Những cảm giác bạn đang trải qua

Hầu hết mọi người đều nhớ rõ khoảnh khắc đó. Thường thì nó xảy ra trong lúc đang thực hiện động tác nâng tạ nặng; ngay khi thanh tạ được hạ xuống ngực, bạn sẽ cảm thấy có tiếng “pop” hoặc cảm giác như có thứ gì đó bị rách ở phía trước vai, kèm theo cơn đau dữ dội. Trong vòng một hoặc hai ngày sau, vết bầm xuất hiện dọc theo mặt trước cánh tay và lan ra khắp vùng ngực; vùng này cũng bị sưng lên.

Khi tình trạng sưng giảm dần, bạn có thể nhận thấy hình dạng vùng ngực mình đã thay đổi. Nếp gấp săn chắc bình thường ở phía trước nách có thể bị phẳng ra hoặc biến mất; cơ bắp ở vùng này cũng có thể bị vón cục lại gần xương ức, trông và cảm giác khác hẳn so với bên còn lại. Các động tác đòi hỏi lực đẩy trở nên yếu đi: việc đóng cánh cửa nặng, đứng dậy khỏi ghế, hoặc đưa tay qua ngực để ôm người khác đều không còn sức mạnh như trước. Một phần của tình trạng yếu lực ban đầu là do cơn đau gây ra; tuy nhiên sự suy giảm thực sự về sức mạnh đẩy thường vẫn tồn tại.

Điều gì đang thực sự xảy ra

Cơ ngực lớn là cơ có hình quạt, nằm ở vùng ngực. Cơ này giúp vận động cánh tay theo ba cách chính: đẩy các vật ra xa khỏi người, ôm cánh tay vào thân mình và xoay cánh tay vào trong. Cơ này dần thu hẹp thành một gân chắc khỏe, gắn vào xương cánh tay (xương cánh tay – humerus), gần vùng vai.

Rách cơ là tình trạng cơ và gân bị đứt; thường xảy ra ngay tại chỗ gân gắn vào xương hoặc gần đó. Nguyên nhân phổ biến là do căng giãn đột ngột và mạnh mẽ trong lúc cơ đang co cứng (điều này thường xảy ra ở cuối động tác nâng tạ trên ghế dài khi hạ tạ xuống). Tình trạng này phổ biến hơn ở nam giới, đặc biệt là những người thường tập tạ; việc sử dụng steroid đồng hóa cũng là yếu tố nguy cơ được công nhận vì nó có thể làm yếu gân.

Mức độ rách cũng khác nhau. Rách hoàn toàn khiến gân tách hẳn khỏi xương; điều này gây ra sự thay đổi rõ rệt về hình dạng ngực và làm mất sức mạnh cơ nhiều nhất. Rách một phần, hoặc rách xảy ra trong thân cơ chứ không phải ở vùng gân, thì vẫn giữ lại phần lớn chỗ gắn vào xương. Loại rách nào bạn gặp phải có ảnh hưởng rất lớn đến phương pháp điều trị thích hợp.

Những biện pháp chúng tôi có thể áp dụng

Tại phòng khám của chúng tôi, Bác sĩ Kieran Hirpara là người chủ trì việc đánh giá chấn thương này; quy trình bắt đầu bằng việc hỏi bệnh kỹ lưỡng và khám lâm sàng để xác nhận chẩn đoán. Đối với trường hợp rách hoàn toàn ở người năng động, phẫu thuật khâu nối thường được khuyến nghị sớm thay vì thử điều trị không phẫu thuật trước; trong khi đó, các trường hợp rách một phần thường được điều trị mà không cần phẫu thuật.

Bước đầu tiên là xác định chính xác bộ phận nào bị rách và ở đâu. Việc này được thực hiện bằng cách khám vai và xác nhận kết quả thông qua chụp MRI, nhờ đó có thể biết rõ rách là một phần hay toàn bộ, đồng thời xác định được là gân bị tách khỏi xương hay chỉ rách bên trong cơ.

