Chuẩn bị cho ca phẫu thuật

Những việc cần chuẩn bị trong vài tuần, vài ngày trước khi phẫu thuật chi trên. Email gửi đến bạn chứa các chi tiết về ca phẫu thuật, quy trình đăng ký nhập viện trực tuyến, những loại thuốc cần ngưng dùng và những loại vẫn tiếp tục sử dụng, cách chuẩn bị cơ thể và môi trường sống tại nhà; đồng thời là những yếu tố thường gây ra việc hoãn ca phẫu thuật.

Phần lớn các yếu tố giúp ca phẫu thuật diễn ra suôn sẻ đã được quyết định trước khi bạn đến bệnh viện. Trang này sẽ đề cập đến những tuần và ngày dẫn đến ca phẫu thuật: những tài liệu chúng tôi gửi cho bạn, những gì chúng tôi cần từ bạn, những việc bạn có thể tự làm, và những thứ cần chuẩn bị sẵn tại nhà. Riêng ngày phẫu thuật – từ lúc làm thủ tục nhập viện cho đến lúc về nhà – sẽ được mô tả chi tiết trên trang Ngày phẫu thuật.

Buổi khám trước phẫu thuật

Trước khi phẫu thuật, bạn sẽ gặp chúng tôi để thảo luận về kế hoạch điều trị. Đây là thời điểm thích hợp nhất để đặt câu hỏi; vì vậy hãy ghi chú chúng ra trước. Một vài câu hỏi hữu ích là: chính xác những gì sẽ được thực hiện, các phương án thay thế là gì, quá trình hồi phục diễn ra như thế nào từng tuần, khi nào bạn có thể lái xe và đi làm, cũng như những rủi ro có thể xảy ra.

Hãy mang theo:

  • Danh sách chi tiết tất cả các loại thuốc bạn đang dùng, kèm liều lượng: thuốc kê đơn, thuốc không kê đơn, thuốc xịt, kem bôi, vitamin, dầu cá, nghệ và bất kỳ loại thực phẩm chức năng nào khác. Hãy ghi chép ra giấy hoặc mang theo vỏ hộp thuốc.
  • Các kết quả chụp chiếu, X-quang hoặc xét nghiệm máu gần đây mà bạn đã nhận được.
  • Thông tin về các bác sĩ khác đang tham gia điều trị cho bạn, đặc biệt là bác sĩ tim mạch hoặc chuyên gia về bệnh tiểu đường.
  • Thông tin về quỹ bảo hiểm y tế của bạn; nếu muốn, hãy mời một người thân đi cùng để hỗ trợ.

Bạn cũng sẽ được yêu cầu ký vào mẫu đồng thuận điều trị. Việc đồng thuận là một cuộc trao đổi, chứ không chỉ là việc ký tên. Nếu có điều gì chưa rõ, hãy nói ra; chúng tôi thà giải thích lại hai lần còn hơn để bạn ký vào văn bản mà mình chưa hiểu rõ.

Email mà chúng tôi gửi cho bạn

Sau khi ca phẫu thuật của bạn được lên lịch, phòng khám sẽ gửi email chứa thông tin chi tiết về ca phẫu thuật. Đây là tài liệu quan trọng nhất mà bạn sẽ nhận được; vì vậy hãy đọc kỹ và lưu giữ nó. Tài liệu này nêu rõ:

