Bệnh lý gân ECU và hiện tượng gân ECU bị kêu lục cục

Những triệu chứng bạn đang gặp phải

Đây là cơn đau ở phía ngón út của mặt sau cổ tay — phía mà dây đeo đồng hồ thường nằm nếu bạn đeo đồng hồ thấp xuống. Cơn đau thường dữ dội nhất khi bạn xoay lòng bàn tay hướng lên trên và gập cổ tay về phía ngón út; điều này chính xảy ra trong các động tác như đánh vợt tennis kiểu backhand, vung gậy golf, hoặc vắt khô vải.

Có hai dạng vấn đề liên quan, và nhiều người gặp phải cả hai dạng này.

Gân bị kích ứng. Cơn đau là dạng âm ỉ dọc theo sợi gân; đôi khi có hiện tượng sưng mà bạn có thể cảm nhận được. Việc ấn vào cổ tay chống lại lực cản cũng gây đau. Cơn đau thường tăng dần theo thời gian do việc sử dụng cổ tay nhiều lần, chứ không xuất hiện đột ngột trong một khoảnh khắc.

Gân bị trượt ra khỏi rãnh chứa nó. Trong trường hợp này, khi xoay cẳng tay bạn sẽ nghe thấy tiếng “cạch”, “lách cách” hoặc tiếng “cộp” rõ rệt; bạn có thể tự mình tạo ra âm thanh này. Thông thường, triệu chứng này xuất hiện sau một sự kiện cụ thể nào đó, chẳng hạn như gậy golf va vào mặt đất hoặc cú đánh backhand mạnh. Cảm giác này có thể gây hoảng sợ, như thể có thứ gì đó đang bị trượt ra khỏi vị trí bình thường.

Điều gì đang thực sự xảy ra

Gân bị ảnh hưởng là gân cơ duỗi cổ tay trụ, thường được gọi tắt là ECU. Gân này chạy từ cơ ở cẳng tay xuống phía bên ngón út của cổ tay, nằm trong một rãnh nông ở đầu xương trụ – tức là xương cẳng tay phía đó. Một lớp mô mỏng, như một “đường hầm” riêng dành cho gân, giúp giữ gân ở đúng vị trí trong rãnh này.

Rãnh này thực sự rất nông, trung bình chỉ sâu khoảng 1,5 mm; vì vậy lớp mô này đóng vai trò chủ yếu trong việc giữ gân cố định. Nếu lớp mô này bị rách hoặc giãn – thường do cử động xoắn mạnh khi lòng bàn tay hướng lên trên và cổ tay gập lại – gân có thể trượt ra khỏi rãnh rồi lại trở về vị trí cũ. Ngược lại, nếu gân và lớp mô bao quanh bị kích ứng và dày lên do chịu tải nhiều lần, việc di chuyển của gân sẽ gây đau đớn; đây là dạng bệnh lý gân.

Có một điều quan trọng cần biết, vì nó ảnh hưởng đến cách diễn giải các xét nghiệm: việc gân di chuyển một chút là hoàn toàn bình thường. Khi các nhà nghiên cứu khảo sát 755 người hoàn toàn không có triệu chứng gì về cổ tay, khoảng 1 trên 15 người có hiện tượng gân ECU bị “nhảy” ra khỏi rãnh, và gần một nửa trong số đó có gân lộ rõ ra khỏi rãnh khi siêu âm – mà không hề đau đớn hay gặp vấn đề gì. Chỉ riêng hiện tượng “nhảy” của gân thôi thì chưa đủ để khẳng định có điều gì bất thường.

Những biện pháp chúng tôi có thể áp dụng

Tại Bệnh viện tư nhân Mater Rockhampton, bác sĩ Kieran Hirpara sẽ đưa ra chẩn đoán chủ yếu dựa trên khám lâm sàng chứ không phải chụp chiếu; điều này cần được giải thích rõ.

Việc diễn giải kết quả chụp chiếu cần hết sức cẩn trọng. Trong một nghiên cứu với hơn 4.000 ca chụp MRI cổ tay, những thay đổi ở gân ECU xuất hiện trong khoảng 1/8 số ca chụp, kể cả ở những người chụp hình vì các lý do hoàn toàn khác. Nếu dựa vào hình ảnh gân ECU bất thường trên MRI để đánh giá, tỷ lệ người bệnh thực sự gặp vấn đề với gân này chỉ khoảng 6%. MRI thực sự hữu ích trong việc loại trừ các nguyên nhân gây đau khác ở vùng cổ tay — ví dụ như rách sụn — nhưng sự bất thường ở gân không phải là bằng chứng duy nhất để chẩn đoán. Siêu âm thường hữu ích hơn, vì chúng tôi có thể quan sát chuyển động của gân theo thời gian thực khi bệnh nhân tái hiện lại động tác gây đau.

