Thông thường, triệu chứng này biểu hiện dưới dạng cơn đau âm ỉ, nhức nhối ở mặt ngoài khuỷu tay; người bệnh thường khó chỉ chính xác vị trí đau. Cơn đau thường trở nên dữ dội hơn khi vận động, đặc biệt là khi ném bóng, tập thể dục dụng cụ hoặc thực hiện bất kỳ hoạt động nào gây áp lực lên cánh tay; cơn đau sẽ giảm bớt khi nghỉ ngơi. Nhiều vận động viên trẻ nhận thấy rằng họ không thể duỗi thẳng khuỷu tay hoàn toàn nữa; việc mất đi vài độ duỗi cuối cùng là hiện tượng phổ biến và thường xuất hiện trước khi cơn đau trở thành vấn đề nghiêm trọng.

Nếu một mảnh nhỏ của bề mặt khớp bắt đầu lỏng ra, khuỷu tay có thể bị kẹt, phát ra tiếng “lục cục” hoặc khó cử động; sau khi vận động, vùng này cũng có thể bị sưng. Tình trạng khuỷu tay bị kẹt hoàn toàn (không thể cử động trong chốc lát) thường là dấu hiệu cho thấy một mảnh nhỏ sụn và xương đã bong ra và trôi nổi bên trong khớp. Tình trạng này thường gặp ở những thanh thiếu niên hay tham gia các môn ném bóng (bóng chày, cricket) hoặc tập thể dục dụng cụ, khi mà mặt ngoài khuỷu tay phải chịu nhiều lực va đập liên tục.

Điều gì đang thực sự xảy ra

Phần ngoài của khuỷu tay là một khối xương tròn gọi là đầu xương cánh tay, được phủ bởi lớp sụn mịn. Khi khuỷu tay phải chịu nhiều lực mạnh trong thời gian dài khi đang phát triển, một vùng nhỏ dưới lớp sụn này có thể bị mất nguồn cung cấp máu và trở nên mềm ra. Đây chính là hiện tượng viêm xương sụn tách rời, hay còn gọi là OCD. Lớp sụn phía trên có thể vẫn giữ nguyên và tự lành, hoặc vùng sụn này có thể nứt, bong lên và cuối cùng tách ra thành một mảnh sụn lỏng lẻo trong khớp.

Cần lưu ý không nhầm lẫn tình trạng này với một bệnh lý nhẹ hơn, có khả năng tự khỏi ở trẻ em gọi là bệnh Panner, thường gặp ở trẻ nhỏ (dưới 10 tuổi), tự hồi phục sau khi nghỉ ngơi và không gây tổn thương lâu dài. OCD thực sự chỉ xảy ra ở trẻ lớn hơn và thanh thiếu niên; nếu mảnh sụn bị tách ra, có thể gây tổn thương khớp vĩnh viễn.

Tình trạng của lớp sụn đóng vai trò cực kỳ quan trọng. Một tổn thương ổn định (lớp sụn vẫn bám chắc, các vùng sụn tăng trưởng vẫn chưa đóng lại) có khả năng lành lại chỉ nhờ nghỉ ngơi. Ngược lại, tổn thương không ổn định (đã nứt, bong lên hoặc đã tách thành mảnh lỏng lẻo) thường không tự lành và hầu như cần phải phẫu thuật.

Những gì chúng tôi có thể làm

Bác sĩ Kieran Hirpara là người đứng đầu chuyên khoa phẫu thuật chi trên tại Bệnh viện tư nhân Mater Rockhampton. Chúng tôi hỗ trợ bệnh nhân từ lúc được bác sĩ đa khoa giới thiệu, tiến hành đánh giá kỹ lưỡng về tiền sử bệnh, khám lâm sàng và chẩn đoán hình ảnh. Đối với các vấn đề về khuỷu tay kéo dài nhiều năm, chúng tôi thường bắt đầu bằng các biện pháp điều trị không phẫu thuật; chỉ chỉ định phẫu thuật khi việc nghỉ ngơi và phục hồi chức năng vẫn không mang lại hiệu quả.

