Những triệu chứng bạn đang gặp phải

Bạn đã bị gãy cổ tay cách đây một thời gian (có thể do ngã và chống tay ra đất), và vết gãy đã lành. Tuy nhiên, cổ tay vẫn chưa trở lại trạng thái bình thường. Cổ tay có thể trông hơi cong, lệch hoặc ngắn hơn so với bên kia; đôi khi còn có một chỗ lồi rõ rệt ở mặt sau cổ tay. Điều gì thực sự đang xảy ra

Xương quay đoạn xa là xương lớn hơn trong hai xương cẳng tay, nằm ở phía ngón tay cái, ngay tại cổ tay. Khi xương này bị gãy và liền lại ở tư thế không đúng, chúng ta gọi đó là sai liền xương: xương đã liền chắc nhưng có hình dạng không bình thường. Dạng phổ biến nhất là đầu xương quay bị nghiêng ra phía sau (hướng về mặt sau bàn tay) và xương trở nên ngắn hơn, khiến nó không còn thẳng hàng với xương trụ lân cận nữa.

Sự thay đổi nhỏ về hình dạng này gây ra nhiều hậu quả. Xương quay ngắn hơn và bị nghiêng làm rối loạn cơ chế nắm chặt, khiến lực nắm giảm đi. Nó cũng làm mất cân bằng khớp nối giữa hai xương cẳng tay gần cổ tay (khớp DRUJ) – khớp giúp chúng ta xoay cẳng tay; điều này khiến việc xoay tay trở nên khó khăn hoặc gây đau. Vì xương quay bị ngắn đi, xương trụ có vẻ “dài” hơn bình thường, gây áp lực lên các xương nhỏ và sụn ở phía ngón út, dẫn đến đau ở phía xương trụ. Nếu vết gãy xảy ra ngay trên bề mặt khớp và liền lại tạo thành chỗ lồi lõm, sụn sẽ bị mòn không đều và có thể gây viêm khớp về lâu dài.

Những gì chúng tôi có thể làm

Tại Bệnh viện tư nhân Mater Rockhampton, bác sĩ Kieran Hirpara dẫn dắt đội ngũ chuyên gia về phẫu thuật chi trên trong việc điều trị các chấn thương cổ tay này. Thông thường, chúng tôi bắt đầu bằng các phương pháp điều trị không phẫu thuật; tuy nhiên nếu cơn đau hoặc tình trạng cứng khớp vẫn tiếp diễn, chúng tôi sẽ cân nhắc phẫu thuật sau khi đánh giá kỹ lưỡng tiền sử bệnh và các kết quả chẩn đoán hình ảnh của bạn.

Không phải mọi trường hợp cổ tay bị biến dạng sau chấn thương đều cần phẫu thuật. Nếu mức độ biến dạng nhẹ và bạn vẫn kiểm soát được tình trạng, việc trị liệu bằng vật lý trị liệu, thay đổi thói quen sinh hoạt, đeo nẹp hỗ trợ và dùng thuốc giảm đau đơn giản là những phương pháp hoàn toàn hợp lý; chúng tôi thường bắt đầu từ đó.

Khi mức độ biến dạng nghiêm trọng và thực sự gây cản trở cho bạn (đau đớn, yếu sức nắm, hạn chế vận động), phương pháp phẫu thuật nhằm khắc phục nguyên nhân gốc rễ là cắt xương chỉnh hình. Nói một cách đơn giản, bác sĩ phẫu thuật sẽ cắt lại xương tại vị trí vết gãy cũ, đưa nó về đúng vị trí sinh lý rồi cố định bằng tấm kim loại và vít. Ở những chỗ cần kéo dài xương, khoảng trống tạo ra có thể được lấp đầy bằng mảnh ghép xương (xương lấy từ chính cơ thể bạn, xương hiến tặng hoặc vật liệu thay thế xương); tuy nhiên với các loại tấm kim loại hiện đại, nhiều ca chỉnh hình giờ đây vẫn có thể lành tốt mà không cần ghép xương. Nếu vấn đề chủ yếu nằm ở xương ở phía ngón út, bác sĩ phẫu thuật có thể rút ngắn xương quay thay thế hoặc kết hợp với phương pháp trên để giảm áp lực lên vùng đó.

Ngày nay, ca phẫu thuật thường được lên kế hoạch trước dựa trên mô hình 3D của chính cổ tay bạn; bác sĩ phẫu thuật cũng có thể sử dụng các mẫu hướng dẫn được chế tạo riêng để đảm bảo đường cắt và việc chỉnh lại xương đúng như kế hoạch; điều này đặc biệt hữu ích khi vết gãy xảy ra ngay trên bề mặt khớp.

Những điều bạn có thể mong đợi

Bạn nên biết trước rằng đây là một ca phẫu thuật phức tạp hơn so với ca phẫu thuật điều trị gãy xương ban đầu: bác sĩ phẫu thuật phải định hình lại xương đã liền thay vì chỉ khắc phục một vết gãy mới, vì vậy quá trình hồi phục sẽ phức tạp và kéo dài hơn. Xương cần phải liền lại ở vị trí mới; quá trình này thường mất vài tháng, và tấm kim loại cố định xương thường được giữ lại.