Phần lớn bệnh nhân hồi phục mà không cần phẫu thuật. Việc điều trị thường bắt đầu bằng việc hạn chế các hoạt động gây kích thích gân, kết hợp với việc đeo nẹp hoặc bó bột trong một thời gian. Đối với trường hợp gân bị trượt ra khỏi rãnh chứa nó, tư thế cổ tay rất quan trọng: thường phải giữ cổ tay hơi ngửa ra sau và cẳng tay xoay sao cho lòng bàn tay hướng xuống dưới, trong khoảng 4–6 tuần; đây là tư thế ngược lại với tình trạng gân bị trượt ra ngoài.

Tiêm steroid vào màng bao gân là bước điều trị hợp lý tiếp theo đối với trường hợp gân bị viêm. Trong nghiên cứu uy tín nhất hiện có, 13 bệnh nhân được chẩn đoán xác định đều có cải thiện triệu chứng; trong đó 10 người hết đau hoàn toàn, và không ai phải phẫu thuật; hầu hết vẫn không còn đau sau 4–5 năm. Một dấu hiệu tích cực là nếu lần tiêm đầu tiên giúp giảm đau hoàn toàn, hiệu quả này thường kéo dài. Ngược lại, nếu chỉ giảm đau một phần, thường sẽ còn sót lại cảm giác đau nhẹ.

Chúng tôi hạn chế số lần tiêm tối đa khoảng ba lần; đồng thời thận trọng hơn với các vận động viên ném bóng hoặc chơi môn vợt, vì có những báo cáo cho rằng việc tiêm steroid nhiều lần quanh vùng gân này có thể làm tăng nguy cơ gân bị rách — dù điều này chưa được chứng minh qua số liệu cụ thể.

Phẫu thuật chỉ được chỉ định cho những bệnh nhân vẫn bị đau do tình trạng gân không ổn định, sau khi đã áp dụng phương pháp bó bột đúng cách trong khoảng hai tháng. Ca phẫu thuật nhằm tái tạo hoặc củng cố màng bao giữ gân trong rãnh chứa nó. Có nhiều kỹ thuật khác nhau và tất cả đều cho kết quả tốt.

Những điều có thể mong đợi

Quá trình hồi phục khi không phẫu thuật phụ thuộc vào mức độ kích ứng của vùng tổn thương. Những vận động viên bị tổn thương nhẹ thường có thể trở lại tập luyện trong vòng hai đến ba tuần; những ca nặng hơn thì mất khoảng sáu đến tám tuần; còn những trường hợp gân trượt hoàn toàn và được điều trị bằng nẹp thì có thể mất năm đến sáu tháng mới có thể tham gia các môn thể thao đòi hỏi sức bền như quần vợt.

Sau phẫu thuật, bệnh nhân thường phải đeo nẹp hoặc bó bột khoảng bốn đến sáu tuần, trong đó cẳng tay được giữ ở tư thế lòng bàn tay hướng xuống dưới. Sau đó mới dần dần phục hồi vận động; sức mạnh cơ bắp bắt đầu cải thiện từ khoảng hai tháng sau phẫu thuật, và các bài tập chuyên biệt cho từng môn thể thao được thực hiện vào tháng thứ ba đến thứ tư. Các nghiên cứu đã ghi nhận rằng hầu hết bệnh nhân có thể trở lại mức độ hoạt động trước đây vào khoảng hai tháng rưỡi đến ba tháng rưỡi.

Có hai điều cần hiểu rõ về phẫu thuật này. Thứ nhất, hầu hết các nghiên cứu đều cho thấy tình trạng gân “nhảy ra ngoài” không tái diễn; tuy nhiên có một nghiên cứu sử dụng phương pháp chụp hình sau phẫu thuật và phát hiện ra rằng ở khoảng một nửa số bệnh nhân, gân vẫn nằm ngoài rãnh của nó — dù những người này cảm thấy hài lòng và kết quả đánh giá sức khỏe cũng tốt. Nói cách khác, phẫu thuật giúp giảm triệu chứng một cách đáng tin cậy, nhưng không phải lúc nào cũng đưa cấu trúc giải phẫu về đúng vị trí ban đầu. Thứ hai, mức cải thiện về sức mạnh nắm và biên độ vận động là không đáng kể; phẫu thuật chủ yếu nhằm giảm đau và ngăn tình trạng gân nhảy ra ngoài, chứ không phải để tăng cường sức mạnh cổ tay.

