Những cảm giác bạn đang trải qua

Các tình trạng này đều xuất phát từ việc dây thần kinh giữa bị chèn ép ở vùng cao, quanh khuỷu tay và phía trên cẳng tay, chứ không phải ở cổ tay – nơi thường gặp tình trạng chèn ép dây thần kinh (hội chứng ống cổ tay). Các triệu chứng của từng tình trạng này cũng khác nhau khá nhiều.

Hội chứng pronator: Thường gây cảm giác đau âm ỉ, mệt mỏi ở phía trước cẳng tay; tình trạng này thường nặng hơn khi bạn xoay cẳng tay nhiều lần (như vặn tuốc-nơ-vít, vặn chìa khóa, vắt khăn) hoặc khi nắm chặt tay. Bạn cũng có thể cảm thấy tê hoặc ngứa ran ở ngón tay cái, ngón trỏ và ngón giữa, cũng như ở lòng bàn tay và vùng gốc ngón cái. Khác với hội chứng ống cổ tay, hội chứng này thường không gây thức giấc vào ban đêm; cảm giác khó chịu chủ yếu tập trung ở cẳng tay chứ không phải ở bàn tay.

Hội chứng Lacertus: Đây là tên gọi mới dành cho một dạng chèn ép dây thần kinh giữa ở vùng cao; nguyên nhân là do một dải mô sợi cứng nằm ở nếp gấp khuỷu tay gọi là lacertus fibrosus. Biểu hiện điển hình là tay nhanh mỏi, cầm nắm vụng về – việc viết chữ, gõ máy tính hay sử dụng dụng cụ trở nên khó khăn – kèm theo đau cẳng tay và vùng dưới nếp gấp khuỷu tay rất nhạy cảm khi ấn. Đây cũng là một trong những tình trạng chúng tôi chú ý kiểm tra khi phẫu thuật hội chứng ống cổ tay không giúp cải thiện các triệu chứng như mong đợi.

Hội chứng dây thần kinh gian xương trước (Anterior Interosseous Nerve – AIN): Tình trạng này lại khác hẳn; thường không gây tê hay ngứa ran. Thay vào đó, nó gây ra tình trạng yếu cơ: các cơ chịu trách nhiệm gập đầu ngón tay cái và ngón trỏ hoạt động không bình thường. Dấu hiệu điển hình là không thể tạo thành ký hiệu “OK” tròn đều: khi cố gắng chạm đầu ngón tay cái vào đầu ngón trỏ, ngón tay lại tạo thành hình dạng phẳng hoặc tam giác. Một số người thấy có giai đoạn đau nhức ở cánh tay hoặc vai trước khi xuất hiện tình trạng yếu cơ.

Điều gì đang thực sự xảy ra

Một dây thần kinh lớn duy nhất là dây thần kinh giữa, chạy từ cổ xuống cánh tay, vượt qua phía trước khuỷu tay và đi vào bàn tay. Dây thần kinh này truyền cả cảm giác (đối với một phần của bàn tay) lẫn sự vận động (đến nhiều cơ).

Trong hội chứng pronator, dây thần kinh bị chèn ép hoặc kích thích khi đi qua khoảng không gian chật hẹp ở khuỷu tay và vùng cẳng tay trên. Nó có thể bị siết chặt bởi một dải mô chặt chẽ gần nếp gấp khuỷu tay (gọi là lacertus fibrosus; khi dải mô này là nguyên nhân, tình trạng này ngày càng được gọi là hội chứng lacertus), hoặc khi đi qua giữa hoặc dưới các cơ có chức năng xoay lòng bàn tay xuống dưới. Vì chỗ bị chèn ép nằm ở vị trí cao (phía trên cổ tay), các triệu chứng thường xuất hiện ở lòng bàn tay và gốc ngón tay cái; đây là dấu hiệu chính giúp phân biệt hội chứng này với hội chứng ống cổ tay.

Trong hội chứng AIN, vấn đề xảy ra ở một nhánh sâu của dây thần kinh giữa chỉ chịu trách nhiệm về sự vận động chứ không mang cảm giác; vì vậy người bệnh chỉ bị yếu cơ mà không bị tê. Ở nhiều người, đây không phải là hiện tượng “bị chèn ép” đơn thuần mà là tình trạng kích thích hoặc viêm dây thần kinh (đôi khi sau khi mắc bệnh do virus hoặc không rõ nguyên nhân); đó cũng là lý do tại sao tình trạng này thường tự khỏi.

