Gân là những sợi dây chắc khỏe trong cơ thể, có chức năng nối cơ với xương và truyền lực từ cơ sang xương để tạo ra chuyển động. Khi bạn gập ngón tay hoặc nâng cánh tay lên, chính các gân là những thành phần vận chuyển lực đó qua khớp. Các gân rất chắc bền, nhưng quá trình lành thương của chúng diễn ra chậm và theo những cơ chế riêng biệt; không phải tất cả các gân đều lành thương theo cùng một cách. Gân gập ngón tay bị đứt và gân cơ chóp xoay ở vai có quá trình lành thương hoàn toàn khác nhau, vì vậy phương pháp phẫu thuật cũng như phục hồi chức năng cho chúng cũng khác biệt. Gân là gì và có chức năng gì?

Gân là một dạng “sợi dây” được tạo thành chủ yếu từ collagen (loại protein cứng cáp giống như chất tạo nên độ dẻo dai cho xương), được sắp xếp thành các bó chặt chẽ dọc theo hướng lực tác động. Một đầu gân hòa quyện vào cơ; đầu kia gắn vào xương. Chức năng của gân rất đơn giản nhưng vô cùng quan trọng: truyền lực mà cơ tạo ra đến xương, giúp khớp cử động. Một số gân còn phải có khả năng trượt: ví dụ như các gân gập ngón tay phải di chuyển qua lại trong những đường hầm hẹp mỗi khi bạn nắm tay lại.

Gân là mô sống, nhưng chỉ ở mức tối thiểu: so với cơ hoặc da, gân chứa ít tế bào hơn và hệ thống mạch máu cũng thưa thớt hơn. Đây là một trong những lý do chính khiến gân lành thương chậm.

Quá trình hồi phục của gân

Khi gân bị cắt đứt hoặc rách, nó hồi phục qua ba giai đoạn chồng chép lên nhau, tương tự như các mô khác:

  1. Giai đoạn viêm (tuần đầu tiên). Máu đông hình thành và các tế bào sửa chữa di chuyển đến vùng tổn thương. Ở giai đoạn này, khớp được giữ lại chủ yếu nhờ các mũi khâu của bác sĩ phẫu thuật; bản thân gân hầu như chưa có khả năng chịu lực.
  2. Giai đoạn sửa chữa (vài tuần). Các tế bào tạo ra collagen mới để lấp đầy khoảng trống, nhưng ban đầu nó còn lộn xộn và yếu, giống như một bó sợi được buộc vội vã chứ chưa thành sợi dây vững chắc.
  3. Giai đoạn tái cấu trúc (vài tháng). Theo thời gian và nhờ việc vận động nhẹ nhàng, khối collagen lộn xộn này dần được thay thế và sắp xếp lại theo hướng kéo của gân, giúp gân phục hồi sức mạnh. Quá trình này kéo dài nhiều tháng, thường là một năm hoặc hơn.

Điểm quan trọng cần lưu ý: Gân hồi phục chủ yếu bằng cách hình thành mô sẹo, chứ không phải tái tạo lại mô gốc một cách hoàn hảo. Vùng gân đã được sửa chữa sẽ không bao giờ trở lại trạng thái nguyên vẹn như ban đầu; vì vậy việc phục hồi chức năng một cách cẩn thận và sự kiên nhẫn là điều vô cùng cần thiết.

