Những triệu chứng bạn đang gặp phải

Cơ tam đầu là cơ lớn nằm ở mặt sau cánh tay. Gân của cơ này bám vào mỏm khuỷu tay (olecranon) và giúp bạn duỗi thẳng cánh tay. Khi gân này bị rách khỏi xương, chấn thương thường xảy ra trong một khoảnh khắc: thường là do ngã khi tay duỗi thẳng, hoặc khi nâng/hất vác vật nặng mà đột nhiên mất kiểm soát.

Lúc đó, bạn có thể cảm thấy đau nhói hoặc nghe thấy tiếng “pop” ở mặt sau khuỷu tay. Sau đó, vùng này sẽ sưng lên, xuất hiện vết bầm tím và trở nên nhạy cảm khi chạm vào. Dấu hiệu rõ rệt nhất là cảm giác yếu khi cố gắng duỗi thẳng cánh tay chống lại trọng lực hoặc lực cản: việc đẩy cửa, nhấc người khỏi ghế, hoặc nâng vật lên cao có thể trở nên khó khăn hoặc không thể thực hiện được. Đây là chấn thương hiếm gặp (gân cơ tam đầu là gân ít bị rách nhất trong cơ thể), và đó cũng là lý do tại sao nó thường bị bỏ sót trong lần khám đầu tiên.

Điều gì đang thực sự xảy ra

Gân cơ tam đầu bị đứt, một phần hoặc hoàn toàn, khỏi chỗ bám trên mỏm khuỷu. Trường hợp đứt hoàn toàn có nghĩa là cơ không còn gắn chặt vào xương nữa; vì vậy lực mà cơ tạo ra không thể truyền đi để duỗi thẳng khuỷu tay. Trường hợp đứt một phần vẫn còn một vài sợi gân bám vào xương, nên sức mạnh của cơ vẫn còn phần nào.

Những trường hợp đứt gân này gần như luôn xảy ra khi gân chịu tải trong lúc cơ đang co mạnh: ví dụ, cánh tay bị ép phải gập lại ngay lúc người bệnh cố gắng duỗi thẳng hoặc giữ vững nó. Một số yếu tố làm tăng nguy cơ gân bị đứt: tình trạng này phổ biến hơn nhiều ở nam giới, đặc biệt là những người thường nâng tạ; việc sử dụng steroid đồng hóa làm suy yếu gân và được coi là yếu tố nguy cơ; các mũi tiêm steroid quanh khuỷu tay, tình trạng viêm màng hoạt dịch khuỷu tay (mỏm khuỷu) tái phát, cùng một số bệnh lý như suy thận mạn tính cũng khiến gân trở nên dễ đứt hơn. Thỉnh thoảng, một mảnh xương nhỏ cũng bị kéo ra cùng với gân; điều này thực ra giúp bác sĩ xác nhận chẩn đoán qua phim X-quang.

Những biện pháp chúng tôi có thể áp dụng

Bác sĩ Kieran Hirpara là bác sĩ phẫu thuật chi trên tại Bệnh viện tư nhân Mater Rockhampton; ông là người dẫn dắt phương pháp điều trị chứng bệnh này của chúng tôi. Thông thường, bệnh nhân được bác sĩ đa khoa giới thiệu đến phòng khám của chúng tôi; nếu vật lý trị liệu viên khuyên bạn nên đến gặp chúng tôi, bạn vẫn cần có giấy giới thiệu từ bác sĩ đa khoa để được hưởng mức hoàn trả chi phí từ Medicare. Bước đầu tiên là chẩn đoán bệnh. Bên cạnh khám lâm sàng, chúng tôi chụp X-quang để tìm kiếm những mảnh xương nhỏ đặc trưng; siêu âm hoặc chụp MRI thường được dùng để xác định xem tình trạng rách gân là một phần hay toàn bộ và mức độ gân bị tụt lại sau đó.

