Những triệu chứng bạn đang gặp phải

Viêm quanh móng là tình trạng nhiễm trùng hoặc viêm ở nếp da bên cạnh móng. Bệnh có thể xuất hiện nhanh hoặc tiến triển từ từ, và hai dạng này gây cảm giác khác nhau.

Dạng xuất hiện nhanh thường bắt đầu sau khi bị xước măng rô, cắn móng hoặc một tổn thương nhỏ ở vùng da bên cạnh móng. Trong vòng một hoặc hai ngày, nếp da trở nên đau khi chạm vào, sưng và đỏ, và mủ có thể tích tụ dưới nếp da đó. Cơn đau nhói và nhức theo nhịp mạch đập, nằm ngay ở mép móng chứ không ở cao hơn trên ngón tay. Những việc hằng ngày có tác động lên đầu ngón tay trở nên khó khăn: nhặt đồng xu, gõ bàn phím, cài cúc áo hoặc cầm ấm đun nước.

Dạng tiến triển chậm hình thành trong sáu tuần hoặc lâu hơn. Tình trạng sưng và đỏ nhẹ hơn, nhưng nếp da vẫn đau và sưng phồng. Nếp da có thể tách ra khỏi móng, và bản thân móng có thể xuất hiện các đường gờ, rãnh hoặc đổi màu. Tình trạng này có xu hướng bùng phát sau khi tay bị ướt trong một thời gian, vì vậy việc rửa chén bát, tắm vòi sen hoặc một ngày làm việc trong môi trường ẩm ướt có thể gây khởi phát. Việc thường xuyên làm công việc tiếp xúc với nước, rửa tay thường xuyên hoặc tổn thương lớp biểu bì móng do làm móng đều có thể khiến tình trạng kéo dài. Nếu không được điều trị, các đợt bùng phát gây đau sẽ tiếp tục tái diễn.

Nếu tình trạng đỏ, sưng và đau lan lên ngón tay, hoặc bạn bị sốt, hãy đến khoa cấp cứu ngay trong ngày. Bạn không cần giấy giới thiệu.

Chuyện gì đang xảy ra thực sự

Móng của bạn nằm trong một túi da vừa khít. Nếp da bao quanh móng ở phần chân móng và hai bên hoạt động giống như một miếng gioăng, giữ móng áp sát vào ngón tay và ngăn nước, bụi bẩn và vi khuẩn lọt vào các lớp bên dưới. Viêm quanh móng là điều xảy ra khi lớp bịt kín đó bị hỏng.

Ở dạng xuất hiện nhanh, vi khuẩn xâm nhập qua chỗ xước măng rô, vết cắn hoặc một vết cắt nhỏ bên cạnh móng. Cơ thể đưa dịch và các tế bào chống nhiễm trùng đến, và mủ tích tụ trong túi kín đó. Vì túi này không thể giãn ra nhiều, chính áp lực này gây ra cảm giác đau nhói, nhức theo nhịp đập ngay ở mép móng.

Dạng tiến triển chậm thì khác. Đây chủ yếu không phải là tình trạng nhiễm trùng. Nó bắt đầu khi lớp biểu bì móng, lớp bịt kín nhỏ ở chân móng, bị tổn thương do công việc tiếp xúc với nước, rửa tay thường xuyên, làm móng hoặc một chất gây kích ứng. Một khi lớp bịt kín đó mất đi, nước và các chất kích ứng thấm vào nếp da và nếp da bị viêm. Nếp da bị sưng không thể áp sát trở lại vào móng, vì vậy nó cứ bị nâng lên và giữ ẩm, và vi khuẩn trú ngụ trong khe hở ẩm ướt đó. Nấm men thường được tìm thấy ở đó giống như một kẻ đi nhờ hơn là nguyên nhân gây bệnh. Tình trạng kích ứng kéo dài cũng làm rối loạn lớp mô nằm dưới lớp biểu bì móng có chức năng tạo ra móng, đó là lý do xuất hiện các đường gờ và rãnh. Mỗi đợt bùng phát lại ngăn lớp bịt kín phục hồi, vì vậy vấn đề cứ tự duy trì.