  • Bệnh viện nào, ngày nào và giờ nhập viện, cũng như địa điểm cụ thể trong bệnh viện mà bạn cần đến khi đến nơi
  • Thời gian nhịn ăn: thời điểm sau đó bạn không được ăn hay uống bất cứ thứ gì. Nếu ca phẫu thuật chỉ sử dụng gây tê tại chỗ, email sẽ ghi rõ bạn không cần phải nhịn ăn
  • Tên bác sĩ gây mê và cách liên hệ với họ nếu bạn có thắc mắc về phương pháp gây mê hoặc các loại thuốc đang dùng; đối với bệnh nhân tư nhân, email cũng hướng dẫn cách hỏi về chi phí gây mê
  • Việc bạn có phải ở lại bệnh viện qua đêm hay về nhà ngay trong ngày; nếu về nhà ngay, bạn cần có người đưa đón
  • Các khoản phí cần thanh toán cho phòng khám và cách thức thanh toán
  • Hướng dẫn sử dụng thuốc áp dụng cho bạn, nếu bạn đã thông báo với chúng tôi rằng đang dùng thuốc chống đông máu, thuốc điều trị tiểu đường, insulin hoặc các loại thuốc tiêm điều trị tiểu đường hay giảm cân

Email này cũng chứa hướng dẫn về quy trình nhập viện trực tuyến của bệnh viện. Bệnh viện sẽ không liên hệ với bạn để yêu cầu thông tin; bạn tự điền mẫu đơn nhập viện trên mạng, và các hướng dẫn đi kèm sẽ hướng dẫn bạn từng bước, bao gồm cả mã số thủ thuật mà mẫu đơn yêu cầu. Vui lòng thực hiện việc này ngay sau khi nhận được email, chứ không phải vào đêm hôm trước. Mẫu đơn yêu cầu thông tin về sức khỏe tổng quát; việc bạn trả lời đầy đủ là rất quan trọng. Đặc biệt, hãy cung cấp thông tin về:

  • Chứng ngưng thở khi ngủ hoặc ngáy to kèm mệt mỏi ban ngày (xem Giấc ngủ, cơn đau và quá trình hồi phục)
  • Máy tạo nhịp tim, stent tim hoặc bất kỳ bệnh lý nào liên quan đến tim, phổi
  • Các dị ứng, kể cả dị ứng với thuốc, băng gạc, keo dán, cao su; đồng thời cho biết bạn có mang theo EpiPen hay không
  • Bất kỳ vấn đề nào bạn hoặc người thân trong gia đình từng gặp phải liên quan đến gây mê
  • Răng lỏng lẻo, răng sứ hoặc răng giả
  • Khả năng bạn đang mang thai

Nếu đã một tuần kể từ khi đặt lịch mà bạn vẫn chưa nhận được thông tin chi tiết về ca phẫu thuật, hoặc nếu gặp khó khăn trong việc hoàn thành thủ tục nhập viện trực tuyến, hãy gọi cho phòng khám.

Thuốc men: Cần ngưng dùng gì và tiếp tục dùng gì

Hầu hết các loại thuốc vẫn được dùng bình thường cho đến sáng ngày phẫu thuật, chỉ cần uống cùng một chút nước. Có một vài loại thuốc cần phải cân nhắc; quyết định này phụ thuộc vào loại phẫu thuật cũng như tình trạng sức khỏe của bạn; vì vậy tuyệt đối không được tự ý ngưng hoặc thay đổi loại thuốc nào. Hướng dẫn cụ thể dành cho bạn sẽ có trong email thông báo chi tiết về ca phẫu thuật; nếu không thấy hướng dẫn đó mà bạn cho rằng nên có, hãy liên hệ với chúng tôi.

