Các triệu chứng đau nhức, tê ngứa như kim châm và đau ngón tay cái rất phổ biến trong thời kỳ mang thai cũng như những tháng đầu sau khi sinh con. Những triệu chứng này có thể gây khó chịu lớn, nhất là khi đôi tay bạn phải hoạt động nhiều hơn bao giờ hết. Tuy nhiên, tin vui là hầu hết các vấn đề này đều tạm thời, xuất phát từ những thay đổi trong thai kỳ và giai đoạn mới làm mẹ; phần lớn các triệu chứng sẽ tự khỏi nhờ các biện pháp đơn giản và thời gian. Tại sao bàn tay bị đau trong thai kỳ

Thai kỳ gây ra nhiều thay đổi trong cơ thể, ảnh hưởng mạnh đến các cấu trúc nhỏ ở cổ tay. Lượng dịch trong các mô tăng lên, đồng thời sự thay đổi nội tiết tố khiến lớp màng bao quanh gân và dây thần kinh trở nên mềm ra và sưng lên. Cổ tay là vùng có nhiều cấu trúc chật chội (dây thần kinh và gân đi qua những đường hầm hẹp), vì vậy ngay cả mức sưng nhỏ cũng có thể gây áp lực lên những bộ phận vốn trước đó hoàn toàn bình thường. Khi kết hợp thêm việc phải liên tục bế và cho con ăn – những hoạt động mới và lặp đi lặp lại – thì việc bàn tay và cổ tay bị đau là điều dễ hiểu.

Điều quan trọng cần ghi nhớ là nguyên nhân gây đau thường tạm thời: khi lượng dịch và sự thay đổi nội tiết tố trong thai kỳ trở lại bình thường sau khi sinh, cơ thể hồi phục dần, áp lực lên các cấu trúc này giảm xuống và các triệu chứng cũng theo đó biến mất.

Hội chứng ống cổ tay khi mang thai

Ống cổ tay là một đường hầm hẹp nằm phía trước cổ tay, có chức năng dẫn dây thần kinh giữa xuống bàn tay. Trong thai kỳ, tình trạng giữ nước khiến các mô bên trong ống cổ tay sưng lên và chèn ép dây thần kinh. Hậu quả là tê và cảm giác kiến bò ở ngón tay cái, ngón trỏ và ngón giữa (cùng nửa ngón áp út); triệu chứng này thường được mô tả như cảm giác kim châm, thường nặng nhất vào ban đêm, thậm chí khiến người mẹ phải thức dậy để lắc tay. Một số phụ nữ nhận thấy triệu chứng rõ rệt nhất trong vài tháng cuối thai kỳ.

Điều đáng mừng là tình trạng này thường thuyên giảm sau khi sinh: đa số phụ nữ thấy các triệu chứng giảm dần trong vòng vài tuần sau sinh, khi lượng dịch dư thừa được đào thải hết. Vì vậy, việc điều trị trong thai kỳ hầu như chỉ mang tính hỗ trợ, nhằm giúp mẹ cảm thấy thoải mái trong lúc cơ thể tự điều chỉnh:

  • Đeo nẹp cổ tay ban đêm là biện pháp chủ đạo. Chiếc nẹp giúp giữ cổ tay thẳng, giúp ống cổ tay luôn được mở rộng nhất khi ngủ; đây thường là phương pháp hiệu quả nhất.
  • Điều chỉnh hoạt động: hạn chế các tư thế hoặc công việc khiến cổ tay phải gập trong thời gian dài, đồng thời nghỉ ngơi thường xuyên.
  • Vật lý trị liệu cho tay: nhận lời khuyên về cách đeo nẹp, tư thế đúng và các kỹ thuật nhẹ nhàng để giảm triệu chứng.
  • Trong những trường hợp triệu chứng nặng và không thuyên giảm, có thể tiêm corticosteroid vào ống cổ tay nhằm giảm đau tạm thời cho đến khi sinh.

Phẫu thuật hầu như không cần thiết trong thai kỳ, bởi vấn đề này thường tự khỏi sau khi em bé chào đời.