Những biện pháp chúng tôi có thể áp dụng

Tại Bệnh viện tư nhân Mater Rockhampton, Tiến sĩ Kieran Hirpara dẫn dắt đội ngũ chuyên gia về chi trên trong việc đánh giá các tình trạng này thông qua việc khai thác bệnh sử và khám lâm sàng cẩn thận. Thông thường, chúng tôi bắt đầu bằng các biện pháp điều trị không phẫu thuật; chỉ chỉ định phẫu thuật nếu các triệu chứng vẫn tiếp diễn dù đã nghỉ ngơi và thay đổi thói quen sinh hoạt.

Đối với tất cả các tình trạng này, bước đầu tiên hầu như luôn là điều trị không phẫu thuật; riêng đối với hội chứng AIN, việc kiên nhẫn chờ đợi thường mang lại kết quả tốt.

Giảm bớt gánh nặng lên chi trên. Điều này bao gồm việc hạn chế những hoạt động gây kích phát triệu chứng (giảm các động tác xoay cổ tay và nắm chặt mạnh), đôi khi sử dụng nẹp để giúp cánh tay nghỉ ngơi, đồng thời cho dây thần kinh thời gian phục hồi. Các biện pháp giảm đau đơn giản cũng giúp làm dịu cơn đau.

Theo dõi và chờ đợi. Tình trạng yếu cơ do hội chứng AIN thường tự cải thiện sau vài tháng; vì vậy phương pháp phổ biến là theo dõi và chờ đợi, thường trong khoảng ba đến sáu tháng, trước khi cân nhắc các biện pháp can thiệp khác. Nhiều người lấy lại sức mạnh cơ bắp trong khoảng thời gian này mà không cần phẫu thuật. Trong quá trình theo dõi, các xét nghiệm như điện cơ đồ hoặc chụp chiếu có thể được thực hiện để xác nhận chẩn đoán và theo dõi tiến triển hồi phục.

Phẫu thuật chỉ được chỉ định cho một số ít trường hợp không đáp ứng với điều trị: hội chứng pronator vẫn gây đau dù đã nghỉ ngơi và thay đổi hoạt động, hoặc tình trạng yếu cơ do AIN không có dấu hiệu cải thiện sau một thời gian theo dõi. Ca phẫu thuật nhằm giải phóng các cấu trúc bị chặt chẽ đang gây áp lực lên dây thần kinh, giúp nó hồi phục. Khi áp lực xuất phát từ dải lacertus, ca phẫu thuật thường có thể thực hiện qua một vết rạch nhỏ dưới gây tê tại chỗ trong khi bệnh nhân vẫn tỉnh táo; điều này cho phép kiểm tra ngay sức mạnh cơ bắp bị ảnh hưởng. Quá trình hồi phục thường diễn ra nhanh chóng.

Những điều có thể mong đợi

Nhìn chung, các tình trạng này có tiên lượng khá tốt. Đặc biệt, hội chứng AIN có xu hướng tự khỏi; đó là lý do chúng tôi không vội vàng chỉ định phẫu thuật. Quá trình hồi phục diễn ra từ từ, mất nhiều tháng thay vì vài tuần; vì vậy cần có sự kiên nhẫn, và tiến triển của bạn sẽ được theo dõi thường xuyên.

Khi các phương pháp điều trị không phẫu thuật không mang lại hiệu quả và cần phải phẫu thuật, việc giải phóng dây thần kinh thường giúp giảm đau trong hội chứng pronator và tạo điều kiện tốt nhất để sức mạnh cơ thể hồi phục ở bệnh nhân mắc hội chứng AIN. Tuy nhiên, dây thần kinh hồi phục rất chậm; sự cải thiện vẫn tiếp tục diễn ra trong nhiều tháng sau đó.

