốt hơn không?”, câu trả lời trung thực là chưa có bằng chứng chứng minh điều đó. Còn nếu câu hỏi là “Phẫu thuật có giúp loại bỏ chỗ lồi trên xương đòn không?”, thì câu trả lời thường là có; đối với một số người, trong một số nghề nghiệp và môn thể thao nhất định, điều này đã đủ quan trọng để trở thành lý do phẫu thuật.

Đó là một lý do hợp lý để tiến hành phẫu thuật. Chỉ là lý do đó khác với điều mà hầu hết mọi người cho rằng họ đang được chỉ định phẫu thuật vì.

Chưa có kỹ thuật nào vượt trội hoàn toàn; một kỹ thuật thì tệ hơn đáng kể

Sau khi quyết định phẫu thuật, việc lựa chọn phương pháp cố định lại nhận được nhiều sự quan tâm nhưng lại chưa có sự phân biệt rõ ràng giữa các phương pháp. Trên 939 bệnh nhân, kết quả điều trị theo cảm nhận của bệnh nhân sau phẫu thuật là tương đương nhau ở tất cả các phương pháp; tuy nhiên, phương pháp cố định bằng mảnh ghép hình móc và dùng dây kim loại lại có tỷ lệ biến chứng cao nhất; còn phương pháp Weaver-Dunn được chỉnh sửa thì có tỷ lệ phải phẫu thuật lại cao nhất [4]. Khi so sánh hai kỹ thuật được sử dụng phổ biến nhất hiện nay trên 399 bệnh nhân, phương pháp dùng nút khâu cho kết quả chức năng tốt hơn và mức độ đau ít hơn so với phương pháp dùng mảnh ghép hình móc; thời gian phẫu thuật, tỷ lệ biến chứng và tình trạng mất cố định giữa hai phương pháp không có sự khác biệt đáng kể [5].

Các nghiên cứu về tái tạo cũng cho kết quả tương tự. Trên 4,473 bệnh nhân, các phương pháp tái tạo bằng vật liệu sinh học hoặc tổng hợp cho kết quả chức năng tốt hơn so với phương pháp cố định xương bằng nẹp kim loại [6]; trên 1,020 bệnh nhân, các phương pháp tái tạo theo cấu trúc giải phẫu cho kết quả tốt nhất, trong đó các phương pháp nội soi vượt trội hơn so với phương pháp mổ mở [7].

Nhìn chung, các phương pháp tái tạo giải phẫu hiện đại có xu hướng cho kết quả tương đương nhau, trong khi các loại vật liệu cố định cứng cáp thế hệ cũ lại có kết quả kém hơn. Trong nhóm các phương pháp hiện đại, sự khác biệt giữa chúng là không đáng kể; do đó, mức độ quen thuộc của bác sĩ phẫu thuật với từng kỹ thuật cũng là cơ sở hợp lý để lựa chọn.

Một trên năm bệnh nhân có tổn thương khác bên trong khớp

Phát hiện này rất đáng chú ý vì những tổn thương này không thể nhìn thấy trên các phim chụp dùng để chẩn đoán. Trong số 860 bệnh nhân bị trật khớp AC cấp tính và đã được phẫu thuật, khoảng một trên năm bệnh nhân có tổn thương nội khớp đi kèm cần can thiệp thêm; phổ biến nhất là các tổn thương môi khớp hoặc cơ chóp xoay. Vì lý do này, các tác giả đề xuất nên tiến hành nội soi khớp ngay cả khi phẫu thuật tái tạo được thực hiện qua đường mổ hở [8].

Phát hiện này cũng giúp giải thích cho một nhóm nhỏ các ca bệnh gây khó khăn: những trường hợp vai vẫn đau sau khi phẫu thuật tái tạo được thực hiện đúng kỹ thuật; có thể do bệnh nhân đã có tổn thương thứ hai ngay từ đầu.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

Section 6 -- Term and Termination.

a. This Public License applies for the term of the Copyright and Similar Rights licensed here. However, if You fail to comply with this Public License, then Your rights under this Public License terminate automatically.

b. Where Your right to use the Licensed Material has terminated under Section 6(a), it reinstates:

1. automatically as of the date the violation is cured, provided it is cured within 30 days of Your discovery of the violation; or

2. upon express reinstatement by the Licensor.

For the avoidance of doubt, this Section 6(b) does not affect any right the Licensor may have to seek remedies for Your violations of this Public License.

c. For the avoidance of doubt, the Licensor may also offer the Licensed Material under separate terms or conditions or stop distributing the Licensed Material at any time; however, doing so will not terminate this Public License.

d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.

d. Nothing in this Public License constitutes or may be interpreted as a limitation upon, or waiver of, any privileges and immunities that apply to the Licensor or You, including from the legal processes of any jurisdiction or authority.
