Những triệu chứng bạn đang gặp phải

Bạn có thể đã nhận thấy rằng một hoặc nhiều ngón tay của mình có hình dạng hơi bất thường. Khớp giữa của ngón tay (khớp lớn nằm ở giữa, không phải khớp gần gốc ngón tay) bị uốn ngược ra sau khiến ngón tay trông hơi cong vồng hay “giống cổ con thiên nga”; trong khi khớp nhỏ gần móng tay nhất lại rủ gập về phía trước. Nhìn từ bên hông, ngón tay tạo thành hình zig-zag nhẹ.

Ở giai đoạn đầu, tình trạng này có thể chỉ gây phiền toái chứ chưa gây đau đớn: ngón tay có cảm giác như bị “kẹt” hoặc bật “tách” khi bạn cố gắng gập lại, và việc nắm tay thành nắm đấm một cách trơn tru hoặc cầm các vật nhỏ cũng trở nên khó khăn. Khi tình trạng này càng trở nên cố định, ngón tay có thể không còn duỗi thẳng hay gập lại như trước nữa; các hoạt động thường ngày như cài cúc áo, nhặt đồng xu, cầm bút cũng trở nên khó khăn và mất sức. Một số người lần đầu nhận ra vấn đề này chỉ vì ngón tay bị vướng khi họ đưa tay vào túi quần hoặc vào găng tay.

Điều gì đang thực sự xảy ra

Ngón tay được giữ cân bằng nhờ một hệ thống gồm các gân và dây chằng nhỏ chạy dọc theo mặt trên, mặt dưới và hai bên mỗi khớp. Thông thường, khớp giữa (khớp PIP) không thể bị uốn ngược ra sau nhờ một dây chằng nhỏ và chắc chắn nằm ở mặt lòng bàn tay gọi là tấm gan tay (volar plate), cùng các bó gân nằm dọc hai bên ngón tay.

Trong trường hợp dị dạng hình cổ thiên nga, sự cân bằng này bị mất. Nếu tấm gan tay bị lỏng lẻo, hoặc các bó gân bên (lateral bands) trượt lên phía mặt trên của khớp, khớp giữa sẽ bị kéo ngược ra sau vào tư thế quá duỗi; do tất cả các gân đều liên kết với nhau nên đầu ngón tay cũng bị kéo rủ xuống. Hai hiện tượng này luôn đi kèm nhau.

Có nhiều nguyên nhân dẫn đến sự mất cân bằng này. Đôi khi nó bắt đầu từ đầu ngón tay: tình trạng ngón tay hình búa (mallet finger) không được điều trị (đầu ngón tay bị rủ xuống sau chấn thương gân) làm thay đổi hướng kéo của các gân và dần dần khiến khớp giữa bị uốn ngược ra sau. Thường thì nguyên nhân là do viêm khớp do viêm, đặc biệt là viêm khớp dạng thấp, làm giãn và yếu đi các dây chằng. Tình trạng này cũng có thể xuất hiện ở người có khớp lỏng lẻo, do chấn thương cũ hoặc các bệnh lý làm tăng độ căng cơ. Dù nguyên nhân là gì, kết quả cuối cùng vẫn là hình dạng ngón tay dễ nhận biết như vậy.

Những giải pháp chúng tôi có thể áp dụng

Bác sĩ Kieran Hirpara là người đứng đầu chuyên khoa phẫu thuật chi trên tại Bệnh viện tư nhân Mater Rockhampton. Chúng tôi hỗ trợ bệnh nhân từ lúc được giới thiệu đến khi xây dựng được kế hoạch điều trị rõ ràng. Quá trình điều trị bắt đầu bằng việc khám lâm sàng kỹ lưỡng nhằm xác định chẩn đoán và đánh giá khả năng vận động của ngón tay. Thông thường, chúng tôi bắt đầu bằng các biện pháp điều trị không phẫu thuật đối với các trường hợp mạn tính; phẫu thuật chỉ được cân nhắc khi các phương pháp này chưa mang lại hiệu quả đủ rõ rệt.

Việc lựa chọn phương pháp điều trị phù hợp phụ thuộc vào mức độ linh hoạt của ngón tay cũng như nguyên nhân gây bệnh; vì vậy bước đầu tiên luôn là đánh giá toàn diện chức năng bàn tay và điều trị các dạng viêm khớp tiềm ẩn.

