Vết thương là gì và da liền lại như thế nào?

Vết thương đơn giản là sự đứt gãy của lớp rào chắn bảo vệ da. Quá trình liền vết thương diễn ra qua nhiều giai đoạn chồng chập lên nhau: đầu tiên máu ngừng chảy và cục máu đông/màng vảy hình thành; sau đó vùng da bị tổn thương bị viêm (đỏ, nóng, hơi sưng) do các tế bào dọn dẹp xuất hiện; tiếp theo cơ thể lấp đầy khoảng trống bằng mô mới và kéo hai mép vết thương lại gần nhau; cuối cùng, trong nhiều tháng, mô mới này được tái cấu trúc và trở nên chắc khỏe thành sẹo.

Một vết mổ được khâu lại cẩn thận (hai mép vết thương được khâu gọn) sẽ liền nhanh hơn và để lại sẹo mịn hơn so với vết thương để mở, để mô mới tự hình thành từ dưới lên; tuy nhiên cả hai đều trải qua cùng một quá trình liền vết thương.

Cơ chế hình thành và trưởng thành của sẹo

Sẹo là vùng mô mà cơ thể tạo ra để khép lại vết thương. Những vết sẹo mới thường nổi lên, cứng và có màu đỏ hoặc hồng vì chứa nhiều mạch máu mới hình thành cùng các sợi collagen sắp xếp lộn xộn. Trong vòng vài tháng đến một năm sau, sẹo dần được tái cấu trúc: các mạch máu giảm đi và collagen sắp xếp lại, khiến sẹo trưởng thành trở nên bằng phẳng hơn, nhạt màu hơn, mềm mại hơn và ít gây chú ý hơn. Đó là lý do tại sao nên đánh giá một vết sẹo sau một năm, chứ không phải sau một tháng.

Hai sự thật cần hiểu rõ về sẹo: thứ nhất, sẹo không bao giờ đạt được độ bền như da nguyên vẹn (tối đa chỉ khoảng 80%); thứ hai, kết quả hình thành sẹo phụ thuộc một phần vào hành vi của bạn (cách chăm sóc vết thương, việc không hút thuốc) và một phần vào các yếu tố nằm ngoài tầm kiểm soát (di truyền, loại da, vị trí trên cơ thể, cũng như mức độ căng trên vết thương).

Những yếu tố giúp vết thương lành tốt

  • Giữ vết thương sạch sẽ và được bảo vệ, đồng thời tuân thủ các hướng dẫn về thay băng/chăm sóc vết thương; nhiễm trùng là nguyên nhân chính gây cản trở quá trình lành thương.
  • Đừng hút thuốc. Nicotine làm co mạch máu, khiến vết thương thiếu oxy; đây là một trong những yếu tố nguy cơ có thể kiểm soát được gây ra tình trạng vết thương không lành.
  • Ăn uống đầy đủ. Việc cung cấp đủ protein, vitamin C và kẽm, cùng việc kiểm soát đường huyết tốt nếu mắc bệnh tiểu đường, sẽ cung cấp những nguyên liệu cần thiết cho quá trình lành thương.
  • Bảo vệ vết sẹo trong giai đoạn hình thành: hạn chế tối đa lực căng lên vết sẹo, và che chắn vết sẹo mới khỏi ánh nắng mặt trời (vì có thể khiến sẹo bị thâm đen vĩnh viễn) trong năm đầu tiên.

Quá trình sửa chữa diễn ra qua bốn giai đoạn chồng chép lên nhau:

  1. Cầm máu (vài phút). Các mạch máu co lại và cục máu đông hình thành, bịt kín vết thương đồng thời tạo ra một khung xương sườn fibrin tạm thời.
  2. Viêm (vài ngày). Các tế bào miễn dịch (đầu tiên là bạch cầu trung tính, sau đó là đại thực bào) loại bỏ vi khuẩn và các mảnh vụn; chúng cũng giải phóng các tín hiệu để kêu gọi các tế bào sửa chữa. Đây là giai đoạn vết thương bị đỏ, sưng và đau nhức.
  3. Tái tạo (vài ngày đến vài tuần). Các tế bào sợi xuất hiện và tạo ra collagen mới; các mạch máu mới mọc lên (tạo thành mô hạt màu hồng); các tế bào da di chuyển lên bề mặt để khép kín vết thương.
  4. Tái cấu trúc (vài tuần đến khoảng một năm). Loại collagen được tạo ra vội vã ở giai đoạn đầu bị phân hủy và thay thế bằng loại collagen chắc khỏe, có cấu trúc tốt hơn; các mạch máu dần teo lại và vết sẹo trở nên ổn định.

Tại sao sẹo yếu hơn da bình thường?

