Những triệu chứng mà bạn đang gặp phải

Đau ở phía ngón út của cổ tay (phía xương trụ) rất phổ biến và có nhiều nguyên nhân có thể gây ra. Trang này nói về nguyên nhân cơ học quan trọng nhất: một tình trạng gọi là hội chứng va chạm xương trụ (còn gọi là chạm trụ – cổ tay).

Cơn đau thường nằm sâu ở rìa ngoài của cổ tay, lệch về phía mu bàn tay. Cơn đau thường nặng hơn khi bạn nắm chặt tay, xoay cẳng tay, hoặc nghiêng cổ tay về phía ngón út, vì vậy những hoạt động thường ngày như mở nắp lọ, vặn chìa khóa, dùng tua vít, vắt khăn, hoặc chống tay để đẩy người đứng dậy khỏi ghế đều có thể gây đau. Nhiều người còn nhận thấy một tiếng “tách” hoặc tiếng “cục” ở cổ tay khi thực hiện một số động tác nhất định, và cổ tay có thể nhức mỏi sau một ngày sử dụng nhiều. Dồn trọng lượng lên cổ tay (khi làm động tác plank, hít đất, hoặc đẩy một cánh cửa nặng) là yếu tố làm nặng điển hình.

Tình trạng này thường tăng dần theo thời gian chứ không bắt đầu từ một chấn thương đơn lẻ, tuy nhiên nó cũng có thể xuất hiện sau khi bị gãy cổ tay.

Chuyện gì đang thực sự xảy ra

Có hai xương cẳng tay chạy đến cổ tay: xương quay (phía ngón cái) và xương trụ (phía ngón út). Để cổ tay vận động trơn tru, đầu của hai xương này nên nằm ở mức gần như ngang nhau. Trong hội chứng va chạm xương trụ, xương trụ tương đối quá dài so với xương quay, một tình trạng mà các bác sĩ gọi là biến thiên xương trụ dương (xương trụ dài hơn xương quay).

Một số người đơn giản là sinh ra đã có xương trụ hơi dài. Ở những người khác, tình trạng này xuất hiện về sau: thường gặp nhất là khi cổ tay bị gãy liền lại hơi ngắn đi, khiến xương quay ngắn hơn trước một chút và xương trụ, trên thực tế, nhô cao hơn.

Dù nguyên nhân là gì, phần dài thêm nhỏ đó khiến đầu xương trụ nghiến vào các xương cổ tay nhỏ nằm cạnh nó (xương nguyệt và xương tháp) mỗi khi cổ tay chịu lực. Giữa các xương này có một đệm sụn gọi là TFCC (phức hợp sụn sợi tam giác), có vai trò như bộ giảm chấn. Sự va chạm lặp đi lặp lại dần làm mòn lớp đệm này, có thể làm rách TFCC, và theo thời gian gây bầm dập, thậm chí hình thành các nang nhỏ trong xương bên dưới. Chính quá trình hao mòn này gây ra cơn đau, tiếng kêu lục cục và cảm giác khó chịu khi cầm nắm và xoay cổ tay. Những gì chúng tôi có thể làm

Người bệnh thường được bác sĩ đa khoa giới thiệu đến phòng khám của chúng tôi; nếu chuyên viên vật lý trị liệu đề nghị bạn đến gặp chúng tôi, bạn vẫn cần giấy giới thiệu từ bác sĩ đa khoa để đủ điều kiện nhận khoản hoàn trả Medicare. Tin vui là hầu hết mọi người đều ổn định với điều trị không phẫu thuật, và đó luôn là bước khởi đầu của chúng tôi.

Làm dịu tình trạng. Các bước đầu tiên rất đơn giản: tạm thời điều chỉnh các hoạt động gây tải lên phía xương trụ của cổ tay (đặc biệt là cầm nắm mạnh, vặn xoắn và chống đỡ trọng lượng), đeo nẹp cổ tay để cho khớp nghỉ ngơi, và dùng thuốc chống viêm để giảm đau và sưng. Đôi khi tiêm steroid vào khớp giúp phá vỡ vòng luẩn quẩn của tình trạng viêm.