Nếu ngón tay vẫn còn linh hoạt, khớp có thể duỗi thẳng và gập lại dễ dàng, chúng tôi thường bắt đầu điều trị mà không cần phẫu thuật. Một chiếc nẹp nhỏ được thiết kế riêng theo hình chữ số tám (đôi khi là loại nẹp kiểu nhẫn bạc gọn gàng) sẽ được đặt ngang khớp giữa, giúp ngăn ngừa hiện tượng ngón tay gập ngược lại mà vẫn cho phép ngón tay vận động bình thường. Nhiều bệnh nhân kiểm soát tốt tình trạng của mình nhờ phương pháp này; trị liệu bàn tay cũng giúp duy trì sự linh hoạt cho các khớp.

Nếu việc đeo nẹp không đủ hiệu quả, hoặc ngón tay dần bị cứng lại, phẫu thuật sẽ giúp khôi phục lại trạng thái cân bằng. Có nhiều phương pháp đã được kiểm chứng, tùy thuộc vào đặc điểm cụ thể của ngón tay: dùng một đoạn gân tự thân để cố định khớp giữa, ngăn không cho nó gập ngược lại (tenodesis); siết chặt các dây chằng lỏng lẻo ở mặt lòng bàn tay; hoặc đưa các gân bị trượt về đúng vị trí ban đầu. Trong trường hợp bề mặt khớp đã bị tổn thương và cứng lại (thường gặp ở bệnh nhân viêm khớp mạn tính), chúng tôi có thể thay vào đó hàn cứng khớp ở một tư thế chức năng tốt, hoặc trong một số trường hợp nhất định sẽ tiến hành thay khớp.

Những điều bạn có thể mong đợi

Nếu được phát hiện sớm, khi ngón tay vẫn còn linh hoạt, tiên lượng điều trị khá tốt; việc đeo nẹp hoặc thực hiện phẫu thuật tái cân bằng có thể giúp ngón tay trở lại trạng thái hoạt động tốt và mượt mà hơn. Tuy nhiên, khi khớp ngày càng cứng lại và bị viêm khớp, các phương án điều trị sẽ trở nên hạn chế; vì vậy việc đi khám sớm trước khi ngón tay bị cố định ở một vị trí là điều rất quan trọng.

Trong trường hợp nguyên nhân gây bệnh là viêm khớp, việc kiểm soát bệnh tốt cùng với sự hỗ trợ của đội ngũ chuyên gia thấp khớp sẽ giúp ngăn ngừa các ngón tay khác bị ảnh hưởng tương tự.

Khi nào nên đi khám bác sĩ

  • Đầu ngón tay bị rủ xuống và không thể duỗi thẳng hoàn toàn sau khi bị va đập hoặc chấn thương (hiện tượng ngón tay dạng “mallet finger”); việc điều trị sớm có thể ngăn ngừa tình trạng ngón tay cong dạng cổ thiên nga sau này.
  • Ngón tay bắt đầu cong ngược về phía sau ở khớp giữa, hoặc có hiện tượng mắc kẹt, phát ra tiếng “cạch” hoặc bị khóa khi gập lại.
  • Tình trạng cứng khớp ngày càng tăng, hoặc ngón tay không còn có thể nắm thành nắm đấm một cách trơn tru nữa.
  • Bệnh nhân mắc viêm khớp dạng thấp hoặc viêm khớp do viêm với những thay đổi về hình dạng ngón tay: nên đi khám để đánh giá sớm trước khi dị dạng trở nên cố định.

Phần này đi sâu hơn mức cần thiết để bạn có thể tự đưa ra quyết định điều trị. Dị dạng ngón tay hình cổ thiên nga đáng để đọc kỹ vì đây không phải là một bệnh lý đơn lẻ; cùng một hình dạng ngón tay có thể xuất hiện do nhiều nguyên nhân khác nhau, xảy ra ở cả hai đầu ngón tay. Việc lựa chọn phương pháp điều trị phù hợp hoàn toàn phụ thuộc vào nguyên nhân gây ra dị dạng đó.

Một dạng biến dạng, nhiều nguyên nhân

Tình trạng ngón tay cong hình cổ thiên nga là khi khớp giữa bị quá duỗi còn đầu ngón tay thì rủ xuống. Tư thế này là kết quả của sự mất cân bằng trong một hệ thống liên kết; nguyên nhân gây ra có thể xuất phát từ bất kỳ đầu mút nào của hệ thống này.

Bắt nguồn từ đầu ngón tay. Chấn thương ngón tay hình búa không được điều trị sẽ khiến cơ chế duỗi ở phía xa bị tách rời [1]. Lực duỗi mà cơ chế này không còn truyền tới đầu ngón tay sẽ được chuyển sang khớp giữa; dần dần khớp này bị kéo vào tư thế quá duỗi. Vì vậy, dạng biến dạng hình cổ thiên nga có thể là hậu quả muộn của một chấn thương xảy ra vài tuần hoặc vài tháng trước đó, tưởng như chỉ ảnh hưởng đến đầu ngón tay.