Yếu tố quyết định là loại collagen. Trong giai đoạn đầu của quá trình hồi phục, cơ thể sản sinh nhiều collagen loại III; loại này được tạo thành nhanh chóng nhưng về mặt cơ học lại yếu và sắp xếp không ngăn nắp. Trong quá trình tái cấu trúc, collagen loại III dần được thay thế bằng collagen loại I – loại vững chắc hơn, được sắp xếp có trật tự và liên kết chéo với nhau. Độ bền chịu kéo của vùng da bị tổn thương cũng tăng dần theo: khoảng nửa mức bình thường sau sáu tuần, đạt đỉnh ở mức 80% sau khoảng ba tháng; tuy nhiên không bao giờ đạt 100%. Ngoài ra, vùng sẹo còn thiếu đi cấu trúc nguyên vẹn của da ban đầu (cùng các nang lông và tuyến mồ hôi). Vì vậy, vết thương đã lành chỉ là một vùng da vững chắc, có thể sử dụng được, chứ không phải là bản sao hoàn hảo của da bình thường.

Tế bào myofibroblast và quá trình co rút vết thương

Tế bào myofibroblast là loại tế bào sửa chữa chuyên biệt; nó vừa mang đặc tính của tế bào sợi vừa giống tế bào cơ. Tế bào này bám chặt vào mô nền vết thương và co lại một cách cơ học, kéo hai mép vết thương lại gần nhau nhằm thu hẹp diện tích cần được che phủ. Điều này rất hữu ích vì giúp vết thương liền nhanh hơn. Tuy nhiên, nếu xảy ra ở những vị trí không thích hợp – chẳng hạn như ở khớp hoặc lòng bàn tay – sự co rút quá mức có thể gây ra tình trạng co cứng mô, làm hạn chế khả năng vận động. Khi sẹo hình thành bất thường: sẹo phì đại và sẹo lồi

Đôi khi sự cân bằng giữa quá trình tổng hợp và phân hủy collagen bị mất cân đối, dẫn đến việc hình thành sẹo phát triển quá mức:

  • Sẹo phì đại có dạng nổi lên và có màu đỏ, nhưng vẫn nằm trong phạm vi vết thương ban đầu; tình trạng này thường cải thiện theo thời gian.
  • Sẹo lồi lại phát triển vượt ra ngoài ranh giới vết thương ban đầu, có đặc tính gần giống như một khối u lành tính; sẹo lồi thường gặp ở một số loại da và vùng cơ thể nhất định (ngực, vai, thùy tai) và rất khó điều trị.

Cả hai loại sẹo này đều là kết quả của phản ứng tăng sinh mô quá mức và kéo dài: lượng collagen hình thành quá nhiều, trong khi quá trình tái cấu trúc lại diễn ra kém hiệu quả.

Liền sẹo theo cơ chế nguyên phát so với thứ phát

Các bác sĩ phẫu thuật phân biệt hai cơ chế liền sẹo. Liền sẹo theo cơ chế nguyên phát là tình trạng vết thương có các mép được khâu lại với nhau; quá trình này diễn ra nhanh và chỉ để lại vết sẹo mỏng. Liền sẹo theo cơ chế thứ phát là tình trạng vết thương được để mở (do mất mô hoặc nhiễm trùng); vết thương dần được lấp đầy từ đáy lên bằng mô hạt rồi co lại: quá trình này chậm hơn và để lại vết sẹo rộng hơn. Việc lựa chọn phương pháp liền sẹo phù hợp (và thời điểm khâu lại vết thương bị nhiễm trùng) là một quyết định phẫu thuật quan trọng.

Những yếu tố hỗ trợ và cản trở quá trình lành vết thương

  • Tuần hoàn máu và lượng oxy là những yếu tố thiết yếu; việc hút thuốc lại làm suy giảm chúng.
  • Tiểu đường, dinh dưỡng kém, tuổi cao, cùng việc sử dụng steroid hoặc thuốc ức chế miễn dịch đều làm chậm quá trình lành thương và tăng nguy cơ nhiễm trùng.
  • Nhiễm trùng khiến vết thương bị kẹt ở giai đoạn viêm và có thể làm vết thương đã liền lại bị rách ra.
  • Lực căng và sự vận động tại vùng vết thương khiến sẹo bị to ra; việc bảo vệ và hỗ trợ vết thương sẽ giúp tạo ra vết sẹo mịn hơn.
  • Thời gian và việc bảo vệ vết thương khỏi ánh nắng quyết định kết quả thẩm mỹ cuối cùng: một vết sẹo sau một năm thường trông đẹp hơn nhiều so với sau một tháng.

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

Section 5 -- Disclaimer of Warranties and Limitation of Liability.

a. d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.
