Khớp ức đòn là khớp nhỏ nằm phía trước ngực, nơi đầu trong của xương đòn gặp gỡ xương ức. Bạn có thể cảm nhận được khớp này dưới dạng một chỗ lồi nhỏ ngay dưới gốc cổ, cách đường giữa cơ thể vài centimet. Khớp này thường dễ bị bỏ qua cho đến khi gặp vấn đề.

Các vấn đề liên quan đến khớp này thường biểu hiện theo vài cách khác nhau. Một số người cảm thấy đau âm ỉ hoặc nhạy cảm ngay tại chỗ lồi đó; thường kèm theo chút sưng, và cơn đau tăng lên khi vươn tay lên cao, nâng vật nặng, đẩy mạnh hoặc nằm nghiêng về phía đó. Những người khác lại cảm nhận được khớp phát ra tiếng “cạch”, trượt hoặc dịch chuyển khi thực hiện một số động tác; đôi khi có một khối lồi lõm xuất hiện rõ rệt. Chuyện gì đang xảy ra thực sự

Khớp ức đòn là khớp xương duy nhất nối toàn bộ cánh tay và vai với phần còn lại của bộ xương. Mọi chuyển động của cánh tay đều được giữ vững nhờ vào khớp nhỏ này, vì vậy nó được cấu tạo rất chắc chắn và được bao quanh bởi các dây chằng mạnh mẽ. Có nhiều yếu tố có thể ảnh hưởng đến khớp này.

Viêm khớp (sự hao mòn bề mặt khớp) là vấn đề phổ biến nhất. Theo thời gian, lớp sụn mịn dần mỏng đi, khớp có thể sưng lên và gây đau khi vận động. Tình trạng này thường gặp ở phụ nữ trung niên, thường không do chấn thương nào gây ra; về bản thân nó thì chỉ gây phiền toái chứ không nguy hiểm.

Tình trạng không ổn định không do chấn thương là khi khớp bị trượt hoặc tạm thời lệch ra khỏi vị trí bình thường mà không có chấn thương thực sự; nguyên nhân thường là do các dây chằng có độ lỏng tự nhiên. Tình trạng này phổ biến nhất ở những người trẻ tuổi, có độ co giãn khớp cao (dễ bị giãn khớp); xương đòn thường bị dịch chuyển về phía trước (gọi là trượt trước), khiến xuất hiện một khối lồi ra khi người ta vận động theo một cách nhất định.

Trật khớp do chấn thương xảy ra khi một lực mạnh đẩy xương đòn hoàn toàn ra khỏi khớp. Nếu xương đòn bị trật về phía trước, tình trạng này gây đau và trông có vẻ bất thường nhưng hiếm khi nguy hiểm. Trường hợp đáng lo ngại nhất là trật khớp về phía sau, khi xương đòn bị đẩy ngược về phía sau, phía sau xương ức, vào vùng chứa khí quản, thực quản và các mạch máu lớn trong lồng ngực. Tình trạng này hiếm gặp nhưng có thể nghiêm trọng. Chúng ta sẽ nói thêm về điều này ở phần cuối.

Những gì chúng tôi có thể làm

Phương pháp điều trị của chúng tôi được hướng dẫn bởi Tiến sĩ Kieran Hirpara, bác sĩ phẫu thuật chi trên tại Bệnh viện Mater Private Rockhampton. Thông thường, bệnh nhân được bác sĩ đa khoa giới thiệu đến phòng khám của chúng tôi; nếu nhà vật lý trị liệu khuyên bạn nên đến gặp chúng tôi, bạn vẫn cần có giấy giới thiệu từ bác sĩ đa khoa để được hưởng mức hoàn trả từ Medicare. Sau đó, chúng tôi tiến hành đánh giá kỹ lưỡng nhằm xác nhận chẩn đoán. Đối với các vấn đề kéo dài lâu năm, chúng tôi thường bắt đầu bằng các biện pháp điều trị không phẫu thuật; chỉ cân nhắc phẫu thuật nếu các biện pháp này không mang lại hiệu quả giảm đau đáng kể.

