Những triệu chứng bạn đang gặp

Chứng rách gân gấp ngón tay thường xảy ra trong một khoảnh khắc mà bạn nhớ rõ, thường là lúc đang nắm chặt trong lúc chơi thể thao. Một ví dụ điển hình là khi bạn nắm lấy áo của đối thủ trong môn bóng bầu dục hoặc bóng đá khi họ cố gắng rút lui; từ đó mà có tên gọi “Jersey finger”. Bạn sẽ cảm thấy đau nhói đột ngột ở đầu ngón tay; sau đó ngón tay sưng lên và có thể xuất hiện vết bầm.

Dấu hiệu đặc trưng là bạn không thể gập được đầu ngón tay. Khớp đầu ngón tay (khớp nhỏ nhất gần móng, gọi là khớp DIP) sẽ không gập lại về phía lòng bàn tay khi bạn cố gắng nắm tay, dù các phần còn lại của ngón tay vẫn cử động được. Ngón tay áp út là ngón thường bị ảnh hưởng nhất. Một số người còn nhận thấy có khối sưng đau nằm dọc theo thân ngón tay hoặc thậm chí ở lòng bàn tay; đó chính là đầu mút của gân vốn đã đứt khỏi xương và bật trở lại như sợi dây cao su được thả ra.

Điều gì đang thực sự xảy ra

Có một gân mạnh có tên là gân gập ngón sâu (Flexor Digitorum Profundus – FDP) chạy dọc mặt lòng của mỗi ngón tay và gắn vào xương nhỏ ở đầu ngón tay. Chức năng của gân này là làm gập khớp ngón tay lại. Trong trường hợp bị chấn thương “jersey finger”, ngón tay bị ép thẳng ra đúng vào lúc cơ đang cố gắng kéo ngón tay lại để gập; điều này khiến gân bị rách khỏi chỗ gắn vào xương đầu ngón tay, đôi khi còn kéo theo cả một mảnh nhỏ xương.

Mức độ mà đầu gân bị rách co lại cũng rất quan trọng; các bác sĩ phẫu thuật phân loại chấn thương này dựa trên yếu tố này (theo phân loại Leddy–Packer). Khi đầu gân chỉ co lại một chút và vẫn nằm trong ngón tay, nó thường vẫn còn được cung cấp máu và có thể chờ điều trị sau. Ngược lại, khi đầu gân co hẳn về phía lòng bàn tay, nó mất đi nguồn cung cấp máu; đồng thời các cấu trúc dẫn hướng gân (hệ thống ròng rọc) bắt đầu bị sẹo hóa và đóng lại. Vì vậy, trường hợp “jersey finger” có đầu gân co hẳn về phía lòng bàn tay được coi là cần can thiệp khẩn cấp: càng để lâu thì việc đưa gân trở lại vị trí cũ và nối lại càng khó khăn. Trong những ca chấn thương có mảnh xương vẫn còn dính vào gân, mảnh xương này thường vẫn giữ nguyên vị trí và đôi khi có thể điều trị muộn hơn.

Những gì chúng tôi có thể làm

Tại phòng khám của chúng tôi, bác sĩ Kieran Hirpara điều trị tình trạng này bằng cách xác nhận chẩn đoán thông qua việc thăm khám kỹ lưỡng tiền sử bệnh và khám lâm sàng. Vì gân thường cần được nối lại trong vòng vài ngày, phẫu thuật thường được khuyến nghị ngay lập tức thay vì thử điều trị không phẫu thuật trước.

Chấn thương gân ngón tay hầu như luôn cần được điều trị bằng phẫu thuật để nối lại gân với xương. Có vài phương pháp thực hiện tùy thuộc vào tình trạng gân bị tổn thương:

  • Nếu gân bị tách rời hoàn toàn khỏi xương, người ta sẽ nối lại bằng cách đặt một móc nhỏ vào xương, hoặc dùng chỉ khâu xuyên qua đầu ngón tay rồi buộc chặt qua một nút nhỏ (phương pháp “nối kéo ra”).
  • Nếu gân kéo theo một mảnh xương, mảnh xương đó sẽ được đặt lại vị trí cũ và cố định bằng một chiếc ốc vít nhỏ, dây kim loại hoặc tấm kim loại mỏng.

Yếu tố quan trọng nhất là thời điểm điều trị. Gân bị co rút vào lòng bàn tay thường cần được phẫu thuật trong vòng khoảng 7 đến 10 ngày, trước khi bị mất độ dài và nguồn cung cấp máu. Những chấn thương có mảnh xương giữ lại thường có thể điều trị muộn hơn một chút. Vì vậy, việc được khám chữa sớm thực sự quyết định phương pháp điều trị phù hợp.