Bước đầu tiên và quan trọng nhất đối với các tổn thương ổn định là ngừng ngay các hoạt động gây kích thích (không ném vật, không chịu tải trọng lên cánh tay) trong vài tháng; sau đó mới dần dần trở lại sinh hoạt bình thường khi kết quả chụp chiếu cho thấy xương đã lành. Ở những vận động viên trẻ có các vùng sụn tăng trưởng vẫn chưa đóng kín, hầu hết các tổn thương này đều lành hoàn toàn nhờ phương pháp này. Việc nghỉ ngơi không phải là “không làm gì cả”; đó chính là phương pháp điều trị.

Khi tổn thương không ổn định, đã hình thành các mảnh xương lỏng lẻo, hoặc không thuyên giảm dù đã nghỉ ngơi, phẫu thuật thường được chỉ định. Phương pháp này hầu như luôn là phẫu thuật nội soi; lựa chọn cụ thể phụ thuộc vào đặc điểm của tổn thương:

  • Dọn dẹp và kích thích quá trình lành thương: loại bỏ các mảnh xương bị tổn thương và tạo ra những lỗ nhỏ trên bề mặt xương (kỹ thuật vi phẫu gãy xương) để lớp mô mới hình thành. Phương pháp này rất hiệu quả với các tổn thương có kích thước nhỏ.
  • Cố định lại mảnh xương bị tách rời: nếu mảnh xương bị tách ra có kích thước lớn và chất lượng tốt, bác sĩ có thể dùng đinh hoặc vít để gắn nó trở lại vị trí ban đầu.
  • Tái tạo bề mặt khớp: đối với các tổn thương lớn, đặc biệt là ở rìa khớp, người ta có thể ghép các mảnh sụn và xương khỏe mạnh vào chỗ khuyết hổng nhằm phục hồi bề mặt khớp (thủ thuật ghép mô).

Bác sĩ phẫu thuật sẽ quyết định phương pháp thích hợp dựa trên kích thước, vị trí tổn thương và khả năng bảo tồn sụn, thường dựa trên kết quả chụp MRI.

Những điều có thể mong đợi

Nhìn chung, tiên lượng khá tốt, đặc biệt là khi bệnh được phát hiện sớm và tổn thương vẫn còn ổn định. Nhiều bệnh nhân trẻ tuổi có tổn thương ổn định có thể hồi phục sau khi nghỉ ngơi và quay lại tập luyện thể thao. Sau phẫu thuật đối với các tổn thương không ổn định, hầu hết bệnh nhân đều giảm đau đáng kể và phục hồi được khả năng vận động; phần lớn vận động viên cũng có thể quay lại thi đấu, dù điều này có thể mất vài tháng và đòi hỏi một chương trình phục hồi chức năng bài bản.

Tuy nhiên, cần lưu ý một vài điểm. Các tổn thương lớn, những tổn thương lan tới rìa ngoài của khớp, cũng như những trường hợp khuỷu tay đã đóng sụn tăng trưởng thường có tiên lượng kém hơn và dễ cần phải phẫu thuật. Có thể vẫn còn một chút mất khả năng duỗi thẳng khuỷu tay; về lâu dài, một tổn thương nghiêm trọng có thể làm tăng nhẹ nguy cơ mắc viêm khớp khuỷu tay sau này. Đó là lý do tại sao việc phát hiện sớm và bảo vệ khớp là vô cùng quan trọng.

Khi nào nên đi khám bác sĩ

  • Đau vùng ngoài khuỷu tay kéo dài ở những vận động viên trẻ tham gia môn ném hoặc thể dục dụng cụ mà không thuyên giảm sau một hoặc hai tuần nghỉ ngơi; nên đi khám trước khi tiếp tục luyện tập.
  • Không thể duỗi thẳng khuỷu tay hoàn toàn: đây là dấu hiệu cảnh báo sớm cần được kiểm tra.
  • Cảm giác vướng, tiếng kêu lục cục hoặc khuỷu tay bị tắc lại: điều này cho thấy có thể có mảnh xương bị lỏng ra và cần chụp hình kiểm tra.
  • Sưng phù sau khi vận động và tình trạng này tái diễn nhiều lần.
  • Bất kỳ triệu chứng nào trong số này ở các vận động viên chưa trưởng thành về xương khớp đều cần được chăm sóc kịp thời, vì những tổn thương được phát hiện khi các vùng sụn tăng trưởng vẫn còn mở có khả năng lành lại tốt nhất mà không cần phẫu thuật.

