Cơ bắp chính là “động cơ” của cơ thể: chúng chuyển hóa các tín hiệu từ não thành chuyển động và lực, đồng thời chiếm phần lớn trọng lượng cơ thể. Khác với sụn, cơ bắp có khả năng tự phục hồi và thậm chí tái tạo nhờ nguồn tế bào gốc chuyên biệt. Tuy nhiên, khả năng phục hồi này cũng có giới hạn: những tổn thương nhẹ thường lành lại tốt, trong khi những vết rách lớn hoặc chấn thương bị bỏ mặc lâu ngày thường để lại sẹo và không thể hồi phục hoàn toàn. Trang này sẽ giải thích một cách dễ hiểu về bản chất của cơ bắp và cơ chế phục hồi của chúng; sau đó, dành cho những ai tò mò, sẽ đi sâu vào cách cơ bắp co lại và lý do vì sao một số chấn thương lại gây ra tình trạng yếu cơ kéo dài.

Cơ là gì và chức năng của nó

Cơ là tập hợp các tế bào dạng sợi dài (sợi cơ) được xếp chồng lên nhau giống như các sợi dây thừng, và được gắn vào xương nhờ các gân. Khi não gửi tín hiệu xuống dây thần kinh, các sợi cơ sẽ co lại, kéo theo gân và làm cho khớp cử động. Đó là toàn bộ chức năng của cơ: chuyển đổi tín hiệu điện và năng lượng hóa học thành lực kéo.

Cơ được cung cấp máu rất dồi dào; vì vậy khi bị rách, cơ sẽ chảy máu và xuất hiện vết bầm. Đồng thời, điều này cũng giúp cơ có khả năng hồi phục tốt hơn nhiều so với các mô ít được cung cấp máu như sụn.

Quy trình hồi phục của cơ

Sau khi bị căng giãn hoặc rách, cơ thể trải qua nhiều giai đoạn hồi phục:

  1. Chảy máu và viêm (những ngày đầu). Vùng bị tổn thương chảy máu và sưng lên; các tế bào dọn dẹp xuất hiện để xử lý. Đây là giai đoạn gây đau đớn và xuất hiện vết bầm.
  2. Tái tạo (những tuần đầu). Các tế bào gốc được kích hoạt và tạo ra các sợi cơ mới để lấp đầy khoảng trống – đó là những sợi cơ thật sự chứ không phải mô sẹo.
  3. Tái cấu trúc (từ vài tuần đến vài tháng). Các sợi cơ mới dần trưởng thành và sắp xếp lại theo cách sử dụng cơ thể; sức mạnh cũng dần hồi phục.

Vấn đề là quá trình tái tạo và hình thành sẹo diễn ra song song. Những tổn thương nhỏ và sạch sẽ thường hồi phục tốt. Ngược lại, những vết rách lớn khiến mô sợi (và đôi khi cả mô mỡ) hình thành nhanh hơn so với tốc độ tạo ra cơ mới; mô sẹo này yếu hơn và kém đàn hồi hơn cơ bình thường. Đó là lý do vì sao những vết rách lớn hoặc những vết thương không được điều trị kịp thời có thể gây ra tình trạng yếu cơ vĩnh viễn.

Những yếu tố giúp cơ bắp hồi phục

  • Vận động sớm và có mức độ. Việc vận động nhẹ nhàng rồi tăng dần mức độ tải trọng sẽ giúp cơ mới được tái tạo và sắp xếp lại đúng trật tự; ngược lại, việc nghỉ ngơi hoàn toàn trong thời gian dài sẽ dẫn đến teo cơ và cứng khớp.
  • Không nên quá sức ngay từ đầu. Việc chịu tải quá mức đối với vùng cơ vừa bị rách sẽ khiến tổn thương nặng thêm và thúc đẩy quá trình hình thành sẹo.
  • Sức khỏe tổng thể tốt. Việc cung cấp đủ protein, không hút thuốc và kiểm soát tốt bệnh tiểu đường đều góp phần hỗ trợ quá trình phục hồi cơ.
  • Điều trị kịp thời các vết rách lớn. Một số trường hợp rách hoàn toàn (chẳng hạn như đứt gân – cơ) cần được phẫu thuật sửa chữa ngay lập tức, trước khi cơ bị co rút và teo đi.

Toàn bộ cơ thể được cấu tạo theo dạng phân cấp, giống như một sợi dây cáp gồm nhiều sợi dây nhỏ hơn: cơ gồm các bó (fascicles) gồm những sợi cơ dài; mỗi sợi cơ là một tế bào lớn chứa đầy các myofibril có dạng sợi chỉ; mỗi myofibril lại là chuỗi các đơn vị co cơ siêu nhỏ gọi là sarcomere. Chính tại sarcomere mà lực được tạo ra. Đây là cấu trúc lặp lại một cách chính xác gồm hai loại sợi xen kẽ nhau: các sợi dày làm từ protein myosin và các sợi mỏng làm từ actin.

Mỗi sợi cơ thuộc về một đơn vị vận động: một tế bào thần kinh và tất cả các sợi cơ mà nó điều khiển. Các chuyển động tinh tế (như cử động của bàn tay) sử dụng các đơn vị vận động nhỏ; các chuyển động mạnh mẽ thì dùng các đơn vị vận động lớn. Các sợi cơ cũng được chia thành các loại: sợi co chậm (chịu mệt mỏi tốt, dùng cho các hoạt động chịu lực và giữ tư thế) và sợi co nhanh (mạnh mẽ nhưng dễ mệt).

