Những triệu chứng bạn đang gặp phải

Nếu bạn thường xuyên ném vật (bóng chày, lao phi tiêu, hoặc bất kỳ môn thể thao nào đòi hỏi việc ném mạnh lên trên), bạn có thể cảm thấy đau nhức ở mặt trong khuỷu tay. Cơn đau thường xuất hiện vào lúc bạn dùng hết sức lực, ngay trước khi cánh tay vung ra để ném. Nhiều vận động viên mô tả cảm giác đau này là kiểu đau âm ỉ, sâu bên trong, ngay tại chỗ gồ lên của xương ở mặt trong khuỷu tay.

Dấu hiệu phổ biến khác là khả năng ném của bạn bị thay đổi: tốc độ ném giảm đi, các cú ném không còn chính xác hoặc mất đi sức mạnh như trước, hoặc đơn giản là bạn không thể ném mạnh hay ném lâu như xưa. Điều gì đang thực sự xảy ra

Một dải mô chắc khỏe gọi là dây chằng bên quay (UCL) nằm ở phía trong khuỷu tay. Chức năng của nó là ngăn không cho khuỷu tay bị mở rộng ra về phía trong khi có lực tác động theo hướng đó; chuyển động này được gọi là valgus. Việc ném bóng gây ra áp lực lớn và liên tục lên đúng dây chằng này. Mỗi lần ném mạnh sẽ làm dây chằng bị giãn ra một chút; sau hàng ngàn lần ném, dây chằng có thể dần bị rách nhỏ, mỏng đi và yếu đi. Trong những trường hợp hiếm hơn, một cú ném quá mạnh có thể làm rách dây chằng ngay lập tức.

Khi UCL bị tổn thương, nó không còn khả năng giữ khuỷu tay vững chắc nữa; kết quả là khớp trở nên hơi lỏng lẻo dưới tác động của các lực khi ném bóng. Cần lưu ý rằng đây chủ yếu là chấn thương thường gặp ở những vận động viên phải ném bóng nhiều lần. Đối với những người không tham gia các hoạt động ném bóng lặp đi lặp lại, việc UCL bị tổn thương hầu như không gây ra vấn đề gì trong sinh hoạt hàng ngày, vì các hoạt động thông thường không tạo ra áp lực lên dây chằng như khi ném bóng.

Phương pháp điều trị phù hợp phụ thuộc vào mức độ tổn thương của dây chằng, cũng như việc bạn có cần quay lại hoạt động ném bóng ở mức độ cao hay không.

Đối với các trường hợp rách dây chằng ở mức độ nhẹ hoặc vừa, cũng như những người không cần tham gia thi đấu ném bóng, chúng tôi ưu tiên các phương pháp điều trị không phẫu thuật. Điều này bao gồm việc tạm ngưng hoạt động ném bóng, sau đó thực hiện chương trình phục hồi chức năng được lên kế hoạch cẩn thận nhằm tăng cường sức mạnh cho cẳng tay, vai và vùng thân trên; điều chỉnh lại kỹ thuật ném bóng; và dần dần đưa bạn trở lại với hoạt động ném bóng thông qua “chương trình ném bóng” có cấu trúc rõ ràng. Đôi khi chúng tôi tiêm các chất như huyết tương giàu tiểu cầu (PRP) để hỗ trợ quá trình lành thương; tuy nhiên mức độ hiệu quả của phương pháp này vẫn chưa được xác nhận chắc chắn.

Phẫu thuật sẽ được cân nhắc khi dây chằng bị rách hoàn toàn, hoặc khi quá trình phục hồi chức năng không giúp các vận động viên ném bóng trở lại thi đấu. Có hai loại phẫu thuật chính:

  • Tái tạo dây chằng UCL (“phẫu thuật Tommy John”). Dây chằng bị tổn thương sẽ được tái tạo bằng cách sử dụng mảnh gân ghép, thường lấy từ cẳng tay hoặc chân của chính bệnh nhân; mảnh gân này được luồn qua các lỗ nhỏ trên xương để tạo thành dây chằng mới vững chắc. Đây là phương pháp phẫu thuật đã được áp dụng lâu năm và chứng minh hiệu quả đối với các vận động viên ném bóng.
  • Sửa chữa dây chằng UCL kết hợp dùng nẹp nội soi. Với những vận động viên trẻ mà dây chằng bị tách rời khỏi xương ở một đầu (thay vì bị rách rải rác), bác sĩ phẫu thuật có thể khâu lại dây chằng và gia cố bằng một loại băng dính chắc chắn. Phương pháp này thường giúp bệnh nhân nhanh chóng trở lại hoạt động ném bóng hơn; tuy nhiên chỉ áp dụng cho những trường hợp rách dây chằng phù hợp.

