Ano ito

Ang suture anchor ay isang maliit na device na humahawak sa mga tahi sa loob ng buto. Nilulutas nito ang isang problema na hindi kayang gawin ng mga tahi lamang: maaari kang magtahi ng soft tissue sa soft tissue, ngunit hindi mo maaaring itahi ang soft tissue sa buto. Ang mga all-suture anchor ang pinakabago, at kasalukuyang malawakang ginagamit sa balikat. Rotator cuff repair

Labral repair

Biceps tenodesis (loop and tack)

Hindi nito patutunugin ang mga airport scanner. Ang mga all-suture at PEEK anchor ay walang laman na metal. Kahit ang mga titanium anchor ay masyadong maliit para mag-trigger ng walk-through detector.

Maaari kang sumailalim sa MRI. Ang mga anchor na kasalukuyang ginagamit ay MRI-safe. Ang mga metal anchor ay maaaring magdulot ng lokal na blur sa imahe malapit sa anchor, na isa sa mga praktikal na bentahe ng mga mas bagong materyales — mas madaling basahin ang scan kung sakaling kailanganin muli ng iyong balikat ang imaging.

Hindi ang mga ito ang naglilimita sa iyong paggaling. Ang anchor ay secure mula sa sandaling mailagay ito; ang paggaling ng tendon-to-bone ang tumatagal ng ilang buwan, at iyon ang pinoprotektahan ng iyong sling at rehabilitation timeline. Ang problemang nilulutas ng mga anchor

Arthroscopy ang nagpalit dito. Sa pamamagitan ng mga keyhole portal, hindi mo maaaring ma-drill at ma-thread nang may katiyakan ang isang tunnel sa isang piraso ng buto na nakikita mo lamang mula sa isang panig. Biomechanically, ang anchor fixation ay nagreresulta sa mas kaunting gap formation sa ilalim ng cyclic loading kaysa sa transosseous repair, na may walang makabuluhang pagkakaiba sa ultimate load to failure [1]. Kaya ang tunnel ay hindi kailanman tunay na natalo. Ginawa lamang itong impractical dahil sa paglipat sa keyhole surgery, at ngayon ay bahagya na itong nagbabalik.

Apat na henerasyon ng anchor

Metal. Ang mga unang anchor ay gawa sa titanium o stainless steel, at napakahusay ng kapit ng mga ito. Ang kanilang mga kawalan ay lumabas kalaunan: nananatili sila sa buto nang permanente, at nagkakalat sila sa mga sumunod na imaging, na mahalaga sa isang joint na malamang na i-scan muli.

Bioabsorbable. Sumunod ang mga polymer anchor na idinisenyo upang matunaw sa loob ng ilang taon, at ang paglipat ay mabilis at halos kabuuan; inilalarawan ng isang review noong 2007 ang isang malaking paglipat mula sa metallic patungo sa bioabsorbable na mga anchor, na isinisisi ito sa mas mataas na complication rates ng metal [3].

Ang hatol na iyon ay hindi lubos na nanatili nang magamit na ang mga polymer mismo. Ano talaga ang PEEK

Ang Polyether ether ketone ay isang semi-crystalline thermoplastic mula sa pamilya ng polyaryletherketone. Hindi ito plastik sa pangkaraniwang kahulugan: ginagamit ito sa aerospace at sa mga spinal implant, natutunaw ito sa paligid ng 340°C, nakakayanan nito ang paulit-ulit na steam sterilisation, at ito ay chemically inert sa loob ng katawan.

Tatlong katangian ang nagpapaliwanag kung bakit nito pinalitan ang metal at ang mga unang absorbables bilang solid-anchor material.

Ito ay radiolucent. Ang PEEK ay hindi lumalabas sa X-ray at hindi nagdudulot ng scatter sa CT o MRI. Ang balikat na kinumpuni gamit ang mga PEEK anchor ay maaaring i-image pagkatapos at mabasa nang malinaw, isang tunay na bentahe sa isang joint na maaaring i-scan muli.

Ang stiffness nito ay malapit sa buto. Ang elastic modulus ng PEEK ay nasa rehiyon ng cortical bone, kung saan ang sa titanium ay humigit-kumulang sampung beses na mas mataas. Ang ginagawa ng PEEK sa buto, na hindi naman wala

Ang mga perianchor cyst, mga cavity na puno ng fluid sa buto sa paligid mismo ng anchor, ay nangyayari sa lahat ng uri ng anchor. Sa paghahambing ng all-suture, bioabsorbable, at PEEK anchors sa 213 na repair, ang mga cyst ay nabuo sa 10.8% ng mga kaso sa kabuuan, na walang makabuluhang pagkakaiba sa pagitan ng tatlong materyales sa mga clinical score, retear rate, o perianchor bone reaction [6].

