Những cảm giác mà bạn đang trải qua

Khi bị lạnh hoặc căng thẳng, các ngón tay của bạn sẽ thay đổi màu sắc và có cảm giác lạ. Một cơn phát tác điển hình gồm ba giai đoạn: đầu tiên các ngón tay trở nên trắng ra, lạnh và như tê liệt; sau đó chuyển sang màu xanh tím khi máu rút đi; cuối cùng chúng ửng đỏ lên và có thể đau nhức hoặc có cảm giác tê rần khi máu ùa trở lại. Trong quá trình này, bạn có thể cảm thấy tê mất cảm giác, có cảm giác kim châm, hoặc đau âm ỉ. Đôi khi hiện tượng tương tự cũng xảy ra ở ngón chân, mũi, tai hoặc môi.

Các cơn phát tác xuất hiện rồi biến mất. Các mạch máu nhỏ cung cấp máu cho các ngón tay có chức năng co lại khi gặp lạnh, nhằm giữ ấm phần trung tâm cơ thể. Ở bệnh Raynaud, các mạch máu này phản ứng quá mức, co lại mạnh hơn mức cần thiết. Hiện tượng này gọi là co thắt mạch. Trong một khoảng thời gian ngắn, rất ít máu đến được da các ngón tay; đó là giai đoạn ngón tay trở nên trắng, lạnh và tê cứng. Khi cơn co mạch giảm dần, máu lại lưu thông trở lại, khiến ngón tay chuyển sang màu xanh tím, rồi đỏ và ấm lên.

Cần biết rằng có hai dạng bệnh này. Raynaud nguyên phát là dạng phổ biến hơn rất nhiều; về cơ bản đây là phản ứng quá mẫn của cơ thể với lạnh mà không do bệnh lý nào gây ra. Bệnh thường xuất hiện ở phụ nữ trẻ, ảnh hưởng đến cả hai bàn tay một cách tương đối đồng đều, và tuy gây phiền toái nhưng không gây hại. Raynaud thứ phát ít gặp hơn nhưng quan trọng hơn, vì cơn co mạch ở đây liên quan đến một bệnh lý khác, thường là các bệnh ảnh hưởng đến mô liên kết như xơ cứng bì hoặc lupus. Các dấu hiệu gợi ý dạng thứ phát gồm: cơn bệnh xuất hiện ở độ tuổi lớn hơn, các cơn chỉ ảnh hưởng đến một bàn tay hoặc vài ngón tay chứ không phải cả hai bàn tay một cách đồng đều, các cơn rất dữ dội, xuất hiện vết lở hoặc vết loét ở đầu ngón tay, hoặc các triệu chứng khác như đau khớp, phát ban. Những dấu hiệu này đáng được bác sĩ khám kỹ lưỡng.

Những gì chúng tôi có thể làm

Đối với hầu hết mọi người, biện pháp chính rất đơn giản và hiệu quả: giữ ấm và tránh các yếu tố gây kích phát.

  • Chủ động chống lại cái lạnh. Hãy đeo găng tay (găng tay mittens giúp giữ các ngón tay gần nhau và ấm hơn), sử dụng túi giữ nhiệt, và mặc nhiều lớp quần áo. Việc giữ ấm toàn thân và vùng thân nhiệt cũng quan trọng không kém việc giữ ấm đôi tay; khi phần trung tâm cơ thể ấm, mạch máu ở các ngón tay sẽ giữ được thông thoáng.
  • Tránh tiếp xúc đột ngột với lạnh. Hãy đeo găng tay khi lấy đồ ra khỏi tủ lạnh hoặc tủ đông, bật sưởi ô tô từ sớm, và cầm lon/chai đồ uống lạnh trong vỏ bọc cách nhiệt.
  • Bỏ thuốc lá. Hút thuốc làm co mạch máu và khiến chứng Raynaud trở nên tồi tệ hơn; việc bỏ thuốc là một trong những điều hữu ích nhất mà bạn có thể làm.
  • Hạn chế dùng caffeine cũng như bất cứ thứ gì làm bạn bị kích thích, vì cả hai đều có thể gây ra các cơn. Việc kiểm soát căng thẳng bằng bất cứ cách nào phù hợp với bạn cũng có ích, vì chính căng thẳng cũng có thể khởi phát hiện tượng co mạch.