Advanced reading: the deeper science (optional)

Phần này đi sâu hơn mức cần thiết để bạn có thể tự đưa ra quyết định điều trị. Bệnh viêm xương sụn vùng đầu xương cánh tay là một chủ đề đáng để tìm hiểu thêm, vì đây chủ yếu là một bệnh tiến triển âm thầm ở khuỷu tay trẻ em: đến khi gây đủ đau đớn khiến các vận động viên trẻ phải đến khám bác sĩ, tổn thương thường đã tiến triển đến mức các phương pháp điều trị đơn giản không còn hiệu quả nữa.

Tình trạng này phổ biến hơn mức các triệu chứng cho thấy

Kết quả sàng lọc cho thấy một bức tranh khác so với những gì ghi nhận được khi khám lâm sàng. Trong số 2,433 vận động viên bóng chày tuổi thiếu niên, tỷ lệ mắc bệnh viêm sụn xương đầu xương cánh tay – kể cả các trường hợp tiềm ẩn – là 3,4%. Những vận động viên bị ảnh hưởng thường bắt đầu chơi từ độ tuổi nhỏ hơn, chơi lâu hơn và gặp phải nhiều cơn đau khuỷu tay hơn [1].

Cụm từ “kể cả các trường hợp tiềm ẩn” là yếu tố quan trọng cần lưu ý. Một tỷ lệ đáng kể các tổn thương này được phát hiện qua việc quan sát chứ không phải do vận động viên tự phản ánh, điều này cho thấy cơn đau là dấu hiệu xuất hiện muộn và không đáng tin cậy ở nhóm vận động viên này – những người vốn cũng ít khi sẵn lòng báo cáo về tình trạng của mình.

Sự chậm trễ ảnh hưởng đến các phương pháp điều trị có thể áp dụng

Hậu quả của việc chậm trễ này thể hiện rõ qua các phương án điều trị. Trên 258 thanh thiếu niên, các tổn thương ở giai đoạn nặng được ghi nhận ở những bệnh nhân có thời gian xuất hiện triệu chứng lâu hơn; tuy nhiên, khi một tổn thương đã cần phẫu thuật, chính các chi tiết cụ thể của phương pháp điều trị chứ không phải thời gian mắc bệnh mới là yếu tố có tương quan mạnh nhất với khả năng trở lại tập luyện thể thao [2].

Có hai dấu hiệu trên phim X-quang cho thấy phương pháp điều trị không phẫu thuật sẽ thất bại: sự phình to của đầu xương quay và tuổi xương ở phía dùng để ném bóng phát triển nhanh hơn so với phía còn lại, cả hai đều liên quan đến bệnh ở giai đoạn nặng ở 245 bệnh nhân [3]. Cả hai dấu hiệu này đều cho thấy khớp khuỷu tay đã thay đổi cấu trúc do chịu tác động của lực, đây là những biểu hiện về mặt cấu trúc chứ không phải về triệu chứng lâm sàng.

Đây là lý do cần chụp hình ảnh cho những vận động viên trẻ bị đau khớp khuỷu tay kéo dài, thay vì chỉ điều trị như một vấn đề liên quan đến mô mềm; đồng thời cần coi trọng sự chênh lệch giữa hai bên cơ thể.

Đối với các tổn thương ổn định, phương pháp phẫu thuật đơn giản là đủ

Trong trường hợp mảnh xương vẫn ổn định và vùng tổn thương nhỏ, vấn đề còn tranh luận là nên chỉ làm sạch vùng tổn thương hay cần khoan xương phía dưới để thúc đẩy quá trình tái tạo sụn sợi. Trên 327 bệnh nhân, cả hai phương pháp làm sạch vùng tổn thương và vi phẫu thuật đều mang lại cải thiện đáng kể về mức độ đau, phạm vi vận động, các chỉ số đánh giá kết quả cũng như khả năng trở lại tập luyện thể thao. Vì kết quả điều trị trung hạn ở cả hai nhóm là tương đương nhau, nên việc chỉ làm sạch vùng tổn thương cũng là một phương pháp hợp lý đối với các vùng tổn thương tương đối nhỏ [4].

