Một chút đau sau phẫu thuật là hiện tượng bình thường và có thể xảy ra. Mục đích của việc kiểm soát cơn đau sau phẫu thuật không phải là để không cảm thấy đau chút nào; mà là giúp bệnh nhân cảm thấy thoải mái đủ để ngủ, thực hiện các bài tập vật lý trị liệu và bắt đầu vận động trở lại.

Những cơn đau có thể gặp phải

Thông thường, cơn đau dữ dội nhất vào ngày phẫu thuật và ngày hôm sau; sau đó sẽ giảm dần trong 1–2 tuần đầu tiên. Đến 4–6 tuần, hầu hết bệnh nhân chỉ còn cảm thấy khó chịu thỉnh thoảng khi vận động. Trong tuần đầu tiên, việc bị đau nhói hoặc âm ỉ vào ban đêm là hiện tượng phổ biến; việc kê tay lên gối thường giúp giảm đau.

Nếu ca phẫu thuật được thực hiện dưới gây tê tại chỗ, vùng phẫu thuật sẽ bị tê khoảng 6–12 giờ sau đó; hiếm khi tình trạng này kéo dài tới 2–3 ngày. Bạn nên uống thuốc giảm đau theo chỉ định trước khi cảm giác tê mất đi, chứ không phải đợi đến khi cơn đau xuất hiện. Sau phẫu thuật vai dưới phương pháp gây tê thần kinh, cảm giác tê cũng thường hết trong vòng 6–12 giờ; tuy nhiên hiếm khi kéo dài tới 2–3 ngày.

Khi các dây thần kinh bắt đầu hồi phục, có thể xuất hiện những cơn đau nhói như bị điện giật. Những cơn đau này gây khó chịu nhưng là hiện tượng bình thường và thường sẽ giảm dần trong vài tuần.

Thuốc giảm đau được kê đơn khi xuất viện

Sau phẫu thuật, bác sĩ sẽ kê đơn thuốc giảm đau cho bạn. Trong hầu hết các trường hợp, thuốc sẽ được phát ngay trước khi bạn rời viện; nếu bạn xuất viện sau giờ làm việc của hiệu thuốc bệnh viện, bạn sẽ nhận được đơn thuốc, đôi khi được gửi qua tin nhắn SMS kèm theo mã in sẵn trong gói hồ sơ xuất viện. Bất kỳ hiệu thuốc nào cũng có thể bán thuốc theo đơn này, kể cả hiệu thuốc của bệnh viện.

Đơn thuốc gồm hai loại thuốc, cách sử dụng của chúng khác nhau:

Thuốc giảm đau thông thường: uống theo lịch định, không chỉ khi cơn đau lên cao:

  • Paracetamol 1 g (hai viên tiêu chuẩn) mỗi 4–6 giờ, không dùng quá 4 g (tám viên) trong 24 giờ
  • Ibuprofen 400 mg uống mỗi 8 giờ, nên uống cùng thức ăn; trừ khi bạn có chống chỉ định như loét dạ dày, bệnh thận, hoặc bác sĩ yêu cầu tránh dùng thuốc chống viêm (bác sĩ phẫu thuật sẽ thông báo nếu việc lành xương là vấn đề cần lưu ý)

Thông thường sẽ có thêm một loại thuốc giảm đau mạnh hơn: phổ biến nhất là tramadol, tapentadol (Palexia) hoặc oxycodone; đôi khi dùng cả hai loại này. Loại thuốc cụ thể sẽ do bác sĩ gây mê quyết định, vì vậy hãy dùng thuốc đúng theo hướng dẫn ghi trên nhãn đơn thuốc. Các nguyên tắc chung trong phác đồ điều trị của tất cả các bác sĩ gây mê:

  • Chỉ dùng thuốc mạnh khi thuốc thông thường không kiểm soát được cơn đau; không dùng thay thế cho thuốc thông thường.
  • Nếu bạn được kê hai loại thuốc mạnh, hãy bắt đầu với loại nhẹ hơn; chuyển sang loại còn lại nếu nó không đủ hiệu quả hoặc gây buồn nôn.
  • Vào ban đêm, thường chỉ cần dùng một viên thuốc mạnh trước khi đi ngủ. Đến tuần thứ hai, hầu hết bệnh nhân chỉ cần dùng thuốc này vào ban đêm, nếu có.
  • Một số bệnh nhân thấy hiệu quả khi dùng thuốc mạnh trước các buổi trị liệu tay hoặc vật lý trị liệu; chuyên viên trị liệu sẽ cho bạn biết ngay buổi hẹn đầu tiên nếu điều này phù hợp với bạn.
  • Cố gắng giảm dần liều lượng thuốc mạnh trong tuần thứ hai. Thuốc này gây táo bón và buồn ngủ; hiệu quả cũng giảm dần nếu dùng lâu ngày.

Đơn thuốc xuất viện được thiết kế có hạn chế về số lượng. Nếu bạn cảm thấy nó không đủ để hỗ trợ quá trình hồi phục, hãy hỏi ý kiến bác sĩ đa khoa ngay sau khi xuất viện thay vì chờ đến khi hết thuốc.

Những tác dụng phụ cần lưu ý

  • Táo bón: Tramadol, Tapentadol và Oxycodone đều gây ra tình trạng này. Hãy ăn nhiều chất xơ và uống thật nhiều nước ngay từ ngày đầu tiên; nếu vẫn chưa đỡ, hãy dùng một gói Movicol mỗi ngày một hoặc hai lần để cải thiện tình trạng.
  • Buồn nôn: Nếu thuốc giảm đau mạnh khiến bạn buồn nôn, hãy chuyển sang loại thuốc khác nếu có thể. Nếu bác sĩ kê đơn Ondansetron khi xuất viện, hãy uống một viên khi bị buồn nôn hoặc nôn mửa.
  • Buồn ngủ: Không nên lái xe hay đưa ra các quyết định quan trọng khi đang dùng các loại thuốc giảm đau mạnh.

Các biện pháp không dùng thuốc

Những phương pháp này có hiệu quả đáng ngạc nhiên khi kết hợp với việc uống thuốc:

  • Chườm đá: đặt túi đá lên vết thương trong 15–20 phút mỗi giờ hoặc bất cứ khi nào cần trong tuần đầu tiên. Tuyệt đối không chườm đá trực tiếp lên da; luôn bọc túi đá trong túi nhựa hoặc khăn (xem Nhiệt vs Đá lạnh)
  • Nâng cao chi: giữ tay cao hơn khuỷu tay, khuỷu tay cao hơn tim. Việc giảm sưng sẽ giúp giảm đau
  • Vận động: các bài tập được chỉ định có thể gây hơi đau nhưng giúp giảm cứng khớp – nguyên nhân gây đau khớp
  • Phân tâm: việc xem những thứ mình thích thực sự giúp giảm cảm giác đau trong những ngày đầu sau phẫu thuật

Khi nào nên liên hệ với chúng tôi

Creative Commons Attribution-NonCommercial 4.0 International Public License

i. identification of the creator(s) of the Licensed Material and any others designated to receive attribution, in any reasonable manner requested by the Licensor (including by pseudonym if designated);

iii. a notice that refers to this Public License;

iv. a notice that refers to the disclaimer of warranties;

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

Section 5 -- Disclaimer of Warranties and Limitation of Liability.

a. Section 7 -- Other Terms and Conditions.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.

d. Nothing in this Public License constitutes or may be interpreted as a limitation upon, or waiver of, any privileges and immunities that apply to the Licensor or You, including from the legal processes of any jurisdiction or authority.