Phương pháp điều trị phụ thuộc vào mức độ rách gân cũng như mức độ sử dụng cánh tay của bệnh nhân:

  • Những trường hợp rách gân một phần nhỏ (dưới khoảng một nửa độ dài gân) ở những người có nhu cầu vận động thấp thường có thể điều trị mà không cần phẫu thuật: bệnh nhân sẽ đeo nẹp trong một thời gian với khuỷu tay hơi gập, sau đó dần dần luyện tập vận động và tăng cường sức mạnh cơ.
  • Các trường hợp rách gân toàn bộ, rách gân một phần khiến người bệnh yếu đi, hoặc rách gân ở những người năng động hoặc có nhu cầu vận động cao thường được điều trị tốt nhất bằng phẫu thuật nhằm nối lại gân với xương. Phẫu thuật này được thực hiện bằng cách luồn các mũi khâu chắc chắn qua những lỗ nhỏ trên xương, hoặc dùng các dụng cụ đặc biệt để gắn gân trở lại vị trí ban đầu. Việc phẫu thuật sớm, trước khi gân bị sẹo và ngắn lại, sẽ mang lại kết quả tốt nhất.

Những điều có thể mong đợi

Khi chấn thương gân tam đầu được phẫu thuật sửa chữa kịp thời, kết quả thường rất tốt: hầu hết bệnh nhân đều lấy lại khả năng duỗi khuỷu tay một cách mạnh mẽ và ổn định, rồi có thể trở lại làm việc và tham gia các hoạt động thể thao. Sau phẫu thuật, khuỷu tay sẽ được bảo vệ trong vài tuần, sau đó mới dần dần được vận động; việc tăng cường sức mạnh cơ bắp chỉ được thực hiện sau đó, nên việc trở lại các hoạt động nâng vác nặng mất vài tháng chứ không phải vài tuần. Đội ngũ bác sĩ của bạn sẽ hướng dẫn mức độ phục hồi phù hợp.

Giống như mọi ca phẫu thuật, vẫn có một số rủi ro cần lưu ý. Những rủi ro chính là tình trạng gân bị tách rời trở lại (tái rách), vì vậy việc bảo vệ khuỷu tay trong giai đoạn đầu và việc tăng dần mức tải trọng là rất quan trọng; ngoài ra còn có vấn đề về quá trình lành da tại vùng khuỷu tay – nơi lớp mô da khá mỏng. Tình trạng cứng khớp và sức mạnh cơ giảm tạm thời trong quá trình phục hồi là những biểu hiện bình thường trong quá trình hồi phục.

Khi nào nên đi khám bác sĩ

  • Đau đột ngột hoặc có tiếng “pop” ở phía sau khuỷu tay sau khi ngã hoặc gắng sức nâng vật nặng, kèm theo sưng và bầm tím. Hãy đi khám ngay lập tức.
  • Gặp khó khăn hoặc yếu sức khi duỗi thẳng khuỷu tay chống lại trọng lực hoặc lực cản; hoặc không thể dùng cánh tay đó để đứng dậy khỏi ghế.
  • Có vùng lõm mềm mà bạn có thể cảm nhận được ngay phía trên khớp khuỷu tay.
  • Đau và yếu ở phía sau khuỷu tay kéo dài mà không thuyên giảm, ngay cả khi bạn vẫn có thể duỗi thẳng cánh tay; tình trạng rách một phần gân cũng có thể gây ra triệu chứng tương tự. Việc khám sớm sẽ mở ra nhiều phương án điều trị và mang lại kết quả tốt hơn.

Advanced reading: the deeper science (optional)

Phần này đi sâu hơn mức cần thiết để bạn đưa ra các quyết định điều trị. Chấn thương rách cơ tam đầu cánh tay vùng xa xứng đáng được đọc thêm vì một lý do khá đặc biệt: tình trạng này hiếm gặp đến mức cơ sở bằng chứng lâm sàng còn rất hạn chế; việc biết rõ mức độ hạn chế đó cũng rất hữu ích khi bạn cân nhắc những thông tin được trình bày.

Bằng chứng hiện có còn hạn chế; điều này cũng được ghi nhận một cách trung thực

Cuộc tổng quan hệ thống quy mô lớn nhất về các phương pháp điều trị phẫu thuật đã bao gồm 560 bệnh nhân [1]. Kết quả cho thấy những bệnh nhân được phẫu thuật sửa chữa có cải thiện về kết quả điều trị; tuy nhiên, nguy cơ tái rách hoặc biến chứng cũng ở mức độ vừa phải. Điều quan trọng hơn là sự khác biệt về kiểu dạng tổn thương, các kỹ thuật phẫu thuật cũng như các tiêu chí đánh giá kết quả khiến việc xác định phương pháp nào hiệu quả hơn trở nên khó khăn [1].

