Khi bác sĩ phẫu thuật yêu cầu thực hiện các phương pháp chẩn đoán hình ảnh, có lúc chúng ta cảm thấy như đang đọc một chuỗi các từ viết tắt khó hiểu: X-quang, siêu âm, MRI, CT. Mỗi phương pháp là một công cụ riêng biệt, cho thấy những thông tin khác nhau; điều này tương tự như sự khác biệt giữa một bức ảnh, một đoạn video và mô hình 3D. Không có phương pháp nào “tốt hơn” các phương pháp còn lại; việc lựa chọn phương pháp nào phụ thuộc hoàn toàn vào những gì chúng ta cần quan sát. Việc hiểu rõ ưu điểm của từng phương pháp chẩn đoán sẽ giúp giảm bớt sự bí ẩn trong các chỉ định y tế, đồng thời giúp bạn hiểu tại sao đôi khi chúng tôi yêu cầu thực hiện nhiều phương pháp cùng lúc.

Điều quan trọng nhất cần hiểu là mỗi loại phương pháp chụp chiếu đều phù hợp với một loại mô cụ thể. Xương, gân, dây chằng, sụn và dây thần kinh đều hiện lên khác nhau trên hình ảnh chụp; không có phương pháp nào có thể hiển thị tất cả chúng một cách hoàn hảo. Chụp X-quang rất hiệu quả để quan sát xương nhưng hầu như không thể phát hiện được gân; siêu âm lại cho hình ảnh gân rất rõ nét nhưng không thể nhìn sâu vào bên trong khớp. Vì vậy, khi lựa chọn phương pháp chụp chiếu, chúng ta thực chất đang đặt ra một câu hỏi: Vấn đề nằm ở xương, ở các mô mềm, hay cả hai? Câu trả lời sẽ quyết định phương pháp chụp nào nên dùng.

Đó cũng là lý do vì sao bạn có thể phải chụp một loại hình ảnh rồi lại được chỉ định chụp thêm loại khác. Thông thường điều này không có nghĩa là kết quả chụp lần đầu có vấn đề; mà là chúng ta đã xác định được phạm vi vấn đề và giờ cần một loại hình ảnh khác để giải đáp nó.

Chụp X-quang là phương pháp chẩn đoán hình ảnh nhanh nhất và phổ biến nhất; nó vẫn là xét nghiệm đầu tiên được chỉ định cho hầu hết các vấn đề liên quan đến xương. Phương pháp này sử dụng một lượng nhỏ bức xạ chiếu qua vùng cần kiểm tra; các mô xương đặc sẽ hiện lên màu trắng rõ rệt, nhờ đó rất hữu ích trong việc phát hiện gãy xương, viêm khớp, tình trạng sắp xếp của các xương và trật khớp. Quy trình này diễn ra nhanh chóng, chi phí thấp, có sẵn ở nhiều nơi, và liều lượng bức xạ cũng rất nhỏ.

Cơ chế hoạt động thực tế. Các tia X được tạo ra bên trong một ống nhỏ nằm phía một bên cơ thể: dòng điện làm nóng sợi dây tóc, khiến các electron bắn ra và va chạm vào mục tiêu kim loại; khi chúng dừng lại, năng lượng này chuyển thành tia X – một dạng “ánh sáng” có năng lượng rất cao. Khác với ánh sáng thông thường, tia X có thể xuyên qua mô mềm một cách dễ dàng. (Tia X thuộc cùng nhóm bức xạ với tia gamma, nhưng được tạo ra bởi các electron chứ không phải từ bên trong nhân nguyên tử.) Khi chùm tia X đi qua cơ thể, các mô xương đặc hấp thụ nhiều bức xạ hơn so với mô mềm hoặc không khí. Một bộ cảm biến phẳng đặt ở phía đối diện sẽ ghi lại mức độ bức xạ đi qua từng điểm: nơi có nhiều tia X tới thì hình ảnh sẽ tối, còn nơi ít tia X tới (phía sau xương) thì hình ảnh sẽ sáng; bản đồ bóng tối này chính là hình ảnh chụp được.

