Sụn là lớp bề mặt mịn, bóng loáng phủ lên các đầu xương bên trong khớp. Đây là một trong những loại mô đặc biệt nhất trong cơ thể: trơn tru hơn cả băng, có khả năng chịu đựng sức nén trong suốt cả đời người; tuy nhiên khác với xương và gân, sụn hầu như không có khả năng tự phục hồi. Chính điều này giải thích tại sao tổn thương sụn và bệnh viêm khớp thoái hóa lại là những vấn đề khó giải quyết, và tại sao việc bảo vệ sụn lại vô cùng quan trọng. Sụn là gì và có chức năng gì?

Loại sụn mà chúng ta đang nói đến, sụn khớp, là lớp phủ cứng, mịn và màu trắng dày chỉ vài milimét nằm ở đầu các xương nơi chúng gặp nhau tạo thành khớp. Chức năng chính của nó là hai điều sau:

  • Nó là bề mặt trượt gần như không có ma sát, giúp các xương trong khớp trượt lên nhau gần như không gặp trở ngại nào.
  • Nó đóng vai trò tấm đệm, phân tán lực tác động lên khớp và bảo vệ xương bên dưới khỏi các chấn thương.

Sụn là mô sống, nhưng thuộc loại đặc biệt: nó không có mạch máu và không có dây thần kinh. Việc không có dây thần kinh là lý do khiến bề mặt sụn bị mòn cũng không gây đau đớn (cơn đau trong bệnh viêm khớp xuất phát từ các cấu trúc khác). Việc thiếu nguồn cung cấp máu lại là nguyên nhân khiến sụn rất dễ bị tổn thương: nó không thể tự lành như da hay xương.

Tại sao sụn khó lành lại?

Ở hầu hết các vùng khác của cơ thể, quá trình lành thương bắt đầu bằng việc chảy máu: cục máu đông hình thành, các tế bào viêm di chuyển đến và các yếu tố tăng trưởng kích hoạt quá trình sửa chữa. Tuy nhiên, sụn không có nguồn cung cấp máu; vì vậy khi bị tổn thương, nó không nhận được bất kỳ yếu tố nào như vậy. Thêm vào đó, số ít tế bào có mặt trong sụn lại bị mắc kẹt trong chất nền xung quanh và không thể di chuyển đến vết thương để sửa chữa nó.

Hệ quả là những vết trầy xước hoặc nứt chỉ giới hạn trong mô sụn thường cứ thế tồn tại mãi. Những tổn thương sâu hơn (xuyên qua sụn và vào tận xương bên dưới) có thể được lấp đầy, nhưng bằng loại “sụn xơ” giống sẹo – loại sụn yếu hơn và kém bền vững so với sụn ban đầu. Những điều giúp bảo vệ sụn khớp

  • Hãy vận động thường xuyên. Sụn khớp được nuôi dưỡng nhờ dịch khớp được bơm vào và thoát ra khi chúng ta vận động; các hoạt động nhẹ nhàng, vừa sức giúp nuôi dưỡng sụn, trong khi việc ít vận động kéo dài lại khiến sụn bị suy dinh dưỡng.
  • Duy trì cân nặng hợp lý. Mỗi kilogram thừa ra đều tạo thêm áp lực lên các bề mặt khớp.
  • Tăng cường sức mạnh cho các cơ quanh khớp. Các cơ khỏe mạnh sẽ gánh vác một phần tải trọng và hấp thụ lực tác động, từ đó bảo vệ sụn khớp.
  • Chăm sóc các chấn thương. Một khớp không ổn định hoặc dễ bị lỏng lẻo sẽ làm mòn sụn nhanh hơn; việc điều trị tình trạng này giúp bảo vệ bề mặt sụn.

Sụn – một mô sống

Sụn khớp thuộc loại sụn hyalin. Nó được cấu tạo từ những tế bào rải rác (chondrocytes) được bao quanh bởi một khối lượng lớn ma trận do chính các tế bào này sản xuất và duy trì. Ma trận chính là yếu tố quyết định mọi chức năng của sụn. Đó là một mạng lưới gồm các sợi collagen loại II, chứa đầy những phân tử có hình dạng giống bàn chải, có khả năng hút nước mạnh mẽ gọi là proteoglycans (chủ yếu là aggrecan), cùng với khoảng 70–80% nước. Mạng lưới collagen giúp sụn không bị kéo giãn; còn aggrecan hút nước vào và tạo ra áp lực phồng lên giúp sụn không bị nén chặt. Về cơ bản, sụn giống như một miếng bọt biển chứa đầy nước, được giữ căng trong mạng lưới sợi.

Sụn cũng có đặc điểm đặc biệt là hoàn toàn tách biệt: không có mạch máu, không có dây thần kinh, không có mạch bạch huyết. Các tế bào chondrocytes nằm trong những khoang nhỏ (gọi là lacunae), từ từ duy trì vùng ma trận xung quanh mình và không thể di chuyển.