Xác nhận chẩn đoán. Song song với điều trị, chúng tôi thường chụp X-quang để đo chính xác sự thẳng hàng giữa hai xương (đôi khi dùng tư thế chụp đặc biệt khi “nắm chặt tay” để làm rõ tình trạng va chạm), và thường chụp MRI để quan sát sụn, TFCC và phần xương bên dưới. Đôi khi, việc nội soi quan sát bên trong khớp (nội soi khớp) được dùng để xác nhận vấn đề và điều trị ngay trong cùng lần đó.

Nếu tình trạng kéo dài. Khi điều trị không phẫu thuật đúng cách vẫn không làm tình trạng ổn định, mục tiêu của phẫu thuật là giảm áp lực lên phía xương trụ của cổ tay. Phương án lâu đời và được kiểm chứng nhất là phẫu thuật cắt xương rút ngắn xương trụ, trong đó bác sĩ phẫu thuật lấy bỏ một lát nhỏ xương trụ để đưa xương về đúng chiều dài và cố định bằng một nẹp vít nhỏ trong khi xương lành. Một lựa chọn ít xâm lấn hơn ở những trường hợp được chọn lọc là thủ thuật “wafer” qua nội soi, mài bớt một lượng nhỏ ở ngay đầu tận của xương trụ qua phẫu thuật nội soi. Cả hai đều có tác dụng bằng cách ngăn xương nghiến vào cổ tay.

Những điều bạn có thể mong đợi

Với hầu hết mọi người, hội chứng va chạm xương trụ là vấn đề chúng tôi có thể kiểm soát được. Các biện pháp không phẫu thuật làm ổn định phần lớn các trường hợp, và các triệu chứng giảm dần khi khớp không còn bị quá tải lặp đi lặp lại.

Khi cần phẫu thuật, rút ngắn xương trụ là một phẫu thuật đáng tin cậy, và các nghiên cứu theo dõi người bệnh trong nhiều năm cho thấy giảm đau tốt, lâu dài và mức độ hài lòng cao. Xương cần thời gian để lành, vì vậy sẽ có một giai đoạn hồi phục vài tuần trong nẹp hoặc bột trong khi chỗ cắt xương liền lại, sau đó trở lại dần việc cầm nắm và chịu lực. Những điều chính cần lưu ý là đôi khi xương liền chậm hơn dự kiến, và nẹp vít nhỏ đôi khi có thể sờ thấy dưới da và thỉnh thoảng được tháo ra sau khi mọi thứ đã lành. Nhìn chung, đại đa số mọi người trở lại các hoạt động hàng ngày và hết đau.

Khi nào nên đi khám

  • Đau cổ tay phía xương trụ không thuyên giảm sau vài tuần, hoặc cứ tái phát khi cầm nắm và vặn xoắn.
  • Đau xuất hiện sau một lần gãy cổ tay trước đây, đặc biệt nếu từ đó cổ tay chưa bao giờ thấy thật sự ổn.
  • Tiếng “tách”, tiếng “cục” hoặc cảm giác vướng kẹt dai dẳng ở phía ngón út của cổ tay, nhất là khi chống đỡ trọng lượng.
  • Cơn đau khiến bạn không làm được công việc hoặc các hoạt động hàng ngày. Đây là lúc nên tìm hiểu điều gì đang gây ra cơn đau và có thể làm gì.

Phần này đi xa hơn mức bạn cần cho các quyết định điều trị của chính mình. Đau cổ tay phía xương trụ đáng để đọc thêm vì đây là một vị trí chứ không phải một chẩn đoán, nhiều vấn đề khác nhau cùng nằm trong một vùng nhỏ, và vì với vấn đề phổ biến nhất trong số đó, phẫu thuật mới hơn và nhỏ hơn hiện có số liệu biến chứng tốt hơn so với phẫu thuật đã được áp dụng lâu nay.

Vì sao góc này của cổ tay khó xử lý

Chỉ trong vài centimet có TFCC, khớp quay – trụ dưới, gân cơ duỗi cổ tay trụ cùng bao gân của nó, khớp đậu – tháp, dây chằng nguyệt – tháp, và điểm mà xương trụ tiếp xúc với các xương cổ tay. Mỗi cấu trúc đều có thể gây đau tại cùng một chỗ, một số cấu trúc thường có bất thường trên hình ảnh ở những người không có triệu chứng, và có thể có hơn một cấu trúc bị liên quan cùng lúc.