Khi nào nên gặp bác sĩ

  • Cổ tay đã lành nhưng vẫn bị đau, yếu hoặc cứng khớp nhiều tháng sau khi gãy xương; việc đánh giá lại độ thẳng hàng của cổ tay là rất cần thiết, vì việc chỉnh sửa sớm thường mang lại kết quả tốt hơn.
  • Đau ở phía ngón út của cổ tay tăng lên khi xoay cổ tay hoặc chống tay xuống mặt phẳng, đặc biệt khi khả năng xoay cẳng tay bị hạn chế hoặc gặp tiếng kêu lục cục.
  • Khả năng nắm chặt hoặc xoay tay ngày càng suy giảm, hoặc gặp khó khăn trong các hoạt động thường ngày như xoay chìa khóa, rót nước hoặc sử dụng dụng cụ.

Phương pháp phẫu thuật ảnh hưởng đến tỷ lệ phải mổ lại

Việc chỉnh sửa tình trạng xương liền sai vị trí đòi hỏi phải cắt bỏ phần xương đã liền, đưa nó về đúng vị trí và cố định bằng nẹp. Nẹp này có thể được đặt ở mặt trước hoặc mặt sau của xương quay; trước đây việc lựa chọn vị trí đặt nẹp được coi là tùy theo sở thích của bác sĩ phẫu thuật.

Tuy nhiên, qua việc so sánh trên 403 bệnh nhân, kết quả cho thấy sự khác biệt rõ rệt: việc đặt nẹp ở mặt trước giúp giảm tỷ lệ phải mổ lại xuống còn 9% so với 28%, đồng thời giảm tỷ lệ phải loại bỏ nẹp xuống còn 3% so với 18%. Tỷ lệ biến chứng nghiêm trọng giữa hai nhóm là tương đương nhau, lần lượt là 5% và 6%. Các tác giả khuyến cáo các bác sĩ phẫu thuật cần nhận thức rõ về nguy cơ cao hơn khi đặt nẹp ở mặt sau sau khi thực hiện phẫu thuật chỉnh sửa xương [1].

Nguyên nhân dẫn đến sự khác biệt này mang tính giải phẫu chứ không phải kỹ thuật. Ở mặt sau cổ tay, các gân duỗi chạy trực tiếp trên bề mặt xương; hầu như không có gì ngăn cách chúng với nẹp kim loại, nên nẹp này dễ gây kích ứng, thường phải được loại bỏ; trong những trường hợp nặng, nó thậm chí có thể làm mòn gân. Ngược lại, ở mặt trước cổ tay, một lớp cơ nằm giữa nẹp và các gân gập, giúp bảo vệ chúng.

Cần lưu ý rằng các biến chứng nghiêm trọng thì không có sự khác biệt giữa hai phương pháp. Ưu điểm của việc đặt nẹp ở mặt trước là giúp tránh được ca mổ thứ hai chỉ để loại bỏ nẹp, chứ không phải giúp ngăn ngừa các biến chứng nguy hiểm.

Tại sao X-quang không thể là yếu tố quyết định

Một đoạn xương quay xa bị liền sẹo sai vị trí có thể được nhận thấy trên mọi phim chụp; dị dạng này dường như cần được chỉnh sửa. Những chỉ số đo lường – mức độ nghiêng của bề mặt khớp, mức độ ngắn đi của xương quay – mới là những điều được nhấn mạnh trong các báo cáo theo dõi.

Tuy nhiên, chính dị dạng đó lại không phải là nguyên nhân gây ra triệu chứng. Những vấn đề mà bệnh nhân gặp phải là mất khả năng xoay cẳng tay, giảm sức nắm, đau ở phía xương trụ của cổ tay, hoặc cảm giác tự ti về ngoại hình. Mối liên hệ giữa các con số trên phim chụp và các triệu chứng này khá lỏng lẻo; đặc biệt ở người cao tuổi, những dị dạng nghiêm trọng thường vẫn được cơ thể chấp nhận mà không gây khó chịu.

Vì vậy, việc thực hiện phẫu thuật cắt xương chỉnh trục được cân nhắc dựa trên các triệu chứng và chức năng vận động, chứ không phải dựa trên hình ảnh trên phim chụp. Một cổ tay vẫn hoạt động ổn định, đáp ứng được nhu cầu sinh hoạt của người bệnh thì không cần phải chỉnh sửa, dù các góc đo trên phim có hơi sai lệch.

Những vấn đề mà ca phẫu thuật này nhằm khắc phục

Có hai nguyên nhân chính gây ra các biến chứng và triệu chứng ở hầu hết các trường hợp gãy xương không liền đúng vị trí.

Thứ nhất là mất khả năng xoay cẳng tay. Khi xương quay liền lại ngắn hơn hoặc bị lệch hướng, nó không còn có thể xoay quanh xương trụ một cách bình thường nữa; việc xoay lòng bàn tay lên trên cũng bị hạn chế – một chuyển động thiết yếu để cầm khay đồ, lấy tiền lẻ hoặc xoay chìa khóa.

Thứ hai là cơn đau ở phía xương trụ do chiều dài tương đối của xương này tăng lên. Khi xương quay bị ngắn lại do biến dạng, xương trụ trở nên dài hơn so với bình thường và bị ép vào các xương cổ tay, gây ra tình trạng va chạm giữa các xương – hậu quả thứ phát của vết gãy. Tình trạng này có thể được điều trị bằng cách khôi phục chiều dài ban đầu cho xương quay hoặc làm ngắn xương trụ; việc chọn phương pháp nào phụ thuộc vào mức độ biến dạng ở từng xương.

Việc hiểu rõ hai cơ chế này giúp giải thích tại sao việc chỉnh sửa vẫn nên được cân nhắc ngay cả nhiều năm sau chấn thương ban đầu: đó là để giải quyết vấn đề cơ học vẫn còn tồn tại, chứ không phải chỉ đơn thuần là sửa chữa vết gãy cũ.
