Những triệu chứng bạn đang gặp phải

Cơ nhị đầu là cơ nằm ở phía trước cánh tay. Đầu dài của gân cơ này chạy dọc theo phía trên xương cánh tay và đi vào khớp vai; chính phần này gây ra các vấn đề ở đây.

Triệu chứng phổ biến nhất là cơn đau âm ỉ, sâu ở phía trước vai, thường xảy ra cách mỏm vai vài centimet. Cơn đau có thể xuất hiện khi bạn vươn tay lên cao, nâng vật nặng hoặc mang đồ vật; đôi khi cơn đau lan xuống phía trước cánh tay. Nhiều người cảm thấy khó xác định chính xác vị trí đau. Vì gân cơ nhị đầu nằm ngay cạnh cơ chóp xoay, cơn đau này thường đi kèm với các vấn đề về cơ chóp xoay; hai tình trạng này rất khó phân biệt nếu không có sự thăm khám lâm sàng.

Đôi khi có những trường hợp nghiêm trọng hơn. Gân cơ bị thoái hóa có thể đứt, thường gặp ở người cao tuổi và đôi khi chỉ gây ra ít đau đớn. Khi điều này xảy ra, phần cơ nhị đầu sẽ chảy xuống dọc cánh tay, tạo thành một khối mềm trông giống như một quả bóng nhỏ hoặc cơ đang co lại. Đây là dấu hiệu “Popeye” đặc trưng. Dù trông có vẻ đáng lo ngại, nhưng riêng dấu hiệu này hiếm khi gây nguy hiểm như vẻ bề ngoài của nó.

Chuyện gì đang thực sự xảy ra

Gân là những sợi dây chắc khỏe nối cơ với xương; giống như bất kỳ sợi dây nào được sử dụng thường xuyên, chúng cũng có thể bị mòn theo thời gian. Trong tình trạng viêm gân cơ nhị đầu, gân đầu dài của cơ nhị đầu bị viêm và mòn tại chỗ nó đi qua một rãnh hẹp ở đỉnh xương cánh tay rồi dẫn vào khớp vai. Chính sự mòn này gây ra cảm giác đau ở phía trước vai. Vì gân này dùng chung không gian chật hẹp với cơ vùng vai, nên cả hai thường bị mòn cùng lúc; đó là lý do tại sao đau cơ nhị đầu và đau cơ vùng vai thường xuất hiện song song.

Nếu tình trạng mòn này tiếp diễn, gân có thể hoàn toàn đứt vỡ. Khi gân đầu dài bị đứt, cơ nhị đầu không còn được giữ chặt ở phía trên nữa, nên nó trượt xuống dọc theo cánh tay và tạo thành chỗ phồng lên giống hình Popeye. Điều an ủi là cơ nhị đầu có hai điểm bám ở vai (đầu dài và đầu ngắn); đầu ngắn vẫn giữ nguyên chức năng. Vì vậy, khi đầu dài bị đứt, cánh tay vẫn giữ được phần lớn sức mạnh; thay đổi chủ yếu chỉ nằm ở hình dáng bên ngoài.

Đây là một điểm phân biệt quan trọng: tình trạng đứt gân ở phía gần vai thường không nghiêm trọng lắm. Ngược lại, nếu gân bị đứt ở phía còn lại của cơ, tức là ở khuỷu tay (gọi là đứt gân cơ nhị đầu ở phía xa), thì đó là tình trạng hoàn toàn khác: nó có thể gây mất sức đáng kể và thường cần can thiệp phẫu thuật. Chỗ phồng lên do đứt gân có thể trông tương tự nhau, nên việc xác định chính xác vị trí đứt gân là rất quan trọng.

Những gì chúng tôi có thể làm

Tại phòng khám của chúng tôi, Bác sĩ Kieran Hirpara sẽ hướng dẫn bệnh nhân theo một quy trình rõ ràng, bắt đầu bằng việc đánh giá kỹ lưỡng để xác nhận chẩn đoán. Thông thường, chúng tôi khuyến nghị bắt đầu bằng các biện pháp điều trị không phẫu thuật đối với các vấn đề đã kéo dài, chỉ chỉ định phẫu thuật khi những biện pháp đó không mang lại hiệu quả giảm đau đáng kể.

