“Thuốc chống đông máu” là thuật ngữ chung chỉ hai nhóm thuốc có cơ chế hoạt động hoàn toàn khác nhau; đối với phẫu thuật chi trên, hướng dẫn điều trị của chúng tôi phụ thuộc vào nhóm thuốc mà bạn đang dùng. Các thuốc chống kết tập tiểu cầu như aspirin và clopidogrel vẫn được tiếp tục sử dụng. Trước hết, máu thực sự đông lại như thế nào?

Để hiểu rõ các loại thuốc chống đông máu, chúng ta cần biết cách máu của bạn thông thường đóng kín các vết rách, vì mỗi loại thuốc lại can thiệp vào một bước khác nhau trong quá trình này.

Khi mạch máu bị cắt hoặc tổn thương, cơ thể sẽ khắc phục tình trạng này qua hai giai đoạn:

  1. Tiểu cầu lấp đầy vết thương: những “cứu tinh” nhanh chóng. Tiểu cầu là những mảnh tế bào nhỏ lưu thông trong máu mà không thực hiện chức năng gì cho đến khi cần thiết. Ngay khi tiếp xúc với thành mạch máu bị tổn thương, chúng trở nên dính vào nhau, tụ lại tại chỗ vết thương và kết cụm lại, tạo thành một nút chặn mềm, tạm thời chỉ trong vài giây đến vài phút. Có thể coi đây là “miếng vá khẩn cấp” của cơ thể.

  2. Quá trình đông máu làm cứng nút chặn đó. Nút chặn do tiểu cầu tạo ra vốn rất yếu. Để củng cố nó, máu sẽ khởi động một phản ứng dây chuyền gọi là quá trình đông máu, bao gồm chuỗi các protein gọi là yếu tố đông máu (phần lớn do gan sản xuất; một số cần vitamin K) lần lượt kích hoạt nhau, giống như một hàng domino. Quá trình này kết thúc bằng việc biến protein fibrinogen thành fibrin – một mạng lưới các sợi mảnh đan xen qua nút tiểu cầu, giúp nó trở thành cục máu đông vững chắc và ổn định.

Về cơ bản, cục máu đông cuối cùng là những tiểu cầu được giữ lại với nhau bởi mạng lưới fibrin. Khi cục máu đông này đóng kín vết thương thì đó là điều mong muốn; nhưng nếu quá trình này xảy ra một cách không mong muốn trong động mạch hoặc tĩnh mạch thì sẽ gây ra nhồi máu cơ tim, đột quỵ hoặc huyết khối tĩnh mạch sâu. Đó là lý do các thuốc chống đông máu được chỉ định để phòng ngừa những tình trạng này.

Đây là điểm mấu chốt cho toàn bộ phần tiếp theo: hai nhóm thuốc chống đông máu tác động vào hai giai đoạn khác nhau. Các thuốc chống kết tập tiểu cầu làm suy yếu bước đầu tiên (hoạt động của tiểu cầu); còn các thuốc chống đông máu lại can thiệp vào bước thứ hai (chuỗi phản ứng của các yếu tố đông máu). Chính sự khác biệt này cũng giải thích tại sao thời điểm dùng thuốc trước phẫu thuật lại khác nhau, vì việc “hủy tác dụng” của tiểu cầu không giống như việc “hủy tác dụng” của các yếu tố đông máu.

Thuốc chống kết tập tiểu cầu

(aspirin, clopidogrel/Plavix, ticagrelor/Brilinta, prasugrel/Effient)

Các thuốc này ngăn chặn những tế bào máu nhỏ gọi là tiểu cầu không bị dính vào nhau để hình thành cục máu đông.

