Gân gót chân là sợi gân chắc khỏe nằm phía sau mắt cá chân, giúp bạn đẩy người khi đi bộ hoặc chạy. Khi gân này bị đứt hoàn toàn, ca phẫu thuật sẽ khâu hai đầu gân bị đứt lại với nhau. Đối với những trường hợp gân vừa mới bị đứt, phẫu thuật có thể được chỉ định ngay lập tức mà không cần thử điều trị bảo tồn trước. Việc điều trị không phẫu thuật bằng nẹp hoặc giày chỉnh hình cũng cho kết quả tương đương phẫu thuật sau một năm, vì vậy chúng tôi sẽ cùng bạn thảo luận cả hai phương án này. Phương pháp phẫu thuật mở giúp giảm nguy cơ gân bị đứt lại, dù lại làm tăng nguy cơ gặp các biến chứng ở vết thương. Mục tiêu chính là giúp gân bám chắc, để bạn có thể chịu lực và trở lại các hoạt động sinh hoạt bình thường.
Trước khi phẫu thuật¶
Trong những ngày trước khi phẫu thuật, hãy làm theo các hướng dẫn chuẩn bị mà chúng tôi đưa ra. Bạn cần ngừng ăn và uống 7 giờ trước khi phẫu thuật. Chúng tôi yêu cầu thời gian 7 giờ này để có thể sắp xếp phẫu thuật sớm hơn nếu lịch phòng mổ diễn ra sớm hơn dự kiến. Bác sĩ phẫu thuật sẽ hướng dẫn bạn loại thuốc thường dùng nào cần ngừng uống và vào thời điểm nào. Hãy mang theo danh sách viết tay về tất cả các loại thuốc bạn đang dùng, đồng thời sắp xếp người đưa bạn về nhà sau phẫu thuật. Hãy mặc quần áo rộng rãi và thoải mái. Các phương pháp chẩn đoán như chụp X-quang, siêu âm hoặc chụp MRI có thể được sử dụng để lên kế hoạch phẫu thuật. Vào ngày phẫu thuật¶
Bạn sẽ đến khu vực tiếp nhận bệnh nhân phẫu thuật của bệnh viện, nơi bạn được làm thủ tục nhập viện và chuẩn bị cho ca mổ. Sau đó, bạn sẽ gặp bác sĩ gây mê – người chịu trách nhiệm làm bạn ngủ và kiểm soát cơn đau. Ca phẫu thuật này được thực hiện dưới gây mê toàn thân. Đôi khi, bác sĩ gây mê sẽ đề xuất thêm phương pháp chặn dây thần kinh vùng để giảm đau sau phẫu thuật; bác sĩ gây mê sẽ trao đổi với bạn về vấn đề này vào ngày hôm đó. Tiếp theo, bạn sẽ được đưa vào phòng mổ để tiến hành ca phẫu thuật.
Sau khi ca mổ kết thúc, bạn sẽ tỉnh dậy tại khu vực hồi sức. Các điều dưỡng sẽ ở bên cạnh bạn cho đến khi tác dụng của thuốc mê hết. Khi tình trạng sức khỏe của bạn ổn định, tùy thuộc vào loại phẫu thuật và diễn biến hồi phục, bạn sẽ được chuyển đến phòng bệnh hoặc xuất viện về nhà.
Quy trình phẫu thuật¶
Gân gót chân là sợi gân chắc khỏe nằm ở phía sau mắt cá chân. Khi bị rách, hai đầu gân tách rời ra giống như sợi dây bị đứt. Ca phẫu thuật này nhằm khâu hai đầu gân lại với nhau.
Bác sĩ phẫu thuật sẽ rạch một đường nhỏ phía sau mắt cá chân, ngay trên vị trí gân. Trong kỹ thuật vá gân ít xâm lấn, vết rạch nhỏ hơn nhiều; các mũi khâu được luồn qua da mà hầu như không cần mở bao gân. Bác sĩ cũng có thể dùng camera mỏng để kiểm tra xem hai đầu gân rách đã được nối đúng cách chưa. Các mũi khâu chắc chắn sẽ nối liền phần gân lành phía trên với xương gót phía dưới, giữ cho hai đầu gân dính vào nhau trong quá trình lành thương.
Sau khi nối hai đầu gân, các mũi khâu được buộc chặt và vết rạch được khâu lại rồi băng bó. Sau phẫu thuật, mắt cá chân của bạn sẽ được giữ ở tư thế bảo vệ nhằm tránh gây áp lực lên vùng vừa được vá gân.
