Những triệu chứng mà bạn đang gặp phải

Hội chứng chèn ép vùng cổ ngực thường biểu hiện qua tình trạng cánh tay có vẻ “không ổn”. Bạn có thể cảm thấy đau ở vùng vai, bên cổ và dọc theo cánh tay; kèm theo tê, kiến bò hoặc cảm giác ngứa ran, thường xuất hiện ở ngón áp út và ngón út. Cánh tay có thể cảm thấy nặng nề, yếu ớt hoặc vụng về, khiến bạn dễ làm rơi đồ vật hoặc mất sức nhanh khi cầm nắm. Một dấu hiệu rất điển hình là các triệu chứng trở nên tồi tệ hơn khi cánh tay được giơ lên cao hoặc vươn ra phía trên (ví dụ như khi phơi quần áo, với tay lấy đồ trên kệ cao, lái xe hoặc đeo túi lên vai), rồi giảm dần khi bạn hạ tay xuống. Một số người còn bị đau đầu ở vùng sau đầu nữa.

Ít gặp hơn, vấn đề lại liên quan đến các mạch máu thay vì dây thần kinh. Nếu tĩnh mạch bị chèn ép, toàn bộ cánh tay có thể đột ngột sưng lên, cảm thấy nặng nề và có sắc xanh tím. Nếu động mạch bị chèn ép (hiếm gặp), bàn tay sẽ trở nên nhợt nhạt, lạnh và đau đớn. Những biểu hiện liên quan đến mạch máu này rất quan trọng vì cần được khám và xử lý ngay lập tức (xem mục cuối cùng).

Điều gì đang thực sự xảy ra

Để đi từ cổ xuống tay, các dây thần kinh và mạch máu phải đi qua một khoảng hở hẹp ở vùng gốc cổ, giữa xương đòn, xương sườn thứ nhất và các cơ thang ở cổ. Khoảng hở này được gọi là “lỗ thoát ngực”. Khi khoảng hở này bị hẹp lại, các cấu trúc này bị chèn ép; chính áp lực này gây ra các triệu chứng lâm sàng.

Có ba dạng bệnh, tùy thuộc vào thành phần nào bị chèn ép. TOS dạng thần kinh (áp lực lên các dây thần kinh, đám rối thần kinh cánh tay) là dạng phổ biến nhất, chiếm đa số các ca bệnh; biểu hiện là đau, tê, cảm giác kiến bò và yếu cơ. TOS dạng tĩnh mạch liên quan đến tĩnh mạch chính, gây ra tình trạng cánh tay sưng phù, nặng nề và có màu xanh tím; đôi khi còn xuất hiện cục máu đông. TOS dạng động mạch rất hiếm gặp, ảnh hưởng đến động mạch, khiến bàn tay trở nên nhợt nhạt và lạnh.

Một vài yếu tố khiến khoảng hở này bị hẹp hơn. Một số người sinh ra đã có xương sườn thừa ở cổ (xương sườn cổ) hoặc một dải sợi liên kết chặt chẽ. Tư thế cũng đóng vai trò quan trọng: vai cúi xuống làm thu hẹp khoảng hở; các cơ cổ và ngực quá căng hoặc dày cũng gây ra tình trạng tương tự. Vì vậy, bệnh hay gặp ở những người phải thực hiện các động tác vung tay lên cao lặp đi lặp lại hoặc vận động viên. Chấn thương vùng cổ hoặc xương đòn trước đó cũng có thể là nguyên nhân khởi phát.

Những gì chúng tôi có thể làm

Tại Bệnh viện tư nhân Mater Rockhampton, Bác sĩ Kieran Hirpara là người dẫn dắt phương pháp điều trị dành cho tình trạng này. Thông thường, bệnh nhân được bác sĩ đa khoa giới thiệu đến phòng khám của chúng tôi; nếu vật lý trị liệu viên khuyên bạn nên đến gặp chúng tôi, bạn vẫn cần có giấy giới thiệu từ bác sĩ đa khoa để được hưởng mức hoàn trả từ Medicare, đồng thời chúng tôi sẽ tiến hành đánh giá kỹ lưỡng nhằm xác nhận chẩn đoán. Đối với những triệu chứng kéo dài, chúng tôi thường bắt đầu bằng các biện pháp điều trị không phẫu thuật và chỉ cân nhắc phẫu thuật nếu các phương pháp đó không mang lại hiệu quả đủ rõ rệt.