Đó là lý do việc đánh giá ở vùng này dựa nhiều hơn vào xác định chính xác vị trí đau và các nghiệm pháp kích phát so với những vùng khác của cổ tay, và vì sao một kết quả chụp ghi nhận nhiều cấu trúc bất thường là kết cục thường gặp nhưng không mấy hữu ích. Đó cũng là lý do những nguyên nhân ít gặp hơn, chẳng hạn va chạm khớp giữa cổ tay, thỉnh thoảng được mô tả là những yếu tố góp phần bị bỏ sót [1].

Hội chứng va chạm xương trụ: vấn đề cơ học với giải pháp cơ học

Nguyên nhân cấu trúc phổ biến nhất là hội chứng va chạm xương trụ, khi xương trụ hơi dài so với xương quay, khiến các xương cổ tay bị ép vào đầu xương trụ khi cầm nắm và khi cẳng tay xoay úp lòng bàn tay xuống. Theo thời gian, điều này làm mòn TFCC và sụn của các xương mà nó tiếp xúc.

Điều trị là rút ngắn xương trụ, hoặc bằng cách cắt và rút ngắn thân xương, hoặc bằng cách lấy bỏ một lát xương mỏng (wafer) ở đầu xương, việc này có thể thực hiện qua nội soi. Tổng hợp 956 người bệnh, thủ thuật wafer qua nội soi và mổ mở có thể là những lựa chọn khả thi thay cho phẫu thuật cắt xương rút ngắn xương trụ vốn phổ biến hơn, dù tính ưu việt lâm sàng chưa được xác lập vào thời điểm đó [2].

Dữ liệu gần đây hơn đã tiến thêm một bước. Khi so sánh thủ thuật wafer qua nội soi với phẫu thuật cắt xương rút ngắn xương trụ trên 311 người bệnh, cả hai đều hiệu quả, nhưng nhóm wafer có ít biến chứng hơn và tỷ lệ mổ lại thấp hơn, khiến các tác giả mô tả đây có thể là lựa chọn ưu việt hơn [3].

Vì sao phẫu thuật cắt xương tích lũy nhiều biến chứng

Lý do không phải vì phẫu thuật được thực hiện kém. Rút ngắn thân xương đòi hỏi phải cắt xương, cố định bằng nẹp vít trong khi xương lành, và chấp nhận rằng xương có thể không liền. Một tổng quan gộp trên 1,423 người bệnh đã xem xét riêng tình trạng không liền xương, cho thấy không có khác biệt giữa đường cắt ngang và đường cắt chéo, và kết luận rằng vì vậy không nên chọn giữa hai kiểu cắt dựa trên nguy cơ không liền xương [4].

Thủ thuật wafer tránh hoàn toàn nhóm vấn đề đó bằng cách lấy bỏ xương ở đầu tận thay vì cắt rời thân xương. Không có ổ gãy nào cần liền và thường không có nẹp vít nào cần tháo sau này, và đó chính là nguồn gốc của sự khác biệt về tỷ lệ mổ lại.

Đổi lại, thủ thuật wafer chỉ lấy bỏ được một lượng xương hạn chế và đòi hỏi khớp quay – trụ dưới phải còn nguyên vẹn, vì vậy rút ngắn thân xương vẫn cần thiết khi cần chỉnh nhiều hơn hoặc khi khớp đó cũng bị liên quan.

Hệ quả thực tế đối với người bệnh

Vì có nhiều cấu trúc có thể là thủ phạm, một điều trị nhắm vào sai cấu trúc sẽ không hiệu quả dù được thực hiện tốt đến đâu. Đây là vùng mà tiêm chẩn đoán, chẩn đoán hình ảnh có mục tiêu và đôi khi nội soi khớp thể hiện rõ giá trị của chúng trước khi đưa ra quyết định, và là nơi cần đạt được một chẩn đoán cụ thể, thay vì “đau cổ tay phía xương trụ”, trước khi lên kế hoạch phẫu thuật.