Khi nào nên đi khám bác sĩ

  • Đau vùng cẳng tay kèm cảm giác tê hoặc ngứa ran ở ngón tay cái, ngón trỏ và ngón giữa, đặc biệt nếu cảm giác này còn xuất hiện ở lòng bàn tay và gốc ngón cái; tình trạng này trở nên tồi tệ hơn khi vặn xoắn hoặc nắm chặt tay.
  • Đột nhiên gặp khó khăn trong việc tạo ra ký hiệu “OK” bình thường, hoặc yếu sức khi cố gắng kẹp đầu ngón cái vào ngón trỏ.
  • Các triệu chứng ở cẳng tay hoặc bàn tay không thuyên giảm dù đã nghỉ ngơi và thay đổi hoạt động, hoặc tình trạng yếu cơ không cải thiện sau vài tháng.
  • Một cơn đau ngắn hạn ở vai hoặc cánh tay kèm theo yếu sức ở tay; trường hợp này nên được khám vì mô hình triệu chứng này có thể là dấu hiệu của vấn đề thần kinh mà việc phát hiện sớm sẽ mang lại nhiều lợi ích.

Phân tích sâu hơn

Advanced reading: the deeper science (optional)

Phần này đi sâu hơn mức cần thiết để bạn tự đưa ra quyết định điều trị. Các hội chứng này đáng để đọc thêm vì có hai cuộc tranh luận đang diễn ra có ảnh hưởng đến phương pháp điều trị: một là chẩn đoán mới nổi lên gần đây (hội chứng lacertus) với những bằng chứng ủng hộ thực sự ấn tượng nhưng lại còn rất hạn chế; hai là vấn đề liên quan đến dây thần kinh gian xương trước, hiện nay người ta cho rằng đây không hẳn là tình trạng chèn ép, nên việc giải áp dây thần kinh nhiều khả năng cũng không phải là giải pháp đúng.

Tình trạng có khả năng không phải do chèn ép

Hội chứng dây thần kinh gian xương trước gây ra tình trạng yếu cơ điển hình; bệnh nhân không thể gập đầu ngón tay cái và ngón trỏ, khiến cử chỉ “OK” biến thành dạng kẹp ngón tay, mà không có cảm giác tê. Điều này là do nhánh thần kinh này chỉ chi phối cơ mà thôi.

Trước đây, người ta cho rằng dây thần kinh này bị chèn ép bởi một dải sợi, và điều trị theo hướng đó. Tuy nhiên, quan điểm hiện nay đã thay đổi: ngày càng có nhiều ý kiến cho rằng đây là tình trạng viêm dây thần kinh và thường tự khỏi sau một thời gian theo dõi lâu dài [1]. Diễn tiến tự nhiên của bệnh gần như luôn dẫn đến sự hồi phục hoàn toàn hoặc gần hoàn toàn; đồng thời không có bằng chứng đáng tin cậy nào cho thấy phẫu thuật có thể thay đổi diễn tiến tự nhiên này. Vì vậy, không nên vội vàng tiến hành phẫu thuật cho đến khi các nghiên cứu chất lượng cao chứng minh được rằng can thiệp sớm giúp cải thiện hoặc đẩy nhanh quá trình hồi phục [2].

Việc thay đổi quan điểm này rất quan trọng vì hai giả thuyết trên dẫn đến những biện pháp điều trị trái ngược nhau. Nếu dây thần kinh bị chèn ép bởi dải sợi, việc trì hoãn điều trị sẽ gây tổn thương vĩnh viễn và cần phải giải phóng dây thần kinh ngay lập tức. Ngược lại, nếu dây thần kinh bị viêm, việc phẫu thuật chỉ tạo thêm vết thương vào một quá trình vốn đã có xu hướng tự hồi phục; mà thời gian hồi phục của dây thần kinh thường kéo dài nhiều tháng, đủ lâu để ngay cả ca phẫu thuật thực hiện ở tháng thứ sáu cũng có vẻ như mang lại hiệu quả.

Các hướng dẫn thực hành hiện nay phản ánh mức độ không chắc chắn này chứ không giải quyết triệt để vấn đề: phẫu thuật giải ép chỉ nên được chỉ định khi các triệu chứng vẫn còn tồn tại sau 6 tháng trong hội chứng cơ vận nhẹ, hoặc sau ít nhất 12 tháng trong hội chứng dây thần kinh gian xương trước [1].