  • Thuốc chống đông máu. Aspirin và clopidogrel (Plavix, cũng như các viên thuốc kết hợp như DuoCover) vẫn tiếp tục dùng; hãy cho chúng tôi biết bạn đang dùng chúng để đội ngũ y tế nắm rõ. Các thuốc chống đông máu như warfarin, Xarelto, Eliquis và Pradaxa thì phải ngưng dùng 48 giờ trước phẫu thuật; email gửi cho bạn sẽ nêu rõ ngày và thời điểm dùng liều cuối cùng. Hãy tuân thủ chính xác hướng dẫn này; thời điểm ngưng dùng được quy định cẩn thận nhằm tránh cả nguy cơ chảy máu trong lúc mổ lẫn nguy cơ hình thành cục máu đông nếu ngưng quá sớm. Trước khi bắt đầu dùng lại, cũng nên hỏi ý kiến bác sĩ.
  • Thuốc điều trị tiểu đường. Không dùng các loại thuốc này vào sáng ngày phẫu thuật. Các thuốc mới hơn thuộc nhóm ức chế SGLT2 (Forxiga, Jardiance, Steglatro) thì phải ngưng ba ngày trước phẫu thuật, vì chúng có thể gây tích tụ axit trong máu một cách nguy hiểm khi cơ thể nhịn ăn.
  • Insulin. Hãy dùng liều insulin tác dụng kéo dài như thường lệ vào đêm trước ngày mổ. Sáng hôm đó, chỉ dùng một nửa liều sáng thông thường, hoặc điều chỉnh theo chỉ số đường huyết của bạn. Tuyệt đối không được ngưng dùng insulin hoàn toàn, và nhớ mang theo cả insulin cùng máy đo đường huyết đến bệnh viện.
  • Các thuốc tiêm điều trị tiểu đường và giảm cân như Ozempic, Wegovy, Mounjaro, Trulicity và Byetta vẫn tiếp tục dùng bình thường; không được ngưng. Điều thay đổi là thời gian nhịn ăn: các thuốc này làm chậm quá trình rỗng dạ dày, nên thời gian nhịn ăn thông thường không đủ để đảm bảo an toàn khi gây mê; vì vậy chúng tôi yêu cầu bạn phải nhịn ăn 24 giờ trước giờ mổ.   • Thuốc kháng viêm dạng viên và một số thực phẩm chức năng. Ibuprofen, naproxen, dầu cá, nghệ, bạch quả và vitamin E liều cao đều có thể làm tăng nguy cơ chảy máu trong lúc phẫu thuật. Chúng tôi sẽ hướng dẫn bạn nên ngưng dùng những loại này và vào thời điểm nào.
  • Các thuốc điều trị huyết áp, tim mạch, tuyến giáp, động kinh và trào ngược dạ dày vẫn tiếp tục dùng như thường lệ vào sáng ngày mổ, trừ khi có chỉ định khác.
  • Thuốc chống trầm cảm và thuốc chống lo âu cũng nên tiếp tục dùng. Việc ngưng đột ngột có thể khiến những ngày xung quanh ca phẫu thuật trở nên khó khăn hơn, chứ không dễ dàng hơn.

Chuẩn bị cơ thể để phục hồi

Chỉ vài tuần thôi cũng đủ tạo ra sự khác biệt rõ rệt trong quá trình hồi phục của bạn.

  • Ngừng hút thuốc và sử dụng thuốc lá điện tử. Nicotine làm hẹp các mạch máu nhỏ mà da, gân và xương cần để phục hồi; đồng thời làm tăng nguy cơ gặp vấn đề về vết thương và làm chậm quá trình lành da. Càng ngừng sớm càng tốt, nhưng ngay cả việc ngừng trong vài tuần cuối cũng có ích. Xem thêm tác động của việc hút thuốc đối với quá trình phục hồi cơ xương khớp.
  • Kiểm soát đường huyết nếu bạn mắc bệnh tiểu đường. Nồng độ đường trong máu cao trong vài tuần trước phẫu thuật làm tăng nguy cơ nhiễm trùng vết thương. Nếu chỉ số đường huyết của bạn không ổn định, hãy gặp bác sĩ gia đình ngay bây giờ chứ không phải sau đó.
  • Ăn uống đầy đủ. Quá trình phục hồi phụ thuộc vào protein; nhiều người đến phẫu thuật với lượng protein nạp vào thấp hơn mức cần thiết. Trang chế độ dinh dưỡng tốt cho quá trình phục hồi cung cấp các mục tiêu thực tế để bạn tham khảo.
  • Hạn chế uống rượu trong tuần trước phẫu thuật; tuyệt đối không uống vào đêm trước ngày mổ. Rượu ảnh hưởng đến tác dụng của thuốc gây mê, thuốc giảm đau và giấc ngủ của bạn.
  • Tiếp tục vận động. Hãy đi bộ và vận động các khớp không được phẫu thuật. Việc duy trì sức khỏe tổng thể giúp việc gây mê an toàn hơn và giúp những tuần đầu hồi phục dễ dàng hơn.

