Nhịn ăn trước phẫu thuật: Những gì có thể ăn và uống, cùng lý do

Tại sao bạn phải ngừng ăn trước khi gây mê, khi nào bạn vẫn có thể nhấp chút nước, và điều gì có thể xảy ra nếu bạn không nhịn ăn. Những quy tắc nhịn ăn rõ ràng giúp dạ dày trống rỗng và giữ phổi của bạn an toàn trong quá trình phẫu thuật.

Vào ngày phẫu thuật, có hai yếu tố quan trọng nhất ảnh hưởng đến sự an toàn của bạn: loại thuốc gây mê được sử dụng và việc bạn tuân thủ nghiêm ngặt các hướng dẫn nhịn ăn đã được đưa ra.

Tại sao chúng tôi yêu cầu bạn nhịn ăn uống

Thuốc gây mê toàn thân làm giãn các cơ vốn có chức năng ngăn không cho thức ăn trong dạ dày trào ngược lên cổ họng. Nếu dạ dày còn đầy, những chất này có thể tràn vào phổi khi bạn mất ý thức, gây nguy hiểm nghiêm trọng. Việc nhịn ăn uống giúp làm rỗng dạ dày, nhờ đó tình trạng này rất khó xảy ra. Các quy định tiêu chuẩn như sau:

  • Tuyệt đối không ăn hay uống gì kể từ thời điểm nhịn ăn được ghi trong email hướng dẫn phẫu thuật. Không ăn bất kỳ loại thực phẩm nào, không uống sữa, nước, trà hay cà phê; cũng không được ăn kẹo hay nhai kẹo cao su. Thời điểm nhịn ăn được tính từ giờ nhập viện: thường là 7 giờ trước đó; nếu nhập viện vào sáng sớm thì thời điểm nhịn ăn là nửa đêm.
  • Nếu bạn đang sử dụng các loại thuốc tiêm điều trị tiểu đường hoặc giảm cân (Ozempic, Wegovy, Mounjaro, Trulicity, Byetta), hãy tiếp tục dùng thuốc như bình thường; tuy nhiên thời gian nhịn ăn sẽ là 24 giờ trước giờ phẫu thuật. Các loại thuốc này làm chậm quá trình rỗng dạ dày, nên thời gian nhịn ăn thông thường là chưa đủ.
  • Nếu ca phẫu thuật chỉ sử dụng thuốc gây tê tại chỗ, trong email sẽ ghi rõ rằng bạn không cần phải nhịn ăn uống.

Nếu sáng hôm phẫu thuật bạn vẫn phải dùng các loại thuốc huyết áp hoặc thuốc chống động kinh, hãy uống chúng cùng một chút nước, trừ khi bác sĩ gây mê có chỉ định khác. Các loại thuốc điều trị tiểu đường không được dùng vào sáng ngày phẫu thuật; thuốc aspirin và clopidogrel vẫn tiếp tục sử dụng; các thuốc chống đông máu như warfarin thì phải ngưng dùng 48 giờ trước phẫu thuật. Các loại gây mê mà chúng tôi sử dụng

Hầu hết các ca phẫu thuật chi trên đều được thực hiện theo một trong ba phương pháp gây mê, đôi khi kết hợp nhiều phương pháp:

  • Gây mê toàn thân: Bạn sẽ hoàn toàn ngủ say. Bác sĩ gây mê sẽ đưa thuốc gây mê vào cơ thể bạn qua một ống tiêm nhỏ ở mu bàn tay, và theo dõi tình trạng của bạn suốt quá trình phẫu thuật.
  • Gây tê vùng: Thuốc gây tê được tiêm quanh các dây thần kinh ở vùng cổ hoặc nách, khiến toàn bộ cánh tay mất cảm giác trong vài giờ. Bạn cũng có thể được dùng thuốc an thần để ngủ trong lúc phẫu thuật, nhưng không cần dùng máy thở.
  • Gây tê tại chỗ: Đối với các ca phẫu thuật nhỏ (như giải phóng ngón tay cò), vùng cần phẫu thuật thường được gây tê và bệnh nhân vẫn tỉnh táo; đôi khi có thể dùng thêm thuốc an thần nhẹ.

Vào ngày phẫu thuật, bác sĩ gây mê sẽ trao đổi với bạn về các phương pháp gây mê có thể áp dụng. Việc lựa chọn phương pháp phụ thuộc vào loại phẫu thuật, tình trạng sức khỏe tổng quát và sở thích cá nhân của bạn.