Hạn chế của phương pháp này lại là mặt trái của ưu điểm: chụp X-quang cho hình ảnh rõ nét về xương nhưng chỉ hiển thị mô mềm (gân, dây chằng và sụn) dưới dạng những vùng mờ màu xám. Vì vậy, kết quả chụp X-quang bình thường không thể loại trừ hoàn toàn chấn thương mô mềm; nó chỉ cho biết xương vẫn nguyên vẹn – điều mà chúng ta thường cần biết trước tiên để yên tâm.

Siêu âm sử dụng sóng âm tần số cao thay vì bức xạ, nên hoàn toàn không có bất kỳ liều phóng xạ nào. Người ta đặt một ít gel lên da rồi di chuyển đầu dò nhỏ trên bề mặt da; phương pháp này đặc biệt hữu ích trong việc quan sát các mô mềm nằm gần bề mặt: gân, nang dịch, các túi chứa dịch và tình trạng viêm.

Cơ chế hoạt động. Đầu dò chứa các tinh thể có khả năng rung động khi được kích thích bằng xung điện nhỏ (hiện tượng áp điện); những xung này tạo ra sóng âm tần số quá cao đến mức tai người không thể nghe thấy. Mỗi khi sóng âm đi qua ranh giới giữa hai loại mô khác nhau, một phần sóng sẽ phản hồi lại. Lúc này đầu dò chuyển sang chế độ “nghe” để ghi nhận các sóng phản hồi này. Máy siêu âm đo thời gian sóng phản hồi trở về (để xác định độ sâu của ranh giới mô) và cường độ sóng (để xác định mức độ sáng của hình ảnh); hàng nghìn dữ liệu như vậy được tổng hợp mỗi giây để tạo thành hình ảnh trực tiếp. Khi hướng đầu dò vào vùng máu đang lưu thông, máy còn có thể đo sự thay đổi tần số của sóng phản hồi (hiệu ứng Doppler) nhằm hiển thị và thậm chí đo lường tốc độ dòng máu.

Ưu điểm nổi bật nhất của siêu âm là khả năng hoạt động thời gian thực. Nhờ có hình ảnh hiển thị ngay lập tức, chúng tôi có thể yêu cầu bạn cử động tay hoặc vai và quan sát trực tiếp sự di chuyển của gân; điều mà hình ảnh tĩnh không thể thực hiện được. Vì vậy, siêu âm rất hữu ích trong chẩn đoán các vấn đề về gân ở cổ tay, bàn tay và vai. Điều quan trọng cần lưu ý là chất lượng hình ảnh phụ thuộc rất nhiều vào kỹ năng của người vận hành: kết quả chẩn đoán có thể thay đổi tùy theo tay nghề người cầm đầu dò; đồng thời siêu âm không thể quan sát được các vùng sâu bên trong khớp hay xuyên qua xương.

Chụp MRI sử dụng một nam châm mạnh (không phát ra bức xạ) để tạo ra những hình ảnh vô cùng chi tiết. Đây là phương pháp chẩn đoán toàn diện vì nó cho thấy cả mô mềm lẫn xương với độ chi tiết cao: dây chằng, sụn, dây thần kinh, tủy xương, thậm chí cả những vết gãy xương ẩn mà chụp X-quang thông thường không phát hiện được. Khi chúng tôi lên kế hoạch phẫu thuật hoặc cần xác định nguyên nhân của một vấn đề mà các phương pháp chẩn đoán khác chưa làm rõ, MRI thường là xét nghiệm quyết định.

Cơ chế hoạt động của máy chụp MRI. Cơ thể con người chủ yếu gồm nước và chất béo, nên chứa rất nhiều hydrogen; nhân của nguyên tử hydro là một proton đơn lẻ có tính chất như một nam châm nhỏ. Bình thường các proton này hướng ngẫu nhiên, nhưng trong từ trường mạnh của máy chụp, chúng sẽ xếp thẳng hàng theo hướng từ trường và dao động với tần số chính xác phụ thuộc vào cường độ từ trường. Máy chụp sau đó phát ra xung sóng vô tuyến có tần số tương ứng; xung này làm các proton lệch khỏi vị trí thẳng hàng và hấp thụ năng lượng. Khi xung sóng ngừng lại, các proton trở lại trạng thái thẳng hàng và phát ra năng lượng dưới dạng tín hiệu vô tuyến yếu; các cuộn thu tín hiệu sẽ ghi nhận những tín hiệu này. Các loại mô khác nhau phát ra tín hiệu với tốc độ khác nhau, tạo nên sự tương phản giữa sụn, dịch và xương. Các gradient từ trường xác định vị trí phát ra tín hiệu trong cơ thể; sau đó phép toán biến đổi Fourier chuyển khối lượng tín hiệu thu được thành hình ảnh chi tiết.