Đối với chứng viêm gân cơ nhị đầu, các bước đầu tiên rất đơn giản và không cần phẫu thuật; chúng giúp phần lớn bệnh nhân cải thiện tình trạng:

  • Thay đổi hoạt động: giảm bớt các động tác nâm cao hoặc nâng vật nặng gây kích thích gân, đồng thời vẫn duy trì sự vận động của vai.
  • Vật lý trị liệu: các bài tập được hướng dẫn nhằm làm dịu gân và cân bằng lại cơ vai, đặc biệt hữu ích khi có sự tham gia của cơ chóp xoay.
  • Thuốc chống viêm: dùng thuốc viên để giảm đau và giảm tình trạng viêm.
  • Tiêm thuốc: đôi khi người ta tiêm corticosteroid quanh vùng gân hoặc vào khớp vai nhằm làm dịu những cơn đau dai dẳng.

Phẫu thuật chỉ được chỉ định trong trường hợp đau kéo dài mà các biện pháp trên không mang lại hiệu quả. Khi cần thiết, có hai phương án chính: có thể cắt bỏ một phần gân để nó không còn tác động lên vùng bị tổn thương (gọi là tenotomy); phương pháp này thường tạo ra vùng phồng giống hình Popeye nhưng giúp giảm đau hiệu quả. Hoặc có thể gắn lại gân ở vị trí thấp hơn dọc theo xương cánh tay (tenodesis); cách này giữ được hình dạng bình thường của cơ, tránh hiện tượng phồng nhưng thời gian hồi phục sẽ lâu hơn. Hai thủ thuật này thường được thực hiện cùng lúc với phẫu thuật cơ chóp xoay vì hai vấn đề này thường xuất hiện đồng thời.

Đối với chứng đứt gân đầu dài phía gần, lời khuyên thông thường là không cần phẫu thuật. Sức mạnh cơ bắp hầu như không bị suy giảm, còn vùng phồng chỉ mang tính thẩm mỹ. Tuy nhiên, một số người trẻ tuổi hoặc năng động, hoặc những người cảm thấy phiền lòng vì hình dạng cơ bắp hoặc tình trạng co cứng cơ, có thể chọn phương pháp gắn lại gân; nhưng đối với phần lớn mọi người, việc không can thiệp là lựa chọn đúng đắn.

Những điều có thể mong đợi

Hầu hết các trường hợp viêm gân cơ nhị đầu đều cải thiện dần nhờ sự kiên nhẫn và việc thực hiện đúng các bài tập. Quá trình hồi phục có thể diễn ra chậm (mất vài tháng thay vì vài tuần), nhưng tiên lượng là tốt; phần lớn bệnh nhân không cần phải phẫu thuật. Trong trường hợp vấn đề ở cơ nhị đầu là một phần của các tổn thương ở cơ chóp xoay, việc hồi phục sẽ đi theo tiến triển của các cơ này; việc điều trị cả hai cùng lúc sẽ mang lại kết quả tốt nhất.

Nếu bạn đã gặp phải tình trạng đứt gân đầu dài phía gần vai và quyết định không phẫu thuật, bạn có thể kỳ vọng cảm giác đau nhức ban đầu sẽ giảm dần trong vài tuần, sức mạnh cơ bắp sẽ phục hồi gần như bình thường, còn chỗ phình ra ở cánh tay vẫn sẽ tồn tại như một dấu hiệu vô hại và vĩnh viễn. Tình trạng này sẽ không trở nên tồi tệ hơn theo thời gian, và cũng không cần phải điều trị gì thêm.