Đối với các tổn thương lớn hoặc không ổn định, các yếu tố quyết định sẽ thay đổi

Khi mảnh sụn đã tách rời hoặc vùng tổn thương có kích thước lớn, cần phải thay thế toàn bộ bề mặt sụn thay vì chỉ sửa chữa một phần, thường là bằng cách ghép một mảnh sụn và xương. Phương pháp này hiệu quả, nhưng có một điều cần lưu ý.

Dựa trên dữ liệu từ 492 bệnh nhân, tỷ lệ bệnh nhân trở lại tập luyện thể thao sau phẫu thuật là khá cao; đặc biệt, những bệnh nhân được ghép mảnh sụn và xương tự thân có khả năng trở lại mức độ thi đấu cao nhất trước phẫu thuật cao hơn so với những người chỉ được làm sạch hoặc cố định vùng tổn thương [5]. Tuy nhiên, một nghiên cứu năm 2024 trên 470 bệnh nhân sau khi ghép mảnh tự thân, dù xác nhận tỷ lệ trở lại thi đấu cao, cũng chỉ ra rằng cần phải tư vấn cho các vận động viên rằng họ có thể chỉ trở lại với mức độ thi đấu thấp hơn hoặc phải thay đổi vị trí thi đấu, đồng thời vị trí tổn thương ở vùng ngoài khớp có thể ảnh hưởng tiêu cực đến kết quả điều trị [6].

Cả hai phát hiện trên đều đúng; sự khác biệt này rất quan trọng đối với các vận động viên trẻ: khả năng họ trở lại tập luyện là rất cao, nhưng việc trở lại đảm nhận cùng một vai trò trong đội tuyển thì không chắc chắn. Kích thước và vị trí của tổn thương – chứ không phải lựa chọn giữa các phương pháp phẫu thuật – mới là yếu tố quyết định tiên lượng điều trị.

In all other cases the Licensor expressly reserves any right to collect such royalties, including when the Licensed Material is used other than for NonCommercial purposes.

Section 3 -- License Conditions.

Your exercise of the Licensed Rights is expressly made subject to the following conditions.

a. Attribution.

1. If You Share the Licensed Material (including in modified form), You must:

a. retain the following if it is supplied by the Licensor with the Licensed Material:

i. identification of the creator(s) of the Licensed Material and any others designated to receive attribution, in any reasonable manner requested by the Licensor (including by pseudonym if designated);

ii. a copyright notice;

iii. a notice that refers to this Public License;

iv. a notice that refers to the disclaimer of warranties;

v. a URI or hyperlink to the Licensed Material to the extent reasonably practicable;

b. indicate if You modified the Licensed Material and retain an indication of any previous modifications; and

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

c. The disclaimer of warranties and limitation of liability provided above shall be interpreted in a manner that, to the extent possible, most closely approximates an absolute disclaimer and waiver of all liability.

Section 6 -- Term and Termination.

a. This Public License applies for the term of the Copyright and Similar Rights licensed here. However, if You fail to comply with this Public License, then Your rights under this Public License terminate automatically.

b. Where Your right to use the Licensed Material has terminated under Section 6(a), it reinstates:

1. automatically as of the date the violation is cured, provided it is cured within 30 days of Your discovery of the violation; or

2. upon express reinstatement by the Licensor.

For the avoidance of doubt, this Section 6(b) does not affect any right the Licensor may have to seek remedies for Your violations of this Public License.

c. For the avoidance of doubt, the Licensor may also offer the Licensed Material under separate terms or conditions or stop distributing the Licensed Material at any time; however, doing so will not terminate this Public License.

d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.

d. Nothing in this Public License constitutes or may be interpreted as a limitation upon, or waiver of, any privileges and immunities that apply to the Licensor or You, including from the legal processes of any jurisdiction or authority.
