Hầu hết chúng ta đều thích uống rượu, và việc nhấp một chút rượu trong bữa tối là điều rất bình thường. Cách rượu bia ảnh hưởng đến xương

Xương không phải là cấu trúc cố định, vô tri vô giác; đó là mô sống luôn trong quá trình bị phá hủy và tái tạo, giống như đội ngũ công nhân đang liên tục sửa chữa mặt đường vậy. Những tế bào đặc biệt gọi là tế bào tạo xương chịu trách nhiệm hình thành xương mới; việc uống rượu nhiều thường xuyên làm suy giảm chức năng của các tế bào này, khiến lượng xương mới được tạo ra ít đi. Theo thời gian, điều này làm mất cân bằng, khiến xương trở nên mỏng và giòn hơn – tình trạng được gọi là loãng xương.

Rượu bia gây hại cho xương qua nhiều cơ chế chồng chéo nhau:

  • Rượu trực tiếp làm chậm hoạt động của các tế bào tạo xương, khiến hệ xương không được tái tạo đầy đủ như bình thường.
  • Rượu làm rối loạn các hormone giúp duy trì sức mạnh cho xương (ví dụ: ở nam giới, rượu có thể làm giảm nồng độ testosterone – yếu tố thiết yếu cho quá trình tạo xương).
  • Rượu cản trở khả năng hấp thụ canxi và vitamin D của cơ thể (những thành phần nguyên liệu và “hệ thống vận chuyển” mà xương cần); điều này xảy ra do rượu ảnh hưởng đến đường tiêu hóa, tuyến tụy và quá trình chuyển hóa vitamin D.

Ngoài ra, rượu còn ảnh hưởng đến khả năng giữ thăng bằng và phán đoán; vì vậy những người uống rượu nhiều thường bị ngã nhiều hơn. Việc có xương yếu cộng với tần suất ngã cao chính là nguyên nhân dẫn đến gãy xương hông và cổ tay.

Điều đáng mừng là: xương có khả năng phục hồi khi việc uống rượu được giảm bớt. Các nghiên cứu cho thấy một phần sức mạnh xương đã mất có thể được phục hồi sau khi ngừng uống rượu nặng. Hệ xương của chúng ta thực ra có khả năng phục hồi tốt hơn nhiều so với những gì ta tưởng.

Rượu và quá trình liền xương

Khi bị gãy xương, quá trình hồi phục giống như một công trình xây dựng lớn, và nó phụ thuộc vào những tế bào tạo xương vốn bị ức chế bởi rượu. Việc uống rượu nhiều hoặc uống quá mức đã được chứng minh là làm chậm quá trình liền xương, khiến mô xương mới hình thành (còn gọi là “vết sẹo xương” nối hai đầu xương gãy) yếu hơn so với mức bình thường. Vì vậy, trong những tuần lễ mà xương đang dần liền lại, việc hạn chế uống rượu sẽ giúp quá trình phục hồi có cơ hội diễn ra tốt nhất, tạo ra mô xương vững chắc.

Rượu, vết thương và nhiễm trùng

Quá trình lành vết thương sau phẫu thuật đòi hỏi hệ miễn dịch phải hoạt động hết công suất: nó phải chống lại vi khuẩn, loại bỏ các mô bị tổn thương và tạo ra lớp da mới cùng các mô nền khỏe mạnh. Việc uống rượu nhiều và thường xuyên làm suy giảm chức năng hệ miễn dịch, từ đó làm chậm quá trình lành vết thương. Kết quả thực tế, được ghi nhận qua nhiều nghiên cứu phẫu thuật, là tỷ lệ nhiễm trùng vết thương và các biến chứng trong quá trình lành thương cao hơn ở những người uống rượu nhiều; ngoài ra vết thương cũng khó lành hơn, chậm liền lại và đôi khi để lại sẹo xấu. Việc giúp hệ miễn dịch hoạt động hiệu quả hơn trong giai đoạn trước và sau phẫu thuật là một trong những cách đơn giản nhất để giảm thiểu nguy cơ này.