Tin vui là đối với dạng TOS do nguyên nhân thần kinh phổ biến, phương pháp điều trị ưu tiên hàng đầu là vật lý trị liệu chứ không phải phẫu thuật. Một chương trình trị liệu có mục tiêu cụ thể nhằm mở rộng vùng lối ra của các cấu trúc thần kinh – mạch máu sẽ mang lại hiệu quả lớn: giúp chỉnh sửa tư thế, tăng cường sức mạnh cho các cơ giữ xương bả vai ở vị trí thích hợp, giãn cơ vùng cổ và ngực bị căng cứng, đồng thời giúp người bệnh tránh những tư thế gây chèn ép. Phương pháp này giúp giảm triệu chứng ở hầu hết bệnh nhân; vì vậy bạn nên kiên trì thực hiện trong vài tháng để đạt kết quả tốt nhất.

Phẫu thuật chỉ được chỉ định trong hai trường hợp. Thứ nhất là các dạng TOS do nguyên nhân mạch máu (tĩnh mạch hoặc động mạch), khi đó việc giải phóng chèn ép là cần thiết và là phương pháp điều trị chính. Thứ hai là trường hợp TOS do nguyên nhân thần kinh mà các liệu pháp vật lý trị liệu đúng cách vẫn không giúp cải thiện tình trạng, gây ảnh hưởng nghiêm trọng đến sinh hoạt hàng ngày. Ca phẫu thuật nhằm mục đích giải phóng chèn ép bằng cách cắt bỏ xương sườn thứ nhất (và bất kỳ xương sườn cổ thừa nào), đồng thời giãn cơ thang – giúp các dây thần kinh và mạch máu có đủ không gian hoạt động. Phẫu thuật có thể thực hiện qua nách, phía trên xương đòn, hoặc kết hợp với kỹ thuật nội soi. Đối với một số bệnh nhân nhất định, phương pháp này mang lại hiệu quả rất tốt.

Những điều có thể xảy ra

Chứng TOS là một chẩn đoán khá phức tạp; không có xét nghiệm nào có thể xác nhận chẩn đoán này một cách chắc chắn. Chẩn đoán chủ yếu dựa trên lâm sàng: dựa vào tiền sử bệnh của bạn cùng các bài kiểm tra trong đó cánh tay được đặt vào những tư thế có thể gây kích phát triệu chứng. Các phương pháp chẩn đoán hình ảnh và nghiên cứu dây thần kinh thường được sử dụng nhiều hơn để loại trừ các nguyên nhân phổ biến hơn (chẳng hạn hội chứng ống cổ tay, hội chứng ống khuỷu tay, hoặc các vấn đề ở cổ/đĩa đệm) chứ không chỉ để xác nhận chứng TOS. Do đó, một phần của quá trình chẩn đoán là phải kiên nhẫn xác định rõ nguyên nhân thực sự gây ra các triệu chứng của bạn; đôi khi kết quả cũng không hề rõ ràng.

Đối với dạng thần kinh, hầu hết bệnh nhân đều cải thiện tình trạng sau khi điều chỉnh tư thế và tập vật lý trị liệu; họ thậm chí không cần phẫu thuật. Tuy nhiên, quá trình này có thể kéo dài nhiều tháng và đòi hỏi bạn phải duy trì các thói quen đã học. Trong trường hợp các dạng mạch máu hoặc những ca thần kinh khó điều trị phải tiến hành phẫu thuật, việc giải ép thường giúp giảm triệu chứng ở hầu hết các bệnh nhân được chọn lọc kỹ lưỡng; thời gian hồi phục thường mất vài tuần. Vì chứng TOS xảy ra ở vị trí gần nguồn gốc nhất của đường đi dây thần kinh (cao hơn so với vùng ống cổ tay hay ống khuỷu tay), đôi khi nó chỉ là một phần của tổng thể vấn đề; vì vậy kế hoạch điều trị sẽ được thiết kế riêng cho từng bệnh nhân.

Khi nào nên đi khám bác sĩ

  • Cánh tay bị sưng đột ngột, nặng nề hoặc có màu xanh tím: điều này có thể là dấu hiệu tắc nghẽn hoặc huyết khối trong tĩnh mạch, cần được đánh giá ngay lập tức, trong cùng ngày.
  • Bàn tay hoặc các ngón tay nhợt nhạt, lạnh, đau, hoặc thay đổi màu sắc: có thể là dấu hiệu của vấn đề về động mạch; cần tìm kiếm sự chăm sóc khẩn cấp.
  • Tê, cảm giác kiến bò, yếu sức hoặc vụng về ở cánh tay hoặc bàn tay mà tình trạng này kéo dài, ngày càng nặng hơn, hoặc ảnh hưởng đến công việc, giấc ngủ và các hoạt động hàng ngày.
  • Teo cơ (mất khối lượng cơ) ở bàn tay, hoặc các triệu chứng tái phát nhiều lần dù đã điều trị vật lý trị liệu đầy đủ: nên tham khảo ý kiến của bác sĩ chuyên khoa.
  • Nếu bạn được chẩn đoán có xương sườn cổ và đang gặp các triệu chứng ở cánh tay, nên đi khám để được đánh giá.