Những yếu tố giúp gân hồi phục

  • Vận động đúng mức. Gân phản ứng với lực tác động. Các bài tập được kiểm soát và tăng dần mức độ (dưới sự hướng dẫn của bác sĩ hoặc chuyên viên vật lý trị liệu) giúp cấu trúc collagen đang hồi phục sắp xếp đúng cách. Nếu vận động quá mức hoặc quá sớm, gân có thể bị rách; còn nếu vận động quá ít thì gân sẽ bị cứng và hình thành mô sẹo làm gân dính lại.
  • Bảo vệ vết khâu gân trong giai đoạn đầu. Ngay cả khi cảm thấy ổn, vết khâu gân vẫn rất yếu trong vài tuần đầu; vì vậy cần tuân thủ việc đeo nẹp và các hạn chế về hoạt động để tránh làm vết khâu bị tách ra.
  • Sức khỏe tổng thể tốt. Không hút thuốc, kiểm soát đường huyết ở bệnh nhân tiểu đường và tránh dùng steroid không cần thiết đều có lợi; đặc biệt, hút thuốc làm suy giảm quá trình hồi phục của gân.
  • Thời gian. Gân là loại mô hồi phục chậm. Để có được sức mạnh thực sự, cần mất vài tháng chứ không phải vài tuần.

Gân như một mô sống

Gân được cấu tạo theo dạng phân cấp của các sợi collagen, giống như một sợi dây thừng gồm nhiều sợi nhỏ hơn nối lại với nhau. Các phân tử collagen tập hợp thành các sợi vi, các sợi vi lại hợp thành sợi lớn, các sợi lớn tạo thành bó sợi, và các bó sợi này hợp thành toàn bộ gân. Ở mỗi cấp độ, một lớp mô liên kết mỏng (gọi là nội gân và ngoại gân bao quanh toàn bộ gân) chứa các mạch máu và dây thần kinh nhỏ. Thành phần collagen chủ yếu là loại I, có độ bền chịu lực căng cực cao và được sắp xếp gần như song song hoàn toàn theo hướng lực tác động.

Khi nghỉ ngơi, các sợi collagen có dạng gợn sóng, uốn lượn. Khi bắt đầu chịu tải, những gợn sóng này sẽ thẳng ra; vì vậy gân có độ đàn hồi nhẹ ở giai đoạn đầu khi chịu lực, trước khi trở nên cứng cáp và chắc chắn (tức là gân có một chút khả năng co giãn tự nhiên). Gân cũng mang tính nhớt đàn hồi: cách ứng xử của nó thay đổi tùy theo tốc độ chịu tải (cứng hơn khi chịu tải nhanh), và nó sẽ từ từ giãn ra dưới tác động của lực liên tục trong thời gian dài.

Các tế bào sống trong gân là tế bào gân, những tế bào hình thoi rải rác nằm xen giữa các bó sợi collagen. Chúng có chức năng duy trì và từ từ tái tạo mô nền. Do số lượng tế bào và mạch máu tương đối ít, cùng với việc một số vùng nhất định của gân thực sự thiếu mạch máu (các vùng “vùng nước” có nguồn cung cấp máu kém), quá trình hồi phục gân diễn ra chậm; đồng thời những vùng này lại là nơi dễ xảy ra rách gân và hồi phục kém.

Cơ chế hồi phục của gân – và lý do hầu hết là qua hình thành sẹo

Ba giai đoạn nêu trên được mô tả chi tiết như sau: Giai đoạn viêm (khoảng tuần đầu tiên) giúp loại bỏ các mảnh vụn và thu hút các tế bào; giai đoạn tăng sinh (khoảng từ 1 đến 3 tuần) là lúc các tế bào sản xuất ra lượng lớn collagen loại III không có trật tự, loại collagen này hình thành nhanh nhưng lại yếu về mặt cơ học; tiếp đến là giai đoạn tái cấu trúc kéo dài (vài tháng, có thể lên tới 18 tháng) trong đó collagen loại III dần được thay thế bằng collagen loại I vững chắc, có cấu trúc sắp xếp đều đặn và liên kết chéo theo hướng chịu lực.