Nếu các cơn co mạch xảy ra thường xuyên, gây đau đớn hoặc nghiêm trọng, có những loại thuốc giúp giãn mạch máu và giảm tần suất xuất hiện của chúng. Loại thuốc phổ biến nhất là nhóm thuốc chẹn kênh canxi (chẳng hạn nifedipine); nếu loại này không phù hợp, vẫn còn nhiều lựa chọn khác. Trong trường hợp mắc chứng Raynaud thứ phát, việc điều trị căn bệnh nền cũng là một phần quan trọng trong kế hoạch điều trị; vì vậy việc chẩn đoán chính xác là rất cần thiết.

Những điều có thể xảy ra

Đối với đa số bệnh nhân (những người mắc chứng Raynaud nguyên phát), tình trạng này chỉ là một phiền toái dễ kiểm soát chứ không gây nguy hiểm. Bằng cách giữ ấm cơ thể hợp lý và tránh các yếu tố kích phát, nhiều người có thể hạn chế tối đa các cơn và vẫn sinh hoạt bình thường; các ngón tay sẽ hồi phục hoàn toàn sau mỗi đợt cơn mà không để lại di chứng nào. Tình trạng này thường tồn tại lâu dài chứ không tự khỏi, nhưng hoàn toàn có thể kiểm soát được; và luôn có thuốc cho những lúc các biện pháp đơn giản chưa đủ hiệu quả.

Với chứng Raynaud thứ phát, cần chú ý nhiều hơn vì chính bệnh lý nền quyết định diễn tiến của bệnh; trong một số trường hợp, việc giảm lưu lượng máu có thể gây tổn thương da đầu ngón tay. Đó là lý do tại sao việc xác định chính xác loại Raynaud mà mình mắc phải là rất quan trọng, nhằm đảm bảo mức độ chăm sóc phù hợp với tình trạng sức khỏe của từng người.

Khi nào nên gặp bác sĩ

Hãy đến gặp bác sĩ để được khám nếu:

  • Các cơn bệnh bắt đầu xuất hiện ở độ tuổi lớn hơn, có mức độ nghiêm trọng cao, hoặc chỉ ảnh hưởng đến một bàn tay hoặc vài ngón tay chứ không phải cả hai bàn tay một cách đồng đều: đây có thể là dấu hiệu của dạng bệnh thứ phát; thường cần làm xét nghiệm máu và kiểm tra các mạch máu nhỏ ở gốc móng tay.

Hiện tượng Raynaud rất đáng để tìm hiểu thêm, bởi vì chỉ có một nhóm thuốc duy nhất có đầy đủ bằng chứng lâm sàng ủng hộ; đồng thời phương pháp điều trị đang thu hút nhiều sự quan tâm hiện nay lại hoạt động dựa trên một cơ chế khá bất ngờ.

Một nhóm thuốc duy nhất có bằng chứng lâm sàng

Bệnh Raynaud là tình trạng co thắt từng đợt của các động mạch nhỏ ở ngón tay; vì vậy mục tiêu điều trị là giữ cho các mạch máu này luôn thông suốt. Người ta đã thử nghiệm nhiều loại thuốc giãn mạch khác nhau.

Một phân tích tổng hợp dựa trên 17 thử nghiệm lâm sàng cho thấy rằng các thuốc chẹn kênh canxi mang lại hiệu quả có ý nghĩa thống kê trong việc giảm tần suất và mức độ nghiêm trọng của các triệu chứng; đồng thời không có bằng chứng nào chứng minh hiệu quả của các thuốc giãn mạch uống khác ngoài nhóm chẹn kênh canxi [1].

Câu thứ hai mới là phần quan trọng. Đây là kết quả âm tính đối với toàn bộ nhóm thuốc thay thế; nó giải thích tại sao việc điều trị thường bắt đầu và tiếp tục sử dụng một nhóm thuốc duy nhất thay vì thử nghiệm nhiều loại thuốc khác nhau.