Hội chứng chèn ép vùng cổ ngực rất đáng để tìm hiểu thêm, vì nó bao gồm nhiều dạng bệnh khác nhau; một dạng có thể chứng minh được một cách khách quan và đôi khi gây nguy hiểm đến chi, còn dạng kia chủ yếu được chẩn đoán dựa trên đánh giá lâm sàng. Hầu hết các tranh luận trong y học đều xoay quanh dạng thứ hai.

Ba tình trạng y khoa có chung một tên gọi

Lỗ thoát ngực là khoảng không hẹp nằm giữa xương đòn và xương sườn thứ nhất; nơi các dây thần kinh chi phối cánh tay, động mạch chủ và tĩnh mạch chính đi qua. Sự chèn ép tại vùng này gây ra ba hội chứng khác nhau.

Hội chứng TOS dạng tĩnh mạch và động mạch là những rối loạn về mạch máu. Chúng ít gặp, có thể được chứng minh một cách khách quan qua các phương pháp chẩn đoán hình ảnh; đồng thời có thể gây nguy hiểm cấp tính: tắc tĩnh mạch dưới đòn dẫn đến tình trạng cánh tay sưng phù và có màu xanh tím; hoặc sự chèn ép động mạch gây ra cảm giác lạnh ở tay, thậm chí có thể dẫn đến tình trạng thuyên tắc mạch. Không ai phủ nhận sự tồn tại của các hội chứng này cũng như việc cần phải điều trị chúng.

Hội chứng TOS dạng thần kinh chiếm tỷ lệ phần lớn các ca bệnh. Trong trường hợp này, các dây thần kinh bị chèn ép gây đau nhức ở vùng vai và cánh tay, tê bì thường xuất hiện ở phía ngón út; các triệu chứng này càng trở nên trầm trọng hơn khi người bệnh giơ tay lên cao. Vấn đề là ở chỗ hầu hết các trường hợp không có xét nghiệm nào có thể xác nhận chẩn đoán: kết quả đo dẫn truyền thần kinh thường vẫn bình thường, còn các phương pháp chẩn đoán hình ảnh cũng không phát hiện được tổn thương nào.

Các tài liệu y khoa hiện đại giải quyết vấn đề này bằng cách phân biệt giữa các dạng TOS có liên quan đến mạch máu và các dạng có thể xác nhận được một cách khách quan với các dạng còn lại [1]; đây là cách tiếp cận thận trọng nhằm thừa nhận rằng các dạng còn lại chủ yếu được chẩn đoán dựa trên sự đánh giá lâm sàng.

Tại sao sự phân biệt này lại quyết định mọi thứ

Trong những trường hợp có thể chẩn đoán một cách khách quan, các quyết định điều trị tuân theo logic thông thường. Ngược lại, trong những trường hợp không thể chẩn đoán được, sẽ xuất hiện hai dạng sai sót: những người thực sự bị chèn ép dây thần kinh phải chờ đợi nhiều năm mà không được chẩn đoán do kết quả xét nghiệm vẫn bình thường; còn những người bị đau cánh tay do nguyên nhân khác lại phải trải qua một ca phẫu thuật phức tạp mà không mang lại hiệu quả gì.

Cả hai sai sót này đều đã được nhận thức rõ, vì vậy tiêu chuẩn chứng cứ cần thiết trước khi tiến hành phẫu thuật ở đây cao hơn so với hầu hết các trường hợp chèn ép dây thần kinh. Đồng thời, trước khi thực hiện bất kỳ ca phẫu thuật nào đối với dạng bệnh do tổn thương thần kinh, người ta phải tiến hành liệu pháp vật lý trị liệu trong thời gian dài, đồng thời điều chỉnh tư thế, các cơ scalene và cơ chế vận động của vùng vai.

