Hầu hết các ca phẫu thuật vai đều được thực hiện kết hợp với chặn dây thần kinh và gây mê toàn thân ở mức độ nhẹ. Chặn dây thần kinh là gì?

Tất cả các dây thần kinh chi phối cánh tay đều đi qua một nhóm dây thần kinh chặt chẽ ngay phía trên xương đòn. Bằng cách sử dụng máy siêu âm để xác định chính xác vị trí của chúng, bác sĩ gây mê sẽ tiêm một lượng nhỏ thuốc tê quanh nhóm dây thần kinh này. Trong lúc phẫu thuật, bạn sẽ không tỉnh táo. Thông thường, người ta sẽ kết hợp thuốc gây mê toàn thân với phương pháp chặn dây thần kinh này, giúp bạn ngủ say suốt ca mổ. Phương pháp chặn dây thần kinh giúp giảm đau; còn thuốc gây mê toàn thân giúp bạn ngủ yên một cách thoải mái.

Tại sao chúng tôi sử dụng phương pháp này cho vai của bạn

Phương pháp gây tê này mang lại hiệu quả vượt xa so với việc chỉ dùng kim tiêm để gây tê:

  • Phương pháp này kiểm soát cơn đau tốt hơn so với việc dùng thuốc viên hoặc gây mê toàn thân đơn thuần.
    Trong nhiều nghiên cứu, những bệnh nhân được gây tê đám rối thần kinh cánh tay có chỉ số đau thấp hơn trong những giờ đầu sau phẫu thuật vai, đồng thời cần ít thuốc giảm đau mạnh hơn. Cảm giác sau khi tiêm

Sau khoảng 20–30 phút kể từ lúc tiêm, cánh tay sẽ trở nên nặng nề, ấm áp, tê cứng và yếu đi. Bạn sẽ không thể nhấc tay lên hay cảm nhận được gì nhiều ở tay; thậm chí có thể không thể cử động các ngón tay. Đây chính là phản ứng mong muốn sau khi tiêm. Tình trạng tê thường kéo dài từ 8 đến 18 giờ, thỉnh thoảng có thể lên tới một ngày. Tuy nhiên, hiện tượng này hoàn toàn tạm thời: cảm giác và khả năng vận động sẽ phục hồi hoàn toàn sau đó.

Tại sao bạn cần đeo đai treo tay

Trong thời gian miếng băng ép vẫn đang hoạt động, cánh tay của bạn sẽ bị tê và mất khả năng cử động. Chiếc đai treo tay có tác dụng giữ và bảo vệ cánh tay, giúp nó không bị lủng lẳng, đung đưa hay va chạm; đồng thời giúp bạn tránh việc tựa người hoặc nằm đè lên cánh tay mà bạn không cảm nhận được. Những điều bạn có thể nhận thấy (đây là những hiện tượng bình thường)

Do các dây thần kinh bị tê nằm gần một vài dây thần kinh khác ở vùng cổ, bạn có thể nhận thấy một vài tác dụng phụ tạm thời, vô hại ở phía đó:

Phương pháp gây tê vùng trên xương đòn mà chúng tôi sử dụng được chọn lựa một phần vì nó thường ít gây ra những tác dụng phụ này hơn so với các phương pháp gây tê ở vùng cao hơn trên cổ. Tất cả các triệu chứng này sẽ biến mất khi tác dụng của thuốc gây tê hết, và không cần điều trị gì thêm.

Khi tác dụng của thuốc gây tê hết – hãy uống thuốc giảm đau sớm

Key Evidence

Brachial plexus blocks reduce pain and opioid use after shoulder surgery. Regional anaesthesia is the cornerstone of modern shoulder-surgery analgesia. A current review of regional techniques for shoulder surgery describes the supraclavicular brachial plexus block as an effective approach that provides dense analgesia of the arm while producing fewer of the unwanted neck-related effects (hoarseness, Horner's syndrome, diaphragm involvement) seen with higher interscalene blocks [1]. Across the broader literature, peripheral nerve blocks consistently lower early post-operative pain scores and reduce opioid consumption compared with general anaesthesia or local infiltration alone [2,3,5].

The benefit is largest in the first hours and translates into better recovery quality. A systematic review and meta-analysis in Anesthesiology found brachial plexus and suprascapular blocks deliver clinically meaningful analgesia for shoulder surgery, supporting their routine use [2]. A focused review of regional blocks for arthroscopic rotator cuff repair reached the same conclusion: blocks improve early pain control and reduce rescue-opioid requirements [3]. Longer-acting block formulations further extend the pain-free window after shoulder replacement [4].

Rebound pain is real and is best managed by pre-emptive oral analgesia. Because a single-shot block wears off after roughly 8–18 hours, patients can experience a sharp surge of pain as sensation returns — usually overnight. A two-centre randomised controlled trial and the wider rebound-pain literature emphasise that patient education and starting regular analgesia before the block resolves are central to a smooth recovery [6]. This is why patients are advised to begin their prescribed pain tablets early rather than waiting for pain to arrive.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.

d. Nothing in this Public License constitutes or may be interpreted as a limitation upon, or waiver of, any privileges and immunities that apply to the Licensor or You, including from the legal processes of any jurisdiction or authority.
