Các dây chằng chính là những “dây đai” của khớp: những sợi mô chắc khỏe nối xương với xương, giữ cho khớp ở đúng vị trí và âm thầm truyền tín hiệu về não bộ về vị trí của khớp trong không gian. Khi một dây chằng bị kéo giãn quá mức hoặc rách (gọi là “chấn thương dây chằng”), khớp có thể cảm thấy lỏng lẻo hoặc dễ bị trật. Dây chằng là gì và có chức năng gì?

Dây chằng là một dải mô ngắn, chắc khỏe, chủ yếu gồm collagen, nối từ xương này sang xương kia qua khớp. Dây chằng có hai chức năng: vừa đóng vai trò là dây giữ để ngăn khớp di chuyển quá xa hoặc theo hướng không đúng, vừa là cảm biến chứa nhiều đầu dây thần kinh giúp truyền thông tin về vị trí và chuyển động của khớp (hiện tượng này gọi là cảm giác bản thể). Chính nhờ khả năng cảm nhận này mà một khớp bị tổn thương dây chằng vẫn có thể cảm thấy không ổn định hoặc thiếu độ tin cậy, dù nhìn bề ngoài có vẻ bình thường.

Quy trình hồi phục của các dây chằng (và lý do một số không hồi phục được)

Các dây chằng bị bong gân hồi phục tương tự như các mô khác: xuất hiện chảy máu và viêm, sau đó collagen mới được hình thành để lấp đầy vết rách; quá trình tái cấu trúc và tăng cường độ bền diễn ra dần dần trong nhiều tháng. Nhiều dây chằng hồi phục tốt theo cách này, đặc biệt khi được bảo vệ bằng nẹp và dần dần tăng dần mức độ vận động.

  • Thời gian. Quá trình tái tạo dây chằng kéo dài nhiều tháng; một chấn thương dây chằng “đã lành” vẫn tiếp tục tăng sức mạnh ngay cả sau khi không còn cảm thấy đau nữa.

Dây chằng scaphoid-lunate nằm bên trong khớp cổ tay, giữa hai xương cổ tay nhỏ, và được bao quanh bởi dịch khớp. Khi dây chằng này bị rách hoàn toàn, dịch khớp sẽ cuốn trôi mọi cục máu đông, khiến không có vật liệu nào để các tế bào sửa chữa có thể di chuyển qua; đồng thời nguồn cung cấp máu cho vùng này rất kém, khiến hai đầu dây chằng bị tách rời. Do đó, tình trạng rách hoàn toàn dây chằng scaphoid-lunate thường không tự lành; nếu không được điều trị kịp thời, các xương cổ tay sẽ dần mất vị trí bình thường và bị hao mòn (hiện tượng mà các bác sĩ gọi là SLAC). Đó là lý do tại sao những chấn thương này cần được phẫu thuật sửa chữa sớm, đặc biệt khi cần duy trì sự ổn định của khớp.

Ngược lại, các dây chằng bên cạnh ngón tay cái hoặc khuỷu tay lại nằm bên ngoài khoang khớp chính. Ban đầu, mảnh ghép này về cơ bản chỉ là collagen không còn sống. Những yếu tố hỗ trợ và cản trở quá trình hồi phục của dây chằng

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

Section 5 -- Disclaimer of Warranties and Limitation of Liability.

a. d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.