Những điều bạn có thể trải qua trong ngày phẫu thuật

Bạn sẽ đến nơi 1–2 giờ trước giờ phẫu thuật đã định. Một điều dưỡng sẽ kiểm tra việc bạn đã nhịn ăn chưa, đặt đường truyền tĩnh mạch và giải thích lại mẫu đồng ý phẫu thuật lần cuối. Bác sĩ gây mê sẽ trực tiếp gặp bạn trước khi bạn được đưa vào phòng mổ.

Sau phẫu thuật, bạn sẽ tỉnh dậy tại khu vực hồi sức. Nếu bạn được gây tê vùng, cánh tay sẽ cảm thấy nặng và tê cứng; điều này là bình thường và kéo dài 8–24 giờ. Cần bảo vệ cánh tay bị tê: hãy đeo nẹp cố định, không tựa người lên cánh tay và giữ ấm cho nó. Cảm giác sẽ dần trở lại, thường đi kèm hiện tượng tê rần như kim châm.

Buồn nôn, buồn ngủ và đau họng (do ống thở) là những triệu chứng thường gặp trong vài giờ đầu sau phẫu thuật. Hãy uống nước từ từ, ăn nhẹ và nghỉ ngơi.

Phần này đi sâu hơn so với những gì bạn cần biết để đưa ra quyết định điều trị. Việc tìm hiểu về gây mê là rất đáng giá; bởi hai vấn đề mà bệnh nhân thường muốn được đảm bảo nhất – đó là nguy cơ tổn thương thần kinh do gây mê và việc “tỉnh táo” có an toàn hơn so với việc ngủ mê hay không – đều có những con số thống kê cụ thể đi kèm. Kết quả thu được lại không hề giống như những gì trực giác chúng ta thường nghĩ.

Nguy cơ gặp phải các vấn đề thần kinh lâu dài do gây tê vùng

Hầu hết các bệnh nhân trải qua phẫu thuật chi trên đều được đề nghị thực hiện gây tê vùng, và hầu hết đều thắc mắc liệu kim tiêm có thể gây tổn thương thần kinh hay không. Đây là câu hỏi hoàn toàn hợp lý; câu trả lời trung thực cần phân biệt rõ: tình trạng thay đổi cảm giác tạm thời không phải là hiếm gặp, còn tổn thương thần kinh kéo dài thì ngược lại.

Một phân tích tổng hợp dựa trên 12.532 bệnh nhân đã xem xét các triệu chứng thần kinh sau phẫu thuật khi áp dụng gây tê vùng cho chi trên. Điểm cốt lõi của nghiên cứu này mang tính phương pháp luận nhưng rất quan trọng đối với bạn: mức độ nguy cơ tổng thể thay đổi rất nhiều tùy thuộc vào cách đo lường [1]. Các thử nghiệm ngẫu nhiên được đánh giá một cách chủ động, trong đó người ta trực tiếp hỏi từng bệnh nhân, cho thấy tỷ lệ tổn thương thần kinh cao hơn nhiều so với các nghiên cứu hồi cứu – nơi chỉ những bệnh nhân phàn nàn rõ ràng mới được tính vào kết quả [1].

Kết luận thực tiễn: Nếu một nghiên cứu đưa ra con số rất thấp về nguy cơ tổn thương thần kinh, hãy hỏi xem liệu người ta có thực sự tìm kiếm các triệu chứng đó hay không. Các triệu chứng thần kinh tự khỏi là khá phổ biến; chúng nên được coi là hiện tượng bình thường chứ không phải biến chứng. Ngược lại, các triệu chứng kéo dài mới thực sự hiếm gặp.

Gây mê tại chỗ không nhất thiết an toàn hơn

Có một quan niệm phổ biến, không chỉ ở bệnh nhân mà còn ở nhiều người khác, rằng việc tránh sử dụng gây mê toàn thân chắc chắn sẽ an toàn hơn, đặc biệt là ở người cao tuổi, và đặc biệt là về nguy cơ gây rối loạn trí nhớ sau phẫu thuật.

Bằng chứng trực tiếp nhất về điều này đến từ các ca phẫu thuật điều trị gãy xương hông; vấn đề này đã được nghiên cứu quy mô lớn vì đối tượng bệnh nhân thường già yếu. Kết quả tổng hợp từ 3,736 bệnh nhân cho thấy gây mê tại chỗ không làm giảm đáng kể tình trạng rối loạn trí nhớ hay tỷ lệ tử vong sau phẫu thuật so với gây mê toàn thân [2].