Hội chứng cơ pronator là một chẩn đoán gây tranh cãi

Hội chứng cơ pronator là tình trạng dây thần kinh giữa bị chèn ép ở vùng cao hơn, quanh khuỷu tay, gây đau cẳng tay kèm tê các ngón tay do dây thần kinh này chi phối. Vì các triệu chứng cảm giác này gần như trùng khớp hoàn toàn với hội chứng ống cổ tay – một bệnh phổ biến hơn nhiều và dễ được xác nhận qua các xét nghiệm dẫn truyền thần kinh – nên việc phân biệt hai bệnh này gặp nhiều khó khăn.

Trong các tài liệu y khoa, hội chứng này được mô tả là một chẩn đoán gây tranh cãi; thường được điều trị không phẫu thuật trong khoảng thời gian dài, khoảng 12 tháng [1]. Những dấu hiệu cho thấy tổn thương nằm ở vùng gần thân mình chứ không phải vùng cổ tay bao gồm: tình trạng tê ở vùng lòng bàn tay gần gốc ngón cái – vùng được chi phối bởi nhánh thần kinh xuất phát trước khi đi qua ống cổ tay, nên không bị ảnh hưởng trong hội chứng ống cổ tay; ngoài ra còn có cảm giác đau nhạy cảm ở cẳng tay và các triệu chứng xuất hiện khi cẳng tay quay ngược lại chống lại lực cản, chứ không phải do tư thế cổ tay vào ban đêm [3] [5].

Hội chứng Lacertus: cái tên mới cho một cấu trúc cổ xưa

Lacertus fibrosus là một dải mô sợi chạy từ gân cơ nhị đầu qua phía trước khuỷu tay; từ lâu nó đã được xem là một trong những cấu trúc có khả năng chèn ép dây thần kinh giữa. Điều mới gần đây là quan điểm cho rằng nó xứng đáng được coi là nguyên nhân chính gây bệnh. Nhóm nghiên cứu do bác sĩ phẫu thuật tay người Thụy Điển Elisabet Hagert dẫn đầu cho rằng nhiều người được chẩn đoán là bị mệt mỏi cẳng tay không rõ nguyên nhân, “bàn tay vụng về”, hoặc đã phẫu thuật giải ép ống cổ tay nhưng không đỡ, thực chất là bị chèn ép dây thần kinh ngay tại vị trí này – tình trạng này hiện được gọi chung là hội chứng Lacertus [6].

Chẩn đoán hội chứng này mang tính lâm sàng thuần túy, vì các xét nghiệm điện sinh lý thường cho kết quả bình thường trong trường hợp chèn ép dây thần kinh giữa ở vùng gần nguồn gốc: yếu ở ba cơ do dây thần kinh giữa chi phối (cơ gập đầu ngón tay cái và ngón trỏ, cùng cơ gập cổ tay), kèm theo cảm giác đau nhạy cảm ngay trên dải mô sợi đó [6]. Phương pháp điều trị được đề xuất là cắt bỏ một phần dải mô này dưới gây tê tại chỗ khi bệnh nhân vẫn tỉnh táo, nhờ đó có thể kiểm tra lại sức mạnh cơ ngay trên bàn mổ chỉ vài phút sau khi tiến hành thủ thuật. Kết quả báo cáo rất ấn tượng: trong nghiên cứu lớn nhất từng công bố, 93 bệnh nhân có điểm đánh giá khuyết tật (thang QuickDASH) trung bình 53 trước phẫu thuật đã giảm xuống còn 8 ngay sau mổ; lực nắm tay tăng từ 16 lên 24 kg; nhiều nhân viên văn phòng có thể trở lại làm việc chỉ trong vài ngày [7].

Những kết quả ấn tượng như vậy cần được xem xét kỹ lưỡng, và những ý kiến hoài nghi cũng đã được công bố. Một nghiên cứu cho thấy cảm giác đau nhạy cảm tại vùng lacertus fibrosus khá phổ biến ngay cả ở những người hoàn toàn khỏe mạnh: trong 72 cánh tay không có triệu chứng, có tới 14 cánh tay (khoảng 1/5) xuất hiện triệu chứng này; điều này làm giảm giá trị chẩn đoán của triệu chứng đau nhạy cảm đơn thuần và khiến việc so sánh với cánh tay còn lại trở nên không đáng tin cậy [8]. Một số nhà nghiên cứu chỉ ra rằng hiện nay thuật ngữ “hội chứng Lacertus” đang được dùng cho hai vấn đề khác nhau: chèn ép dây thần kinh thực sự và tình trạng quá tải cơ ở cùng vị trí; việc dùng chung một tên gọi có thể khiến việc phân loại bệnh trở nên mơ hồ [9]. Cho đến nay vẫn chưa có thử nghiệm ngẫu nhiên nào; thời gian theo dõi trong các nghiên cứu hiện có cũng còn ngắn; việc chẩn đoán một bệnh lý mà không có xét nghiệm khách quan rồi điều trị bằng phẫu thuật có phản hồi tức thì chính là môi trường lý tưởng để các hiệu ứng chọn lọc và hiệu ứng giả dược làm méo mó kết quả.