Một vài điều thực tế cần lưu ý để bạn không cảm thấy bất ngờ khi chụp:

  • Quy trình chụp mất khá nhiều thời gian, thường khoảng 20 đến 40 phút; bạn cần nằm yên để có hình ảnh rõ nét.
  • Bạn sẽ nằm trong một đường hầm; máy chụp phát ra nhiều tiếng ồn lớn, gồm cả tiếng gõ và va đập. Bạn sẽ được cung cấp nút bịt tai hoặc tai nghe.
  • Nếu bạn dễ bị sợ không gian kín, vui lòng thông báo cho chúng tôi trước; có nhiều cách giúp bạn cảm thấy dễ chịu hơn; đôi khi việc sử dụng máy chụp khác hoặc dùng thuốc an thần nhẹ cũng hữu ích.
  • Nếu bạn có các loại thiết bị kim loại cấy ghép (như máy tạo nhịp tim hoặc các vật liệu kim loại cũ), hãy cho chúng tôi biết. Hầu hết các loại đinh vít, tấm kim loại cấy ghép đều an toàn, nhưng chúng tôi luôn kiểm tra kỹ lưỡng; vì vậy hãy nêu rõ mọi thứ bạn đang sử dụng.

Chụp CT sử dụng nhiều tia X từ các góc độ khác nhau; máy tính sau đó xử lý các hình ảnh này thành các mặt cắt chi tiết, thậm chí là mô hình 3D. Giống như phim X-quang thông thường, hình ảnh chụp CT cũng tập trung vào cấu trúc xương, nhưng với độ chi tiết cao hơn nhiều. Vì vậy, phương pháp này rất hữu ích trong việc chẩn đoán các trường hợp gãy xương phức tạp (khi xương bị vỡ thành nhiều mảnh) và để lên kế hoạch phẫu thuật theo không gian ba chiều. Đối với những vết gãy khó xử lý ở cổ tay, khuỷu tay hoặc vai, chụp CT có thể cho thấy chính xác vị trí các mảnh xương, giúp chúng tôi lên kế hoạch phục hồi một cách chính xác.

Cơ chế hoạt động. Máy chụp CT về cơ bản là một thiết bị phát tia X có khả năng quay tròn: ống phát tia X và vòng các cảm biến quay quanh cơ thể bệnh nhân trong lúc bệnh nhân từ từ di chuyển qua lỗ hình vòng tròn của máy, thu nhận hình ảnh tia X từ hàng trăm góc độ khác nhau. Mỗi hình ảnh riêng lẻ chỉ là một bóng đổ phẳng; tuy nhiên, máy tính sẽ tổng hợp tất cả các hình ảnh này lại, tính toán mật độ mô tại từng điểm, từ đó tái tạo thành các “lát cắt” ngang có thể ghép lại thành mô hình 3D.

Tuy nhiên, phương pháp này sử dụng lượng bức xạ nhiều hơn so với phim X-quang thông thường; vì vậy chúng tôi chỉ chỉ định chụp CT khi những chi tiết bổ sung thực sự giúp thay đổi kế hoạch điều trị.