Chăm sóc da vùng cánh tay sẽ được phẫu thuật:

  • Không cạo lông cánh tay trong tuần trước phẫu thuật. Dao cạo có thể để lại những vết xước nhỏ mà vi khuẩn dễ xâm nhập vào. Nếu cần loại bỏ lông, nhân viên y tế sẽ cắt tỉa trong lúc phẫu thuật.
  • Hãy thông báo cho chúng tôi về bất kỳ vết cắt, trầy xước, phát ban, mụn nhọt hay nhiễm trùng nào trên cánh tay đó, dù chỉ là vết nhỏ. Phẫu thuật trên vùng da bị nhiễm trùng hoặc tổn thương là không an toàn; đây là một trong những nguyên nhân phổ biến khiến ca mổ phải hoãn lại.
  • Gỡ sơn móng tay và móng giả trên bàn tay sẽ được phẫu thuật. Chúng che khuất vùng móng tay – nơi các bác sĩ dùng để kiểm tra tuần hoàn máu của bạn.
  • Giữ da sạch sẽ và dưỡng ẩm; tránh xăm mình hoặc xỏ khuyên trên cánh tay đó trong vòng một tháng trước phẫu thuật.

Chuẩn bị nhà cửa trước khi phẫu thuật

Sau khi phẫu thuật chi trên, bạn sẽ phải thực hiện mọi việc chỉ bằng một tay, thường là khi cánh tay được đeo nẹp, bó bột hoặc nạng cố định. Việc chuẩn bị nhà cửa trước khi phẫu thuật sẽ giúp bạn tránh được nhiều phiền toái sau này.

  • Sắp xếp người đưa đón và ở lại cùng bạn đêm đầu tiên. Điều này là bắt buộc. Bạn không thể tự lái xe sau khi gây mê, và cũng không nên ở một mình qua đêm.
  • Lên kế hoạch về việc di chuyển. Bạn không thể lái xe vào ngày phẫu thuật, cũng như khi cánh tay vẫn đang đeo nẹp, bó bột hoặc nạng cố định. Đối với phẫu thuật vai, thời gian này ít nhất là sáu tuần, bất kể tay nào được phẫu thuật. Trang Lái xe sau phẫu thuật vai và chi trên liệt kê thời hạn cụ thể tùy theo từng loại phẫu thuật. Hãy sắp xếp phương tiện di chuyển hoặc chuẩn bị sẵn thời gian không dùng xe hơi trước ngày phẫu thuật.
  • Sắp xếp công việc. Thời gian nghỉ việc phụ thuộc vào cả tính chất công việc lẫn loại phẫu thuật. Hãy trao đổi sớm với người sử dụng lao động; trang Trở lại làm việc sẽ giúp bạn lên kế hoạch cho cuộc trò chuyện này.
  • Chuẩn bị đồ dùng và thực phẩm. Hãy nấu sẵn và đông lạnh một vài bữa ăn. Đặt những vật dụng thường dùng hàng ngày trong bếp và phòng tắm ở độ cao vòng eo để bạn không phải với lên cao hay cúi xuống khi chỉ dùng một tay. Nên mua dầu gội và xà phòng dạng vòi bơm; nếu phẫu thuật vai, hãy chuẩn bị sẵn bọt biển cán dài.
  • Quần áo. Chọn những chiếc áo cổ rộng, mở phía trước và giày dễ mang vào. Hãy luyện tập việc mặc/ cởi áo bằng một tay trước khi thực sự cần làm điều đó khi đang đeo nẹp.
  • Giấc ngủ. Sau phẫu thuật vai, hầu hết mọi người phải ngủ ở tư thế nửa nằm nửa ngồi trong vài tuần đầu. Bạn nên chuẩn bị sẵn ghế có thể ngả lưng hoặc chồng gối thành hình nêm. Tham khảo trang Đeo nẹp tay khi ngủ.
  • Trẻ em, thú cưng và các công việc nặng. Trong hai tuần đầu sau phẫu thuật, hãy nhờ người giúp đỡ với những việc cần dùng cả hai tay hoặc sức mạnh nắm: chăm sóc trẻ nhỏ, dắt chó lớn, mua sắm, đổ rác, v.v.