Nếu mủ không được xử lý, mủ có thể lan xuống dưới móng hoặc vòng sang phía bên kia của ngón tay.

Những biện pháp chúng tôi có thể áp dụng

Bác sĩ Kieran Hirpara, bác sĩ phẫu thuật chi trên tại Bệnh viện tư nhân Mater Rockhampton, thường bắt đầu với các phương pháp ít xâm lấn nhất phù hợp với tình trạng của bệnh nhân. Thông thường, bệnh nhân được bác sĩ đa khoa giới thiệu đến phòng khám chúng tôi; nếu nhà vật lý trị liệu khuyên bạn nên đến gặp chúng tôi, bạn vẫn cần có giấy giới thiệu từ bác sĩ đa khoa để được hưởng mức hoàn trả từ Medicare. Đối với dạng tiến triển chậm, việc tự chăm sóc được ưu tiên hàng đầu. Bảo vệ nếp da quanh móng quan trọng hơn bất kỳ loại kem nào: giữ cho tay khô ráo, đeo găng tay khi làm việc tiếp xúc với nước, và ngừng cạy hoặc cắt lớp biểu bì móng. Chúng tôi có thể kê đơn kem hoặc thuốc mỡ steroid để bạn xoa vào nếp da, giúp làm dịu tình trạng viêm. Đối với dạng này, các loại kem bôi lên nếp da có hiệu quả tốt hơn thuốc viên. Hãy kiên trì trong vài tuần, và tiếp tục duy trì thói quen giữ tay khô ráo và được bảo vệ ngay cả khi da đã ổn định, vì lớp bịt kín cần thời gian để phục hồi.

Đối với dạng xuất hiện nhanh có mủ, chúng tôi có thể kê đơn thuốc kháng sinh dạng viên, đôi khi kèm theo ngâm tay trong nước muối ấm tại nhà. Hơi ấm có thể giúp một ổ mủ nhỏ tự thoát ra. Nếu mủ đã tích tụ dưới nếp da, thường cần phải tháo mủ ra chứ không chỉ dùng kháng sinh đơn thuần. Điều trị sớm có thể giúp tình trạng thuyên giảm trong vòng vài ngày mà không cần nhổ móng.

Nếu dạng tiến triển chậm không thuyên giảm sau khi đã áp dụng các biện pháp này một cách đầy đủ, phẫu thuật sẽ được cân nhắc. Ca mổ cắt bỏ một mảnh hình lưỡi liềm của nếp da bị dày lên và viêm ở chân móng, giúp nếp da lành lại áp sát vào móng và bịt kín trở lại. Nếu bản thân móng đã có các đường gờ hoặc mọc không đều, việc nhổ bỏ tấm móng cùng lúc sẽ giúp cải thiện kết quả. Chúng tôi sẽ trao đổi với bạn xem phẫu thuật có phù hợp với ngón tay của bạn hay không và cùng nhau quyết định.

Những điều có thể xảy ra

Dạng xuất hiện nhanh thường thuyên giảm tốt khi được điều trị sớm. Nhiều người khỏi bệnh mà không cần nhổ móng. Khi mủ được tháo ra và nếp da có thể lành lại, cơn đau sẽ dịu đi và ngón tay trở lại bình thường trong vòng vài ngày đến vài tuần.

Dạng tiến triển chậm cần nhiều thời gian hơn. Đây là tình trạng cứ xuất hiện rồi biến mất, bùng phát sau khi làm việc tiếp xúc với nước và ổn định khi nếp da được bảo vệ. Với sự chăm sóc đều đặn, trong đó việc giữ tay khô ráo và giữ nguyên lớp biểu bì móng quan trọng hơn bất kỳ loại kem nào, khe hở giữa nếp da và móng sẽ liền lại trong khoảng 6 đến 8 tuần và lớp bịt kín tự phục hồi. Một số người cần duy trì thói quen này trong vài tháng trước khi nếp da ổn định hẳn. Nếu để nguyên không điều trị, các đợt bùng phát gây đau sẽ tiếp tục tái diễn, và móng có thể dày lên và xuất hiện các đường gờ hoặc rãnh lâu dài.

