“Gãy xương do loãng xương” là gì?

Gãy xương do loãng xương là tình trạng xương bị gãy do một cú ngã có lực tác động thấp, tức là ngã từ độ cao ngang bằng hoặc thấp hơn chiều cao người đứng thẳng – loại va chạm mà một bộ xương khỏe mạnh ở người trẻ tuổi thường sẽ không gây ra vết thương gì. Khi xương bị gãy dưới tác động nhỏ như vậy, nguyên nhân thường là do xương đã trở nên mỏng và yếu đi; tình trạng này gọi là loãng xương. Cú ngã chỉ là yếu tố khởi phát; nguyên nhân thực sự là do xương yếu.

Ở chi trên, hai dạng gãy xương do loãng xương phổ biến nhất là gãy cổ tay (gãy xương quay đoạn xa – phần xương cẳng tay ngay phía trên cổ tay) và gãy vai (gãy xương cánh tay đoạn gần – phần trên cùng của xương cánh tay). Cả hai đều xảy ra theo cùng cơ chế: người bệnh vội đưa tay ra để hứng cú ngã, hoặc ngã đè trực tiếp lên vai; đồng thời cả hai đều thường là dấu hiệu cảnh báo đầu tiên cho thấy xương đang bị mỏng đi. Rất nhiều người không hề có triệu chứng gì của bệnh loãng xương cho đến khi một cú ngã nhẹ gây ra gãy xương; trên phim X-quang, vết gãy này trông nghiêm trọng hơn nhiều so với mức độ ngã vốn có.

Tại sao ngay cả một vết gãy xương đơn lẻ cũng đáng để can thiệp

Điều khiến hầu hết mọi người ngạc nhiên là: sau khi đã bị một lần gãy xương do loãng xương, nguy cơ bị gãy xương lần nữa (kể cả những vết gãy nghiêm trọng hơn ở hông hoặc cột sống) sẽ tăng gấp đôi. Việc bị gãy cổ tay ở độ tuổi 60–70 không chỉ đơn thuần là vết thương cần bó bột rồi bỏ mặc; đó là dấu hiệu sớm, dễ nhận thấy của một vấn đề nếu không được xử lý sẽ dẫn đến gãy xương hông – một chấn thương khó hồi phục hơn nhiều.

Vì vậy, chúng tôi coi vết gãy xương do loãng xương là tín hiệu cần can thiệp trên hai phương diện cùng lúc. Việc chỉ điều trị vết gãy hiện tại có thể giải quyết vấn đề ngay lúc này nhưng không làm giảm nguy cơ xảy ra trong tương lai. Hai nhiệm vụ cần thực hiện là: làm cho xương trở nên chắc khỏe hơn và ngăn ngừa những cú ngã tiếp theo.

Bước 1: Làm cho xương trở nên chắc khỏe hơn

Việc đầu tiên cần làm là xác định mức độ chắc khỏe (hoặc yếu) của xương bạn, rồi mới có biện pháp can thiệp phù hợp.

  • Đánh giá tình trạng xương. Xét nghiệm tiêu chuẩn là chụp DEXA, một phương pháp chụp X-quang không gây đau và sử dụng liều bức xạ thấp (thường chụp vùng hông và cột sống) để đo mật độ xương. Kết quả xét nghiệm sẽ cho biết bạn có mắc chứng loãng xương hay không và mức độ nghiêm trọng ra sao.
  • Bổ sung vitamin D và canxi. Xương được tạo thành từ canxi; cơ thể bạn không thể hấp thụ canxi đầy đủ nếu thiếu vitamin D. Nhiều người cao tuổi bị thiếu hụt vitamin D mà không hề hay biết. Chỉ cần một xét nghiệm máu đơn giản và việc bổ sung vitamin D nếu cần là có thể khắc phục tình trạng này; đây là một trong những biện pháp cơ bản và dễ thực hiện nhất.
  • Cân nhắc sử dụng thuốc bảo vệ xương. Trong trường hợp mật độ xương thấp, có nhiều loại thuốc đã được chứng minh là có hiệu quả trong việc làm chậm quá trình mất xương hoặc thúc đẩy tái tạo xương, từ đó giảm đáng kể nguy cơ gãy xương tiếp theo. Việc có nên dùng thuốc hay không cần được thảo luận kỹ với bác sĩ đa khoa hoặc chuyên gia.

Hiện nay, nhiều bệnh viện đã thành lập dịch vụ hỗ trợ bệnh nhân gãy xương, một nhóm chuyên trách việc theo dõi những người vừa bị gãy xương do loãng xương và đảm bảo họ được đánh giá sức khỏe xương một cách đầy đủ, thay vì việc đánh giá này bị bỏ sót giữa các công việc chăm sóc vết thương. Phòng ngừa ngã lần nữa: hai biện pháp quan trọng

Việc tăng cường sức mạnh xương giúp giảm thiểu tổn thương khi bị ngã; còn việc ngăn chặn ngã thì loại bỏ hoàn toàn nguyên nhân gây ra tai nạn. Hầu hết các trường hợp ngã thực chất đều có thể phòng tránh được. Những biện pháp hiệu quả nhất lại khá đơn giản và dễ thực hiện.