Khái niệm quan trọng cần nhớ là đây là quá trình hồi phục qua hình thành sẹo, chứ không phải tái tạo. Cơ thể chỉ lấp đầy khoảng trống thay vì tái tạo lại cấu trúc gân nguyên vẹn, có trật tự như ban đầu. Yếu tố quyết định biến khối sẹo yếu ớt ban đầu thành cấu trúc gân vững chắc và có trật tự chính là lực cơ học. Đây cũng là cơ sở lý luận cho các phương pháp phục hồi chức năng gân hiện đại: việc áp dụng lực có kiểm soát vào thời điểm thích hợp thực chất là cách “hướng dẫn” khối sẹo phát triển thành gân bình thường.

Quá trình hồi phục của gân duỗi: cơ chế nội tại và ngoại tại, cùng “công việc gập ngón tay”

Các gân duỗi giúp gập các ngón tay là một trường hợp đặc biệt, vì chúng phải trượt qua một đường hầm hẹp (là bao gân duỗi cùng các ròng rọc) để thực hiện chức năng. Điều này tạo ra những thách thức lớn trong phẫu thuật gân duỗi, đồng thời dẫn đến hai cơ chế hồi phục khác nhau:

  • Hồi phục nội tại diễn ra nhờ các tế bào bản thân gân, được nuôi dưỡng bởi dịch khớp nằm trong bao gân và những nếp gấp mạch máu nhỏ gọi là vincula. Phần lớn các chất dinh dưỡng cho gân duỗi không đến từ mạch máu mà từ dịch khớp này; mỗi khi ngón tay gập lại và duỗi ra, dịch khớp sẽ được bơm vào gân (quá trình gọi là thấm dịch), giúp nuôi dưỡng gân đang hồi phục. Quá trình hồi phục nội tại giúp gân liền lại mà không dính vào các mô xung quanh, nhờ đó gân vẫn có thể trượt tự do.
  • Hồi phục ngoại tại xảy ra khi các tế bào và mô sẹo từ bên ngoài – từ bao gân và các mô lân cận – xâm nhập vào gân. Cơ chế này cũng giúp gân liền lại, nhưng lại khiến gân dính chặt vào bao gân, hình thành những mô dính ngăn cản gân trượt; kết quả là ngón tay không thể gập hoặc duỗi hoàn toàn.

Phương pháp phẫu thuật hiện đại kết hợp với việc vận động sớm có mục đích đẩy quá trình hồi phục hướng về cơ chế nội tại và hạn chế hình thành mô dính.

Đây là lúc “công việc gập ngón tay” phát huy vai trò: đó là lực mà cơ bắp phải tạo ra để thực sự gập ngón tay. Nó không chỉ là trọng lượng cần di chuyển, mà còn bao gồm lực cản trở sự trượt của gân: ma sát trong bao gân, kích thước của vết khâu phẫu thuật, tình trạng sưng nề sau mổ, cùng các ròng rọc gân chặt chẽ. Nếu “công việc gập ngón tay” (lực cản) vượt quá khả năng chịu đựng của vết khâu, vết khâu sẽ bong ra hoặc đứt, hoặc ngón tay hoàn toàn không thể cử động và mô dính sẽ hình thành. Vì vậy, bác sĩ phẫu thuật phải cân bằng giữa hai yêu cầu đối lập: vết khâu phải đủ chắc chắn để chịu được vận động sớm, nhưng cũng phải mỏng và trơn nhẵn để giảm thiểu lực cản khi gập ngón tay. Chỉ khi đáp ứng được cả hai điều kiện này, việc vận động sớm mới mang lại hiệu quả.

Tại sao khả năng chịu lực của vùng được vá chữa gân lại giảm đi trước khi tăng lên

Đây là điều trái ngược với trực giác nhưng lại giải thích toàn bộ quy trình phục hồi chức năng một cách thận trọng. Việc vá chữa gân không khiến gân ngày càng mạnh lên đều đặn từ ngày đầu tiên; thay vào đó, khả năng chịu lực tuân theo một đường cong hình chữ U:

  • Trong những ngày đầu, gần như toàn bộ sức mạnh chịu lực đến từ các chỉ khâu; chính mô gân hầu như không góp phần gì vào việc chịu lực.
  • Trong vòng một đến ba tuần đầu, hai đầu gân bị đứt thực chất lại mềm đi: cơ thể hấp thụ và tái cấu trúc collagen ngay tại vùng vá chữa trước khi collagen mới hình thành đầy đủ. Do đó, toàn bộ vùng vá chữa đạt mức yếu nhất vào những tuần đầu (thường là từ cuối tuần thứ nhất đến tuần thứ ba), dù vết thương trông đã lành và ngón tay dường như hoạt động bình thường.
  • Sau đó, sức mạnh mới tăng dần. Khi collagen loại III chưa được sắp xếp lại được thay thế bằng collagen loại I có cấu trúc thẳng hàng và liên kết chéo, vùng vá chữa dần hồi phục và vượt qua mức sức mạnh ban đầu; đến khoảng tuần thứ 12, khả năng chịu lực đã đủ để thực hiện hầu hết các hoạt động sinh hoạt hàng ngày, và quá trình hoàn thiện vẫn tiếp diễn trong nhiều tháng sau đó.

Chính sự suy giảm này là lý do phục hồi chức năng cho tay phải được thực hiện một cách rất cẩn trọng; việc “ngón tay cảm thấy bình thường sau ba tuần” thực chất là một cạm bẫy: đó thường là thời điểm vùng vá chữa dễ bị tổn thương nhất. Việc vận động nhẹ nhàng, có kiểm soát trong giai đoạn đầu giúp gân di chuyển trơn tru (thúc đẩy quá trình lành thương) và sắp xếp collagen mới một cách đúng hướng mà không gây quá tải lên vùng vá chữa đang ở mức yếu nhất về mặt sinh học. Những trường hợp gân bị đứt lại sau khi đã được vá chữa thường xảy ra vào những tuần đầu này, cũng vì lý do đó.

Quá trình lành thương giữa gân và xương: cơ vùng vai và mô gắn kết gân-xương

Tình trạng rách cơ vùng vai là một vấn đề hoàn toàn khác, bởi vì trong trường hợp này gân phải lành lại và gắn kết với xương. Điểm nối giữa gân và xương là một cấu trúc đặc biệt gọi là mô gắn kết gân-xương (enthesis).

Trong mô khỏe mạnh, mô gắn kết gân-xương có dạng sự chuyển tiếp dần dần, được chia thành khoảng bốn vùng: gân → sụn sợi chưa vôi hóa → sụn sợi đã vôi hóa → xương. Sự chuyển tiếp từ từ này từ gân mềm, dẻo dai sang xương cứng, cứng nhắc giúp phân tán lực tác động; nhờ đó gân mềm không bị xé rách khỏi xương cứng tại một ranh giới đột ngột.

Vấn đề là sau khi bị rách, cơ thể không thể tái tạo lại mô gắn kết gân-xương có cấu trúc dần dần như vậy. Vết thương sau khi phẫu thuật lành lại thành sẹo sợi mạch máu nằm giữa gân và xương; loại sẹo này về mặt cơ học yếu hơn so với cấu trúc gồm bốn vùng ban đầu. Đây là một trong những nguyên nhân khiến vết rách cơ vùng vai có thể tái phát dù phẫu thuật đã được thực hiện đúng kỹ thuật.