Phẫu thuật cho dạng tiến triển chậm cắt bỏ phần nếp da bị dày lên để nếp da có thể lành lại áp sát vào móng. Những ngón tay có móng vẫn còn trơn nhẵn thường lành chỉ với phẫu thuật nếp da. Khi móng đã có các đường gờ hoặc mọc không đều, việc nhổ bỏ tấm móng cùng lúc sẽ giúp tăng khả năng vấn đề không tái phát. Thường không cần dùng kháng sinh sau phẫu thuật đối với tình trạng này ở những người không có vấn đề sức khỏe nào khác.

Có một vài điều bạn nên biết trong quá trình điều trị. Nhiễm trùng ở tay có thể dẫn đến các biến chứng khó xử lý, vì vậy đừng bỏ qua một ngón tay không cải thiện. Nếu các triệu chứng không thuyên giảm, ngày càng nặng hơn qua nhiều tuần, khiến bạn thức giấc vào ban đêm, hoặc khiến bạn không thể làm việc hay sử dụng bàn tay, hãy đến gặp bác sĩ đa khoa hoặc đề nghị được bác sĩ chuyên khoa thăm khám.

Khi nào nên đi khám bác sĩ

Hầu hết các ca nhiễm trùng nếp da quanh móng đều thuyên giảm nhờ được chăm sóc sớm, nhưng một số dấu hiệu cho thấy bạn không nên chờ đợi. Hãy đến khoa cấp cứu ngay trong ngày nếu ngón tay của bạn trở nên nóng, đỏ, sưng và đau, đặc biệt là khi kèm theo sốt, hoặc nếu mủ đang lan lên ngón tay. Bạn không cần giấy giới thiệu. Nhiễm trùng ở tay có thể dẫn đến các biến chứng khó xử lý một khi đã phát triển, vì vậy việc được chăm sóc ngay trong ngày là rất quan trọng.

Hãy đến gặp bác sĩ đa khoa nếu tình trạng đau và sưng đã kéo dài sáu tuần hoặc lâu hơn, hoặc nếu nếp da cứ bùng phát sau khi làm việc tiếp xúc với nước dù đã được giữ khô ráo và bảo vệ. Hãy đề nghị được bác sĩ chuyên khoa thăm khám nếu nếp da vẫn bị nâng lên và sưng phồng, móng xuất hiện các đường gờ hoặc rãnh, hoặc một vấn đề về móng không đáp ứng với các loại kem bôi. Nếp da quanh móng không thuyên giảm sau khi đã được điều trị đúng cách cần được kiểm tra kỹ hơn, vì đôi khi các nguyên nhân khác bị nhầm lẫn với tình trạng này.

Việc tìm hiểu kỹ về tình trạng viêm quanh móng là rất đáng giá, bởi một thuật ngữ duy nhất lại bao hàm hai tình trạng bệnh lý có biểu hiện khác nhau, đáp ứng với các phương pháp điều trị trái ngược nhau, và thường bị nhầm lẫn với nhau. Ngoài ra, phương pháp phẫu thuật dành cho dạng mãn tính của bệnh này lại đơn giản hơn, cổ điển hơn và có cơ sở khoa học vững chắc hơn nhiều so với những gì hầu hết mọi người tưởng tượng.

Hai tình trạng, một tên gọi

Viêm quanh móng cấp tính là tình trạng nhiễm trùng do vi khuẩn ở vùng da quanh móng, thường do Staphylococcus aureus gây ra; tình trạng này phát triển trong vòng 24 đến 48 giờ sau khi lớp niêm phong giữa móng và da bị phá vỡ [1]. Vùng da bị ảnh hưởng sẽ đỏ, nóng, căng và đau dữ dội; đồng thời xuất hiện mủ.