Nhịn ăn

Email chứa thông tin về ca phẫu thuật sẽ nêu rõ thời điểm bạn cần ngừng ăn và uống. Vui lòng tuân theo thời gian được ghi trong email; thời điểm này được xác định dựa trên giờ nhập viện và các loại thuốc mà bạn đang sử dụng. Việc nhịn ăn là yêu cầu an toàn bắt buộc khi gây mê, chứ không phải là thủ tục hình thức; việc không tuân thủ có thể là nguyên nhân phổ biến khiến ca phẫu thuật bị hoãn vào ngày dự kiến. Tuần trước phẫu thuật: danh sách kiểm tra

  • Đã nhận được email chứa chi tiết phẫu thuật, đã đọc và lưu ở nơi dễ tìm.
  • Đã hoàn tất thủ tục nhập viện trực tuyến; đã nhập các mã số cần thiết.
  • Đã ghi chép danh sách thuốc; biết rõ loại nào cần ngưng dùng và vào thời điểm nào.
  • Nếu bạn đang dùng warfarin, Xarelto, Eliquis hoặc Pradaxa: đã ghi ngày dùng liều cuối cùng từ email vào sổ tay.
  • Đã sắp xếp người đưa đón, lái xe đưa về nhà và ở lại qua đêm đầu tiên sau phẫu thuật.
  • Nơi làm việc đã biết ngày phẫu thuật; đã sắp xếp việc giúp đỡ khi bạn không thể tự lái xe.
  • Đã mua thuốc, chuẩn bị sẵn bữa ăn trong tủ đông; sắp xếp lại nhà cửa để thuận tiện khi chỉ dùng một tay.
  • Da vùng cánh tay vẫn nguyên vẹn; chưa cạo lông; đã tẩy sạch sơn móng tay.
  • Thời gian nhịn ăn theo hướng dẫn trong email đã được ghi lại ở nơi dễ nhìn thấy vào đêm trước phẫu thuật.
  • Nếu bạn mắc bệnh tiểu đường: đã chuẩn bị sẵn insulin và máy đo đường huyết. Nếu mang theo EpiPen: đã đóng gói và sẵn sàng để đưa cho nhân viên y tế.

Vui lòng gọi cho chúng tôi trước ngày phẫu thuật nếu:

  • Bạn chưa nhận được email chứa thông tin phẫu thuật, hoặc không thể hoàn tất thủ tục nhập viện trực tuyến
  • Bạn bị ốm trong những ngày trước khi phẫu thuật: cảm lạnh, ho, sốt hoặc rối loạn tiêu hóa
  • Cánh tay mà chúng tôi sẽ phẫu thuật có vết cắt, phát ban, mụn nhọt hoặc bất kỳ dấu hiệu nhiễm trùng nào
  • Bạn không chắc nên dùng hay ngưng dùng các loại thuốc
  • Bạn vô tình ăn hoặc uống trong thời gian phải nhịn ăn
  • Tình trạng sức khỏe của bạn có thay đổi kể từ lần cuối chúng tôi khám cho bạn
  • Bạn đơn giản là thay đổi ý định, hoặc muốn thảo luận lại về quyết định này

Những trường hợp này đều không gây phiền toái gì cả. Chúng tôi thà nhận được cuộc gọi từ bạn vào ngày hôm trước còn hơn là phải cho bạn về nhà mà không thực hiện phẫu thuật vào sáng hôm đó.