Cách tiếp cận hợp lý nhất, như đã nêu trên trang này, là: dải mô sợi này thực sự tồn tại và có khả năng chèn ép dây thần kinh; khi xuất hiện mẫu triệu chứng đặc trưng gồm yếu cơ và cảm giác đau nhạy cảm, việc cắt bỏ có mục tiêu là một ca phẫu thuật nhỏ với thời gian hồi phục nhanh. Sự nhiệt tình cũng như những hoài nghi đều là một phần của các tài liệu nghiên cứu hiện nay.

Tại sao việc kiên nhẫn chờ đợi là hợp lý chứ không chỉ là thái độ thận trọng

Đối với cả hai tình trạng bệnh, phương pháp điều trị được khuyến nghị là theo dõi lâu dài; điều này thật khó chấp nhận khi các triệu chứng vẫn còn tồn tại. Có hai yếu tố khiến cách tiếp cận này là hợp lý.

Thứ nhất là diễn tiến tự nhiên của bệnh: đối với hội chứng dây thần kinh gian xương trước, việc hồi phục là kết quả mong đợi ngay cả khi không can thiệp y tế [2]. Thứ hai là chẩn đoán thay thế trong trường hợp liệt đau khởi phát đột ngột là chứng teo cơ do đau thần kinh, hay còn gọi là hội chứng Parsonage-Turner – một tình trạng viêm dây thần kinh thường bắt đầu bằng cơn đau vai hoặc cánh tay dữ dội kéo dài vài ngày đến vài tuần, sau đó mới xuất hiện yếu cơ khi cơn đau giảm bớt [4]. Đây là một bệnh lý y khoa chứ không phải do nguyên nhân cơ học; vì vậy việc giải ép dây thần kinh cũng không có tác dụng gì.

Do đó, việc xuất hiện cơn đau dữ dội trước khi xuất hiện triệu chứng yếu cơ là đặc điểm quan trọng nhất cần được ghi nhận chính xác, vì nó giúp xác định chẩn đoán đúng đắn, loại trừ khả năng do chèn ép dây thần kinh.

To the extent this Public License may be interpreted as a contract, You are granted the Licensed Rights in consideration of Your acceptance of these terms and conditions, and the Licensor grants You such rights in consideration of benefits the Licensor receives from making the Licensed Material available under these terms and conditions.

Section 1 -- Definitions.

a. Adapted Material means material subject to Copyright and Similar Rights that is derived from or based upon the Licensed Material and in which the Licensed Material is translated, altered, arranged, transformed, or otherwise modified in a manner requiring permission under the Copyright and Similar Rights held by the Licensor. For purposes of this Public License, where the Licensed Material is a musical work, performance, or sound recording, Adapted Material is always produced where the Licensed Material is synched in timed relation with a moving image.

b. Adapter's License means the license You apply to Your Copyright and Similar Rights in Your contributions to Adapted Material in accordance with the terms and conditions of this Public License.

c. Copyright and Similar Rights means copyright and/or similar rights closely related to copyright including, without limitation, performance, broadcast, sound recording, and Sui Generis Database Rights, without regard to how the rights are labeled or categorized. For purposes of this Public License, the rights specified in Section 2(b)(1)-(2) are not Copyright and Similar Rights.

d. Effective Technological Measures means those measures that, in the absence of proper authority, may not be circumvented under laws fulfilling obligations under Article 11 of the WIPO Copyright Treaty adopted on December 20, 1996, and/or similar international agreements.

e. Exceptions and Limitations means fair use, fair dealing, and/or any other exception or limitation to Copyright and Similar Rights that applies to Your use of the Licensed Material.

