Bệnh gút thường khởi phát đột ngột vào ban đêm. Một khớp (đôi khi là vùng gốc ngón tay cái, khớp ngón tay, cổ tay hoặc mỏm khuỷu tay) đột nhiên trở nên đau dữ dội chỉ trong vài giờ. Khớp đó có vẻ nóng, đỏ và sưng phù; thậm chí chỉ cần chạm nhẹ vào cũng gây đau đớn dữ dội, đến mức ngay cả trọng lượng của tấm chăn cũng khiến người bệnh cảm thấy không chịu nổi. Đây là cơn bùng phát bệnh, và lần đầu tiên thường xuất hiện ở bàn chân; tuy nhiên bàn tay và cổ tay cũng là những vị trí phổ biến.

Điều gì đang thực sự xảy ra

Bệnh gút là một vấn đề ảnh hưởng toàn thân, biểu hiện rõ nhất ở các khớp. Cơ thể chúng ta sản xuất ra một chất thải gọi là axit uric (urate). Khi nồng độ axit uric trong máu quá cao, nó có thể tạo thành những tinh thể nhỏ, sắc nhọn và lắng đọng trong các khớp cũng như các mô mềm xung quanh, bao gồm cả bàn tay và cổ tay.

Hệ miễn dịch coi những tinh thể này là những tác nhân xâm nhập và tấn công chúng; phản ứng này gây ra tình trạng viêm đột ngột, dữ dội mà chúng ta cảm nhận được qua các cơn đau gút. Nếu nồng độ urate trong máu vẫn cao mà không được kiểm soát trong nhiều năm, các tinh thể sẽ tích tụ lại thành những khối cứng (gọi là tophi). Theo thời gian, những khối này có thể làm mòn xương, tổn thương gân và thỉnh thoảng gây chèn ép dây thần kinh. Kiểm soát được nồng độ urate là cách kiểm soát được bệnh gút.

Những gì chúng tôi có thể làm

Việc điều trị gồm hai phần riêng biệt, và cả hai đều rất quan trọng.

Kiểm soát cơn đau cấp tính. Cơn đau cấp tính có thể được làm dịu nhờ thuốc chống viêm, thường là thuốc viên chống viêm không steroid (NSAID), colchicine, hoặc liệu trình dùng steroid ngắn ngày. Những loại thuốc này phát huy hiệu quả tốt nhất khi được dùng sớm; vì vậy bạn nên cùng bác sĩ lập kế hoạch điều trị trước khi cơn đau tiếp theo xảy ra.

Hạ nồng độ axit uric lâu dài. Đây mới là phương pháp chữa khỏi bệnh gút về lâu dài. Việc dùng thuốc viên hàng ngày, phổ biến nhất là allopurinol, sẽ từ từ hạ nồng độ axit uric trong máu xuống dưới mức mục tiêu. Khi duy trì mức này đủ lâu, các tinh thể axit uric sẽ dần tan biến, các cơn bùng phát chấm dứt và các khối tophi cũng teo nhỏ lại. Chế độ ăn uống và lối sống cũng có tác động tích cực (giảm bia và rượu mạnh, hạn chế đồ uống có đường, duy trì cân nặng hợp lý, uống đủ nước); tuy nhiên đối với hầu hết mọi người, việc dùng thuốc hàng ngày vẫn là yếu tố quyết định.

Phẫu thuật chỉ được chỉ định trong những trường hợp hiếm gặp, chẳng hạn như khi có khối tophi lớn xuyên qua da, gây cản trở hoạt động của gân hoặc chèn ép dây thần kinh (ví dụ gây ra các triệu chứng hội chứng ống cổ tay). Ngay cả trong những trường hợp đó, việc tiếp tục điều trị y tế nhằm hạ nồng độ axit uric vẫn là điều bắt buộc.

Những điều bạn có thể mong đợi

Bệnh gút là một trong số ít các dạng viêm khớp mà chúng ta hoàn toàn có thể kiểm soát được. Nếu nồng độ axit uric luôn được duy trì dưới mức mục tiêu trong thời gian dài, các cơn bùng phát sẽ trở nên hiếm gặp rồi chấm dứt; những khối cứng hiện có cũng dần tan đi. Quá trình này diễn ra chậm: mất vài tháng đến vài năm. Bạn thường phải dùng thuốc hạ axit uric suốt đời, vì việc ngưng thuốc sẽ khiến các tinh thể axit uric tái hình thành.