Khi nào nên đi khám bác sĩ

  • Đau vùng phía trước vai không thuyên giảm, đặc biệt khi vươn tay lên cao hoặc nâng vật nặng: nên được đánh giá, vì tình trạng này thường là dấu hiệu của vấn đề ở cơ nhị đầu hoặc cơ vùng vai, và có thể điều trị hiệu quả.
  • Sự phồng lên đột ngột ở vùng cánh tay kèm theo yếu cơ: cần được kiểm tra để xác nhận đó chỉ là phần đầu dài của cơ nhị đầu ở vai – vô hại – chứ không phải là rách cơ ở khuỷu tay; trường hợp sau là chấn thương khác có thể cần phẫu thuật.
  • Rách cơ nhị đầu ở người trẻ tuổi hoặc người năng động: ngay cả khi rách ở vị trí gần vai cũng nên được đánh giá, nhằm xác nhận chẩn đoán và thảo luận xem việc phẫu thuật sửa chữa có phù hợp với bạn hay không.
  • Cơn co cứng hoặc đau nhức cơ nhị đầu kéo dài sau khi đã có chẩn đoán rách cơ: thường là tình trạng nhẹ, nhưng nếu gây phiền toái thì vẫn có thể điều trị được.

Phần này đi sâu hơn mức cần thiết để bạn có thể tự đưa ra quyết định điều trị. Các vấn đề liên quan đến đầu dài của gân cơ nhị đầu đáng để bạn đọc thêm, bởi có một tình huống đặc biệt trong phẫu thuật: các phương pháp điều trị tiêu chuẩn đều yêu cầu tách rời gân này, mặc dù chức năng của nó chưa từng được chứng minh một cách thuyết phục ở vai người sống; hơn nữa, các bằng chứng khoa học cho thấy việc mất đi gân này cũng không gây ảnh hưởng gì.

Gân có chức năng chưa được chứng minh

Đầu dài của gân cơ nhị đầu chạy từ đỉnh ổ vai, xuyên qua khớp, rồi đi xuống theo rãnh trên xương cánh tay. Vì đi trực tiếp qua khớp, người ta từ lâu đã cho rằng gân này giúp giữ đầu xương cánh tay ở đúng vị trí trong ổ vai.

Tuy nhiên, bằng chứng thực tế lại yếu hơn so với giả định này. Các nghiên cứu cơ sinh học cho thấy gân này góp phần duy trì sự ổn định của khớp vai theo mọi hướng; nhưng các nghiên cứu trên cơ thể sống vẫn chưa xác nhận được tác dụng ổn định này, và mức tải trọng sinh lý cần thiết vẫn chưa được xác định rõ [1].

Điều này rất quan trọng vì nó giải thích tại sao các phẫu thuật liên quan đến gân này vẫn mang lại hiệu quả. Nếu gân thực sự đảm nhiệm vai trò ổn định quan trọng, việc cắt bỏ nó sẽ gây ra hậu quả nghiêm trọng. Nhưng dường như điều đó không xảy ra.

Cắt gân và gắn lại gân cho kết quả tương đương nhau

Hai phương pháp phẫu thuật này là cắt gân (giải phóng gân để nó tự co lại) và cố định gân (giải phóng gân rồi gắn nó vào xương cánh tay ở vị trí thấp hơn). Phương pháp cố định gân phức tạp hơn và thường được coi là phương pháp tốt hơn.

Dựa trên dữ liệu từ 650 bệnh nhân, không có sự khác biệt nào về kết quả chức năng sau phẫu thuật giữa hai phương pháp cắt gân và cố định gân. Những khác biệt duy nhất chỉ nằm ở hai điểm: hiện tượng “dị dạng Popeye” – tức là phần cơ ở cánh tay bị phồng lên – và tình trạng đau co thắt ở rãnh cơ nhị đầu; cả hai hiện tượng này đều xuất hiện thường xuyên hơn sau khi thực hiện phương pháp cắt gân [2]. Một phân tích có hệ thống trước đó cũng đi đến kết luận tương tự, cho thấy kết quả điều trị tương đương nhau giữa hai phương pháp; điểm khác biệt chính duy nhất là vấn đề thẩm mỹ [3].

Vì vậy, việc lựa chọn phương pháp phẫu thuật thực chất không phụ thuộc vào mức độ hoạt động của cánh tay. Điều quan trọng là yếu tố thẩm mỹ, tình trạng đau co thắt, và mức độ can thiệp phẫu thuật mà bệnh nhân mong muốn; cách tiếp cận này trung thực hơn so với việc gọi một phương pháp là “tốt hơn”.