Bắt nguồn từ khớp giữa. Nếu tấm gan tay – tức là dây chằng dày có chức năng ngăn khớp giữa bị quá duỗi – bị giãn hoặc rách, khớp sẽ bị lệch về phía sau. Các dải bên (lateral bands) khi đó sẽ nằm phía trên trục khớp và càng làm tăng thêm hiện tượng duỗi; trong khi đó gân gấp sâu bị dư chùng, và độ chùng này làm gập đầu ngón tay.

Bắt nguồn từ tình trạng lỏng khớp toàn thân hoặc bệnh viêm. Ở những người có tính lỏng khớp bẩm sinh, tấm gan tay vốn đã lỏng lẻo chứ không phải do chấn thương. Ở bệnh nhân viêm khớp dạng thấp, tình trạng viêm màng hoạt dịch theo thời gian làm suy yếu các cấu trúc giữ khớp ổn định; dị dạng này thường là một phần của một kiểu biến dạng rộng hơn trên nhiều ngón tay [3].

Tại sao việc phân biệt nguyên nhân lại quyết định phương pháp điều trị

Mục tiêu điều trị là ngăn khớp giữa bị quá duỗi; vị trí được chỉnh sửa phải tương ứng với vị trí gây ra tổn thương.

Trong trường hợp dị dạng bắt nguồn từ đầu ngón tay, việc chỉ điều chỉnh khớp giữa sẽ không giải quyết được sự mất cân bằng gây ra tình trạng này. Khi tấm gan tay bị tổn thương, có thể thực hiện thủ thuật tenodesis bằng cách sử dụng một đoạn của gân gấp nông (superficialis) để hạn chế tình trạng quá duỗi [2]. Nếu nguyên nhân là do viêm, thì bệnh lý nền sẽ quyết định liệu các phương pháp tái tạo có hiệu quả hay không; các thủ thuật liên quan đến mô mềm ở bàn tay đang trong giai đoạn viêm thường có xu hướng bị giãn ra theo thời gian.

Thông thường, người ta sẽ sử dụng nẹp hình vòng bạc hoặc nẹp hình số 8 – loại nẹp giúp chặn vài độ quá duỗi cuối cùng trong khi vẫn cho phép ngón tay cử động. Phương pháp này hiệu quả hơn so với vẻ bề ngoài của nó, bởi việc ngăn chặn một phạm vi chuyển động nhỏ ấy sẽ cắt đứt cơ chế gây ra dị dạng.

Tính linh hoạt so với tính cố định – đây là vấn đề quyết định nữa

Vấn đề thứ hai là liệu ngón tay có thể được duỗi thẳng một cách thụ động hay không. Dạng biến dạng linh hoạt – tức là dạng có thể chỉnh sửa được bằng tay – có thể đáp ứng tốt với việc đeo nẹp hoặc cân bằng lại các mô mềm. Ngược lại, dạng biến dạng cố định, nơi khớp không còn di chuyển được trong phạm vi vận động bình thường, đã hình thành tình trạng co rút khớp; chỉ áp dụng các thủ thuật liên quan đến mô mềm thì không thể khắc phục được tình trạng này [4].

Ý nghĩa thực tiễn của điều này cũng tương tự như trong trường hợp biến dạng boutonnière; đó là lý do vì sao cả hai dạng biến dạng này đều cần được phát hiện sớm: chúng có xu hướng tiến triển và ngày càng trầm trọng hơn. Thời gian mà việc đeo nẹp có thể giúp giữ ngón tay linh hoạt ở trạng thái thẳng là dài hơn nhiều so với thời gian mà phẫu thuật có thể giúp khôi phục lại ngón tay đã bị biến dạng cố định.

Lưu ý về dạng biến dạng liên quan

Tình trạng Boutonnière về một khía cạnh nào đó là hình ảnh phản chiếu của dạng biến dạng trên: khớp giữa bị gập lại, đầu ngón tay bị duỗi quá mức. Tình trạng này xảy ra do tổn thương dải trung tâm (central slip) của gân duỗi chứ không phải do tổn thương tấm gan tay. Hai dạng biến dạng này được mô tả riêng biệt; việc nhầm lẫn giữa chúng có hậu quả thực sự vì tư thế nẹp cho hai dạng này trái ngược nhau.