Các biến chứng rất hiếm gặp nhưng vẫn có thể xảy ra: tê hoặc vùng da bị đau do kích ứng dây thần kinh nhỏ ở mặt sau cổ tay, tình trạng cứng khớp; hiếm khi gặp phải hội chứng đau kéo dài.

Nếu tình trạng gân nhảy ra ngoài không gây đau đớn, có lẽ không cần phải điều trị. Những người bị tình trạng này mà không đau thường vẫn có thể duy trì mức độ hoạt động cao nhờ việc dùng băng dán và theo dõi định kỳ.

Khi nào nên đi khám

Hãy đến gặp bác sĩ đa khoa nếu cơn đau ở phía ngón út của cổ tay không thuyên giảm sau vài tuần giảm bớt các hoạt động gây ra đau, hoặc nếu cơn đau ảnh hưởng đến việc chơi thể thao hoặc làm việc của bạn.

Bạn nên tìm lời khuyên từ bác sĩ phẫu thuật tay nếu cổ tay phát ra tiếng “cạch” hoặc cảm giác đau khi bạn xoay cẳng tay; nếu cơn đau xuất hiện sau một chấn thương do vặn xoắn; hoặc nếu bạn đã thử nghỉ ngơi, đeo nẹp và tiêm thuốc nhưng không thấy cải thiện rõ rệt.

Hãy đi khám ngay lập tức nếu cổ tay trở nên nóng, đỏ và sưng; nếu bạn bị tê hoặc yếu ở tay; hoặc nếu cảm thấy cổ tay đột ngột mất sức và không còn khả năng cử động – triệu chứng cuối cùng này có thể là dấu hiệu của tình trạng gân bị rách.

Advanced reading: the deeper science (optional)

Phần này đi sâu hơn mức cần thiết để bạn có thể tự đưa ra quyết định điều trị. Các vấn đề liên quan đến cơ duỗi cổ tay trụ bên đáng được nghiên cứu kỹ hơn, bởi hai dấu hiệu thường dùng để xác nhận chẩn đoán – đó là tín hiệu bất thường trên hình ảnh chụp và hiện tượng gân bị “vụt” ra khi cổ tay xoay – lại rất phổ biến ngay cả ở những người hoàn toàn không có triệu chứng gì.

Kết quả MRI bất thường không nhất thiết là dấu hiệu bệnh lý nghiêm trọng như vẻ bề ngoài

Gân ECU chạy trong một rãnh ở mặt sau xương quay, phía ngón út của cổ tay. Khi bệnh nhân bị đau ở vùng này và kết quả chụp MRI cho thấy có thay đổi tín hiệu ở gân đó, người ta thường kết luận ngay rằng đó là nguyên nhân gây đau.

Tuy nhiên, dữ liệu thực tế lại không ủng hộ điều đó. Sau khi phân tích 4.301 hình ảnh MRI cổ tay, người ta nhận thấy rằng những thay đổi tín hiệu ở gân ECU khá phổ biến và thường không gây triệu chứng; đồng thời những thay đổi này hiếm khi liên quan đến tình trạng viêm gân có triệu chứng, dẫn đến giá trị dự đoán dương tính thấp [1].

“Giá trị dự đoán dương tính thấp” là cụm từ then chốt ở đây. Nó có nghĩa là trong số tất cả các trường hợp có kết quả MRI cho thấy thay đổi tín hiệu ở gân ECU, chỉ có một tỷ lệ nhỏ là do gân này gây ra đau đớn. Chính vì vậy, kết quả chụp MRI không thể tự mình xác định chẩn đoán; nó cần phải đi kèm với dấu hiệu đau tại đúng vị trí đó và tình trạng đau tăng lên khi thực hiện các động tác vận động theo hướng cụ thể.

Hiện tượng “lách” gân là hiện tượng phổ biến ngay cả ở những người hoàn toàn khỏe mạnh

Dấu hiệu xác nhận thứ hai là hiện tượng gân bị “lách” ra khỏi rãnh chạy theo khi cẳng tay xoay. Trong một nghiên cứu trên 755 người không có triệu chứng, hiện tượng lách gân ECU xuất hiện với tỷ lệ cao đến mức không thể coi đây là dấu hiệu đặc trưng của bệnh lý được [2].