Những gì được loại bỏ và các bằng chứng liên quan

Phẫu thuật giúp giảm áp lực bằng cách cắt bỏ xương sườn thứ nhất, cắt đứt các cơ thang hoặc kết hợp cả hai phương pháp.

Khi so sánh các phương pháp trên 2.010 bệnh nhân, các chỉ số Disabilities of the Arm, Shoulder and Hand và Derkash cho thấy kết quả tốt hơn đáng kể sau khi thực hiện phẫu thuật cắt cơ thang mà không cắt xương sườn; ngược lại, tỷ lệ biến chứng cao hơn được ghi nhận sau khi cắt xương sườn thứ nhất [2].

Tuy nhiên, nhiều nghiên cứu trước đây lại ủng hộ việc thực hiện phẫu thuật kết hợp. Trên 532 bệnh nhân, phương pháp kết hợp cắt xương sườn thứ nhất qua đường nách và cắt cơ thang qua đường cổ được đánh giá là cách giảm áp lực triệt để nhất, giúp cải thiện triệu chứng tốt hơn và giảm tỷ lệ tái phát [3]; tỷ lệ tái phát khi áp dụng phương pháp này làm phẫu thuật đầu tiên chỉ ở mức 5% đến 10% [4].

Dữ liệu hiện có chưa thể hòa giải hoàn toàn hai quan điểm trên. Phân tích so sánh gần đây ủng hộ việc thực hiện ít thao tác phẫu thuật hơn; trong khi kinh nghiệm lâm sàng của một bác sĩ phẫu thuật lại ủng hộ việc thực hiện nhiều thao tác hơn, với lập luận rằng việc giảm áp lực không triệt để mới là nguyên nhân dẫn đến tái phát. Điểm chung giữa hai quan điểm là tình trạng tái phát hội chứng chèn ép mạch máu và dây thần kinh vùng cổ tay – cẳng tay là vấn đề khó giải quyết hơn nhiều so với lần mắc bệnh đầu tiên; vì vậy việc đưa ra quyết định đúng đắn ngay từ đầu còn quan trọng hơn cả việc lựa chọn kỹ thuật phẫu thuật.

Ý nghĩa thực tiễn

Với một chẩn đoán thường mang tính lâm sàng, một ca phẫu thuật mà phạm vi can thiệp tối ưu vẫn còn gây tranh cãi, cùng vấn đề tái phát khó kiểm soát, đây là trường hợp mà việc tham khảo ý kiến thứ hai trước khi phẫu thuật là điều hợp lý chứ không nên coi là sự đối đầu; đặc biệt là đối với dạng bệnh do tổn thương thần kinh mà chưa có bằng chứng xác nhận khách quan.

In all other cases the Licensor expressly reserves any right to collect such royalties, including when the Licensed Material is used other than for NonCommercial purposes.

Section 3 -- License Conditions.

Your exercise of the Licensed Rights is expressly made subject to the following conditions.

a. Attribution.

1. If You Share the Licensed Material (including in modified form), You must:

a. retain the following if it is supplied by the Licensor with the Licensed Material:

i. identification of the creator(s) of the Licensed Material and any others designated to receive attribution, in any reasonable manner requested by the Licensor (including by pseudonym if designated);

ii. a copyright notice;

iii. a notice that refers to this Public License;

iv. a notice that refers to the disclaimer of warranties;

v. a URI or hyperlink to the Licensed Material to the extent reasonably practicable;

b. indicate if You modified the Licensed Material and retain an indication of any previous modifications; and

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

c. The disclaimer of warranties and limitation of liability provided above shall be interpreted in a manner that, to the extent possible, most closely approximates an absolute disclaimer and waiver of all liability.

Section 6 -- Term and Termination.

a. This Public License applies for the term of the Copyright and Similar Rights licensed here. However, if You fail to comply with this Public License, then Your rights under this Public License terminate automatically.

b. Where Your right to use the Licensed Material has terminated under Section 6(a), it reinstates:

1. automatically as of the date the violation is cured, provided it is cured within 30 days of Your discovery of the violation; or

2. upon express reinstatement by the Licensor.

For the avoidance of doubt, this Section 6(b) does not affect any right the Licensor may have to seek remedies for Your violations of this Public License.

c. For the avoidance of doubt, the Licensor may also offer the Licensed Material under separate terms or conditions or stop distributing the Licensed Material at any time; however, doing so will not terminate this Public License.

d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.

d. Nothing in this Public License constitutes or may be interpreted as a limitation upon, or waiver of, any privileges and immunities that apply to the Licensor or You, including from the legal processes of any jurisdiction or authority.