Cũng cần lưu ý rằng phần lớn các trường hợp rách cơ vùng vai mang tính thoái hóa chứ không hoàn toàn do chấn thương: theo tuổi tác, gân, đặc biệt là vùng “vùng nguy cơ cao” cách điểm nối giữa gân cơ supra-spinatus và xương khoảng một centimet, sẽ trở nên yếu đi, bị sờn rách và rách ra mà không cần tác động chấn thương nào (các trường hợp rách cơ thực sự rất hiếm gặp ở người dưới 50 tuổi). Phẫu thuật giúp gắn lại gân vào “vùng tiếp xúc” trên xương, nhưng bác sĩ phải đối mặt với tình trạng lão hóa và chất lượng mô kém. Vì vậy, quá trình lành thương cơ vùng vai chịu ảnh hưởng lớn bởi các yếu tố sinh học: ca phẫu thuật chỉ khôi phục lại cấu trúc giải phẫu; việc mô gắn kết gân-xương có lành thật sự hay không còn phụ thuộc vào chất lượng gân, kích thước vết rách, tuổi tác, thói quen hút thuốc, bệnh đái tháo đường và cách chăm sóc, vận động vùng phẫu thuật. Đây là trường hợp hoàn toàn đối lập với tình trạng đứt gân duỗi ở bàn tay trẻ tuổi: cùng loại mô, nhưng vấn đề lành thương lại hoàn toàn khác.

Những yếu tố hỗ trợ và cản trở quá trình hồi phục của gân

  • Tải trọng, được điều chỉnh đúng mức. Việc áp dụng tải trọng một cách có kiểm soát và tăng dần là công cụ hiệu quả nhất để tăng cường sức mạnh và sự ổn định của gân. Tải trọng quá mức ngay từ đầu có thể làm đứt gân vừa được phục hồi; ngược lại, tải trọng quá ít sẽ gây ra tình trạng cứng khớp và hình thành mô sẹo. Nghệ thuật của quá trình phục hồi nằm ở việc điều chỉnh mức tải trọng phù hợp.
  • Tuần hoàn máu. Những vùng gân có nguồn cung cấp máu tốt sẽ hồi phục nhanh hơn; còn những vùng thiếu máu (như một số khu vực của cơ chóp xoay) thì hồi phục chậm hơn nhiều.
  • Hút thuốc, bệnh đái tháo đường, việc sử dụng steroid và tuổi tác đều làm suy giảm khả năng hồi phục của gân, đồng thời làm tăng nguy cơ đứt gân hoặc tái đứt gân.
  • Thời gian. Gân là loại mô hồi phục chậm; để đạt được sức mạnh đáng kể cần mất vài tháng, còn quá trình tái cấu trúc hoàn toàn có thể kéo dài hơn một năm.

b. produce, reproduce, and Share Adapted Material for NonCommercial purposes only.

2. Exceptions and Limitations. For the avoidance of doubt, where Exceptions and Limitations apply to Your use, this Public License does not apply, and You do not need to comply with its terms and conditions.

3. Term. The term of this Public License is specified in Section 6(a).

4. Media and formats; technical modifications allowed. The Licensor authorizes You to exercise the Licensed Rights in all media and formats whether now known or hereafter created, and to make technical modifications necessary to do so. The Licensor waives and/or agrees not to assert any right or authority to forbid You from making technical modifications necessary to exercise the Licensed Rights, including technical modifications necessary to circumvent Effective Technological Measures. For purposes of this Public License, simply making modifications authorized by this Section 2(a) (4) never produces Adapted Material.

5. Downstream recipients.

a. Offer from the Licensor -- Licensed Material. Every recipient of the Licensed Material automatically receives an offer from the Licensor to exercise the Licensed Rights under the terms and conditions of this Public License.

b. No downstream restrictions. You may not offer or impose any additional or different terms or conditions on, or apply any Effective Technological Measures to, the Licensed Material if doing so restricts exercise of the Licensed Rights by any recipient of the Licensed Material.

6. No endorsement. b. Other rights.

1. Section 3 -- License Conditions.

Your exercise of the Licensed Rights is expressly made subject to the following conditions.

a. Attribution.

1. If You Share the Licensed Material (including in modified form), You must:

a. retain the following if it is supplied by the Licensor with the Licensed Material:

i. identification of the creator(s) of the Licensed Material and any others designated to receive attribution, in any reasonable manner requested by the Licensor (including by pseudonym if designated);

ii. a copyright notice;

iii. a notice that refers to this Public License;

iv. a notice that refers to the disclaimer of warranties;

v. a URI or hyperlink to the Licensed Material to the extent reasonably practicable;

b. indicate if You modified the Licensed Material and retain an indication of any previous modifications; and

c. indicate the Licensed Material is licensed under this Public License, and include the text of, or the URI or hyperlink to, this Public License.

