Vết thương sạch sẽ và đang lành là nền tảng cho quá trình hồi phục tốt. Hai tuần đầu sau phẫu thuật là thời điểm vết thương dễ bị nhiễm trùng nhất, đồng thời cũng dễ bị rách ra nếu bị kéo giãn hoặc tác động mạnh. Nếu bạn có bất kỳ thắc mắc nào về vết thương, hãy liên hệ với phòng khám; việc chụp ảnh vết thương và gửi qua email để các bác sĩ xem xét thường rất hữu ích.

Những điều có thể xảy ra

Sưng phù sau phẫu thuật chi trên thường khiến bệnh nhân lo lắng, nhưng đây là hiện tượng bình thường. Sau phẫu thuật tay hoặc cánh tay, toàn bộ cánh tay có thể bị sưng; sau phẫu thuật vai thì cả cánh tay và bàn tay cũng bị sưng. Tùy vào mức độ chấn thương và loại phẫu thuật, tình trạng sưng phù có thể kéo dài từ vài tuần đến vài tháng. Nếu sưng phù kéo dài, không thuyên giảm hoặc gây cứng khớp, hãy liên hệ phòng khám để được tư vấn.

Vết thương có thể rỉ dịch hoặc chảy máu trong vài ngày, đặc biệt là các vết thương ở tay và vết thương sau phẫu thuật nội soi vai. Hiện tượng rỉ dịch thường hết sau khoảng 3–5 ngày; nếu vẫn tiếp diễn sau thời gian đó thì cần gọi điện cho bác sĩ.

Ban đầu, một số vùng trên cánh tay hoặc bàn tay có thể bị tê. Thuốc gây tê tại chỗ được dùng trong phẫu thuật thường hết tác dụng sau khoảng 6–12 giờ; hiếm khi tình trạng tê kéo dài đến 2–3 ngày. Tình trạng tê do gây tê thần kinh vai thường kéo dài từ 8 đến 18 giờ, đôi khi lên đến một ngày. Nếu cánh tay của bạn vẫn còn tê hoàn toàn sau khoảng 24 đến 30 giờ, hãy liên hệ với chúng tôi.

Băng gạc

Sau khi xuất viện, bạn sẽ có sẵn băng gạc được dán lên vết thương. Thông thường, không nên tháo băng gạc ra: băng gạc sẽ phát huy tác dụng tốt nhất khi không bị can thiệp, và hầu hết các loại băng gạc sẽ được giữ nguyên cho đến lần tái khám vết thương. Ngoại lệ là những loại băng gạc bị bẩn hoặc thấm đẫm máu: những loại này cần được thay thế (các loại băng gạc chống thấm nước dùng để thay thế có bán tại hầu hết các hiệu thuốc). Nếu bạn không chắc liệu có cần thay băng gạc hay không, hãy gửi hình ảnh cho chúng tôi trước khi tháo băng ra.

Loại băng gạc sử dụng phụ thuộc vào loại phẫu thuật mà bạn đã trải qua:

  • Phẫu thuật tay và khuỷu tay: thường là băng gạc len và vải crepe có lớp băng gạc bên dưới. Băng gạc len có thể tháo ra sau 48–72 giờ; còn lớp băng gạc bên dưới thì giữ nguyên cho đến khoảng ngày thứ 10. Hãy giữ cho băng gạc ở tay và khuỷu tay luôn khô ráo cho đến lần tái khám vết thương.
  • Phẫu thuật vai nội soi: dùng nhiều miếng băng gạc chống thấm nước nhỏ; những miếng băng này sẽ được giữ nguyên trong 10 ngày.
  • Phẫu thuật thay khớp vai: dùng một miếng băng gạc dài; miếng băng này có thể giữ nguyên trong khoảng 10–14 ngày. Nếu bạn phải thay băng gạc (đặc biệt là khi vết thương vẫn tiết dịch sau 2–3 ngày), hãy liên hệ với phòng khám để được tư vấn. Sau 10–14 ngày có thể tháo băng ra, nhưng nếu cảm thấy thoải mái thì có thể giữ lại lâu hơn.

Tắm rửa

Sau khi phẫu thuật vai, các lớp băng gạc đã được xử lý chống thấm nước; vì vậy bạn có thể tắm ngay từ ngày đầu tiên: hãy để nước chảy qua lớp băng gạc nhưng không được ngâm hay chà xát vùng đó.

Sau phẫu thuật tay và khuỷu tay, hãy giữ cho lớp băng gạc khô ráo cho đến khi đi khám vết thương: hãy cho cánh tay vào trong túi nhựa rồi dán kín lại khi tắm, hoặc sử dụng màng che tắm bán sẵn tại hiệu thuốc.

Không được tắm bồn, bơi lội hay sử dụng phòng tắm hơi cho đến khi vết thương lành hẳn (thông thường mất 2–3 tuần); đồng thời tránh mọi hoạt động có thể khiến vết thương bị ngâm nước hoặc bị bẩn.