Viêm bao gân de Quervain (“ngón cái của mẹ”)

Viêm bao gân de Quervain là tình trạng sưng đau ở các gân nằm phía bên ngón tay cái của cổ tay, nơi chúng đi qua một đường hầm chật hẹp gần xương cổ tay. Cơn đau xuất hiện ở phía bên ngón tay cái của cổ tay, thường tăng lên khi nắm chặt, nâng vật nặng hoặc xoay cổ tay; cơn đau đôi khi lan sang ngón tay cái và cẳng tay.

Tình trạng này được gọi là “ngón cái của mẹ” vì nó có liên quan mật thiết đến việc liên tục nâng, bế và ôm em bé – những hành động khiến cổ tay phải chịu tải hàng chục lần mỗi ngày, thường trong các tư thế không thuận lợi. Những thay đổi nội tiết tố trong thai kỳ và thời kỳ cho con bú cũng góp phần gây ra bệnh. Khác với hội chứng ống cổ tay thường xuất hiện trước khi sinh, viêm bao gân de Quervain thường xuất hiện hoặc trở nặng sau khi sinh, trong những tháng đầu chăm sóc trẻ sơ sinh.

Thông thường bệnh sẽ thuyên giảm; phương pháp điều trị cũng khá đơn giản:

  • Nẹp ngón tay cái (nẹp spica): giúp cố định ngón tay cái và cổ tay, đồng thời để đầu ngón tay cái tự do; phương pháp này giúp các gân bị viêm được nghỉ ngơi và là nền tảng trong điều trị.
  • Thay đổi cách nâng bế trẻ: nên dùng động tác nâng từ phía dưới, giữ cổ tay thẳng và để cẳng tay đảm nhận sức nâng; thay vì nắm giữ bằng ngón cái và cổ tay duỗi căng ra. Chuyên viên trị liệu tay có thể hướng dẫn những thay đổi kỹ thuật đơn giản nhưng hiệu quả rõ rệt.
  • Trong trường hợp đau dai dẳng, việc tiêm steroid vào đường hầm chứa gân bị tổn thương thường mang lại hiệu quả tốt và có thể chữa khỏi hoàn toàn.

Nhiều trường hợp bệnh sẽ tự khỏi khi những nhu cầu chăm sóc trẻ trong những tháng đầu giảm bớt; đối với các bà mẹ cho con bú, tình trạng này thường hết hẳn sau khi ngừng cho con bú.

Những cơn đau khác có thể xuất hiện và biến mất

Vài vấn đề nhỏ liên quan đến tay và cổ tay cũng có thể xuất hiện vào giai đoạn này; tất cả đều khá phổ biến và thường chỉ tạm thời:

  • Cơn đau và sưng ở cổ tay, bàn tay: do tình trạng tích nước tương tự; triệu chứng này thường nặng hơn về cuối ngày và giảm dần sau khi sinh.
  • Ngón tay bị kẹt (trigger finger): hiện tượng ngón tay hoặc ngón cái bị vướng, phát ra tiếng “cạch” hoặc bị khóa lại khi gập lại; nguyên nhân là do sưng quanh gân. Tình trạng này có thể xuất hiện trong thai kỳ hoặc những tháng sau sinh; thường sẽ thuyên giảm với nẹp, vật lý trị liệu cho tay hoặc, nếu cần thiết, tiêm steroid.

Những phương pháp nào an toàn khi mang thai và cho con bú?

Một nỗi lo phổ biến là liệu có phương pháp nào có thể áp dụng một cách an toàn trong thời kỳ mang thai hoặc cho con bú hay không. Tin vui là các phương pháp điều trị được ưu tiên cũng là những phương pháp an toàn nhất:

  • Đeo nẹp, vật lý trị liệu cho tay và điều chỉnh các hoạt động hàng ngày hoàn toàn không gây rủi ro cho mẹ và thai nhi; đây là những biện pháp chúng tôi thường áp dụng đầu tiên. Thông thường, chỉ cần những biện pháp này là đủ.
  • Tiêm steroid và phẫu thuật chỉ được sử dụng trong số ít trường hợp không đáp ứng với các phương pháp trên. Các phương pháp này sẽ được thảo luận riêng với từng bệnh nhân, đồng thời cân nhắc thời điểm thực hiện so với tình trạng mang thai hoặc cho con bú, nhằm đảm bảo mọi quyết định được đưa ra dựa trên đầy đủ thông tin và phù hợp với hoàn cảnh cụ thể của từng người.