f. Licensed Material means the artistic or literary work, database, or other material to which the Licensor applied this Public License.

g. Licensed Rights means the rights granted to You subject to the terms and conditions of this Public License, which are limited to all Copyright and Similar Rights that apply to Your use of the Licensed Material and that the Licensor has authority to license.

h. Licensor means the individual(s) or entity(ies) granting rights under this Public License.

i. NonCommercial means not primarily intended for or directed towards commercial advantage or monetary compensation. For purposes of this Public License, the exchange of the Licensed Material for other material subject to Copyright and Similar Rights by digital file-sharing or similar means is NonCommercial provided there is no payment of monetary compensation in connection with the exchange.

j. Share means to provide material to the public by any means or process that requires permission under the Licensed Rights, such as reproduction, public display, public performance, distribution, dissemination, communication, or importation, and to make material available to the public including in ways that members of the public may access the material from a place and at a time individually chosen by them.

k. Sui Generis Database Rights means rights other than copyright resulting from Directive 96/9/EC of the European Parliament and of the Council of 11 March 1996 on the legal protection of databases, as amended and/or succeeded, as well as other essentially equivalent rights anywhere in the world.

l. You means the individual or entity exercising the Licensed Rights under this Public License. Your has a corresponding meaning.

Section 2 -- Scope.

a. License grant.

1. Subject to the terms and conditions of this Public License, the Licensor hereby grants You a worldwide, royalty-free, non-sublicensable, non-exclusive, irrevocable license to exercise the Licensed Rights in the Licensed Material to:

a. reproduce and Share the Licensed Material, in whole or in part, for NonCommercial purposes only; and

b. produce, reproduce, and Share Adapted Material for NonCommercial purposes only.

2. Exceptions and Limitations. For the avoidance of doubt, where Exceptions and Limitations apply to Your use, this Public License does not apply, and You do not need to comply with its terms and conditions.

3. Term. The term of this Public License is specified in Section 6(a).

4. Media and formats; technical modifications allowed. The Licensor authorizes You to exercise the Licensed Rights in all media and formats whether now known or hereafter created, and to make technical modifications necessary to do so. The Licensor waives and/or agrees not to assert any right or authority to forbid You from making technical modifications necessary to exercise the Licensed Rights, including technical modifications necessary to circumvent Effective Technological Measures. For purposes of this Public License, simply making modifications authorized by this Section 2(a) (4) never produces Adapted Material.

5. Downstream recipients.

a. Offer from the Licensor -- Licensed Material. Every recipient of the Licensed Material automatically receives an offer from the Licensor to exercise the Licensed Rights under the terms and conditions of this Public License.

b. No downstream restrictions. You may not offer or impose any additional or different terms or conditions on, or apply any Effective Technological Measures to, the Licensed Material if doing so restricts exercise of the Licensed Rights by any recipient of the Licensed Material.

6. No endorsement. Nothing in this Public License constitutes or may be construed as permission to assert or imply that You are, or that Your use of the Licensed Material is, connected with, or sponsored, endorsed, or granted official status by, the Licensor or others designated to receive attribution as provided in Section 3(a)(1)(A)(i).

b. Other rights.

1. Moral rights, such as the right of integrity, are not licensed under this Public License, nor are publicity, privacy, and/or other similar personality rights; however, to the extent possible, the Licensor waives and/or agrees not to assert any such rights held by the Licensor to the limited extent necessary to allow You to exercise the Licensed Rights, but not otherwise.

2. Patent and trademark rights are not licensed under this Public License.

3. To the extent possible, the Licensor waives any right to collect royalties from You for the exercise of the Licensed Rights, whether directly or through a collecting society under any voluntary or waivable statutory or compulsory licensing scheme. In all other cases the Licensor expressly reserves any right to collect such royalties, including when the Licensed Material is used other than for NonCommercial purposes.

Section 3 -- License Conditions.

Your exercise of the Licensed Rights is expressly made subject to the following conditions.

a. Attribution.