Viêm quanh móng mãn tính, theo quy ước là tình trạng kéo dài hơn sáu tuần, thực chất không phải là nhiễm trùng. Đây là dạng viêm da dị ứng ở vùng da quanh móng, do tiếp xúc thường xuyên với nước và các chất kích thích gây ra; việc mất đi lớp niêm phong do lớp da chết tạo ra tạo điều kiện cho các chất kích thích xâm nhập tiếp, tạo thành một vòng luẩn quẩn không dứt [2]. Vi khuẩn Candida thường được phát hiện trong các mẫu bệnh phẩm lấy từ vùng da này; vì lý do đó mà tình trạng này từ lâu vẫn được coi là nhiễm nấm. Tuy nhiên, hiện nay người ta hiểu rằng sự hiện diện của Candida chỉ là kết quả của việc chúng cư trú trên vùng da đã bị tổn thương, chứ không phải nguyên nhân gây bệnh.

Thử nghiệm đã làm thay đổi cách hiểu về dạng bệnh mãn tính này

Bằng chứng quyết định cho vấn đề này là một thử nghiệm ngẫu nhiên, mù đôi, sử dụng phương pháp “double-dummy”, so sánh hiệu quả của steroid bôi ngoài da (methylprednisolone aceponate) với hai loại thuốc kháng nấm dùng đường toàn thân. Trong số 48 móng được điều trị bằng steroid bôi ngoài da, có 41 móng có cải thiện hoặc khỏi bệnh; con số tương ứng với nhóm dùng terbinafine là 30/57 móng, và với nhóm dùng itraconazole là 29/64 móng. Kết quả này cho thấy ưu thế có ý nghĩa thống kê rõ rệt của steroid so với các thuốc kháng nấm, với p-value là [3].

Chính trong thử nghiệm này còn có một quan sát mang tính quyết định hơn nữa. Sự hiện diện của nấm Candida không liên quan chặt chẽ đến mức độ hoạt động của bệnh; việc loại bỏ nấm Candida chỉ giúp 2 trong số 18 bệnh nhân có kết quả chữa khỏi lâm sàng, những người này đều mang nấm này ngay từ đầu [3].

Điều này khó có thể giải thích dựa trên giả thuyết bệnh do nấm gây ra; ngược lại lại dễ hiểu nếu xét theo giả thuyết bệnh có cơ chế viêm: nấm Candida chỉ là “hành khách” xuất hiện trong môi trường da bị tổn thương, chứ không phải nguyên nhân gây bệnh. Điều này khiến chúng ta phải nhìn nhận tình trạng này như một vấn đề về hàng rào bảo vệ da: lớp cuticle đóng vai trò như lớp niêm phong; việc tiếp xúc thường xuyên với nước làm hỏng lớp này; các nếp gấp da bị sưng phù, không thể khép lại được nữa, tạo điều kiện cho vi sinh vật xâm nhập và phát triển. Việc điều trị các vi sinh vật này thực chất không tác động gì đến cơ chế bệnh sinh.

Hệ quả thực tiễn của điều này thì không mấy hấp dẫn, và cũng là phần mà hầu hết bệnh nhân thường bỏ qua: biện pháp hiệu quả nhất chính là giữ cho tay luôn khô ráo và tránh tiếp xúc với các chất gây kích ứng. Trong trường hợp này, việc đeo găng tay còn hiệu quả hơn cả việc dùng thuốc.

Viêm quanh móng cấp tính: cách dẫn lưu mủ và mức độ can thiệp cần thiết

Khi mủ đã hình thành, chỉ dùng kháng sinh thôi là không đủ; cần phải dẫn lưu mủ ra ngoài [1]. Phương pháp truyền thống là nâng vạt da quanh móng lên khỏi móng để giảm áp lực do áp xe gây ra, đồng thời cắt bỏ một phần móng nơi mủ đã tích tụ phía dưới.

Một phương pháp thay thế giúp bảo tồn móng là kỹ thuật cuộn vạt kiểu Thụy Sĩ: vạt da quanh móng được nâng lên và cuộn lại quanh sợi chỉ khâu thay vì cắt rạch hay cắt bỏ; sau đó giữ nguyên trong vài ngày rồi mới mở ra [4]. Ưu điểm của phương pháp này là có thể dẫn lưu mủ lan rộng khắp vùng quanh móng mà không làm tổn thương móng hay vạt da quanh móng; điều này đặc biệt quan trọng khi phương pháp truyền thống đòi hỏi phải rạch một vết cắt rộng.