Mô học của sụn khớp gồm ba vùng: vùng bề mặt (các tế bào dẹt), vùng giữa và vùng sâu (các tế bào xếp thành cột), cho thấy sự thay đổi hình dạng của tế bào sụn theo độ sâu.

Sụn là một trong những vật liệu trơn trượt nhất được biết đến: hệ số ma sát của nó chỉ khoảng 0,001, thấp hơn cả ma sát giữa hai lớp băng. Hiện tượng này xảy ra nhờ hai cơ chế phối hợp với nhau.

Thứ nhất, sụn mang tính hai pha: đó là một ma trận rắn thấm đầy dịch. Khi khớp chịu tải, lượng nước bị giữ lại trong sụn không thể thoát ra ngay lập tức; do đó áp lực dịch tăng lên và giúp chịu tải – ban đầu là hơn 90% tổng tải trọng. Lực chịu tải đến từ áp lực dịch chứ không phải từ cấu trúc rắn, tương tự như việc đứng trên chiếc nệm nước vậy. Khi điểm tiếp xúc thay đổi (do cơ thể vẫn đang chuyển động), dịch sẽ chảy ra phía trước và lại được hút trở lại phía sau. Hiện tượng tăng áp lực dịch nội tại này là nguyên nhân chính giúp sụn chịu được tải trọng lớn mà hầu như không gây ma sát hay mài mòn.

Thứ hai, bề mặt sụn có một lớp phân tầng màng bôi trơn gồm nhiều phân tử như lubricin (còn gọi là PRG4) và axit hyaluronic có trong dịch khớp; lớp màng này giữ cho hai bề mặt không bao giờ tiếp xúc trực tiếp với nhau.

Ma trận sụn cũng được sắp xếp thành các vùng: ở vùng trên cùng (vùng bề mặt), các sợi collagen chạy song song với bề mặt, giúp giữ lớp màng bôi trơn và chống lại lực cắt; ở vùng giữa, các sợi này có cấu trúc lộn xộn hơn; còn ở vùng sâu, chúng mọc thẳng đứng và gắn vào lớp sụn vôi hóa (được ngăn cách bởi một đường gọi là đường tidemark). Lớp này giúp liên kết chắc chắn giữa sụn mềm và xương cứng; đây cũng là thách thức kỹ thuật tương tự như việc nối mô mềm với mô cứng mà mô gân bám vào xương phải giải quyết.

Tại sao sụn hầu như không thể tự lành

Khi kết hợp các yếu tố lại với nhau, nguyên nhân dẫn đến khả năng hồi phục kém của sụn trở nên dễ hiểu:

  • Không có nguồn cung cấp máu: điều này dẫn đến không có cục máu đông, không có các tế bào viêm, và thiếu hụt các yếu tố tăng trưởng cần thiết để thúc đẩy quá trình sửa chữa ở các mô khác.
  • Các tế bào sụn hầu như bất lực: số lượng tế bào sụn rất ít; chúng bị “giam cầm” trong ma trận sụn và không thể di chuyển hay nhân lên đủ để lấp đầy vùng bị tổn thương.
  • Độ sâu của tổn thương quyết định phản ứng sửa chữa. Một tổn thương chỉ ở lớp sụn sẽ không tiếp cận được nguồn máu hay tủy xương, nên hầu như không có phản ứng sửa chữa nào xảy ra. Ngược lại, tổn thương xuyên qua toàn bộ độ dày sụn tới tận xương dưới sụn sẽ cho phép các tế bào tủy xương và máu tràn vào; chúng lấp đầy khoảng trống, nhưng tạo thành sụn xơ (chứa chủ yếu collagen loại I, ít proteoglycan). Loại sụn này kém bền vững về mặt cơ học và dễ bị mòn nhanh hơn so với sụn hyalin thực thụ.

Đây là điều gây thất vọng lớn nhất trong phẫu thuật sụn: chúng ta có thể lấp đầy chỗ hổng, nhưng không thể tạo ra loại sụn đúng như bản gốc.

Cơ chế cung cấp dinh dưỡng cho sụn

Do không có mạch máu riêng, sụn được nuôi dưỡng nhờ quá trình khuếch tán từ dịch khớp, và quá trình này được thúc đẩy bởi động tác và lực tác động lên khớp. Mỗi khi khớp chịu lực rồi lại được giải phóng lực, dịch và các chất dinh dưỡng sẽ được đẩy vào và ra khỏi sụn (tương tự như cách các chuyển động của ngón tay giúp nuôi dưỡng gân cơ đang hồi phục). Vì vậy, sụn thực sự cần có các chuyển động tuần hoàn để duy trì sức khỏe: việc bất động hoặc thiếu lực tác động trong thời gian dài sẽ làm suy thoái sụn, trong khi các hoạt động vừa phải và đều đặn sẽ giúp bảo vệ nó. Câu nói của các chuyên viên vật lý trị liệu rằng “chuyển động là liều thuốc” ở đây có cơ sở sinh học thực sự.