Ý nghĩa thực tiễn là: nếu bệnh nhân nhận thấy hiện tượng gân “lách” và cổ tay cũng đau đúng tại vị trí đó, thì điều này có ý nghĩa lâm sàng. Ngược lại, nếu hiện tượng này chỉ được phát hiện tình cờ khi khám cổ tay mà cổ tay lại đau ở những chỗ khác, thì có lẽ đó chỉ là biến thể bình thường của cơ thể.

Khi kết hợp với kết quả chụp MRI, mọi dữ liệu đều cho thấy một khuôn mẫu nhất quán: gân này tạo ra những kết quả hình ảnh bất thường ngay cả ở những cổ tay bình thường. Việc chẩn đoán trong trường hợp này phụ thuộc nhiều vào sự tương quan giữa các dấu hiệu hơn là chỉ dựa vào từng dấu hiệu riêng lẻ.

Điều này rất quan trọng vì cổ tay ulna có nhiều nguyên nhân gây đau có thể xảy ra.

Gân ECU nằm ngay cạnh TFCC, khớp quay-ulna đoạn xa và khớp pisotriquetral; tất cả các cấu trúc này đều gây đau ở cùng một vùng nhỏ trên cổ tay. Việc cho rằng nguyên nhân gây đau là do gân ECU có hình dạng bất thường là điều dễ xảy ra; tuy nhiên, dựa trên các số liệu nêu trên, điều này thường không đúng. Đây là lý do chính khiến chúng ta cần thận trọng trước khi xác định gân ECU là thủ phạm gây đau.

Điều trị bảo tồn là phương pháp chủ đạo và mang lại hiệu quả

Khi chẩn đoán chính xác, phương pháp điều trị thường không mấy hấp dẫn. Việc kết hợp các biện pháp điều trị bảo tồn và phục hồi chức năng có thể giúp bệnh nhân hết triệu chứng và trở lại với các hoạt động mong muốn [3].

Có hai đặc điểm riêng biệt liên quan đến gân này ảnh hưởng đến quá trình phục hồi chức năng. Thứ nhất, gân chịu tải dưới tác động của sự xoay cẳng tay kết hợp với sự lệch trục về phía xương quay – tư thế xuất hiện khi đánh gôn, đánh vợt hoặc nâng vật nặng; do đó việc điều chỉnh hoạt động ở đây là thay đổi hướng chịu tải thay vì đơn thuần là nghỉ ngơi. Thứ hai, vì gân nằm trong một màng bao có thể bị giãn hoặc rách, dẫn đến tình trạng trật gân, nên việc cố định thường được thực hiện ở tư thế xoay lòng bàn tay xuống dưới và cổ tay duỗi thẳng; tư thế này giúp giữ gân nằm đúng rãnh của nó trong lúc màng bao hồi phục.

To the extent possible, the Licensor waives any right to collect royalties from You for the exercise of the Licensed Rights, whether directly or through a collecting society under any voluntary or waivable statutory or compulsory licensing scheme. In all other cases the Licensor expressly reserves any right to collect such royalties, including when the Licensed Material is used other than for NonCommercial purposes.

Section 3 -- License Conditions.

Your exercise of the Licensed Rights is expressly made subject to the following conditions.

a. Attribution.

1. If You Share the Licensed Material (including in modified form), You must:

a. retain the following if it is supplied by the Licensor with the Licensed Material:

i. identification of the creator(s) of the Licensed Material and any others designated to receive attribution, in any reasonable manner requested by the Licensor (including by pseudonym if designated);

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

Section 6 -- Term and Termination.

a. This Public License applies for the term of the Copyright and Similar Rights licensed here. However, if You fail to comply with this Public License, then Your rights under this Public License terminate automatically.

b. Where Your right to use the Licensed Material has terminated under Section 6(a), it reinstates:

1. automatically as of the date the violation is cured, provided it is cured within 30 days of Your discovery of the violation; or

2. upon express reinstatement by the Licensor.

For the avoidance of doubt, this Section 6(b) does not affect any right the Licensor may have to seek remedies for Your violations of this Public License.

c. For the avoidance of doubt, the Licensor may also offer the Licensed Material under separate terms or conditions or stop distributing the Licensed Material at any time; however, doing so will not terminate this Public License.

d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.

d. Nothing in this Public License constitutes or may be interpreted as a limitation upon, or waiver of, any privileges and immunities that apply to the Licensor or You, including from the legal processes of any jurisdiction or authority.