  • Loại bỏ các mối nguy hiểm trong nhà. Đây là nơi xảy ra nhiều vụ ngã nhất. Hãy gỡ bỏ hoặc cố định các tấm thảm lỏng lẻo, dây điện chằng chịt; dọn sạch đồ đạc trên lối đi và cầu thang; cải thiện ánh sáng (đặc biệt là ở cầu thang và lối đi tới phòng tắm vào ban đêm); lắp tay vịn ở cầu thang và thanh vịn trong phòng tắm. Việc đặt đèn ngủ giữa giường và nhà vệ sinh cũng giúp ngăn chặn rất nhiều ca ngã.
  • Chọn giày dép phù hợp. Những đôi giày vừa vặn, có đế chống trượt và cài chặt sẽ tốt hơn nhiều so với dép lê, giày rộng thùng thình hoặc việc đi chân trần. Đau chân và việc chọn giày không phù hợp đều làm suy giảm khả năng giữ thăng bằng.
  • Kiểm tra thị lực định kỳ. Hãy đi khám mắt và thay kính nếu cần. Nếu bạn đeo kính hai tròng hoặc kính đa tròng, hãy lưu ý rằng chúng có thể làm mờ các cạnh bậc thang; việc sử dụng một cặp kính riêng cho việc đi cầu thang hoặc ra ngoài đôi khi rất hữu ích.
  • Xem xét lại các loại thuốc đang dùng. Một số loại thuốc gây buồn ngủ, chóng mặt hoặc làm giảm huyết áp khi đứng dậy; nguy cơ ngã cũng tăng lên nếu bạn dùng nhiều loại thuốc cùng lúc. Dược sĩ hoặc bác sĩ đa khoa có thể xem lại danh sách thuốc và điều chỉnh hoặc đơn giản hóa chúng nếu cần.
  • Kiểm soát chứng chóng mặt và huyết áp khi đứng dậy. Nếu bạn cảm thấy chóng mặt khi ngồi dậy hoặc đứng lên, hãy báo cho bác sĩ; tình trạng này thường có thể điều trị được. Việc đứng dậy từ từ và dừng lại một chút trước khi bước đi là thói quen đơn giản nhưng rất hữu ích.
  • Tập luyện tăng cường sức mạnh cơ bắp và khả năng giữ thăng bằng: biện pháp hiệu quả nhất. Các phương pháp như Thái Cực Quyền hoặc các lớp tập luyện có cấu trúc (bác sĩ vật lý trị liệu hoặc bác sĩ đa khoa có thể giới thiệu cho bạn địa điểm phù hợp) sẽ giúp bạn vững vàng hơn; ngoài ra còn giúp xương luôn chịu lực và khỏe mạnh. Việc duy trì hoạt động thể chất là điều có lợi, chứ không gây nguy hiểm.

Ngã không phải là “điều tất yếu khi về già”

Người ta thường cho rằng việc ngã chỉ là điều xảy ra khi tuổi tác tăng lên; vì vậy mà họ dần thu hẹp phạm vi sinh hoạt để tránh điều đó: ít ra ngoài, ít làm việc, ít vận động. Bản năng này thực chất lại khiến mọi thứ tệ hơn, bởi càng ít vận động thì cơ thể càng yếu đi và mất thăng bằng. Ngã không phải là điều không thể tránh khỏi trong quá trình lão hóa; phần lớn các trường hợp ngã có thể được phòng ngừa nhờ những biện pháp thiết thực nêu trên. Mục tiêu không phải là bảo vệ bản thân quá mức, mà là duy trì sức mạnh, sự vững vàng và tự tin để tiếp tục làm những điều mình yêu thích.

Khi nào nên đi khám hoặc tìm sự giúp đỡ

  • Sau bất kỳ ca gãy xương do loãng xương nào, hãy hỏi bác sĩ đa khoa hoặc đội ngũ phẫu thuật về việc đánh giá sức khỏe xương (chụp DEXA và kiểm tra mức vitamin D, canxi, cũng như xem liệu việc dùng thuốc bảo vệ xương có phù hợp với bạn hay không). Đừng để cuộc trò chuyện dừng lại ở việc bó bột thôi.
  • Hãy đến gặp bác sĩ đa khoa nếu bạn thường xuyên bị ngã, cảm thấy mất thăng bằng hoặc chóng mặt, choáng váng, đặc biệt là khi đứng dậy. Đây là những vấn đề hoàn toàn có thể được đánh giá và cải thiện.
  • Nếu bạn bị ngã và lo ngại rằng mình có thể đã gãy xương (đau đột ngột, sưng tấy, biến dạng rõ rệt ở chi, hoặc không thể cử động chi được): hãy đi khám ngay lập tức.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.