Một số phương pháp vá gân cho phép bệnh nhân chịu trọng lượng lên chân sớm hơn so với kỹ thuật vá gân mở truyền thống. Bác sĩ phẫu thuật sẽ giải thích cho bạn biết phương pháp nào sẽ được áp dụng và lý do tại sao.
Sau khi phẫu thuật¶
Bạn sẽ tỉnh dậy tại khu vực hồi sức với các y tá ở gần bên cạnh trong lúc thuốc gây mê dần hết tác dụng. Mắt cá chân của bạn sẽ được băng bó và giữ ở tư thế bảo vệ nhằm tránh gây căng thẳng cho vết khâu. Chúng tôi sẽ cho bạn dùng thuốc giảm đau để giúp bạn cảm thấy thoải mái hơn; trước khi xuất viện, chúng tôi sẽ kiểm tra lại việc lưu thông máu, cảm giác và khả năng vận động của bàn chân. Trong 24 giờ đầu tiên sau khi về nhà, cần có người ở bên cạnh bạn. Đội ngũ y tế sẽ thông báo cho bạn biết là bạn có thể về nhà ngay trong ngày hay phải ở lại bệnh viện qua đêm. Trong hai tuần đầu, không đặt trọng lượng cơ thể lên bàn chân trừ khi các bác sĩ chỉ định khác. Chúng tôi sẽ giữ băng gạc trên vết thương khoảng 10 ngày; vui lòng đừng tự ý tháo băng ra trước thời hạn đó trừ khi có chỉ định từ chúng tôi. Chúng tôi sẽ thay hoặc gỡ băng khi bạn đến tái khám.
Quá trình hồi phục¶
Trong vài ngày đầu, mắt cá chân của bạn sẽ bị đau và sưng. Việc dùng thuốc giảm đau sẽ giúp bạn cảm thấy dễ chịu hơn; đồng thời việc nghỉ ngơi với chân được nâng cao cũng giúp giảm sưng. Hiện tượng bầm tím quanh mắt cá chân và lan xuống bàn chân là hiện tượng bình thường và sẽ dần mờ đi theo thời gian.
Khi xuất viện, mắt cá chân của bạn sẽ được giữ ở tư thế bảo vệ nhằm tránh gây áp lực lên vùng vừa được phẫu thuật. Trong hai tuần đầu, bạn không nên đặt trọng lượng lên chân trừ khi các bác sĩ chỉ định khác. Chúng tôi sẽ giữ băng gạc trên vết thương khoảng 10 ngày rồi mới thay hoặc tháo ra khi bạn tái khám. Sau đó, chuyên viên vật lý trị liệu sẽ hướng dẫn bạn thực hiện các động tác vận động nhẹ nhàng cho mắt cá chân. Ban đầu, bạn sẽ được gập bàn chân xuống dưới một cách tự do, đồng thời tránh để bàn chân gập lên trên quá mức. Khi vết thương lành dần, bạn sẽ dần đặt nhiều trọng lượng lên chân và bắt đầu đi bộ xa hơn. Một số ca phẫu thuật cho phép bạn chịu trọng lượng sớm hơn những ca khác; bác sĩ phẫu thuật sẽ thông báo cho bạn kế hoạch cụ thể.

Những biến chứng có thể xảy ra¶
Hầu hết bệnh nhân đều hồi phục tốt, nhưng đôi khi vẫn có thể gặp phải các vấn đề. Bác sĩ phẫu thuật và đội ngũ y tế sẽ theo dõi sát sao để phát hiện sớm bất kỳ bất thường nào.
Các vấn đề về vết thương là điều cần chú ý nhất. Vùng da phủ lên gân gót chân có nguồn cung cấp máu kém, nên vết cắt có thể lâu lành hoặc các mép vết thương bị tách ra. Hãy chú ý nếu thấy vùng da đỏ lan rộng ra từ vết thương, có dịch tiết, đau tăng dần hoặc sốt. Nếu gặp bất kỳ triệu chứng nào, hãy gọi cho phòng khám ngay thay vì chờ đến lần khám tiếp theo. Một số bệnh lý như tiểu đường và việc hút thuốc làm tăng nguy cơ gặp vấn đề về vết thương; vì vậy chúng tôi sẽ trao đổi với bạn về các yếu tố nguy cơ cá nhân trước khi phẫu thuật.