Toxin botulinum – tại sao nó không hoạt động theo cách mà bạn tưởng tượng

Phương pháp điều trị gây được nhiều sự quan tâm trong các trường hợp khó điều trị là việc tiêm toxin botulinum quanh các động mạch ở gốc ngón tay. Việc sử dụng phương pháp này đối với hiện tượng Raynaud không đáp ứng với điều trị y tế đã cho thấy kết quả khả quan, và nó được coi là phương án không xâm lấn thay thế cho phẫu thuật, với hiệu quả đã được chứng minh qua các nghiên cứu quy mô nhỏ [2].

Cơ chế tác động của nó rất đáng để tìm hiểu vì trái ngược với trực giác thông thường. Toxin botulinum vốn được biết đến với khả năng gây liệt cơ; vì vậy người ta thường cho rằng nó làm giãn cơ nằm trong thành động mạch. Tuy nhiên, hiểu biết hiện nay lại khác: nó can thiệp vào các tín hiệu thần kinh giao cảm điều khiển sự co mạch, đồng thời dường như còn tác động lên các đường dẫn truyền cảm giác đau. Đây là lý do tại sao hiệu quả điều trị đôi khi bao gồm cả việc giảm đau, mức độ giảm đau này có thể vượt xa mức thay đổi về lưu lượng máu.

Cần lưu ý cách mô tả trung thực trong các bằng chứng nghiên cứu: “các nghiên cứu quy mô nhỏ”. Đây là một phương án đầy hứa hẹn dành cho những bệnh nhân đã thử đủ mọi phương pháp điều trị y tế, nhưng chưa phải là phương pháp đã được công nhận rộng rãi.

Sự khác biệt quyết định mọi thứ

Câu hỏi quan trọng nhất là hiện tượng này thuộc dạng nguyên phát, xảy ra một cách độc lập, hay thứ phát do một bệnh lý nền gây ra – thường là các bệnh tự miễn về mô liên kết như xơ cứng bì hoặc lupus.

Sự khác biệt này không chỉ mang tính lý thuyết. Bệnh Raynaud nguyên phát chỉ gây phiền toái, có tiên lượng tốt và không gây tổn thương mô. Ngược lại, bệnh Raynaud thứ phát có thể tiến triển thành loét và mất mô ở đầu ngón tay; đồng thời bệnh lý nền cũng cần được xác định và điều trị riêng.

Những dấu hiệu đáng lo ngại bao gồm: khởi phát sau khoảng 30 tuổi, tình trạng không đối xứng giữa hai bàn tay, có loét hoặc sẹo lõm ở đầu ngón tay, cùng sự bất thường của các mao mạch ở vùng nếp móng. Những dấu hiệu này đòi hỏi phải được kiểm tra kỹ lưỡng thay vì chỉ được xem là vô hại; đó là lý do tại sao khi bệnh Raynaud lần đầu xuất hiện, người bệnh thường cần được thăm khám kỹ thay vì chỉ được khuyên đeo găng tay.

Vị trí của phẫu thuật trong điều trị

Đối với các trường hợp bệnh thứ phát nặng có nguy cơ mất mô, phẫu thuật cắt giao cảm quanh động mạch – tức là loại bỏ các sợi thần kinh giao cảm khỏi bề mặt các động mạch ngón tay – là phương án phẫu thuật được lựa chọn. Phương pháp này tác động lên cùng một con đường truyền tín hiệu như độc tố botulinum nhưng theo một cách vĩnh viễn. Phẫu thuật này chỉ được chỉ định trong các trường hợp thiếu máu cục bộ nguy kịch và loét không lành, chứ không dùng cho những người bị ngón tay lạnh và khó chịu; điều này phù hợp với trình tự điều trị đã nêu ở trên: càng đi sâu vào các bước điều trị, mục tiêu càng chuyển từ việc mang lại sự thoải mái sang việc cứu vãn mô.