Vào thời điểm phẫu thuật

Phẫu thuật là thời điểm mà rượu bia có ảnh hưởng lớn nhất, vì nó tác động lên nhiều yếu tố cùng lúc:

  • Chảy máu. Rượu bia làm rối loạn quá trình đông máu bình thường, nên có tác dụng tương tự như thuốc chống đông máu. Điều này có thể khiến lượng máu mất đi trong và sau phẫu thuật nhiều hơn mức dự kiến.
  • Thuốc gây mê và thuốc giảm đau. Rượu bia làm thay đổi cách cơ thể xử lý các loại thuốc này, khiến phản ứng trong quá trình phẫu thuật trở nên khó dự đoán; đôi khi cần điều chỉnh liều lượng. Tương tác này càng quan trọng sau phẫu thuật: việc kết hợp rượu bia với các thuốc giảm đau opioid (loại mạnh) gây nguy hiểm nghiêm trọng, làm tăng tình trạng buồn ngủ và làm chậm nhịp thở; còn việc uống rượu cùng paracetamol sẽ gây tổn hại nặng cho gan. Vì vậy, tốt nhất là tránh uống rượu bia khi đang dùng những loại thuốc này.
  • Cơn cai rượu trong bệnh viện. Đây là điều mà hầu hết mọi người không ngờ tới. Nếu cơ thể đã quen với việc uống rượu thường xuyên, việc đột ngột ngừng uống (như khi nhập viện) có thể gây ra các triệu chứng cai rượu, từ run rẩy, đổ mồ hôi, tim đập nhanh cho đến rối loạn ý thức (mê sảng) và thỉnh thoảng là co giật. Đây là một nguy cơ y tế thực sự, nhưng hoàn toàn có thể kiểm soát được nếu đội ngũ y tế biết theo dõi các dấu hiệu này. Đó là lý do chính khiến chúng tôi cần biết một cách trung thực mức độ bạn uống rượu: không phải để phán xét, mà để đảm bảo sự an toàn và thoải mái cho bạn.

Tại sao sự trung thực với đội ngũ y tế lại quan trọng

Có thể bạn cảm thấy ngượng ngùng khi phải trả lời chính xác về mức độ uống rượu của mình khi được hỏi, và nhiều người có xu hướng làm tròn số xuống một cách bản năng. Xin hãy cưỡng lại bản năng đó. Bác sĩ phẫu thuật và bác sĩ gây mê không hề ghi chép lại con số này; họ sử dụng nó để đưa ra các quyết định quan trọng: cần theo dõi tình trạng chảy máu ở mức độ nào, lên kế hoạch giảm đau ra sao, và có nên áp dụng biện pháp ngăn ngừa tình trạng nghiện rượu để bạn không gặp vấn đề sức khỏe trong bệnh viện hay không. Giảm hoặc ngưng uống rượu trước và sau phẫu thuật

Đây là tin vui, và có nhiều bằng chứng khoa học ủng hộ: việc giảm hoặc ngưng uống rượu vài tuần trước và sau phẫu thuật giúp cải thiện đáng kể quá trình hồi phục và giảm nguy cơ biến chứng. Các nghiên cứu trên những người ngưng uống rượu khoảng 4–8 tuần quanh thời điểm phẫu thuật cho thấy họ ít gặp các vấn đề sau mổ hơn. Bạn không cần phải thay đổi thói quen uống rượu mãi mãi để nhận được lợi ích này; chỉ cần ngưng uống trong một khoảng thời gian nhất định quanh ca phẫu thuật cũng đã mang lại hiệu quả rồi.

Một vài lưu ý thực tế:

  • Nếu có thể, hãy lên kế hoạch ngưng hoặc hạn chế uống rượu trong vài tuần trước phẫu thuật, và tiếp tục duy trì điều này trong thời gian vết thương và xương đang hồi phục.
  • Đừng uống rượu khi đang dùng thuốc giảm đau opioid hoặc paracetamol: sự kết hợp này mới là yếu tố gây nguy hiểm thực sự.

Khi nào nên tìm sự trợ giúp sớm

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

1. automatically as of the date the violation is cured, provided it is cured within 30 days of Your discovery of the violation; or

2. upon express reinstatement by the Licensor.

For the avoidance of doubt, this Section 6(b) does not affect any right the Licensor may have to seek remedies for Your violations of this Public License.

c. For the avoidance of doubt, the Licensor may also offer the Licensed Material under separate terms or conditions or stop distributing the Licensed Material at any time; however, doing so will not terminate this Public License.

d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.

d. Nothing in this Public License constitutes or may be interpreted as a limitation upon, or waiver of, any privileges and immunities that apply to the Licensor or You, including from the legal processes of any jurisdiction or authority.