Tin vui là hầu hết các vấn đề liên quan đến khớp ức đòn đều tự khỏi mà không cần phẫu thuật.

Đối với tình trạng viêm khớp và tình trạng mất ổn định khớp hướng về phía trước, phương pháp điều trị ưu tiên là các biện pháp không phẫu thuật và thường mang lại kết quả tốt: điều chỉnh các hoạt động gây kích phát triệu chứng, sử dụng thuốc giảm đau và chống viêm đơn giản, cùng vật lý trị liệu nhằm ổn định khớp và tăng cường sức mạnh cơ vùng xung quanh. Nếu khớp bị viêm gây đau vẫn tiếp tục gây phiền toái sau khi đã áp dụng các biện pháp trên đúng mức, việc tiêm corticosteroid vào khớp có thể làm giảm đau và giúp xác nhận chắc chắn rằng khớp chính là nguyên nhân gây đau.

Phẫu thuật chỉ là giải pháp ngoại lệ, chứ không phải là phương pháp thường dùng. Nó chỉ được áp dụng cho những bệnh nhân mà cơn đau hoặc tình trạng mất ổn định không thuyên giảm dù đã điều trị không phẫu thuật đúng cách. Tùy theo tình trạng bệnh, phẫu thuật có thể nhằm ổn định khớp (tái tạo các dây chằng để giữ xương đòn ở đúng vị trí) hoặc, trong trường hợp viêm khớp nặng, cắt bỏ phần đầu xương đòn bị thoái hóa nhằm loại bỏ bề mặt gây đau ra khỏi khớp. Các phương pháp này đều được cân nhắc kỹ lưỡng, vì khớp này nằm ngay cạnh nhiều cấu trúc quan trọng trong vùng ngực.

Trường hợp trật khớp ức đòn hướng về phía sau là tình huống không thể chậm trễ. Thông thường cần tiến hành đặt lại khớp khẩn cấp; do phía sau khớp có nhiều cấu trúc quan trọng, thủ thuật này thường được thực hiện trong phòng mổ với sự hỗ trợ sẵn sàng của bác sĩ phẫu thuật ngực hoặc mạch máu, phòng trường hợp có biến chứng xảy ra.

Những điều có thể mong đợi

Đối với bệnh viêm khớp và tình trạng mất ổn định khớp thường gặp, tiên lượng là khả quan. Nhờ việc thay đổi thói quen vận động, tập vật lý trị liệu và thời gian, phần lớn bệnh nhân cảm thấy thoải mái đủ để tiếp tục sinh hoạt bình thường; nhiều người thậm chí không cần điều trị gì thêm. Các khớp linh hoạt thường dần ổn định trở lại khi các cơ xung quanh trở nên khỏe mạnh hơn và bệnh nhân biết được những chuyển động nào nên tránh.

Khi cần phẫu thuật, thủ thuật này có thể mang lại hiệu quả rất tốt đối với những bệnh nhân phù hợp; tuy nhiên quá trình hồi phục đòi hỏi sự kiên nhẫn: cần bảo vệ khớp trong một thời gian rồi mới dần dần trở lại các hoạt động bình thường trong vài tháng. Bác sĩ phẫu thuật sẽ giải thích chi tiết kế hoạch điều trị phù hợp với tình trạng của bạn.

Trường hợp trật khớp phía sau được xử lý kịp thời thường có kết quả tốt sau khi khớp đã được đưa trở lại vị trí bình thường. Yếu tố quan trọng nhất là tốc độ: cần đưa bệnh nhân đi khám và chỉnh lại khớp càng sớm càng tốt.

Khi nào nên gặp bác sĩ

Hãy đến gặp bác sĩ nếu bạn gặp phải các triệu chứng sau:

  • Đau, sưng hoặc cảm giác nhạy cảm kéo dài ở khớp phía trước ngực mà không thuyên giảm; hoặc có khối u di chuyển ra vào liên tục.
  • Khớp có cảm giác không ổn định, thường xuyên bị trật ra khỏi vị trí khi thực hiện một số động tác, gây hạn chế khả năng vận động.
  • Đau sau chấn thương ở vùng phía trước vai hoặc ngực; đặc biệt nếu khớp có vẻ ngoài hoặc cảm giác không bình thường.