Nhiễm trùng có thể xảy ra ở vết thương hoặc sâu hơn quanh gân. Thông thường, người bệnh sẽ cảm thấy đau sâu, nhói theo nhịp mà thuốc giảm đau thông thường không làm dịu đi; kèm theo cảm giác nóng rát, sưng và đỏ da. Các mũi khâu giúp giữ hai đầu gân lại với nhau trong quá trình lành thương; tuy nhiên gân vẫn có thể bị rách trở lại trước khi đủ chắc khỏe. Tình trạng này thường xảy ra khi có lực đẩy mạnh đột ngột, vấp ngã hoặc trượt chân. Bạn sẽ cảm thấy tiếng “cạch” hoặc “rắc” rõ rệt ở phía sau mắt cá chân, kèm theo cảm giác yếu đi và khó thực hiện động tác đẩy người. Nếu gặp phải tình huống này, hãy gọi ngay cho phòng khám hoặc đến phòng cấp cứu.
Dây thần kinh chạy dọc phía ngoài mắt cá chân có thể bị kích thích trong quá trình phẫu thuật. Điều này gây ra cảm giác tê, ngứa ran hoặc nóng rát dọc theo mặt ngoài bàn chân. Hãy báo lại với bác sĩ trong lần tái khám tới; nếu triệu chứng nặng, hãy gọi sớm hơn.
Sau chấn thương và phẫu thuật, huyết khối có thể hình thành trong các tĩnh mạch sâu ở bắp chân. Các dấu hiệu cảnh báo gồm sưng đột ngột, đau nhức hoặc cảm giác nặng nề ở bắp chân; nếu cục máu đông di chuyển lên phổi sẽ gây khó thở hoặc đau ngực. Mô sẹo đôi khi có thể dính gân vào các mô xung quanh, gây cảm giác “lục cục” hoặc lạo xạo, hoặc cứng khi vận động. Hãy đề cập vấn đề này trong lần tái khám tới.

Anatomy & Pathophysiology¶
Bony Anatomy¶
- The ankle mortise is formed by the tibial plafond, medial malleolus, and lateral malleolus [3].
- The ankle mortise articulates with the dome of the talar body [3].
- The talar dome is wider anteriorly and narrower posteriorly [3].
- The ankle mortise widens 1 to 1.5 mm during motion from plantar flexion to dorsiflexion [3].
- Medial and superior clear spaces appear wider with the foot in plantar flexion [3].
- The distal fibula has a convex medial surface that articulates with the concave incisura fibularis of the distal lateral tibia [3].
- The fibula rotates approximately 2 degrees within the incisura during ankle motion and ambulation [3].
- Ankle dorsiflexion results in external rotation and proximal translation of the fibula [3].
Ligamentous Anatomy¶
- The lateral ankle ligaments function as restraints to varus and inversion forces at the ankle [3].
- The anterior talofibular ligament (ATFL) originates from the anteroinferior aspect of the lateral malleolus, 1 cm proximal to its tip, and extends to the lateral aspect of the talar neck [3].
- The calcaneofibular ligament (CFL) extends from the tip of the lateral malleolus to the lateral aspect of the calcaneus [3].
- The posterior talofibular ligament (PTFL) extends from the posterior lateral malleolus to the posterolateral talus [3].
- The ATFL is the weakest ankle ligament [3].
- The PTFL is the strongest ankle ligament [3].
- The distal tibiofibular joint and fibula provide stability against lateral talar translation [3].
- The deltoid ligament complex is the primary ankle stabilizer during stance [3].
- The deep deltoid ligament extends from the apex of the medial malleolus to the medial talar body [3].
- The deep deltoid ligament functions primarily to resist lateral talar translation and external rotation [3].
- The posterior deep deltoid is the most important component of the deep deltoid ligament [3].
- The superficial deltoid ligament extends from the distal medial malleolus to the navicular bone, sustentaculum tali of the calcaneus, medial talus, and spring ligament [3].
- The superficial deltoid ligament functions primarily to resist valgus and eversion ankle forces [3].
- The deltoid ligament consists of superficial and deep layers, with at most six bands of which only three are constant: the tibionavicular ligament, tibiospring ligament, and deep posterior tibiotalar ligament [8].
- The tibiocalcaneal portion of the superficial deltoid ligament is the strongest component and resists eversion of the calcaneus [8].