Nếu chấn thương không được phát hiện hoặc chỉ được nhận biết sau nhiều tuần, nhiều tháng, việc nối lại gân đơn giản sẽ không còn khả thi. Lúc này, các phương pháp điều trị phức tạp hơn mới được áp dụng: dùng mảnh ghép để tái tạo gân, hoặc nếu vấn đề nằm ở khớp đầu ngón tay, người ta sẽ cố định khớp này lại để tạo ra ngón tay ổn định và không còn đau. Sau mọi ca phẫu thuật, việc thực hiện chương trình vật lý trị liệu cho tay một cách có giai đoạn là điều thiết yếu nhằm giúp gân vận động an toàn mà không bị đứt ra trong quá trình hồi phục.

Những điều có thể mong đợi

Khi tình trạng gân gấp ngón tay bị tổn thương được phát hiện sớm và điều trị đúng cách, tiên lượng rất tốt; hầu hết bệnh nhân đều phục hồi được khả năng vận động mạnh mẽ và hữu ích của đầu ngón tay. Tuy nhiên, quá trình hồi phục không diễn ra nhanh chóng. Trong vài tuần đầu sau phẫu thuật, gân gấp ngón tay vừa được phục hồi vẫn còn rất dễ tổn thương; vì vậy bạn sẽ phải đeo nẹp bảo vệ và tuân theo chương trình vật lý trị liệu do chuyên viên chỉ định nhằm dần dần, một cách an toàn, khôi phục lại vận động. Việc vội vã trong quá trình này là nguyên nhân chính dẫn đến thất bại trong phục hồi. Thông thường, phải mất vài tháng mới có thể lấy lại sức nắm chắc và thực hiện các hoạt động nặng hơn; bác sĩ phẫu thuật và chuyên viên trị liệu sẽ hướng dẫn thời điểm thích hợp để bạn trở lại tập luyện thể thao hoặc làm việc chân tay.

Cần lưu ý rằng ngay cả khi phẫu thuật thành công, khớp đầu ngón tay vẫn có thể cứng hơn so với bên còn lại. Những ca chấn thương được điều trị muộn, hoặc cần ghép gân hoặc hợp nhất khớp, thường có mức độ vận động của đầu ngón tay bị hạn chế nhiều hơn; đó là lý do việc điều trị sớm mang lại lợi ích rất lớn.

Khi nào nên đi khám bác sĩ

  • Bạn không thể gập đầu ngón tay sau khi bị chấn thương do nắm chặt vật gì đó. Đây là dấu hiệu chính của tình trạng đứt gân gấp ngón tay; cần đi khám ngay lập tức: đừng chờ xem tình trạng có tự khỏi hay không. Những triệu chứng tưởng chừng như ngón tay bị “kẹt” hoặc bong gân thực chất có thể là do gân bị đứt rời khỏi xương; việc phẫu thuật sửa chữa sớm sẽ mang lại kết quả tốt nhất.
  • Xuất hiện khối sưng đau ở ngón tay hoặc lòng bàn tay sau chấn thương, đặc biệt kèm theo tình trạng không thể gập đầu ngón tay.
  • Đau dữ dội, sưng hoặc bầm tím ở ngón tay sau chấn thương khi chơi thể thao hoặc nắm chặt vật gì đó mà tình trạng không thuyên giảm.
  • Bất kỳ chấn thương ngón tay nào khiến vận động, cảm giác hoặc hình dạng ngón tay có vẻ bất thường: khi không chắc chắn, hãy đi khám ngay, vì khoảng thời gian để đạt được kết quả điều trị tốt nhất là rất ngắn.

Chấn thương gân ngón tay (Jersey finger) là trường hợp đáng để tìm hiểu thêm, vì đây là dạng chấn thương tay mà việc chậm trễ trong điều trị sẽ làm giảm đáng kể các phương án điều trị; gân cũng không giữ nguyên vị trí nơi bị rách. Hơn nữa, kết quả so sánh các phương pháp phục hồi trong phòng thí nghiệm lại chỉ ra một hướng điều trị, trong khi các yếu tố lâm sàng lại gợi ý hướng khác.

Tại sao thời gian lại quan trọng hơn bình thường trong trường hợp này

Gân gấp sâu các ngón tay có chức năng gập đầu ngón tay. Trong chấn thương này, gân bị bong ra khỏi chỗ bám trên đốt ngón tay cuối; thường xảy ra khi ngón tay mắc vào quần áo trong lúc nắm chặt vật gì đó – đây cũng là cơ chế gây chấn thương điển hình khiến nó có tên gọi như vậy.