Chỉ khâu

Hầu hết các vết thương đều được khâu lại bằng chỉ tự tiêu. Một số loại chỉ chạy dưới da, có một đoạn nhỏ “đuôi” ở mỗi đầu vết thương; phần chỉ nằm dưới da không cần cắt bỏ, còn các đoạn “đuôi” lộ ra ngoài thì có thể cắt ngang mức da sau khoảng 10 ngày. Một số vết thương khác lại dùng những dải nhựa nhỏ gọi là Steri-Strips; chúng sẽ tự bong ra sau 1–2 tuần. Đối với những vết thương lớn hơn, có thể cần phải cắt chỉ trong lần tái khám. Nếu bạn không thể quay lại phòng khám để thay băng, bác sĩ đa khoa của bạn thường sẵn lòng cắt hoặc gỡ chỉ cho bạn.

Các bước theo dõi sau phẫu thuật

Nhân viên phòng khám sẽ liên hệ với bạn trong vòng hai đến ba ngày sau phẫu thuật để cung cấp thông tin về các buổi tái khám. Thông thường, các bước theo dõi như sau:

  • Kiểm tra vết thương vào tuần thứ 1–2 cùng điều dưỡng viên của phòng khám. Nếu bạn sống ở nơi xa, việc đến gặp bác sĩ đa khoa để chăm sóc vết thương có thể thuận tiện hơn; hãy mang theo bản tóm tắt sau phẫu thuật chứa các chi tiết liên quan đến ca mổ của bạn.
  • Tái khám với bác sĩ Hirpara vào tuần thứ 6–8 sau phẫu thuật: có thể sớm hơn nếu vết thương cần được theo dõi kỹ lưỡng hơn.
  • Đối với các ca phẫu thuật về ngón tay và gân (bệnh Dupuytren, gãy ngón tay, phục hồi gân và các ca tương tự), bệnh nhân thường được giới thiệu đến chuyên viên trị liệu tay, người sẽ chịu trách nhiệm toàn bộ công việc: chăm sóc vết thương, cắt chỉ, xử lý sẹo và phục hồi chức năng. Dịch vụ trị liệu tay chỉ có ở một số địa điểm thuộc vùng Trung Queensland; nơi nào không có chuyên viên trị liệu tay, thì chuyên viên vật lý trị liệu sẽ đảm nhiệm công tác phục hồi chức năng, còn điều dưỡng viên của phòng khám (hoặc bác sĩ đa khoa) sẽ phụ trách việc chăm sóc vết thương.

Dấu hiệu của quá trình lành thương bình thường

Trong vài ngày đầu, bạn có thể gặp các triệu chứng sau:

  • Vùng da quanh vết thương hơi đỏ nhẹ
  • Một lượng nhỏ dịch trong suốt hoặc màu hồng thấm vào băng gạc
  • Cảm giác căng, ngứa hoặc “tê rần” khi các dây thần kinh bắt đầu hồi phục
  • Bầm tím quanh vết thương; đôi khi lan xuống cánh tay

Trong những tuần tiếp theo, vết sẹo sẽ hơi nổi lên, có màu hồng và cứng; sau đó dần dần phẳng và nhạt màu đi trong vòng 6–12 tháng.

Các dấu hiệu bất thường – hãy gọi cho chúng tôi

  • Vùng da đỏ lan rộng ra ngoài mép vết thương
  • Vết thương có cảm giác nóng khi chạm vào
  • Dịch tiết có màu vàng, xanh lá cây hoặc có mùi khó chịu
  • Cơn đau ngày càng tăng thay vì giảm sau vài ngày đầu
  • Sốt (nhiệt độ trên 38°C)
  • Vết thương bị hở ra

Tất cả những dấu hiệu này có thể là biểu hiện sớm của nhiễm trùng. Đừng chờ đến lịch hẹn đã đặt; hãy gọi cho chúng tôi ngay lập tức.

Chăm sóc sẹo sau khi vết thương đã lành

Khi các mũi khâu đã được cắt bỏ và vết thương khô ráo, kín miệng (thường là 2 đến 4 tuần sau phẫu thuật), bạn có thể bắt đầu mát-xa vùng sẹo:

  • 5–10 phút, hai lần mỗi ngày. Trong 2–4 tuần đầu, hãy mát-xa dọc theo đường rạch; sau đó có thể mát-xa theo mọi hướng.
  • Ấn mạnh vừa đủ để vùng sẹo chuyển từ màu hồng sang màu nhạt, nhưng không được mạnh đến mức gây đau. Tuyệt đối không mát-xa vùng sẹo còn hở hoặc có dấu hiệu nhiễm trùng.
  • Việc mát-xa còn có tác dụng giảm độ nhạy cảm: hiện tượng sẹo mới trở nên rất nhạy cảm là điều bình thường; việc chạm vào thường xuyên sẽ giúp giảm tình trạng này.
  • Dùng kem dưỡng ẩm không mùi (như sorbolene, kem vitamin E, lô hội) để làm mềm sẹo và giúp việc mát-xa dễ dàng hơn.
  • Nếu sẹo bị lồi, hãy dán một dải băng Fixomull hoặc Hypafix lên đường rạch trong tối đa 2 tuần (thay băng mỗi ngày; bạn vẫn có thể mát-xa lên trên lớp băng này). Hầu hết các hiệu thuốc đều có bán loại băng này.
  • Bảo vệ khỏi ánh nắng là rất quan trọng: sẹo mới rất dễ bị cháy nắng, và tia UV có thể khiến sẹo bị thâm đen vĩnh viễn. Trong năm đầu tiên, hãy che chắn sẹo bằng kem chống nắng SPF 30 trở lên, phấn kẽm, quần áo hoặc tránh nắng.