Vì hầu hết các tình trạng này đều tự khỏi sau khi sinh, kế hoạch điều trị thông thường là giúp mẹ cảm thấy thoải mái bằng các biện pháp nhẹ nhàng và để thời gian giúp giải quyết vấn đề.

Khi nào nên gặp bác sĩ

Vui lòng liên hệ với chúng tôi, hoặc gặp bác sĩ đa khoa, nữ hộ sinh hoặc đội ngũ của chúng tôi nếu:

  • Tình trạng tê hoặc ngứa ran trở nên nghiêm trọng hoặc kéo dài liên tục, thay vì chỉ xuất hiện và biến mất vào ban đêm.

Advanced reading: the deeper science (optional)

Phần này đi sâu hơn mức cần thiết để bạn tự đưa ra quyết định điều trị. Các bệnh lý về bàn tay trong thai kỳ rất đáng để tìm hiểu thêm, bởi mối liên hệ giữa thai kỳ và các bệnh lý này là có thật và hiện đã được xác nhận qua số liệu thống kê rất lớn; đồng thời có một yếu tố có thể thay đổi được lại làm tăng nguy cơ mắc cả hai vấn đề phổ biến này.

Mang thai là một yếu tố nguy cơ thực sự, chứ không phải sự trùng hợp về thời điểm

Hội chứng ống cổ tay và viêm bao gân de Quervain thường xuất hiện trong thời kỳ mang thai và vài tháng sau khi sinh; người ta dễ cho rằng điều này là do gánh nặng chăm sóc trẻ sơ sinh. Tuy nhiên, một nghiên cứu trên 715.337 bệnh nhân đã chứng minh rõ ràng hơn: Mang thai là yếu tố nguy cơ đáng kể gây ra các bệnh lý ở tay, liên quan đến khả năng mắc viêm bao gân de Quervain cao hơn [1].

Quy mô mẫu nghiên cứu này rất quan trọng, vì nó giúp phân biệt giữa một mối liên quan thực sự và chỉ là cảm nhận chủ quan. Tình trạng giữ nước và những thay đổi nội tiết tố trong thai kỳ có thể làm tăng áp lực bên trong ống cổ tay, gây sưng màng bao gân; đồng thời việc bế đứa trẻ ngày càng lớn cũng khiến ngón tay cái phải chịu tải trọng liên tục. Các dữ liệu thu được xác nhận rõ mối tương quan này mà không cần phải làm rõ cơ chế cụ thể.

Tiểu đường thai kỳ làm tăng nguy cơ mắc cả hai bệnh lý

Bộ dữ liệu này xác định tiểu đường thai kỳ là yếu tố nguy cơ đáng kể đối với cả hội chứng ống cổ tay và viêm bao gân de Quervain [1].

Đây là thông tin hữu ích nhất ở đây, vì nó liên kết một vấn đề về tay với tình trạng đang được theo dõi trong thai kỳ. Thông tin này cũng phù hợp với những hiểu biết hiện có ngoài thời kỳ mang thai: tiểu đường là yếu tố nguy cơ đã được xác nhận đối với cả hai bệnh lý; những thay đổi ở mô liên kết gây ra tình trạng này dường như diễn ra trong khoảng thời gian ngắn hơn dự kiến.

Những trường hợp mang thai nào có nguy cơ mắc bệnh de Quervain cao hơn?

Đối với bệnh de Quervain sau sinh, các yếu tố nguy cơ trong thai kỳ được xác định là lần mang thai đầu tiên và thai kỳ kéo dài hơn 40 tuần. Đáng chú ý là cân nặng của trẻ sơ sinh cũng như số lượng trẻ sinh ra không làm tăng nguy cơ mắc bệnh [2].