Nguyên tắc chung cho cả hai phương pháp là vết rạch phải nhằm mục đích giảm áp lực ở vùng quanh móng chứ không được đi vào phần thịt ngón tay, vì phần thịt này là một khoang riêng biệt; việc mở khoang này sẽ biến tình trạng viêm quanh móng đơn giản thành một vết thương phức tạp hơn nhiều.

Phẫu thuật tạo túi mô ở vùng mô móng

Đối với tình trạng viêm mô móng mãn tính không thuyên giảm, phương pháp phẫu thuật này đã được mô tả từ năm 1976 và về cơ bản vẫn không thay đổi cho đến nay.

Kỹ thuật của Keyser và Eaton là cắt bỏ một mảnh hình lưỡi liềm của lớp mô móng phía gần chân móng bị dày lên, loại bỏ mô viêm mà vẫn tránh làm tổn thương đến lớp mô mầm; sau đó để vết thương hở để tự lành. Quá trình lành thương này tạo ra sự co rút, giúp lớp mô móng trở lại vị trí ban đầu trên móng và phục hồi lại cấu trúc vốn bị tổn thương do bệnh [5].

Cấu trúc giải phẫu là yếu tố quyết định tầm quan trọng của vùng rìa này. Dưới lớp mô móng là một túi mô mầm dày khoảng 2–3 mm có chức năng tạo ra móng; lớp mô dưới da phía trên túi này lại điều chỉnh bề mặt móng. Nếu lớp mô này bị tổn thương do nhiễm trùng, áp lực hoặc chấn thương, quá trình tạo móng sẽ bị rối loạn tương ứng: những đợt bệnh ngắn ngày gây ra các đường gờ ngang trên móng; còn bệnh kéo dài sẽ dẫn đến các rãnh dọc và móng dày lên – những đặc điểm điển hình của viêm mô móng mãn tính [5].

Vì vậy, việc bảo tồn lớp mô mầm là điều cần thiết để ngăn ngừa dị dạng móng vĩnh viễn; đồng thời việc để vết thương hở mới tạo ra sự co rút giúp phục hồi lại cấu trúc mô móng. Nếu khâu kín vết thương thì mục đích điều trị sẽ không đạt được.

Phương pháp điều trị giúp ngăn ngừa tái phát

Dữ liệu kết quả điều trị hữu ích nhất là từ một nhóm nhỏ bệnh nhân có sự so sánh nội bộ rõ ràng. Như đã báo cáo, những ngón tay có dấu hiệu bất thường ở móng được điều trị chỉ bằng phương pháp mổ mở thì vẫn có một tỷ lệ nhỏ bị tái phát; ngược lại, nhóm ngón tay tương tự nhưng được điều trị bằng phương pháp mổ mở kèm theo việc loại bỏ móng thì không gặp hiện tượng này. Các ngón tay không có dấu hiệu bất thường ở móng thì chỉ cần điều trị bằng phương pháp mổ mở là lành [6].

Vì số lượng bệnh nhân trong nhóm này ít và việc điều trị được thực hiện theo trình tự chứ không phải ngẫu nhiên, nên đây chỉ là dấu hiệu gợi ý chứ chưa phải bằng chứng chắc chắn. Toàn văn báo cáo cũng không có sẵn cho chúng tôi, nên các con số trên được tổng hợp từ các tài liệu nghiên cứu thứ cấp chứ không phải trích trực tiếp từ bài báo gốc. Tuy nhiên, điều này đưa ra một nguyên tắc cụ thể trong quá trình phẫu thuật: móng có hình dạng bất thường hoặc gồ ghề là dấu hiệu cho thấy bệnh đã ảnh hưởng đến mô móng bên dưới; vì vậy ở những ngón tay này cần phải loại bỏ luôn móng. Còn với những ngón tay có móng bình thường thì có thể giữ lại.

Đây là một phát hiện rất hữu ích và dễ áp dụng đối với một bệnh lý phổ biến như thế này; cũng vì vậy mà trước khi quyết định phẫu thuật, việc kiểm tra tình trạng móng – chứ không chỉ vùng nếp gấp da – là điều rất cần thiết.