Một dạng xét nghiệm khác — các nghiên cứu về dây thần kinh

Không phải mọi xét nghiệm đều là việc chụp hình. Nếu nghi ngờ có vấn đề liên quan đến dây thần kinh (tê, ngứa ran hoặc yếu cơ), chúng tôi có thể chỉ định các xét nghiệm dẫn truyền thần kinh hoặc điện cơ đồ (EMG). Thay vì chụp hình cơ thể, các xét nghiệm này đo lường mức độ hoạt động thực tế của dây thần kinh và cơ bắp bằng cách gửi các tín hiệu điện nhỏ dọc theo dây thần kinh rồi ghi lại phản ứng. Tổng kết — những gì bạn có thể mong đợi

Điều an ủi là đội ngũ bác sĩ sẽ chọn loại chụp chiếu phù hợp với những gì chúng ta cần quan sát; hầu hết các ca chụp đều diễn ra nhanh chóng và không gây đau đớn. X-quang và CT: những “bóng” thể hiện mật độ mô

Cả hai phương pháp đều sử dụng bức xạ ion hóa (chụp CT sử dụng nhiều hơn nhiều so với một lần chụp X-quang); vì vậy chúng chỉ được dùng một cách thận trọng.

Siêu âm: nghe những tiếng vang

Đầu dò siêu âm phát ra các xung âm tần số rất cao vào cơ thể, rồi thu nhận những tiếng vang phản xạ lại từ các bề mặt mô. Máy tính chuyển đổi thời điểm và cường độ của những tiếng vang này thành hình ảnh động theo thời gian thực. Phương pháp này không sử dụng bức xạ, và nhờ tính chất thời gian thực mà nó rất hữu ích trong việc quan sát các mô mềm nông và theo dõi chuyển động của các cấu trúc; ví dụ như sự trượt của gân hoặc tình trạng rách gân khi bệnh nhân vận động.

MRI: Các nguyên tử hydro quay tròn

MRI sử dụng nam châm mạnh và sóng vô tuyến để tác động lên các nguyên tử hydro trong cơ thể (chủ yếu có trong nước và mỡ), sau đó ghi nhận tín hiệu yếu ớt mà chúng phát ra khi trở lại trạng thái cân bằng. Các mô khác nhau phát ra tín hiệu này với tốc độ khác nhau; nhờ việc xác định thời điểm đo lường, người ta có thể “điều chỉnh trọng số” cho hình ảnh chụp: chụp kiểu T2-weighted khiến dịch trong cơ thể trở nên sáng rõ, vì vậy tình trạng sưng, viêm và nhiều chấn thương đều hiện rõ trên hình ảnh; còn chụp kiểu T1-weighted thì thể hiện cấu trúc giải phẫu và mô mỡ. Kết quả là hình ảnh chi tiết tuyệt vời về các mô mềm (dây chằng, sụn, đĩa đệm, tủy xương) mà không gây phơi nhiễm phóng xạ.

Tại sao việc lựa chọn phương pháp chụp chiếu lại quan trọng

Mỗi phương pháp chụp chiếu sẽ cho thấy những thông tin khác nhau; do đó, phương pháp chụp phù hợp nhất phụ thuộc vào mục đích khám: kiểm tra sự thẳng hàng của xương hoặc gãy xương (chụp X-quang), quan sát gân đang chuyển động ở vùng gần bề mặt da (siêu âm), phân tích cấu trúc xương phức tạp (CT), hoặc quan sát chi tiết các mô mềm và tủy xương (MRI). f. Licensed Material means the artistic or literary work, database, or other material to which the Licensor applied this Public License.

g. Licensed Rights means the rights granted to You subject to the terms and conditions of this Public License, which are limited to all Copyright and Similar Rights that apply to Your use of the Licensed Material and that the Licensor has authority to license.

h. Licensor means the individual(s) or entity(ies) granting rights under this Public License.

i. NonCommercial means not primarily intended for or directed towards commercial advantage or monetary compensation. For purposes of this Public License, the exchange of the Licensed Material for other material subject to Copyright and Similar Rights by digital file-sharing or similar means is NonCommercial provided there is no payment of monetary compensation in connection with the exchange.

j. Share means to provide material to the public by any means or process that requires permission under the Licensed Rights, such as reproduction, public display, public performance, distribution, dissemination, communication, or importation, and to make material available to the public including in ways that members of the public may access the material from a place and at a time individually chosen by them.

l. You means the individual or entity exercising the Licensed Rights under this Public License. Your has a corresponding meaning.

Section 2 -- Scope.

a. License grant.

