Giấc ngủ kém là một trong những vấn đề phổ biến nhất – và thường bị xem nhẹ nhất – trong quá trình hồi phục sau phẫu thuật chi trên. Cơn đau, việc không thể tìm được tư thế thoải mái cho cánh tay, cùng chiếc đai cố định đều góp phần khiến giấc ngủ không ngon. Điều an ủi là tình trạng này chỉ tạm thời, diễn ra theo một khuôn mẫu khá dễ dự đoán, và có rất nhiều biện pháp mà bạn có thể áp dụng để cải thiện nó.

Những điều bạn có thể mong đợi

Giấc ngủ thường sẽ tạm thời xấu đi trước khi cải thiện, và thời điểm cụ thể phụ thuộc vào loại phẫu thuật mà bạn trải qua.

  • Sau phẫu thuật sửa chữa cơ chóp xoay: Giấc ngủ thường tệ nhất vào khoảng hai tuần sau mổ, khi vùng vai còn rất đau nhức. Hầu hết bệnh nhân sẽ phục hồi lại giấc ngủ như trước khi mổ vào khoảng sáu tuần, và tình trạng rối loạn giấc ngủ hầu như biến mất sau ba tháng.
  • Sau phẫu thuật thay khớp vai: Bạn nên biết rằng phần lớn bệnh nhân cũng gặp khó khăn về giấc ngủ trước khi phẫu thuật; tình trạng mất ngủ phổ biến ở hầu hết những người bị viêm khớp vai. Tại sao giấc ngủ trở nên khó khăn sau phẫu thuật

Có nhiều yếu tố góp phần gây ra tình trạng này:

  • Việc nằm xuống làm tăng áp lực lên chi vừa được phẫu thuật; vì vậy cảm giác đau nhức thường dữ dội hơn vào ban đêm so với ban ngày.
  • Bạn không thể nằm ở tư thế quen thuộc. Việc ngủ trên bên có vết mổ gây khó chịu hoặc bị hạn chế; những người có thói quen ngủ nghiêng đặc biệt cần thời gian để thích nghi.
  • Chiếc đai treo cánh tay giúp bảo vệ cánh tay nhưng khiến việc lăn người và tìm tư thế ngủ thoải mái trở nên khó khăn.

Những điều hữu ích

Tư thế nằm

  • Sau phẫu thuật vai, nhiều bệnh nhân ngủ ngon hơn khi nằm ở tư thế nâng cao: dùng ghế bành có tựa lưng hoặc xếp nhiều gối phía sau lưng và đầu trong vài tuần đầu. Nằm phẳng thường khiến cơn đau vai trở nên tồi tệ hơn.
  • Đặt cánh tay vừa phẫu thuật lên gối để tránh việc cánh tay bị kéo căng hoặc thõng xuống.
  • Đeo nẹp cánh tay vào ban đêm đúng như hướng dẫn (xem Cách đeo nẹp). Điều này giúp bảo vệ vết khâu và ngăn bạn lăn người nằm đè lên cánh tay.

Giảm đau trước khi đi ngủ

  • Uống thuốc giảm đau theo lịch trình để thuốc phát huy tác dụng trước khi bạn nằm xuống, chứ không phải sau khi cơn đau làm bạn tỉnh giấc (xem Cách kiểm soát cơn đau sau phẫu thuật).
  • Trong tuần đầu hoặc hai tuần đầu sau phẫu thuật, chỉ cần dùng một viên thuốc giảm đau đặc trị trước khi ngủ là đủ.
  • Chườm đá và nâng cao cánh tay vào buổi tối giúp giảm sưng và cơn đau ban đêm (xem Nóng hay lạnh?).

Thói quen ngủ

  • Duy trì giờ đi ngủ và thức dậy đều đặn, ngay cả khi bạn ở nhà và ít vận động.
  • Tránh uống cà phê sau bữa trưa và hạn chế sử dụng các thiết bị màn hình trong giờ trước khi ngủ.
  • Một căn phòng tối, mát mẻ và yên tĩnh cùng thói quen thư giãn nhẹ nhàng trước khi ngủ thực sự rất hữu ích.

Thuốc và giấc ngủ

Phần lớn các rối loạn giấc ngủ đều được điều trị hiệu quả nhất bằng cách kiểm soát cơn đau thay vì dùng thuốc ngủ. Chúng tôi cố gắng tránh việc sử dụng các thuốc an thần thường xuyên, vì chúng nhanh chóng mất tác dụng và không tương thích tốt với các loại thuốc giảm đau mạnh.

Sau phẫu thuật vai, hiện nay chúng tôi thường kê đơn melatonin trong khoảng sáu tuần đầu. Nếu bạn mắc chứng ngưng thở khi ngủ

Nếu bạn đang sử dụng máy CPAP hoặc đã được chẩn đoán mắc chứng ngưng thở khi ngủ do tắc nghẽn, vui lòng thông báo cho chúng tôi và bác sĩ gây mê trước khi phẫu thuật, đồng thời mang theo máy của bạn đến bệnh viện. Những bệnh nhân mắc chứng ngưng thở khi ngủ có tỷ lệ biến chứng y tế ngắn hạn hơi cao hơn sau phẫu thuật thay khớp vai; tuy nhiên kết quả dài hạn của khớp vẫn không khác biệt. Việc kiểm soát tình trạng bệnh một cách tốt trong giai đoạn phẫu thuật là yếu tố then chốt.

Creative Commons Attribution-NonCommercial 4.0 International Public License

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

Section 5 -- Disclaimer of Warranties and Limitation of Liability.

a. Section 8 -- Interpretation.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable.