Thoái hóa khớp: Khi sụn bị phá hủy

Thoái hóa khớp (Osteoarthritis – OA) thường được gọi là “hiện tượng mòn và rách sụn”, nhưng cách gọi này chưa phản ánh đúng bản chất của bệnh: đây là một quá trình sinh học tích cực, chứ không chỉ đơn thuần là sự mài mòn cơ học.

Các yếu tố như quá tải cơ học, chấn thương, lão hóa hoặc sự mất ổn định của khớp sẽ kích hoạt các tế bào sụn (chondrocytes) chuyển sang chế độ phá hủy. Chúng tăng cường sản xuất các enzyme phân hủy mô liên kết (MMPs như MMP-13, và aggrecanases như ADAMTS-4, ADAMTS-5) – những enzyme này phá vỡ collagen loại II và aggrecan nhanh hơn tốc độ tái tạo của chúng. Hệ quả là mô sụn mất đi các phân tử proteoglycan giữ nước, trở nên mềm và dễ rách. Các tế bào sụn cũng thay đổi đặc tính (hiện tượng gọi là phì đại); sụn bắt đầu bị vôi hóa, cho phép mạch máu xâm nhập; xương dưới sụn dày lên và hình thành các gai xương (osteophytes). Lớp màng bao khớp (synovium) cũng bị viêm nhẹ, giải phóng các chất như IL-1 và TNF – những chất này tiếp tục thúc đẩy chu trình phá hủy. Vì sụn không có khả năng tái sinh, quá trình này hầu như không thể đảo ngược; vì vậy việc điều trị thoái hóa khớp chủ yếu tập trung vào giảm áp lực lên khớp, tăng cường sức mạnh cơ bắp và kiểm soát cân nặng; khi sụn đã hoàn toàn mất đi, phương pháp điều trị duy nhất là thay khớp.

Phục hồi sụn: tại sao lại khó đến vậy?

Có nhiều phương pháp phẫu thuật nhằm khôi phục bề mặt sụn bị tổn thương; lý do khiến không phương pháp nào mang lại kết quả hoàn hảo nằm ngay ở các yếu tố sinh học sau:

  • Phương pháp vi nứt tạo ra những lỗ nhỏ trên xương dưới sụn để các tế bào tủy xương thâm nhập vào. Những tế bào này hình thành sụn sợi, vốn hữu ích trong ngắn hạn nhưng thường bị thoái hóa sau vài năm.
  • Cấy ghép mảnh sụn-xương (OATS/mosaicplasty) lấy những mảnh sụn và xương từ vùng khớp chịu lực ít hơn. Đây là loại sụn hyalin thật sự, nhưng nguồn cung cấp lại hạn chế.
  • Liệu pháp tế bào (ACI/MACI) nuôi cấy các tế bào sụn của chính bệnh nhân trong phòng thí nghiệm rồi cấy lại; phương pháp này có triển vọng nhưng đòi hỏi kỹ thuật phức tạp và vẫn chưa hoàn thiện.

Không phương pháp nào có thể tái tạo lại được lớp sụn hyalin nguyên bản với cấu trúc từng vùng và khả năng bôi trơn như ban đầu. Khi toàn bộ sụn của một khớp bị mòn hết, giải pháp tối ưu vẫn là thay khớp.

Những yếu tố có lợi và có hại đối với sụn

  • Vận động và tải trọng vừa phải giúp nuôi dưỡng và duy trì sụn; việc bất động kéo dài lại gây hại cho sụn.
  • Thừa cân và tình trạng khớp không ổn định làm tăng tốc quá trình thoái hóa; ngược lại, các cơ vùng xung quanh khỏe mạnh giúp bảo vệ sụn.
  • Các chấn thương, đặc biệt là những chấn thương làm mất ổn định khớp hoặc tổn thương bề mặt sụn, làm tăng nguy cơ mắc bệnh viêm khớp thoái hóa về lâu dài.
  • Một sự thật khắc nghiệt cần ghi nhớ: một khi đã bị tổn thương nghiêm trọng, sụn không thể mọc lại. Gần như không có mô nào khác mà việc phòng ngừa lại quan trọng hơn thế.

a. for the avoidance of doubt, Section 2(a)(1) grants You the right to extract, reuse, reproduce, and Share all or a substantial portion of the contents of the database for NonCommercial purposes only;

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

Section 6 -- Term and Termination.

a. This Public License applies for the term of the Copyright and Similar Rights licensed here. However, if You fail to comply with this Public License, then Your rights under this Public License terminate automatically.

b. Where Your right to use the Licensed Material has terminated under Section 6(a), it reinstates:

1. automatically as of the date the violation is cured, provided it is cured within 30 days of Your discovery of the violation; or

2. upon express reinstatement by the Licensor.

For the avoidance of doubt, this Section 6(b) does not affect any right the Licensor may have to seek remedies for Your violations of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.

d. Nothing in this Public License constitutes or may be interpreted as a limitation upon, or waiver of, any privileges and immunities that apply to the Licensor or You, including from the legal processes of any jurisdiction or authority.