Sau khi bị tách rời, gân sẽ co ngược lên phía trên ngón tay; mức độ co ngược này quyết định phương pháp điều trị. Nếu gân vẫn nằm gần đầu ngón tay, được duy trì nhờ nguồn máu và hệ thống các cái móc giữ gân, thì có thể nối lại một cách dễ dàng. Ngược lại, nếu gân co ngược vào lòng bàn tay, nguồn máu nuôi sẽ bị đứt, màng bọc mà gân phải đi qua bắt đầu bị sẹo hóa, và chính cơ cũng dần bị ngắn lại.

Các yếu tố tiên lượng bệnh cũng phản ánh chính điều này: mức độ co ngược của gân về phía gốc ngón tay, thời gian kể từ khi xảy ra chấn thương, và sự hiện diện của mảnh xương vỡ đi kèm [1]. Việc có mảnh xương vỡ lại là tin vui, vì điều đó thường có nghĩa là gân vẫn bám gần đầu ngón tay và chưa bị co ngược xa.

Đó là lý do tại sao chấn thương gân gấp sâu các ngón tay cần được điều trị ngay lập tức; dù vậy, bệnh nhân có thể cảm thấy mức độ cấp bách này không tương xứng với thực tế là ngón tay không quá đau và vẫn còn hoạt động được phần lớn.

Dễ bị bỏ sót; nguyên nhân là do cấu trúc giải phẫu

Ngón tay trông gần như bình thường. Gân flexor superficialis vẫn nguyên vẹn, nên khớp giữa vẫn có thể gập lại; khi quan sát một cách sơ qua khi bàn tay nắm lại, ta không thấy có gì bất thường.

Phương pháp kiểm tra đáng tin cậy duy nhất là giữ khớp giữa ở tư thế thẳng rồi yêu cầu người bệnh gập chỉ đầu ngón tay. Nếu gân flexor profundus bị đứt, động tác này sẽ không thể thực hiện được; và nó hoàn toàn không thể thực hiện được chứ không chỉ yếu đi. Điều này rất đáng lưu ý vì chấn thương này thường bị nhầm thành chứng bong gân trong tuần đầu tiên sau chấn thương – chính là tuần mà các phương án điều trị còn nhiều nhất.

Phòng thí nghiệm ưu tiên sử dụng móc khâu; trong thực hành lâm sàng thì hai phương pháp đều được dùng

Hiện có hai phương pháp sửa chữa chủ yếu: một là luồn sợi chỉ qua xương rồi buộc vào nút cố định trên đinh vít; hai là đặt móc khâu trực tiếp vào xương.

Khi tổng hợp các nghiên cứu sinh cơ học trên 201 mẫu, phương pháp sửa chữa bằng móc khâu cho thấy độ cứng ban đầu của cấu trúc cao hơn và hiện tượng hở khe ít xảy ra so với phương pháp dùng nút cố định, trong khi không có sự khác biệt đáng kể về lực gây hỏng cấu trúc tối đa [2].

Tuy nhiên, các nghiên cứu lâm sàng lại chỉ ra rằng các phương pháp sửa chữa dùng nhiều sợi chỉ mang lại kết quả tốt hơn, rằng hiện tượng hở khe có thể xảy ra ở phương pháp dùng nút cố định, và rằng nguy cơ móc khâu bị tách ra khỏi xương vẫn là vấn đề đáng lo ngại [1].

Như vậy, mỗi phương pháp đều có nhược điểm riêng: cấu trúc dùng nút cố định dễ bị giãn ra và hở khe; còn móc khâu có thể bị tách khỏi xương nhỏ. Ưu điểm về độ cứng được ghi nhận trong phòng thí nghiệm là có thật, nhưng chỉ giải quyết được một trong hai cơ chế gây hỏng cấu trúc. Điều này minh họa rõ ràng rằng việc vượt trội về mặt sinh cơ học không đồng nghĩa với việc mang lại kết quả lâm sàng tốt hơn.

Những gì sẽ mất đi nếu không được phẫu thuật khắc phục

Khi gân profundus không được phục hồi, đầu ngón tay sẽ không thể gập lại được một cách chủ động. Đối với nhiều người, tình trạng này vẫn chấp nhận được; trong những trường hợp chấn thương được phát hiện muộn, việc chấp nhận tình trạng đó là một lựa chọn hợp lý. Các phương án thay thế vào thời điểm đó là ghép gân từng bước hoặc cố định khớp đầu ngón tay – cả hai đều là những ca phẫu thuật phức tạp hơn nhiều so với việc nối gân sớm.

Hậu quả về mặt chức năng mang tính cụ thể chứ không phổ quát: khả năng nắm các vật nhỏ và việc khép tay lại quanh tay cầm. Đó là sự khác biệt giữa một bàn tay vẫn có thể hoạt động và một bàn tay hoạt động một cách hoàn hảo; mức độ quan trọng của điều này hoàn toàn phụ thuộc vào mục đích sử dụng bàn tay đó.
