Gãy xương ngón tay là tình trạng gãy một trong những xương dài nằm trong lòng bàn tay – những xương nối cổ tay với các ngón tay, chính là những khúc xương mà bạn có thể cảm nhận được dưới dạng các khớp ngón tay. Dạng gãy phổ biến nhất thường xảy ra sau khi đấm vào vật cứng; nó ảnh hưởng đến xương nằm phía sau khớp ngón tay út. Tình trạng này rất phổ biến nên còn có biệt danh là “gãy xương võ sĩ quyền anh”.

Thông thường sẽ có một thời điểm rõ ràng gây ra chấn thương: đấm vào vật cứng, ngã đập tay xuống đất, hoặc bị chèn ép hoặc vặn xoắn tay. Ngay sau đó, mặt sau bàn tay sẽ đau nhức, sưng và bầm tím; khớp ngón tay có thể trông bị dẹt hoặc “lõm” so với phía còn lại. Việc nắm tay gây đau đớn, và sức nắm của bạn cũng trở nên yếu đi. Gãy xương có thể xảy ra ở cổ xương (vị trí ngay phía sau khớp ngón tay, nơi thường gặp ở chứng gãy xương kiểu võ sĩ quyền anh), dọc theo thân xương (phần giữa của xương), hoặc ở gốc xương (gần cổ tay). Sau khi gãy, các đầu xương bị gãy có thể bị lệch thành góc, ngắn lại hoặc xoay tròn.

Một mức độ lệch góc nhất định thường không sao, vì bàn tay có khả năng thích nghi tốt; đặc biệt là các xương bàn tay thuộc ngón áp út và ngón út có thể chịu được mức độ uốn cong khá lớn mà không gây ra vấn đề gì, do các khớp ở những ngón này vốn có tính linh hoạt cao. Tuy nhiên, bàn tay lại không chịu được tình trạng xoay tròn (chúng tôi gọi là hiện tượng quay). Những gì chúng tôi có thể làm

Bác sĩ Kieran Hirpara là bác sĩ phẫu thuật chi trên tại Bệnh viện tư nhân Mater Rockhampton; ông sẽ hướng dẫn bạn trong suốt quá trình điều trị, từ khâu đánh giá ban đầu cho đến khi hồi phục. Thông thường, bệnh nhân được bác sĩ đa khoa giới thiệu đến phòng khám của chúng tôi; nếu vật lý trị liệu viên khuyên bạn nên gặp chúng tôi, bạn vẫn cần có giấy giới thiệu từ bác sĩ đa khoa để được hưởng hoàn trả chi phí từ Medicare. Tin vui là hầu hết các trường hợp gãy xương ngón tay đều lành tốt mà không cần phẫu thuật.

Đối với đa số các ca gãy xương (bao gồm cả các ca gãy xương ngón tay do đấm đá), cách xử lý rất đơn giản: cố định và vận động sớm. Tùy vào mức độ gãy xương, chúng tôi có thể dùng nẹp nhẹ, bó bột, hoặc chỉ cần buộc ngón tay bị thương vào ngón bên cạnh; sau đó khuyến khích bạn bắt đầu vận động nhẹ nhàng ngay từ sớm. Việc vận động sớm là điều có chủ đích: nó giúp ngăn ngừa tình trạng cứng khớp; các nghiên cứu cho thấy việc bó cố định đơn giản kết hợp vận động sớm cũng mang lại hiệu quả tương đương việc bó bột dày cho các ca gãy xương ngón tay thông thường [4, 5].

Phẫu thuật chỉ được chỉ định cho những ca gãy xương không thể tự lành nếu không can thiệp. Các lý do chính để phẫu thuật gồm: ngón tay bị xoay trục, xương bị gãy với góc lệch nhiều hoặc bị ngắn lại, nhiều xương ngón tay bị gãy cùng lúc, vết gãy xương lan vào khớp, hoặc có vết thương hở tại chỗ gãy. Khi tiến hành phẫu thuật, chúng tôi có thể dùng các dây kim loại nhỏ luồn qua da (dây Kirschner), hoặc đặt một tấm nẹp nhỏ cùng ốc vít qua đường mổ. Nhiều ca phẫu thuật này có thể thực hiện khi bệnh nhân hoàn toàn tỉnh táo: tay được gây tê cục bộ để không đau, bệnh nhân vẫn tỉnh táo và thoải mái; nhờ đó chúng tôi có thể yêu cầu bệnh nhân vận động ngón tay trên bàn mổ để xác nhận rằng độ xoay đã được chỉnh sửa đúng mức trước khi kết thúc ca phẫu thuật.

Những điều bạn có thể mong đợi

Hầu hết các trường hợp gãy xương ngón tay đều liền lại trong khoảng bốn đến sáu tuần, và bàn tay sẽ có thể thực hiện hầu hết các hoạt động bình thường ngay sau đó. Ngay cả khi xương liền lại với một chút cong vênh, khớp ngón tay có thể trông kém nổi bật hơn trước một chút, nhưng bàn tay vẫn hoạt động hoàn toàn bình thường.