Ang isang mas detalyadong paghahambing ay hindi gaanong pabor sa PEEK. Sa 73 na repair, sa kabila ng pagiging non-absorbable ng dalawa, ang mga all-suture anchor ay nagdulot ng mas kaunting osseous reaction kaysa sa PEEK, kung saan napagtanto ng mga may-akda na ang pagpili ng anchor ay dapat isaalang-alang hindi lamang ang initial fixation strength kundi pati na rin ang post-operative biological response [7].

Ang nakaka-assure na bahagi ay kung ano ang tila kahulugan ng mga cyst na ito. Partikular sa paligid ng mga PEEK anchor, ang pagbuo ng cyst ay nag-i-stabilize sa loob ng anim na buwan at walang kaugnayan sa functional scores o sa retear [8]. Nagkakaroon ito ng epekto sa revision. Ang anchorless transosseous repair, na hindi nag-iiwan ng anumang implant, ay ganap na umiiwas sa peri-implant cyst formation, at iginigiit ng mga may-akda na ang tunay na bentahe ay nasa prospekto ng karagdagang operasyon, kung saan ang hindi nagambalang buto ay nagbibigay ng mas maraming opsyon [9]. Bakit nanalo ang all-suture anchors sa buto, hindi sa lakas

Ang intuwisyon ay ang isang soft anchor ay dapat na mas mahina kaysa sa isang solid anchor. Hindi ito totoo. Ang tunay na argumento para sa mga ito ay ang butas. Ang isang solid anchor ay nangangailangan ng socket na kasing-laki ng body nito; ang isang all-suture anchor ay nangangailangan ng butas na humigit-kumulang kalahati ng diameter. Sa greater tuberosity, kung saan ang buto ay madalas na malambot, at kung saan ang isang revision ay maaaring mangailangan balang araw ng isang bagong lugar para sa fixation, ang dami ng buto na tinanggal sa unang pagkakataon ay isang tunay na konsiderasyon.

Sinuportahan ng clinical evidence ang paggamit sa mga ito kung saan ito pinaka-importante. Ang double-row suture-bridge cuff repair na gumagamit ng all-suture anchors para sa medial row ay nagresulta sa katulad na mahusay na outcomes sa parehong repair na gumagamit ng solid medial-row anchors [11].

Hindi rin sila ligtas sa cyst phenomenon, kung saan humigit-kumulang 40% ng mga pasyente pagkatapos ng double-row suture-bridge repair gamit ang all-suture anchors, na may mababang anchor insertion angle at isang malaking mediolateral tear na natukoy bilang mga risk factor [12]. Ang hindi gaanong kapansin-pansing rebolusyon ay nasa sinulid. Ang mga modernong suture ay hinabi mula sa ultra-high-molecular-weight polyethylene, ang FiberWire class ng mga materyales, at ang resulta ay isang suture na mas malakas nang husto kaysa sa tisyu na pinagdadaanan nito.

Ang flat na suture tape ang direktang tugon: ang mas malawak na footprint laban sa tendon ay nangangahulugan ng mas mababang pressure bawat unit area para sa parehong tension. Ang teorya ay tama at sumasang-ayon ang laboratoryo. Ang mga pasyente ay hindi gaanong sumasang-ayon: bagaman biomechanically superior, ang mga suture tape ay nagpakita ng katulad na retear rates at postoperative function kumpara sa mga kumbensyonal na round suture [13].

Kapag ang fixation ay sapat na ang lakas, ang pagpapalakas pa nito ay hindi na nagpapabago sa outcome; ang limiting factor ay naging ang biology ng paghilom ng tendon sa buto, na walang implant ang tumutugon.

Konpigurasyon: kung saan naroon ang mga natitirang pagkakaiba

Ang single-row repair ay naglalagay ng isang linya ng mga anchor sa gilid ng tendon. Ang mga double-row at suture-bridge construct ay nagdaragdag ng ikalawa at mas lateral na row, na nag-compress ng malawak na bahagi ng tendon laban sa buto. Sa kabuuan ng ebidensya, ang mga double-row at suture-bridge repair ay may mas mababang retear rates kaysa sa single-row repair sa karamihan ng mga kategorya ng laki ng punit, na may walang pagkakaiba sa pagitan ng double-row at suture bridge [14].

Ang mga knot-tying kumpara sa mga knotless design ay nagpapakita ng walang makabuluhang pagkakaiba sa mga retear rate [15].