Để so sánh, các thông tin về các bệnh lý vai và đầu gối trên trang web này dựa trên dữ liệu từ hàng chục nghìn bệnh nhân. Trong khi đó, đối với các bệnh lý này, toàn bộ các nghiên cứu hiện có chỉ ghi nhận vài trăm bệnh nhân thuộc nhiều dạng tổn thương khác nhau. Điều đó không có nghĩa là các phương pháp điều trị không hiệu quả; chỉ là những khẳng định chắc chắn về sự khác biệt nhỏ giữa các phương pháp đều chưa có cơ sở vững chắc, và cần được xem xét một cách thận trọng khi gặp phải.

Hiệu quả của phương pháp phẫu thuật sửa chữa đã được ghi nhận rõ ràng

Những bằng chứng có tính nhất quán cao nhất lại liên quan đến câu hỏi chính. Trên 318 bệnh nhân – bao gồm hơn 90% là quân nhân đang tại ngũ, vận động viên và những người lao động bình thường – tất cả đều đã có thể quay trở lại tập luyện thể thao hoặc tiếp tục công việc sau khi được phẫu thuật sửa chữa cơ tam đầu cánh tay [2].

Nhóm bệnh nhân này rất quan trọng trong việc diễn giải kết quả. Đây là những người phải làm việc trong môi trường thể chất nặng nhọc; việc họ có thể quay lại làm việc được xác nhận một cách khách quan chứ không phải chỉ dựa trên lời khai cá nhân. Vì vậy, con số này mang tính thuyết phục cao hơn so với các con số tương tự thu được từ nhóm dân số nói chung.

Chỉ có một so sánh kỹ thuật cho thấy sự khác biệt

Mặc dù nhìn chung các kỹ thuật phẫu thuật không thể phân biệt được với nhau, nhưng có một so sánh lại cho thấy sự khác biệt. Trên 56 bệnh nhân, các kết quả được ghi nhận bởi bệnh nhân đều tích cực đối với cả phương pháp sử dụng móc khâu và phương pháp sửa chữa qua đường hầm xương; tuy nhiên phương pháp sửa chữa bằng móc khâu cho kết quả vượt trội đáng kể trong các xét nghiệm đo sức mạnh cơ, tỷ lệ biến chứng và tỷ lệ tái rách [3].

Số lượng mẫu nghiên cứu còn hạn chế; vì vậy kết quả này nên được xem như một tín hiệu tiềm năng chứ không phải kết luận chắc chắn. Đây cũng chính là điều mà các nghiên cứu tổng quan quy mô lớn hơn muốn nhấn mạnh về toàn bộ các tài liệu nghiên cứu trong lĩnh vực này.

Tại sao việc chẩn đoán sớm lại quan trọng trong trường hợp này

Cơ tam đầu là cơ duy nhất có chức năng duỗi khuỷu tay chống lại lực cản; vì vậy, tình trạng đứt hoàn toàn cơ này có ý nghĩa chức năng rất lớn, điều mà các trường hợp rách một phần không có. Các trường hợp đứt hoàn toàn nên được điều trị bằng phương pháp phẫu thuật nối lại cơ, đặc biệt là khi thực hiện ngay trong giai đoạn cấp tính [4].

Có hai yếu tố khiến chấn thương này dễ bị bỏ sót. Thứ nhất, khả năng duỗi cánh tay vẫn còn được duy trì nhờ các điểm bám cơ còn lại, nên cánh tay không có vẻ như mất chức năng hoàn toàn; thứ hai, tình trạng sưng và bầm tím ở phía sau khuỷu tay thường được cho là hậu quả của chính cú ngã. Phương pháp kiểm tra thực tế là yêu cầu bệnh nhân duỗi cánh tay chống lại lực hấp dẫn; việc không thể giữ cánh tay thẳng khi có lực đẩy xuống, chứ không phải chỉ cảm giác đau nhức, mới là dấu hiệu quan trọng cần chú ý.