Cái giá của việc chờ đợi là có thật: tophi tích tụ ở bàn tay càng nhiều thì càng hạn chế những gì bàn tay có thể làm, vì vậy hạ nồng độ urate sớm sẽ bảo vệ sức nắm và chức năng bàn tay của bạn. Những người kiên trì điều trị thường đạt kết quả rất tốt.

Khi nào nên đi khám bác sĩ

  • Khớp bị nóng, sưng và đau lần đầu tiên: hãy đi khám bác sĩ ngay lập tức. Nhiễm trùng khớp có thể có biểu hiện giống hệt bệnh gút và là tình trạng cấp cứu; vì vậy cần phân biệt hai trường hợp này.
  • Các cơn đau tái phát hoặc xuất hiện ngày càng thường xuyên: đây là dấu hiệu cần bắt đầu điều trị làm giảm nồng độ axit uric.
  • Các khối cứng xuất hiện quanh khớp, hoặc một khối cứng vỡ ra ngoài da hoặc rỉ ra chất giống bột vôi.
  • Tê, cảm giác kiến bò hoặc yếu ở tay: có thể do các chất lắng đọng đang chèn ép dây thần kinh; cần được đánh giá kỹ.

Bệnh gút thực chất là gì: các tinh thể urat

Axit uric là sản phẩm thải tự nhiên được tạo ra khi cơ thể phân hủy các chất gọi là purine (có trong tế bào và một số loại thực phẩm). Khi nồng độ urat trong máu duy trì ở mức cao, nó sẽ kết tủa và hình thành những tinh thể nhỏ dạng kim gọi là tinh thể natri urat đơn (MSU) trong và quanh các khớp. Các tinh thể này dễ hình thành hơn ở những nơi có nhiệt độ thấp; đó là một phần lớn lý do khiến ngón chân cái – khớp xa nhất và có nhiệt độ thấp nhất trên cơ thể – là vị trí bị ảnh hưởng đầu tiên điển hình.

Đây là một tình trạng gây tổn thương thực sự ở khớp, chứ không chỉ gây đau đớn đơn thuần. Dựa trên dữ liệu từ 5,478 bệnh nhân, viêm khớp cấp tính ở khớp bàn-ngón chân thứ nhất là tình trạng rất phổ biến trong bệnh gút; nó gây ảnh hưởng đáng kể đến mức độ đau đớn và khả năng vận động, đồng thời có tác động rõ rệt lên cấu trúc cũng như chức năng của khớp đó [1].

Cơ chế inflammasome: Tại sao cơn viêm cấp tính lại xuất hiện đột ngột và dữ dội?

Cơn viêm bắt đầu khi các tế bào thực bào miễn dịch (đại thực bào) thực bào (nuốt) các tinh thể MSU. Những tinh thể này kích hoạt một phức hợp cảnh báo nội tại gọi là inflammasome NLRP3, từ đó kích hoạt enzyme caspase-1; enzyme này giải phóng chất trung gian gây viêm mạnh mẽ là interleukin-1β (IL-1β). IL-1β làm gia tăng tình trạng viêm tại khớp, dẫn đến hiện tượng đỏ, nóng, sưng và đau dữ dội chỉ trong vài giờ. Sau vài ngày, phản ứng viêm này tự hết dần.

Cơ chế này giúp giải thích một điều quan trọng về mặt lâm sàng: cơn viêm là phản ứng miễn dịch đối với các tinh thể, chứ không phải do chính các tinh thể gây ra. Việc kiểm soát cơn viêm và loại bỏ các tinh thể là hai vấn đề riêng biệt, đòi hỏi các loại thuốc khác nhau và được xử lý ở các giai đoạn thời gian khác nhau.