1. If You Share the Licensed Material (including in modified form), You must:

a. retain the following if it is supplied by the Licensor with the Licensed Material:

i. identification of the creator(s) of the Licensed Material and any others designated to receive attribution, in any reasonable manner requested by the Licensor (including by pseudonym if designated);

ii. a copyright notice;

iii. a notice that refers to this Public License;

iv. a notice that refers to the disclaimer of warranties;

v. a URI or hyperlink to the Licensed Material to the extent reasonably practicable;

b. indicate if You modified the Licensed Material and retain an indication of any previous modifications; and

c. indicate the Licensed Material is licensed under this Public License, and include the text of, or the URI or hyperlink to, this Public License.

2. You may satisfy the conditions in Section 3(a)(1) in any reasonable manner based on the medium, means, and context in which You Share the Licensed Material. For example, it may be reasonable to satisfy the conditions by providing a URI or hyperlink to a resource that includes the required information.

3. If requested by the Licensor, You must remove any of the information required by Section 3(a)(1)(A) to the extent reasonably practicable.

4. If You Share Adapted Material You produce, the Adapter's License You apply must not prevent recipients of the Adapted Material from complying with this Public License.

Section 4 -- Sui Generis Database Rights.

Where the Licensed Rights include Sui Generis Database Rights that apply to Your use of the Licensed Material:

a. for the avoidance of doubt, Section 2(a)(1) grants You the right to extract, reuse, reproduce, and Share all or a substantial portion of the contents of the database for NonCommercial purposes only;

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

Section 5 -- Disclaimer of Warranties and Limitation of Liability.

a. UNLESS OTHERWISE SEPARATELY UNDERTAKEN BY THE LICENSOR, TO THE EXTENT POSSIBLE, THE LICENSOR OFFERS THE LICENSED MATERIAL AS-IS AND AS-AVAILABLE, AND MAKES NO REPRESENTATIONS OR WARRANTIES OF ANY KIND CONCERNING THE LICENSED MATERIAL, WHETHER EXPRESS, IMPLIED, STATUTORY, OR OTHER. THIS INCLUDES, WITHOUT LIMITATION, WARRANTIES OF TITLE, MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT, ABSENCE OF LATENT OR OTHER DEFECTS, ACCURACY, OR THE PRESENCE OR ABSENCE OF ERRORS, WHETHER OR NOT KNOWN OR DISCOVERABLE. WHERE DISCLAIMERS OF WARRANTIES ARE NOT ALLOWED IN FULL OR IN PART, THIS DISCLAIMER MAY NOT APPLY TO YOU.

b. TO THE EXTENT POSSIBLE, IN NO EVENT WILL THE LICENSOR BE LIABLE TO YOU ON ANY LEGAL THEORY (INCLUDING, WITHOUT LIMITATION, NEGLIGENCE) OR OTHERWISE FOR ANY DIRECT, SPECIAL, INDIRECT, INCIDENTAL, CONSEQUENTIAL, PUNITIVE, EXEMPLARY, OR OTHER LOSSES, COSTS, EXPENSES, OR DAMAGES ARISING OUT OF THIS PUBLIC LICENSE OR USE OF THE LICENSED MATERIAL, EVEN IF THE LICENSOR HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH LOSSES, COSTS, EXPENSES, OR DAMAGES. WHERE A LIMITATION OF LIABILITY IS NOT ALLOWED IN FULL OR IN PART, THIS LIMITATION MAY NOT APPLY TO YOU.

c. The disclaimer of warranties and limitation of liability provided above shall be interpreted in a manner that, to the extent possible, most closely approximates an absolute disclaimer and waiver of all liability.

Section 6 -- Term and Termination.

a. This Public License applies for the term of the Copyright and Similar Rights licensed here. However, if You fail to comply with this Public License, then Your rights under this Public License terminate automatically.

b. Where Your right to use the Licensed Material has terminated under Section 6(a), it reinstates:

1. automatically as of the date the violation is cured, provided it is cured within 30 days of Your discovery of the violation; or

2. upon express reinstatement by the Licensor.

For the avoidance of doubt, this Section 6(b) does not affect any right the Licensor may have to seek remedies for Your violations of this Public License.

c. For the avoidance of doubt, the Licensor may also offer the Licensed Material under separate terms or conditions or stop distributing the Licensed Material at any time; however, doing so will not terminate this Public License.

d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.

d. Nothing in this Public License constitutes or may be interpreted as a limitation upon, or waiver of, any privileges and immunities that apply to the Licensor or You, including from the legal processes of any jurisdiction or authority.