1. Subject to the terms and conditions of this Public License, the Licensor hereby grants You a worldwide, royalty-free, non-sublicensable, non-exclusive, irrevocable license to exercise the Licensed Rights in the Licensed Material to:

2. Exceptions and Limitations. For the avoidance of doubt, where Exceptions and Limitations apply to Your use, this Public License does not apply, and You do not need to comply with its terms and conditions.

3. Term. The term of this Public License is specified in Section 6(a).

4. Media and formats; technical modifications allowed. The Licensor authorizes You to exercise the Licensed Rights in all media and formats whether now known or hereafter created, and to make technical modifications necessary to do so. 5. Downstream recipients.

b. No downstream restrictions. You may not offer or impose any additional or different terms or conditions on, or apply any Effective Technological Measures to, the Licensed Material if doing so restricts exercise of the Licensed Rights by any recipient of the Licensed Material.

b. Other rights.

1. Section 3 -- License Conditions.

1. If You Share the Licensed Material (including in modified form), You must:

a. retain the following if it is supplied by the Licensor with the Licensed Material:

i. identification of the creator(s) of the Licensed Material and any others designated to receive attribution, in any reasonable manner requested by the Licensor (including by pseudonym if designated);

ii. a copyright notice;

iii. a notice that refers to this Public License;

iv. a notice that refers to the disclaimer of warranties;

v. a URI or hyperlink to the Licensed Material to the extent reasonably practicable;

b. indicate if You modified the Licensed Material and retain an indication of any previous modifications; and

Section 4 -- Sui Generis Database Rights.

Where the Licensed Rights include Sui Generis Database Rights that apply to Your use of the Licensed Material:

a. for the avoidance of doubt, Section 2(a)(1) grants You the right to extract, reuse, reproduce, and Share all or a substantial portion of the contents of the database for NonCommercial purposes only;

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

Section 5 -- Disclaimer of Warranties and Limitation of Liability.

a. THIS INCLUDES, WITHOUT LIMITATION, WARRANTIES OF TITLE, MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT, ABSENCE OF LATENT OR OTHER DEFECTS, ACCURACY, OR THE PRESENCE OR ABSENCE OF ERRORS, WHETHER OR NOT KNOWN OR DISCOVERABLE. WHERE DISCLAIMERS OF WARRANTIES ARE NOT ALLOWED IN FULL OR IN PART, THIS DISCLAIMER MAY NOT APPLY TO YOU.

b. TO THE EXTENT POSSIBLE, IN NO EVENT WILL THE LICENSOR BE LIABLE TO YOU ON ANY LEGAL THEORY (INCLUDING, WITHOUT LIMITATION, NEGLIGENCE) OR OTHERWISE FOR ANY DIRECT, SPECIAL, INDIRECT, INCIDENTAL, CONSEQUENTIAL, PUNITIVE, EXEMPLARY, OR OTHER LOSSES, COSTS, EXPENSES, OR DAMAGES ARISING OUT OF THIS PUBLIC LICENSE OR USE OF THE LICENSED MATERIAL, EVEN IF THE LICENSOR HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH LOSSES, COSTS, EXPENSES, OR DAMAGES. c. The disclaimer of warranties and limitation of liability provided above shall be interpreted in a manner that, to the extent possible, most closely approximates an absolute disclaimer and waiver of all liability.

Section 6 -- Term and Termination.

a. This Public License applies for the term of the Copyright and Similar Rights licensed here. However, if You fail to comply with this Public License, then Your rights under this Public License terminate automatically.

b. Where Your right to use the Licensed Material has terminated under Section 6(a), it reinstates:

1. automatically as of the date the violation is cured, provided it is cured within 30 days of Your discovery of the violation; or

2. upon express reinstatement by the Licensor.

For the avoidance of doubt, this Section 6(b) does not affect any right the Licensor may have to seek remedies for Your violations of this Public License.

c. For the avoidance of doubt, the Licensor may also offer the Licensed Material under separate terms or conditions or stop distributing the Licensed Material at any time; however, doing so will not terminate this Public License.

d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.

d. Nothing in this Public License constitutes or may be interpreted as a limitation upon, or waiver of, any privileges and immunities that apply to the Licensor or You, including from the legal processes of any jurisdiction or authority.