- The deep portion of the deltoid ligament is organized into two short, thick, discrete bands: the anterior and posterior deep tibiotalar ligaments [8].

- Valgus tilting of the talus within the mortise requires complete rupture of both the superficial and deep deltoid ligaments [8].
Neurovascular Anatomy¶
- The superficial peroneal nerve penetrates the deep fascia and lies subcutaneously 8 to 10 cm proximal to the tip of the lateral malleolus, anterior to the subcutaneous border of the fibula shaft [6].
- The deep peroneal nerve accompanies the anterior tibial artery between the tendons of the anterior tibial and extensor digitorum longus muscles, just lateral to the extensor hallucis longus tendon [6].
- The saphenous nerve is located just medial or posterior to the saphenous vein in a slightly deeper plane, 3 to 5 cm proximal to the tip of the medial malleolus [6].
Biomechanics¶
- The ankle joint is responsible for most sagittal plane motion of the foot and ankle [3].
- Ankle plantar flexion ranges from 23 to 48 degrees [3].
- Ankle dorsiflexion ranges from 10 to 23 degrees [3].
- The ankle joint also contributes to inversion, eversion, and rotation [3].
Investigations¶
- MRI is sensitive for detecting osteochondral lesions of the ankle, but the edema pattern frequently overestimates the severity of the injury [12, 13].
- Linear fluid signal deep to subchondral bone on MRI indicates an unstable osteochondral lesion [12, 13].
- MRI has a sensitivity of 92% for predicting stable versus unstable osteochondral lesions [12, 13].
- CT scans are helpful for evaluating bony lesions, determining the integrity of subchondral bone, identifying cysts, and preoperative planning for osteochondral lesions [12, 13].
- AP, mortise, and lateral weight-bearing ankle x-rays may not demonstrate subtle osteochondral lesions [12, 13].
- Advanced imaging is often helpful in the diagnosis of foot and ankle injuries when combined with a thorough clinical examination [16].
- MRI is used for the evaluation of chronic Achilles tendon ruptures [1].
- MRI is used for the evaluation of traumatic ligamentous injuries of the ankle and foot [1].
- MRI is used for the evaluation of musculotendinous structures of the ankle [1].

Treatment¶
Operative Technique: Lindholm Repair¶
- The patient is positioned prone for the procedure [14].
- A posterior curvilinear incision is made extending from the midcalf to the calcaneus [14].
- The deep fascia is incised in the midline to expose the tendon rupture [14].
- Ragged ends of the tendon are debrided [14].
- Tendon ends are apposed using a box type of mattress suture made of heavy nonabsorbable suture material or wire [14].
- Fine interrupted sutures are also used to appose the tendon ends [14].
- Two flaps are fashioned from the proximal tendon and gastrocnemius aponeurosis [14].
- Each flap is approximately 1 cm wide and 7 to 8 cm long [14].
- The flaps are left attached at a point 3 cm proximal to the site of rupture [14].
- Each flap is twisted 180 degrees on itself so that its smooth external surface lies next to the subcutaneous tissue [14].
- The flaps are turned distally over the rupture [14].
- Each flap is sutured to the distal stump of the tendon and to one another [14].
- The flaps cover the site of rupture completely [14].
- The wound is closed with care taken to approximate the tendon sheath over the site of repair [14].
Operative Technique: Lynn Repair¶
- The tendon sheath is opened in the midline [14].
- The foot is held in 20 degrees of plantar flexion during the repair [14].
- The ends of the Achilles tendon are sewn together with 2-0 absorbable sutures without excising the irregular edges [14].
- If the plantaris tendon is intact, its insertion on the calcaneus is divided [14].
- The plantaris tendon is fanned out to form a membrane using forceps, beginning distally [14].
- The fanned-out plantaris membrane is placed over the repair of the Achilles tendon [14].
- The plantaris membrane is sutured in place with interrupted sutures [14].
- When possible, the Achilles tendon is covered for 2.5 cm both proximal and distal to the repair [14].
- If the plantaris tendon is ruptured, it is dissected free from the Achilles tendon for several centimeters [14].
- The ruptured plantaris tendon is divided proximally using a tendon stripper [14].
- The plantaris tendon is pulled distally into the incision and fanned out as a free graft [14].
- The sheath of the Achilles tendon is closed as far distally as possible without tension [14].
Postoperative Care¶
- Postoperative care for these repairs is the same as that used after treatment of acute rupture of the Achilles tendon [14].