Con số quyết định mọi thứ

Việc điều trị lâu dài có một mục tiêu có thể đo lường được: đưa nồng độ axit uric trong huyết thanh xuống dưới mức mà các tinh thể có thể hình thành, và duy trì ở mức đó. Theo thực hành quốc tế, mục tiêu là giữ nồng độ axit uric dưới 0.36 mmol/L (6 mg/dL); đối với những bệnh nhân có hạt tophi thì mục tiêu là dưới 0.30 mmol/L (5 mg/dL). Ở những mức này, các tinh thể hiện có sẽ tan chứ không chỉ ngừng hình thành thêm nữa.

Các bằng chứng y khoa ủng hộ việc điều trị nhằm đạt được mục tiêu này, thay vì chỉ kê đơn liều cố định rồi hy vọng kết quả tốt. Trong một thử nghiệm lâm sàng ngẫu nhiên có 183 người tham gia, việc tăng dần liều allopurinol hàng tháng cho đến khi đạt mục tiêu đã giúp giảm đáng kể tỷ lệ người bị cơn gút tái phát cũng như kích thước trung bình của các hạt tophi trong vòng 24 tháng; phương pháp này cũng được dung nạp tốt [2].

Nguyên tắc tương tự cũng được áp dụng ở cấp độ dịch vụ y tế: một thử nghiệm ngẫu nhiên tại Vương quốc Anh về mô hình chăm sóc do điều dưỡng viên thực hiện, kết hợp giáo dục bệnh nhân và chiến lược điều trị nhằm đạt mục tiêu nồng độ axit uric, đã cho kết quả kiểm soát axit uric tốt hơn hẳn so với phương pháp chăm sóc thông thường; số cơn gút giảm đi và các hạt tophi tan biến nhiều hơn sau hai năm [3].

Từ đó suy ra hai điều: Liều allopurinol chưa từng được điều chỉnh dựa trên kết quả xét nghiệm máu thì chưa thể coi là phương pháp điều trị thực sự; và câu nói “bệnh gút của tôi đã được kiểm soát” thực chất là ám chỉ một con số cụ thể, chứ không phải việc trong tháng này không có cơn gút nào xảy ra.

Tại sao việc điều trị thường bị ngưng trong những tháng đầu

Việc bắt đầu điều trị làm giảm nồng độ urat có thể gây ra các cơn đau. Khi các tinh thể urat tan ra, chúng giải phóng các chất mà inflammasome phản ứng lại; vì vậy những tháng đầu điều trị có thể khiến tình trạng bệnh trở nên tồi tệ hơn so với trước đó. Đây là lý do phổ biến nhất khiến người bệnh ngưng điều trị và cho rằng phương pháp này không hiệu quả, trong khi thực tế các cơn đau này lại là dấu hiệu cho thấy điều trị đang phát huy tác dụng. Vì vậy, liều điều trị thường được bắt đầu ở mức thấp, tăng dần theo thời gian, và thường đi kèm với thuốc chống viêm dự phòng trong những tháng đầu.

Hai điều mà bác sĩ phẫu thuật chi trên cần chú ý

Bệnh gút không chỉ xuất hiện ở bàn chân; ở cánh tay, nó có thể giống với các bệnh lý cần phẫu thuật. Đã có các trường hợp ghi nhận về viêm màng gân do gút tại chỗ bám xa của gân cơ nhị đầu, gây rách gân một phần, biểu hiện giống như một ca đứt gân cơ nhị đầu đơn thuần ở người mà tiền sử bệnh gút hóa ra mới là lời giải thích [4]. Tình trạng này hiếm gặp, nhưng đây là loại chẩn đoán chỉ được đưa ra bởi người đã nghĩ đến nó.

May mắn thay, một nỗi lo phổ biến lại không xảy ra. Dựa trên dữ liệu từ 684,964 người tham gia nghiên cứu, bệnh gút không làm tăng nguy cơ gãy xương; đồng thời các thuốc hạ nồng độ axit uric được sử dụng sớm cũng không gây ra tác dụng có hại hay có lợi nào đối với nguy cơ gãy xương [5].

3. If requested by the Licensor, You must remove any of the information required by Section 3(a)(1)(A) to the extent reasonably practicable.