Trong cả hai loại phẫu thuật, bác sĩ phẫu thuật cũng sẽ kiểm tra dây thần kinh nằm ở mặt trong khuỷu tay; đồng thời bảo vệ hoặc dịch chuyển dây thần kinh này nếu nó bị chèn ép.

Những điều có thể mong đợi

Đối với hầu hết các vận động viên ném bóng đã trải qua phẫu thuật, tiên lượng là khá tốt: phần lớn họ có thể trở lại mức độ ném như trước, mặc dù cần phải kiên nhẫn. Phẫu thuật tái tạo dây chằng (phẫu thuật Tommy John) thường đòi hỏi quá trình hồi phục kéo dài khoảng một năm hoặc lâu hơn trước khi có thể ném trở lại ở mức thi đấu; trong khi đối với những bệnh nhân phù hợp, việc phẫu thuật sửa chữa kết hợp với đeo nẹp nội bộ có thể giúp hồi phục nhanh hơn. Dù là phương pháp nào, việc trở lại thi đấu phụ thuộc chủ yếu vào một chương trình phục hồi chức năng dài hạn và có hệ thống, chứ không chỉ nhờ vào ca phẫu thuật; việc vội vàng trở lại hoạt động là nguyên nhân chính gây ra những biến chứng.

Việc hồi phục không phải lúc nào cũng diễn ra như mong đợi: một số ít vận động viên không thể trở lại mức độ thi đấu trước đó; các triệu chứng liên quan đến dây thần kinh hoặc tình trạng của mảnh ghép đôi khi vẫn cần được theo dõi và điều trị tiếp. Tuy nhiên, với việc chẩn đoán chính xác, lựa chọn phương pháp điều trị thích hợp và sự kiên trì trong quá trình phục hồi, kết quả điều trị cho chấn thương này được xem là một trong những thành công nổi bật nhất trong phẫu thuật thể thao.

Khi nào nên đi khám bác sĩ

  • Đau phía trong khuỷu tay khi ném bóng, đặc biệt nếu cơn đau tái phát nhiều lần hoặc ngày càng nặng hơn theo thời gian.
  • Giảm tốc độ ném, độ chính xác hoặc sức bền mà việc nghỉ ngơi không giúp cải thiện được.
  • Cảm giác “lụp bụp” hoặc đau dữ dội ở phía trong khuỷu tay trong lúc ném; sau đó cánh tay cảm thấy yếu hoặc mất vững.
  • Tê, ngứa ran hoặc yếu ở ngón áp út và ngón út.
  • Đau phía trong khuỷu tay ở trẻ em hoặc thanh thiếu niên hay tham gia các hoạt động ném bóng: vùng sụn tăng trưởng ở trẻ có thể bị tổn thương do lực ném, vì vậy cần được đánh giá kịp thời.

Advanced reading: the deeper science (optional)

Phần này đi sâu hơn mức cần thiết để bạn tự đưa ra quyết định điều trị. Chấn thương dây chằng bên ulna đáng để đọc thêm, bởi ca phẫu thuật mà hầu hết mọi người đều biết đến: phẫu thuật Tommy John — giờ đây không còn là lựa chọn duy nhất nữa. Phương pháp thay thế mới này đòi hỏi một thứ khác thay vì kết quả điều trị: đó là thời gian.

Phẫu thuật tái tạo là phương pháp được công nhận rộng rãi, và kết quả điều trị khá ổn định

Phẫu thuật tái tạo bao gồm việc thay thế dây chằng bị rách bằng mảnh ghép gân; thường là gân palmaris longus lấy từ cẳng tay của chính bệnh nhân. Trên 6.671 bệnh nhân, hầu hết các bác sĩ phẫu thuật đã sử dụng mảnh ghép gân tự thân theo cấu hình hình số 8 thông qua kỹ thuật Jobe được cải tiến. Tỷ lệ phải phẫu thuật lại khi dùng mảnh ghép gân từ người hiến dường có vẻ cao hơn so với dùng mảnh ghép tự thân; tuy nhiên các tác giả cho rằng điều này có thể do số lượng nghiên cứu về mảnh ghép gân từ người hiến dường còn hạn chế chứ không hẳn là sự khác biệt thực sự [1].

Sau thời gian theo dõi trung bình 1.104 bệnh nhân, phẫu thuật tái tạo mang lại kết quả tốt về mặt lâm sàng cũng như theo đánh giá của bệnh nhân; tỷ lệ biến chứng và phải phẫu thuật lại cũng thấp. Các vận động viên bóng chày cho biết họ có tỷ lệ trở lại thi đấu ở mức độ như trước chấn thương rất cao, đồng thời duy trì được sự nghiệp thể thao lâu dài [2]. Tỷ lệ trở lại thi đấu nằm trong khoảng 80% đến 90%, và phần lớn các vận động viên có thể tham gia thi đấu đầy đủ sau 12 đến 18 tháng [3].