Advanced reading: the deeper science (optional)

Phần này dành cho những độc giả muốn biết rõ những khía cạnh nào trong quá trình chuẩn bị phẫu thuật thực sự tạo ra sự khác biệt có thể đo lường được. Hầu hết các nghiên cứu được đề cập ở đây đến từ lĩnh vực thay khớp hông và khớp gối; do số lượng bệnh nhân lớn nên vấn đề này dễ được nghiên cứu hơn. Những kết luận này có thể áp dụng cho phẫu thuật chi trên, nhưng không nên giả định rằng các con số cũng tương tự.

Cuộc trò chuyện quan trọng hơn cả định dạng

Kết quả nghiên cứu đáng chú ý nhất lại rất đơn giản: những kỳ vọng của bệnh nhân về kết quả phẫu thuật có thể thay đổi nhờ việc trao đổi về vấn đề này trước khi mổ; đồng thời những kỳ vọng ấy lại ảnh hưởng đến mức độ hài lòng của họ sau phẫu thuật. Các thử nghiệm ngẫu nhiên về các buổi hướng dẫn trước phẫu thuật thay khớp hông và khớp gối cho thấy rằng những buổi học có cấu trúc đã làm thay đổi kỳ vọng của bệnh nhân về quá trình hồi phục của họ [1]. Ngoài ra, việc áp dụng chương trình giáo dục bài bản trước phẫu thuật thay khớp cũng giúp rút ngắn thời gian nằm viện [2].

Nội dung của cuộc trò chuyện cũng rất quan trọng. Một thử nghiệm ngẫu nhiên về đoạn video ngắn dành cho bệnh nhân trước phẫu thuật, trong đó đề cập cụ thể đến các loại thuốc giảm đau opioid, cho thấy những người xem video này đã sử dụng ít thuốc opioid hơn đáng kể trong tuần đầu tiên sau khi thay khớp gối [3]. Đây là một tác động lớn đạt được từ một can thiệp rất nhỏ; cũng vì lý do này mà chúng tôi dành thời gian trước ca phẫu thuật để hướng dẫn bạn cách sử dụng các loại thuốc giảm đau, chứ không chỉ tập trung vào chính ca mổ. Đối với phẫu thuật tay, một phân tích có hệ thống cho thấy có một số bằng chứng cho thấy việc giáo dục bệnh nhân về opioid trước mổ là hữu ích; tuy nhiên các nghiên cứu trong lĩnh vực này còn ít và chất lượng còn hạn chế [4].

Công nghệ nhiều hơn không đồng nghĩa với việc hiểu biết nhiều hơn

Hai nghiên cứu đã làm giảm bớt sự hào hứng này. Ở những bệnh nhân được phẫu thuật nội soi khớp háng, việc tư vấn bằng mô hình 3D được in riêng cho từng bệnh nhân không giúp họ hiểu rõ hơn hay hài lòng hơn so với việc chỉ sử dụng các hình ảnh chụp quét thông thường [5]. Ngoài ra, các buổi giáo dục tiền phẫu thuật được tổ chức trong các chuyến đi phẫu thuật từ thiện cũng chỉ giúp bệnh nhân nâng cao hiểu biết về phẫu thuật thay khớp một cách hạn chế [6]. Kết luận thực tiễn là: việc giải thích một cách đơn giản, từ tốn và cho phép bệnh nhân đặt câu hỏi mới là yếu tố quan trọng; các công cụ hỗ trợ chỉ mang tính tùy chọn mà thôi.

Hãy cẩn trọng khi đọc thông tin

Nhiều người chuẩn bị bằng cách tìm kiếm trên mạng, nhưng chất lượng thông tin tìm được lại không đồng đều. Một cuộc khảo sát các video trên YouTube về các phương pháp tiêm điều trị trong ngoại khoa chỉ ra rằng nội dung khá thiếu nhất quán; phần lớn các video này do người dùng cá nhân tạo ra chứ không phải các tổ chức y tế [7].