Hãy coi đây là tình trạng khẩn cấp: gọi xe cứu thương hoặc đến ngay khoa cấp cứu gần nhất nếu sau khi bị chấn thương mạnh hoặc tác động mạnh vào vai hoặc ngực, bạn có các triệu chứng sau:

  • Khó thở, cảm giác bị ép ngực hoặc nghẹn thở, hoặc thay đổi giọng nói.
  • Khó nuốt hoặc đau khi nuốt.
  • Sưng, đổi màu da, lạnh hoặc tê dại ở cánh tay, hoặc mạch máu ở cánh tay đó yếu đi.

Những triệu chứng này có thể là dấu hiệu của tình trạng trật khớp vai phía sau gây chèn ép khí quản, thực quản hoặc các mạch máu lớn phía sau xương ức. Tình trạng này ít gặp nhưng cần được đánh giá ngay tại bệnh viện. Đừng chờ đợi xem các triệu chứng có tự hết hay không.

Advanced reading: the deeper science (optional)

Phần này đi sâu hơn so với những gì bạn cần biết để đưa ra quyết định điều trị. Các vấn đề liên quan đến khớp ức đòn rất đáng để tìm hiểu kỹ, bởi vì một dạng biến chứng của chúng là tình trạng khẩn cấp thực sự; tình trạng này có thể trông bình thường khi quan sát từ bên ngoài, và khoảng thời gian mà phương pháp điều trị đơn giản nhất có hiệu quả chỉ tính bằng vài giờ mà thôi.

Trật khớp phía sau mới là loại đáng lo ngại

Khớp ức đòn nằm ở vùng gốc cổ, nơi xương đòn gặp xương ức. Ngay phía sau khớp này là các mạch máu lớn, khí quản và thực quản.

Trật khớp phía trước sẽ đẩy xương đòn ra phía trước, tạo thành một khối lồi rõ rệt. Tình trạng này trông có vẻ nghiêm trọng nhưng thường lành tính. Ngược lại, trật khớp phía sau đẩy xương đòn về phía sau, hướng về các cấu trúc nêu trên; do đó hầu như không có dấu hiệu gì thấy được bên ngoài, đôi khi chỉ có một chỗ lõm nhẹ. Mức độ nghiêm trọng của bệnh lại tỷ lệ nghịch với những biểu hiện bên ngoài.

Các triệu chứng gợi ý trật khớp phía sau gồm: khó nuốt, thở gấp, thay đổi giọng nói, hoặc tĩnh mạch ở tay hoặc cổ bị giãn nở. Nếu có bất kỳ triệu chứng nào sau chấn thương vùng phía trước vai, cần được đánh giá ngay lập tức. Chụp X-quang thông thường không đáng tin cậy đối với khớp này; thường phải chụp CT mới đủ chính xác.

Cửa sổ thời gian 48 giờ

Khi phát hiện ra tình trạng trật khớp phía sau, xương đòn thường có thể được đưa trở lại vị trí ban đầu mà không cần mở khớp. Trên 140 bệnh nhi tuổi thiếu niên, cả phương pháp chỉnh nắn kín và phương pháp phẫu thuật đều cho thấy hiệu quả rất cao; trong đó việc chỉnh nắn kín đạt hiệu quả tốt nhất nếu được thực hiện trong vòng chưa đầy 48 giờ sau chấn thương [1].

Đây là một con số cụ thể và có thể áp dụng thực tế. Sau khoảng hai ngày, các mô mềm bắt đầu hình thành quanh mảnh xương bị trật, khiến khả năng thành công của phương pháp chỉnh nắn kín giảm dần; từ đó biến một thủ thuật đơn giản thành ca phẫu thuật mở gần các mạch máu lớn. Đây là lý do quan trọng nhất để thực hiện chẩn đoán hình ảnh sớm thay vì chỉ theo dõi mà không can thiệp, nhất là khi cơ chế chấn thương phù hợp.