Hai kết quả âm tính này cũng quan trọng không kém các kết quả dương tính. Việc sinh ra em bé nặng cân hay sinh đôi đều không làm tăng nguy cơ mắc bệnh; điều này đi ngược lại lời giải thích đơn giản “việc nâng vác nhiều gây ra bệnh” và cho thấy môi trường nội tiết tố mới là yếu tố chính. Lần mang thai đầu tiên có thể phản ánh việc người mẹ chưa quen với các kỹ thuật nâng vác; hoặc đơn giản là sau khi đã mắc bệnh, cách họ nâng vác sẽ thay đổi về sau.

Những gì bằng chứng y khoa hiện chưa làm rõ

Về mặt hướng dẫn điều trị thì các nghiên cứu khoa học còn khá hạn chế. Một phân tích về hội chứng ống cổ tay ở phụ nữ mang thai cho thấy tỷ lệ mắc bệnh thay đổi đáng kể tùy theo tiêu chuẩn chẩn đoán và thiết kế nghiên cứu; quan trọng hơn, hiện còn rất ít bằng chứng chất lượng cao về các chiến lược điều trị tối ưu trong và sau thai kỳ. Các tác giả của nghiên cứu này kêu gọi tiến hành các thử nghiệm lâm sàng được thiết kế kỹ lưỡng [3].

Như vậy, nguy cơ mắc bệnh đã được xác nhận rõ ràng, nhưng phương pháp điều trị thì chưa được xác định chắc chắn. Trên thực tế, điều này ủng hộ việc áp dụng phương pháp điều trị bảo tồn trong suốt thai kỳ và giai đoạn hậu sản sớm; bởi nhiều trường hợp sẽ tự khỏi khi các thay đổi sinh lý trở lại bình thường. Tuy nhiên, cần hiểu rằng đây là kết luận dựa trên diễn biến tự nhiên của bệnh chứ không phải từ các bằng chứng từ thử nghiệm lâm sàng.

Từ đó dẫn đến hai hệ quả thực tiễn. Thứ nhất, những triệu chứng vẫn còn tồn tại lâu sau giai đoạn hậu sản thì ít có khả năng tự khỏi hơn; vì vậy cần được đánh giá lại thay vì cứ tiếp tục chờ đợi. Thứ hai, vì những tình trạng này khá phổ biến, thực sự xảy ra và chỉ kéo dài trong thời gian ngắn ở hầu hết phụ nữ, nên nên đề cập đến chúng trong các buổi khám thai định kỳ hoặc sau sinh, thay vì âm thầm chịu đựng như một phần không thể tránh khỏi của việc làm cha mẹ.

To the extent possible, the Licensor waives any right to collect royalties from You for the exercise of the Licensed Rights, whether directly or through a collecting society under any voluntary or waivable statutory or compulsory licensing scheme. In all other cases the Licensor expressly reserves any right to collect such royalties, including when the Licensed Material is used other than for NonCommercial purposes.

Section 3 -- License Conditions.

Your exercise of the Licensed Rights is expressly made subject to the following conditions.

a. Attribution.

1. If You Share the Licensed Material (including in modified form), You must:

a. retain the following if it is supplied by the Licensor with the Licensed Material:

i. identification of the creator(s) of the Licensed Material and any others designated to receive attribution, in any reasonable manner requested by the Licensor (including by pseudonym if designated);

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

Section 6 -- Term and Termination.

a. This Public License applies for the term of the Copyright and Similar Rights licensed here. However, if You fail to comply with this Public License, then Your rights under this Public License terminate automatically.

b. Where Your right to use the Licensed Material has terminated under Section 6(a), it reinstates:

1. automatically as of the date the violation is cured, provided it is cured within 30 days of Your discovery of the violation; or

2. upon express reinstatement by the Licensor.

For the avoidance of doubt, this Section 6(b) does not affect any right the Licensor may have to seek remedies for Your violations of this Public License.

c. For the avoidance of doubt, the Licensor may also offer the Licensed Material under separate terms or conditions or stop distributing the Licensed Material at any time; however, doing so will not terminate this Public License.

d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.

d. Nothing in this Public License constitutes or may be interpreted as a limitation upon, or waiver of, any privileges and immunities that apply to the Licensor or You, including from the legal processes of any jurisdiction or authority.