2. You may satisfy the conditions in Section 3(a)(1) in any reasonable manner based on the medium, means, and context in which You Share the Licensed Material. 3. If requested by the Licensor, You must remove any of the information required by Section 3(a)(1)(A) to the extent reasonably practicable.

Section 4 -- Sui Generis Database Rights.

Where the Licensed Rights include Sui Generis Database Rights that apply to Your use of the Licensed Material:

a. for the avoidance of doubt, Section 2(a)(1) grants You the right to extract, reuse, reproduce, and Share all or a substantial portion of the contents of the database for NonCommercial purposes only;

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

Section 5 -- Disclaimer of Warranties and Limitation of Liability.

a. UNLESS OTHERWISE SEPARATELY UNDERTAKEN BY THE LICENSOR, TO THE EXTENT POSSIBLE, THE LICENSOR OFFERS THE LICENSED MATERIAL AS-IS AND AS-AVAILABLE, AND MAKES NO REPRESENTATIONS OR WARRANTIES OF ANY KIND CONCERNING THE LICENSED MATERIAL, WHETHER EXPRESS, IMPLIED, STATUTORY, OR OTHER. THIS INCLUDES, WITHOUT LIMITATION, WARRANTIES OF TITLE, MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT, ABSENCE OF LATENT OR OTHER DEFECTS, ACCURACY, OR THE PRESENCE OR ABSENCE OF ERRORS, WHETHER OR NOT KNOWN OR DISCOVERABLE. WHERE DISCLAIMERS OF WARRANTIES ARE NOT ALLOWED IN FULL OR IN PART, THIS DISCLAIMER MAY NOT APPLY TO YOU.

b. TO THE EXTENT POSSIBLE, IN NO EVENT WILL THE LICENSOR BE LIABLE TO YOU ON ANY LEGAL THEORY (INCLUDING, WITHOUT LIMITATION, NEGLIGENCE) OR OTHERWISE FOR ANY DIRECT, SPECIAL, INDIRECT, INCIDENTAL, CONSEQUENTIAL, PUNITIVE, EXEMPLARY, OR OTHER LOSSES, COSTS, EXPENSES, OR DAMAGES ARISING OUT OF THIS PUBLIC LICENSE OR USE OF THE LICENSED MATERIAL, EVEN IF THE LICENSOR HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH LOSSES, COSTS, EXPENSES, OR DAMAGES. WHERE A LIMITATION OF LIABILITY IS NOT ALLOWED IN FULL OR IN PART, THIS LIMITATION MAY NOT APPLY TO YOU.

c. The disclaimer of warranties and limitation of liability provided above shall be interpreted in a manner that, to the extent possible, most closely approximates an absolute disclaimer and waiver of all liability.

Section 6 -- Term and Termination.

a. This Public License applies for the term of the Copyright and Similar Rights licensed here. However, if You fail to comply with this Public License, then Your rights under this Public License terminate automatically.

b. Where Your right to use the Licensed Material has terminated under Section 6(a), it reinstates:

1. automatically as of the date the violation is cured, provided it is cured within 30 days of Your discovery of the violation; or

2. upon express reinstatement by the Licensor.

For the avoidance of doubt, this Section 6(b) does not affect any right the Licensor may have to seek remedies for Your violations of this Public License.

c. For the avoidance of doubt, the Licensor may also offer the Licensed Material under separate terms or conditions or stop distributing the Licensed Material at any time; however, doing so will not terminate this Public License.

d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.

d. Nothing in this Public License constitutes or may be interpreted as a limitation upon, or waiver of, any privileges and immunities that apply to the Licensor or You, including from the legal processes of any jurisdiction or authority.