  • Aspirin và clopidogrel (cũng như prasugrel) có tác dụng không thể đảo ngược: một khi thuốc tiếp xúc với tiểu cầu, tiểu cầu đó sẽ bị vô hiệu hóa vĩnh viễn. Cơ thể không thể khôi phục lại chức năng của nó. Cách duy nhất để trở lại trạng thái đông máu bình thường là tạo ra những tiểu cầu mới; mỗi tiểu cầu chỉ sống khoảng 7–10 ngày, và mỗi ngày có khoảng 10% tiểu cầu được thay thế. Vì vậy, việc ngưng dùng thuốc trong thời gian ngắn hầu như không mang lại hiệu quả gì; còn nếu ngưng lâu hơn thì lại khiến stent tim hoặc cơ thể dễ gặp nguy cơ đột quỵ trong suốt một tuần. Đó là lý do chúng tôi không yêu cầu bạn ngưng dùng các thuốc này: nguy cơ chảy máu nhỏ không đáng kể so với nguy cơ hình thành cục máu đông.
  • Ticagrelor có tác dụng có thể đảo ngược: thuốc chỉ tạm thời gắn vào tiểu cầu chứ không làm vô hiệu hóa vĩnh viễn, nên thời gian thải trừ ra khỏi cơ thể nhanh hơn so với các thuốc không thể đảo ngược.
  • Aspirin và clopidogrel vẫn phải tiếp tục dùng. Bạn không nên ngưng các thuốc này trước khi phẫu thuật, kể cả các dạng thuốc kết hợp (DuoCover, DuoPlidogrel, Piax Plus Aspirin). Thuốc chống đông máu

Các thuốc này tác động ở các bước tiếp theo trong quá trình đông máu, lên các yếu tố đông máu trong máu chứ không phải lên tiểu cầu.

Warfarin (Coumadin, Marevan). Warfarin ngăn chặn gan sản xuất nhiều yếu tố đông máu phụ thuộc vào vitamin K. Thuốc này không loại bỏ các yếu tố đông máu đã có sẵn trong máu; chúng phải được sử dụng hết và thải trừ tự nhiên, quá trình này mất khoảng 5 ngày. Theo hướng dẫn của chúng tôi, bạn nên ngừng dùng warfarin 48 giờ trước phẫu thuật; vì vậy vào ngày phẫu thuật thuốc vẫn còn một phần tác dụng. Các loại thuốc viên mới hơn: DOACs (apixaban/Eliquis, rivaroxaban/Xarelto, dabigatran/Pradaxa, edoxaban). Mỗi loại chỉ ức chế một yếu tố đông máu cụ thể; cơ thể thải trừ chúng nhanh chóng, với thời gian bán hủy khoảng 12 giờ. Do khả năng thải trừ nhanh như vậy, 48 giờ là khoảng thời gian đủ để thuốc hầu như được loại bỏ hoàn toàn khỏi cơ thể. Tại sao lại có thời điểm quy định như vậy — tóm tắt trong một dòng

Có hai quy tắc, mỗi quy tắc dành cho một nhóm thuốc.

Thuốc chống kết tập tiểu cầu (aspirin, clopidogrel và các loại thuốc phối hợp): tiếp tục uống như bình thường. Việc ngưng dùng thuốc trong thời gian ngắn hầu như không mang lại lợi ích gì, còn ngưng lâu thì rất nguy hiểm; vì vậy chúng tôi không yêu cầu bệnh nhân ngưng dùng thuốc.

Những việc bạn cần làm

  • Hãy mang theo danh sách chi tiết tất cả các loại thuốc bạn đang dùng đến buổi đánh giá tiền phẫu thuật, bao gồm cả aspirin và các thực phẩm bổ sung như dầu cá – những thứ cũng có thể ảnh hưởng đến khả năng đông máu.

Sau khi phẫu thuật

Các thuốc chống đông máu sẽ được dùng trở lại khi nguy cơ chảy máu đã giảm xuống; đôi khi là trong vòng một ngày, đôi khi lâu hơn sau các ca phẫu thuật có nguy cơ chảy máu cao. Bác sĩ sẽ thông báo chính xác thời điểm dùng thuốc trở lại. Cục máu đông mà thuốc chống đông ngăn chặn không hề biến mất chỉ vì bạn đã trải qua phẫu thuật; vì vậy việc dùng thuốc trở lại đúng thời điểm cũng quan trọng không kém việc ngưng dùng trước đó.

d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.