Do vị trí gần các cấu trúc quan trọng này, việc chỉnh nắn trật khớp phía sau thường được thực hiện khi có sự hỗ trợ của đội ngũ phẫu thuật tim mạch – không phải vì thủ thuật này thường gặp biến chứng, mà vì hậu quả nếu có sai sót sẽ xảy ra ngay lập tức.

Phương pháp tái tạo khớp cho tình trạng mất ổn định là hiệu quả; kỹ thuật thực hiện không quyết định kết quả

Đối với tình trạng mất ổn định tái phát hoặc mãn tính, người ta tiến hành tái tạo khớp bằng cách sử dụng mảnh ghép gân. Trên 164 bệnh nhân, phương pháp này đã giúp cải thiện đáng kể các chỉ số sức khỏe do bệnh nhân tự đánh giá cũng như khả năng trở lại sinh hoạt thường ngày; tỷ lệ biến chứng vẫn ở mức thấp. Điều đáng chú ý là hiệu quả này không phụ thuộc vào kỹ thuật được áp dụng, dù là phương pháp khoan đơn lớp hay hai lớp xương, hoặc việc sử dụng mảnh ghép từ người hiến hay mảnh ghép tự thân [2].

Khi nhiều biến thể kỹ thuật khác nhau đều cho kết quả tương đương nhau, có thể hiểu rằng việc tạo ra một cấu trúc khớp ổn định mới là yếu tố quan trọng hơn so với cách thức thực hiện kỹ thuật đó.

Viêm khớp ở đây có thể điều trị được nhưng thường bị bỏ qua

Những thay đổi thoái hóa tại khớp này là nguyên nhân thực sự gây đau ở vùng gốc cổ, thường bị bỏ qua; đặc biệt là ở phụ nữ trung niên. Nguyên nhân này thường bị cho là xuất phát từ chính vùng cổ. Việc phẫu thuật trên 107 bệnh nhân cho thấy đây là phương pháp an toàn, giúp giảm đau hiệu quả và mang lại tỷ lệ hài lòng cao; tuy nhiên các tài liệu y khoa hiện tại chưa xác định rõ liệu phẫu thuật mở hay phẫu thuật nội soi là phương pháp ưu việt hơn [3].

Điều quan trọng cần lưu ý là tình trạng đau nhức khu trú, kéo dài đúng tại vị trí khớp này cần được đề cập rõ ràng; vì đây là vùng nhỏ dễ bị bỏ sót khi bệnh nhân mô tả triệu chứng là đau vai hoặc đau cổ.

4. If You Share Adapted Material You produce, the Adapter's License You apply must not prevent recipients of the Adapted Material from complying with this Public License.

Section 4 -- Sui Generis Database Rights.

Where the Licensed Rights include Sui Generis Database Rights that apply to Your use of the Licensed Material:

a. for the avoidance of doubt, Section 2(a)(1) grants You the right to extract, reuse, reproduce, and Share all or a substantial portion of the contents of the database for NonCommercial purposes only;

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

Section 6 -- Term and Termination.

a. This Public License applies for the term of the Copyright and Similar Rights licensed here. However, if You fail to comply with this Public License, then Your rights under this Public License terminate automatically.

b. Where Your right to use the Licensed Material has terminated under Section 6(a), it reinstates:

1. automatically as of the date the violation is cured, provided it is cured within 30 days of Your discovery of the violation; or

2. upon express reinstatement by the Licensor.

For the avoidance of doubt, this Section 6(b) does not affect any right the Licensor may have to seek remedies for Your violations of this Public License.

c. For the avoidance of doubt, the Licensor may also offer the Licensed Material under separate terms or conditions or stop distributing the Licensed Material at any time; however, doing so will not terminate this Public License.

d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.

d. Nothing in this Public License constitutes or may be interpreted as a limitation upon, or waiver of, any privileges and immunities that apply to the Licensor or You, including from the legal processes of any jurisdiction or authority.