Đây là một kết quả thực sự gây ngạc nhiên và cần được diễn giải một cách thận trọng. Kết quả này không có nghĩa là việc lựa chọn phương pháp gây mê không quan trọng; nó chỉ có nghĩa là việc lựa chọn nên dựa trên tình trạng tim, phổi, đường thở của bệnh nhân cũng như loại phẫu thuật cần thực hiện, chứ không nên dựa trên niềm tin chung rằng một phương pháp nào đó an toàn hơn. Việc gây mê toàn thân hiện đại ở những bệnh nhân khỏe mạnh là rất an toàn; ngược lại, phương pháp gây mê tại chỗ cũng không phải là giải pháp “vô hại” hoàn toàn.

Khi nào các kỹ thuật gây tê thực sự phát huy tác dụng

Lý do chính để ưu tiên gây tê vùng không phải là độ an toàn, mà là việc kiểm soát cơn đau. Một phân tích hệ thống trên 1.872 bệnh nhân trải qua phẫu thuật nối lại xương cánh tay gần cho thấy gây tê vùng là phương án hiệu quả để giảm đau sau mổ [3]; những giờ đầu sau phẫu thuật vai là giai đoạn đau dữ dội nhất, và kỹ thuật gây tê vùng lại giúp kiểm soát đau đúng trong khoảng thời gian này.

Cũng có nhiều phương pháp gây tê khác nhau. Đối với phẫu thuật nội soi vai, một nghiên cứu trên 1.382 bệnh nhân cho thấy việc gây tê dây thần kinh trên mỏm vai không mang lại hiệu quả kiểm soát đau tốt hơn so với gây tê vùng liên thang cơ, nhưng lại có ít tác dụng phụ hơn [4]. Gây tê vùng liên thang cơ thường khiến cánh tay yếu đi tạm thời và có thể ảnh hưởng đến dây thần kinh chi phối cơ hoành; điều này đặc biệt quan trọng nếu chức năng hô hấp của bệnh nhân đã kém. Khi gặp tình huống này, việc lựa chọn phương pháp gây tê khác không phải là sự thỏa hiệp, mà là lựa chọn phù hợp hơn với từng ca phẫu thuật.

Tại sao phải nhịn ăn trước phẫu thuật

Việc nhịn ăn tồn tại vì một lý do duy nhất: khi bệnh nhân bị gây mê, đường thở không thể tự bảo vệ mình; vì vậy các chất chứa trong dạ dày có thể trào ngược xuống phổi. Tình trạng này rất hiếm gặp nhưng rất nguy hiểm; quy định nhịn ăn chính là biện pháp phòng ngừa rẻ tiền và hiệu quả.

Sai lầm phổ biến là coi việc “không được ăn uống gì” như một cuộc thi. Việc nhịn ăn kéo dài không làm tăng độ an toàn; ngược lại, tình trạng mất nước khiến bệnh nhân cảm thấy mệt mỏi hơn, việc đặt ống tiêm cũng khó khăn hơn, và hoàn toàn không có lợi gì cho dạ dày. Các loại chất lỏng trong suốt thì thoát khỏi dạ dày nhanh hơn thức ăn; một số bệnh viện cho phép bệnh nhân uống chúng gần giờ phẫu thuật hơn. Chúng tôi yêu cầu bệnh nhân nhịn ăn, nhịn uống trong bảy giờ – chỉ một lần và theo một quy định duy nhất – vì lý do thực tiễn: lịch mổ đôi khi được sắp xếp sớm hơn; bệnh nhân đã tuân thủ đúng thời gian nhịn ăn sẽ được chuyển sang ca phẫu thuật sớm hơn thay vì phải chờ đợi. Tuyệt đối không bắt đầu nhịn ăn sớm hơn thời gian được chỉ định: mỗi giờ nhịn thêm chỉ gây cảm giác khát nước mà thôi. Nếu ca phẫu thuật bị hoãn vào ngày hôm đó, hãy hỏi bác sĩ thay vì ngồi đó khát nước với suy nghĩ rằng nhịn càng lâu càng tốt.

iii. a notice that refers to this Public License;

iv. a notice that refers to the disclaimer of warranties;

v. a URI or hyperlink to the Licensed Material to the extent reasonably practicable;

b. indicate if You modified the Licensed Material and retain an indication of any previous modifications; and

a. for the avoidance of doubt, Section 2(a)(1) grants You the right to extract, reuse, reproduce, and Share all or a substantial portion of the contents of the database for NonCommercial purposes only;

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

Section 5 -- Disclaimer of Warranties and Limitation of Liability.

a.