Phương pháp thay thế này nhanh hơn; đó là toàn bộ lý do để ưu tiên nó

Trong những trường hợp dây chằng bị bong ra khỏi một đầu gắn kết và mô dây chằng vẫn còn trong tình trạng tốt, người ta có thể khâu vá thay vì phải thay thế dây chằng; ngày càng nhiều ca phẫu thuật sử dụng thêm nẹp nội soi để hỗ trợ quá trình khâu vá này.

Sự khác biệt nằm ở thời gian hồi phục. Phương pháp khâu vá có nẹp hỗ trợ cho phép vận động viên trở lại thi đấu sau trung bình 6 tháng; tỷ lệ thành công trong việc trở lại thi đấu ở những bệnh nhân được chọn lựa phù hợp là rất cao. Ngược lại, phương pháp tái tạo dây chằng đòi hỏi thời gian hồi phục từ 12 đến 18 tháng [4]. Một nghiên cứu năm 2025 cho thấy phương pháp khâu vá ngày càng được ưa chuộng; phương pháp này được đánh giá là an toàn và hiệu quả, với tỷ lệ biến chứng thấp cùng tỷ lệ vận động viên trở lại thi đấu cao [5].

Đối với các vận động viên trung học hay đại học có một “cửa sổ cơ hội” thi đấu cụ thể, việc rút ngắn thời gian nghỉ dưỡng xuống một nửa không phải là vấn đề nhỏ; điều đó có thể quyết định việc họ mất đi một mùa giải hay thậm chí thay đổi cả sự nghiệp. Tuy nhiên, cụm từ “được chọn lựa phù hợp” lại mang ý nghĩa rất quan trọng: việc khâu vá chỉ thực hiện được khi dây chằng bị bong ra theo mô hình nhất định và phần dây chằng còn lại vẫn khỏe mạnh; tình trạng này chỉ chiếm một tỷ lệ nhỏ trong tổng số các ca chấn thương, và được xác định thông qua hình ảnh chẩn đoán cùng kết quả khám phẫu thuật, chứ không phải do mong muốn của bệnh nhân.

Không phải mọi trường hợp rách dây chằng đều cần phẫu thuật

Việc điều trị phụ thuộc vào mức độ rách dây chằng. Các trường hợp rách một phần mức độ nhẹ thường được điều trị không cần phẫu thuật, còn phương pháp tái tạo dây chằng mới là tiêu chuẩn điều trị cho các trường hợp rách hoàn toàn [3]. Dây chằng này chịu tải chủ yếu do lực kéo valgus trong các động tác ném vượt đầu; vì vậy, tình trạng rách một phần vẫn có thể tương thích với các hoạt động sinh hoạt hàng ngày trong thời gian dài, và chỉ bộc lộ triệu chứng khi người bệnh bắt đầu lại các động tác ném.

Phẫu thuật chỉnh sửa không mang lại kết quả như mong đợi

Nếu ca phẫu thuật tái tạo đầu tiên thất bại, cần hạ thấp kỳ vọng đối với ca phẫu thuật thứ hai. Phẫu thuật chỉnh sửa tái tạo là thủ thuật hiếm gặp; kết quả của nó không khả quan bằng phẫu thuật tái tạo lần đầu, đồng thời việc chẩn đoán, kỹ thuật thực hiện và quá trình phục hồi cũng trở nên phức tạp hơn nhiều [6].

Sự chênh lệch này là lý do quan trọng nhất để thực hiện đúng đắn ca phẫu thuật đầu tiên cùng quá trình phục hồi đi kèm, bao gồm cả việc hoàn thành chương trình phục hồi dù có vẻ kéo dài một cách không cần thiết đối với một dây chằng đã được thay thế. Mảnh ghép phải dần hình thành lại thành dây chằng; quá trình này tuân theo nhịp độ sinh học chứ không phụ thuộc vào cảm giác tại khớp khuỷu tay.

In all other cases the Licensor expressly reserves any right to collect such royalties, including when the Licensed Material is used other than for NonCommercial purposes.

Section 3 -- License Conditions.

Your exercise of the Licensed Rights is expressly made subject to the following conditions.

1. If You Share the Licensed Material (including in modified form), You must:

Section 6 -- Term and Termination.

a. This Public License applies for the term of the Copyright and Similar Rights licensed here. However, if You fail to comply with this Public License, then Your rights under this Public License terminate automatically.

b. Where Your right to use the Licensed Material has terminated under Section 6(a), it reinstates:

1. automatically as of the date the violation is cured, provided it is cured within 30 days of Your discovery of the violation; or

2. upon express reinstatement by the Licensor.

For the avoidance of doubt, this Section 6(b) does not affect any right the Licensor may have to seek remedies for Your violations of this Public License.

c. For the avoidance of doubt, the Licensor may also offer the Licensed Material under separate terms or conditions or stop distributing the Licensed Material at any time; however, doing so will not terminate this Public License.

d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.