Đối với những người năng động mà gân bị rách hoàn toàn, phẫu thuật nối lại gân vào xương thường được khuyến nghị. Ca phẫu thuật này giúp phục hồi sức mạnh khi đẩy và khôi phục lại hình dạng bình thường của vùng ngực; các bằng chứng lâm sàng cho thấy gân sau khi được phẫu thuật thường hồi phục sức mạnh tốt hơn so với trường hợp để tự lành. Yếu tố quan trọng nhất ở đây là thời điểm: kết quả tốt nhất đạt được khi phẫu thuật sớm, lý tưởng là trong vài tuần đầu sau chấn thương khi gân vẫn còn khỏe mạnh, chưa hình thành sẹo hay bị kéo lùi. Đây là lý do chính khiến việc đi khám sớm là rất cần thiết thay vì chờ xem tình trạng có tự cải thiện hay không.

Các trường hợp rách một phần, rách nằm trong thân cơ, hoặc những người lớn tuổi, có mức độ vận động thấp thường được điều trị rất hiệu quả mà không cần phẫu thuật: sau một thời gian nghỉ ngơi, bệnh nhân sẽ thực hiện chương trình phục hồi chức năng có kiểm soát để tái lập khả năng vận động và sức mạnh cơ. Phương pháp này giúp phục hồi chức năng sinh hoạt hàng ngày; tuy nhiên, so với ca phẫu thuật nối gân bị rách hoàn toàn thì sức mạnh đẩy tối đa vẫn bị giảm đi đôi chút. Trong trường hợp rách hoàn toàn bị bỏ sót ở giai đoạn đầu, vẫn có thể tiến hành phẫu thuật ở giai đoạn muộn hơn; tuy nhiên ca phẫu thuật sẽ phức tạp hơn và kết quả cũng kém ổn định hơn. Chính vì vậy việc đánh giá sớm là vô cùng quan trọng.

Những điều bạn có thể mong đợi

Nếu bạn trải qua phẫu thuật, quá trình hồi phục thường mất vài tháng chứ không phải vài tuần. Ban đầu, vùng được phẫu thuật sẽ được bảo vệ bằng cách sử dụng dây đeo cổ tay trong khoảng sáu tuần, đồng thời chỉ cho phép thực hiện các chuyển động nhẹ nhàng dưới sự hướng dẫn nhằm giúp gân gắn chắc trở lại với xương. Sau đó, việc tăng cường sức mạnh mới được thực hiện từ từ; các bài tập nặng hơn sẽ được hạn chế một cách có chủ đích. Các bài tập đẩy và nâng tạ trên ghế thường bị cấm trong khoảng sáu đến chín tháng. Hầu hết mọi người có thể trở lại các hoạt động sinh hoạt thường ngày sau khoảng sáu tuần, và có thể tham gia các môn thể thao hoặc tập luyện nặng hơn sau bốn đến sáu tháng; sức mạnh cơ thể vẫn tiếp tục cải thiện cho đến tận một năm sau phẫu thuật.

Việc kiên nhẫn chờ đợi sẽ mang lại kết quả tốt: hầu hết những người được phẫu thuật sớm đều lấy lại được sức mạnh đẩy mạnh mẽ, hình dáng ngực trở lại bình thường và có thể tiếp tục tham gia các hoạt động yêu thích. Những người không phẫu thuật cũng thường đạt được khả năng vận động ổn định trong sinh hoạt hàng ngày, dù sức mạnh tối đa có thể giảm đi một chút. Khi nào nên đi khám bác sĩ

  • Có tiếng “pop” hoặc cảm giác như có thứ bị rách ở vùng ngực hoặc phía trước vai khi nâng vật nặng hoặc gắng sức mạnh: cần đi khám ngay, vì kết quả phẫu thuật tốt nhất đạt được khi tiến hành sửa chữa sớm.
  • Vết bầm tím lan xuống cánh tay hoặc ngực, sưng phù rõ rệt, hoặc sự thay đổi rõ rệt về hình dạng ngực hoặc nếp gấp phía trước nách.
  • Tình trạng yếu cơ kéo dài khi đẩy hoặc đưa tay qua phía trước cơ thể sau khi cơn đau ban đầu đã giảm bớt.
  • Bất kỳ chấn thương gây rách nào mà bạn không chắc mức độ nghiêm trọng: việc chụp hình sớm sẽ giúp phân biệt rách một phần hay rách hoàn toàn; quyết định này cần được thực hiện kịp thời.