Cơ chế co cơ

Cơ chế co cơ được gọi là cơ chế “sợi trượt”. Khi tín hiệu thần kinh đến khớp thần kinh-cơ (nơi tiếp nối giữa dây thần kinh và cơ; xem cách hoạt động của dây thần kinh), chất hóa học được giải phóng khiến sợi cơ phát ra xung điện riêng của mình. Xung điện này làm tăng nồng độ canxi trong sợi cơ, từ đó làm lộ ra các vị trí liên kết trên các sợi actin. Các đầu myosin sau đó bám vào actin và kéo chúng vào trong (giống như đội kéo co cùng nhau kéo sợi dây), khiến các sợi dày và mỏng trượt qua nhau và làm cho sarcomere ngắn lại. Khi quá trình này diễn ra ở hàng tỷ sarcomere, toàn bộ cơ sẽ co lại. Tất cả các hoạt động này đều phụ thuộc vào ATP – nguồn năng lượng chính của tế bào; khi lượng ATP cạn kiệt, cơ sẽ mệt mỏi.

Cơ chế hồi phục cơ bắp: tế bào vệ tinh so với sẹo

Bí mật trong quá trình phục hồi cơ bắp nằm ở tế bào vệ tinh – những tế bào gốc dự trữ nằm sát bên từng sợi cơ. Sau khi bị tổn thương, các tế bào vệ tinh được kích hoạt, nhân lên và hợp nhất để tạo thành những sợi cơ mới. Quá trình hồi phục diễn ra qua ba giai đoạn: giai đoạn phá hủy (chảy máu và viêm), giai đoạn tái tạo (tế bào vệ tinh xây dựng lại sợi cơ), và giai đoạn tái cấu trúc (sợi cơ mới trưởng thành dưới tác động của lực cơ học).

Tuy nhiên, quá trình tái tạo này phải đối mặt với sự cạnh tranh từ xơ hóa. Các phân tử gây viêm và các tín hiệu sinh học, đặc biệt là TGF-β1, thúc đẩy các tế bào sợi sản xuất collagen tạo thành mô sẹo. Trong trường hợp tổn thương nhỏ, quá trình tái tạo giành chiến thắng. Ngược lại, với những tổn thương lớn hoặc tái diễn nhiều lần, mô sẹo (đôi khi kèm mô mỡ) sẽ chiếm ưu thế, tạo thành vùng mô có tính cơ học kém hơn so với cơ bắp thật. Đây là lý do vì sao những chấn thương cơ nặng thường không hồi phục hoàn toàn.

Tình trạng thâm nhập mỡ và cơ vùng cơ chóp xoay

Hiện tượng này có ý nghĩa lâm sàng quan trọng trong cơ vùng cơ chóp xoay ở vai. Khi gân cơ chóp xoay bị rách mà không được điều trị, cơ gắn liền với gân dần co rút, teo đi và bị thay thế bởi mỡ (hiện tượng thâm nhập mỡ). Sự thay đổi này gần như không thể đảo ngược, ngay cả khi sau đó gân được phẫu thuật nối lại; cơ đã bị thoái hóa sẽ không bao giờ phục hồi hoàn toàn sức mạnh ban đầu. Đây là một trong những lý do chính khiến các bác sĩ phẫu thuật đặc biệt quan tâm đến thời điểm thực hiện các ca nối gân: việc phẫu thuật trước khi cơ bị thoái hóa sẽ mang lại kết quả tốt hơn nhiều.

Dùng thì còn, không dùng thì mất: teo cơ và sự thích nghi

Cơ bắp luôn được điều chỉnh cho phù hợp với nhu cầu sử dụng. Khi chịu tải trọng tăng dần, cơ sẽ phì đại: các sợi cơ to ra và mạnh lên (các tế bào vệ tinh cung cấp nhân tế bào để hỗ trợ sự phát triển này). Ngược lại, khi không được sử dụng (do nằm liệt giường, đeo nẹp cố định hoặc dây thần kinh bị tổn thương), cơ sẽ teo đi rất nhanh; chỉ trong vài tuần đã mất đi kích thước và sức mạnh. Cơ bị cắt đứt liên kết với dây thần kinh (mất thần kinh chi phối) sẽ teo nhanh nhất; nếu dây thần kinh không hồi phục kịp thời, cơ cuối cùng sẽ bị thay thế bởi mỡ và mô sợi. Đó là lý do tại sao việc phục hồi chức năng và duy trì hoạt động cho cơ bắp trong quá trình hồi phục lại quan trọng đến vậy.

Những yếu tố hỗ trợ và cản trở quá trình hồi phục cơ

  • Tải trọng có kiểm soát là yếu tố kích thích chính cho sự tái tạo và sắp xếp lại các sợi cơ; việc áp dụng tải trọng quá mức hoặc quá sớm có thể gây rách và hình thành sẹo.
  • Tuần hoàn máu ở cơ rất tốt; vì vậy cơ hồi phục nhanh hơn so với sụn hoặc các phần bên trong gân.
  • Hút thuốc, bệnh đái tháo đường, tuổi tác cao và việc sử dụng corticosteroid đều làm suy giảm khả năng phục hồi và thúc đẩy tình trạng teo cơ.
  • Thời gian và mức độ tổn thương quyết định giới hạn phục hồi: các chấn thương nhẹ có thể hồi phục hoàn toàn; còn các vết rách lớn hoặc cơ đã bị thoái hóa thì không thể hồi phục được.

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

Section 5 -- Disclaimer of Warranties and Limitation of Liability.

a. Section 7 -- Other Terms and Conditions.

Section 8 -- Interpretation.