Dù bạn có phẫu thuật hay không, yếu tố quan trọng giúp đảm bảo kết quả điều trị tốt là việc vận động bàn tay sau đó: vận động nhẹ nhàng, sớm, và dưới sự hướng dẫn cần thiết của chuyên viên trị liệu tay. Khi nào nên đi khám bác sĩ

Ngoài ra, dạng biến dạng khiến nhiều người lo lắng nhất – đó là tình trạng các khớp ngón tay bị “dẹt” đi – lại là dạng biến dạng mà bàn tay có thể thích nghi tốt nhất.

Tại sao góc lệch có thể được chấp nhận, và tại sao mức độ chấp nhận lại khác nhau tùy ngón tay

Gãy xương ở cổ xương bàn tay thường khiến đầu xương nghiêng xuống phía lòng bàn tay, khiến khớp ngón tay mất đi độ nhô ra tự nhiên. Tình trạng này trông có vẻ bất thường và dễ bị hiểu lầm là cần phải chỉnh sửa.

Yếu tố quyết định việc góc lệch có gây vấn đề hay không là mức độ di động còn lại của khớp tương ứng ở phía cổ tay. Các xương bàn tay của ngón trỏ và ngón giữa về cơ bản được cố định tại gốc, nên góc lệch ở những vị trí này khó được bù đắp. Trong khi đó, các xương bàn tay của ngón áp út và ngón út lại nằm trên các khớp có khả năng xoay lớn, giúp hấp thụ một mức độ góc lệch đáng kể mà không gây mất chức năng vận động.

Đó là lý do tại sao gãy xương bàn tay ngón út – dạng chấn thương phổ biến nhất – có thể chấp nhận được mức góc lệch lớn hơn nhiều so với cùng chấn thương ở hai ngón tay bên cạnh; cũng vì thế mà dạng biến dạng trông khá nghiêm trọng này thường được điều trị mà không cần phẫu thuật.

Xoay trục là dạng biến dạng thực sự đáng lưu ý

Ngoại lệ duy nhất là hiện tượng xoay trục; điều này cần được hiểu rõ vì rất dễ bị bỏ sót trên phim X-quang. Khi xương ngón tay bị gãy và liền lại với mức độ xoay trục nhất định, các ngón tay sẽ chéo vào nhau khi nắm tay – hiện tượng này gọi là “hiện tượng kéo cắt”, gây cản trở khả năng nắm vật và không thuyên giảm theo thời gian.

Để đánh giá mức độ xoay trục, ta yêu cầu bệnh nhân nắm tay lại rồi quan sát xem các móng tay có thẳng hàng và các ngón tay có hướng về phía nhau không; không cần đo đạc trên phim X-quang. Trường hợp gãy xương mà góc độ trên phim vẫn chấp nhận được nhưng khi khám lâm sàng lại thấy hiện tượng kéo cắt thì cần phải chỉnh sửa; ngược lại thì thường không cần thiết.

Đối với các trường hợp cần cố định xương, bằng chứng khoa học còn hạn chế

Đối với những ca gãy xương thực sự cần phẫu thuật, phương pháp cố định bằng vít nội tủy – tức là đặt vít dọc theo thân xương từ vị trí khớp ngón – ngày càng được ưa chuộng vì tránh được việc đặt nẹp kim loại dưới các gân duỗi. Sau khi khảo sát 1,014 bệnh nhân, kết quả cho thấy các biến chứng sau khi cố định gãy xương ngón tay bằng vít nội tủy tương đối hiếm gặp [1].

Tuy nhiên, các bằng chứng so sánh lại không mạnh mẽ như mức độ phổ biến của phương pháp này. Một nghiên cứu trước đó trên 502 bệnh nhân kết luận rằng phương pháp cố định nội tủy có thể có vai trò nhất định trong điều trị một số loại gãy xương ngón tay, nhưng chưa có khuyến nghị chắc chắn nào có thể đưa ra dựa trên các nghiên cứu hiện có [2].

Cả hai kết luận trên đều có thể đúng: kỹ thuật này an toàn, nhưng chưa được chứng minh là vượt trội hơn các phương pháp khác. Xét rằng phần lớn các ca gãy xương ngón tay đều lành tốt mà không cần phẫu thuật, quyết định quan trọng nhất vẫn là có nên mổ hay không, chứ không phải chọn loại vật liệu cố định nào.

Tình trạng liền xương sai vị trí có thể được điều chỉnh, điều này làm thay đổi cách đánh giá lâm sàng

Trong những trường hợp xương gãy liền ở tư thế không đúng, gây ra các vấn đề chức năng thực sự, phương pháp cắt xương chỉnh hình là một lựa chọn khả thi. Các ca cắt xương chỉnh hình hầu như luôn liền xương mà không gặp biến chứng, phần lớn bệnh nhân đều có sự cải thiện hoặc việc chỉnh sửa dị dạng được thực hiện, đồng thời vận động ngón tay cũng được cải thiện — mặc dù nghiên cứu cũng ghi nhận rằng nhiều bệnh nhân, dù đã có tiến triển tốt, vẫn còn một số hạn chế nhất định [3].

Đây là một lập luận hợp lý để áp dụng phương pháp điều trị không phẫu thuật trong những trường hợp chưa rõ ràng. Việc không phẫu thuật ngay lúc này không có nghĩa là “chấp nhận mọi hậu quả có thể xảy ra”; thực chất là “phẫu thuật sau này, đối với số ít bệnh nhân thực sự cần can thiệp”, với một thủ thuật có tỷ lệ liền xương thành công cao.