Phương pháp phẫu thuật phục hồi mang lại kết quả tốt hơn so với điều trị không phẫu thuật

Trên 664 bệnh nhân, việc phục hồi gân cơ ngực lớn mang lại kết quả vượt trội đáng kể so với điều trị không phẫu thuật; các chỉ số về chức năng vận động, sức mạnh cơ khi vận động theo tốc độ cố định và sức mạnh cơ tĩnh đều được cải thiện một cách có ý nghĩa thống kê; tỷ lệ biến chứng là 14,21% [1].

Cả hai nội dung trên cần được hiểu là một câu hoàn chỉnh. Đây là phương pháp phẫu thuật giúp phục hồi sức mạnh cơ bắp một cách rõ rệt – điều mà phương pháp điều trị không phẫu thuật không làm được; đồng thời cứ khoảng 7 bệnh nhân thì có 1 người gặp phải biến chứng. Đối với hầu hết các chấn thương gân ở vùng này, ưu thế của phẫu thuật ít rõ rệt hơn và tỷ lệ biến chứng cũng thấp hơn; riêng trường hợp này cả hai chỉ số đều cao hơn mức trung bình.

Sự suy giảm sức mạnh cơ là điều quan trọng vì vai trò của cơ ngực lớn. Cơ này chịu trách nhiệm thực hiện các động tác khép cánh tay ngang thân và xoay trong, cũng như các động tác đẩy. Khi gân cơ này bị rách mà không được phục hồi, thường xuất hiện dị dạng rõ rệt và mất đi khả năng đẩy mạnh; vì vậy chấn thương này chủ yếu gặp ở những người thường xuyên phải thực hiện các động tác nâng, đẩy.

Vị trí của vết rách quyết định phương pháp điều trị

Không phải tất cả các trường hợp đứt cơ ngực đều giống nhau. Phân loại hiện đại hiện nay dựa trên thời điểm chấn thương, vị trí chấn thương và thuật ngữ chuẩn để mô tả mức độ vết rách, nhằm phản ánh đúng cấu trúc cơ-gân và hỗ trợ việc lập kế hoạch phẫu thuật một cách hiệu quả [2].

Sự khác biệt quan trọng nằm ở chỗ: nếu vết rách xảy ra tại chỗ gân bám vào xương cánh tay thì có thể khâu nối lại dễ dàng; ngược lại, nếu vết rách nằm ngay trong thân cơ thì không có gân nào để cố định, nên thường chỉ cần điều trị nội khoa. Giữa hai trường hợp này là vùng tiếp giáp cơ-gân – nhóm trường hợp khó xử lý nhất.

Thời điểm xảy ra chấn thương cũng rất quan trọng: gân bị đứt sẽ co rút và hình thành sẹo; vì vậy trong các trường hợp đứt cơ đã kéo dài, có thể cần phải ghép gân thay vì chỉ khâu nối trực tiếp.

Tại sao chẩn đoán thường bị bỏ sót ở giai đoạn sớm

Ngay sau khi bị chấn thương, vùng ngực và cánh tay trên xuất hiện nhiều vết bầm tím và sưng nề, làm che khuất những thay đổi về hình dạng vùng cơ. Các dấu hiệu đặc trưng như mất nếp gấp nách phía trước, sự mất cân đối của cơ khi thực hiện động tác ép chống lại lực cản, và vùng lõm có thể sờ thấy sẽ trở nên rõ ràng hơn khi sưng nề giảm đi; lúc này chấn thương có thể đã bị chẩn đoán nhầm là căng cơ thông thường.

Vì việc phẫu thuật sửa chữa sẽ dễ dàng hơn nhiều nếu thực hiện ngay từ đầu, nên nếu có tiếng “pop” rõ ràng khi thực hiện động tác ép mạnh kèm theo vết bầm tím, cần tiến hành chụp hình thay vì chỉ theo dõi quan sát thông thường.