Trong phương pháp tenodesis, các yếu tố kỹ thuật không tạo ra sự khác biệt

Có nhiều tranh luận xoay quanh việc nên cố định gân ở đâu và bằng cách nào; tuy nhiên dường như điều này không mấy quan trọng. Trên 707 bệnh nhân, phương pháp cố định gân dưới cơ ngực và trên cơ ngực đều mang lại kết quả cải thiện tương tự, không có sự khác biệt về tỷ lệ thất bại điều trị; tuy nhiên phương pháp cố định gân nội soi lại gây ra tình trạng cứng khớp nhiều hơn trong giai đoạn sớm sau phẫu thuật [4]. Khi so sánh phương pháp cố định gân nội soi với phương pháp mở trên 476 bệnh nhân, cả hai đều cho kết quả khả quan mà không có sự khác biệt nào đáng kể [5].

Các nghiên cứu trong phòng thí nghiệm thực sự chỉ ra sự khác biệt về độ bền của cấu trúc sau phẫu thuật: việc sử dụng vít chặn và nhiều mũi khâu giúp tăng độ vững chắc; đồng thời việc cố định gân dưới hay trên cơ ngực cũng có tính chất sinh học tương đương khi đã loại trừ các yếu tố gây nhiễu [6]. Tuy nhiên, độ bền này lại không dẫn đến sự khác biệt lâm sàng đáng kể; đây là một hiện tượng thường gặp trong phẫu thuật vai và cần được ghi nhận khi có phương pháp nào được mô tả là “vững chắc hơn”.

Việc chẩn đoán mới là phần thực sự khó khăn

Do các phương pháp điều trị đều cho kết quả tương tự nhau, phần lớn sự không chắc chắn nằm ở giai đoạn đầu: việc xác định xem gân cơ nhị đầu có phải là nguyên nhân gây đau hay không. Gân này nằm cạnh cơ chóp xoay, và các triệu chứng của hai tình trạng này có sự trùng lặp.

Các phương pháp khám lâm sàng đã được kiểm chứng so với những gì phát hiện được qua nội soi khớp. Việc thực hiện thử nghiệm uppercut kết hợp với việc phát hiện điểm đau ở rãnh cơ nhị đầu mang lại độ nhạy và độ đặc hiệu cao nhất trong số các thủ thuật đã được biết đến [7]. Điều này rất hữu ích vì đây là sự kết hợp nhiều phương pháp; không có xét nghiệm đơn lẻ nào có thể đưa ra chẩn đoán chắc chắn.

Hệ quả thực tiễn là: tình trạng tổn thương gân cơ nhị đầu chỉ có thể được xác định một cách chắc chắn khi các kết quả khám lâm sàng, hình ảnh học và mô hình triệu chứng đều trùng khớp với nhau. Nếu các yếu tố này không đồng nhất, gân cơ nhị đầu có thể chỉ là nguyên nhân phụ; việc phẫu thuật gân này nhiều khả năng cũng không giải quyết được vấn đề.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

Section 6 -- Term and Termination.

a. This Public License applies for the term of the Copyright and Similar Rights licensed here. However, if You fail to comply with this Public License, then Your rights under this Public License terminate automatically.

b. Where Your right to use the Licensed Material has terminated under Section 6(a), it reinstates:

1. automatically as of the date the violation is cured, provided it is cured within 30 days of Your discovery of the violation; or

2. upon express reinstatement by the Licensor.

For the avoidance of doubt, this Section 6(b) does not affect any right the Licensor may have to seek remedies for Your violations of this Public License.

c. For the avoidance of doubt, the Licensor may also offer the Licensed Material under separate terms or conditions or stop distributing the Licensed Material at any time; however, doing so will not terminate this Public License.

d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.

d. Nothing in this Public License constitutes or may be interpreted as a limitation upon, or waiver of, any privileges and immunities that apply to the Licensor or You, including from the legal processes of any jurisdiction or authority.
