Nhiễm trùng đầu ngón tay là tình trạng nhiễm khuẩn xảy ra ở phần mô mềm ở đầu ngón tay – nơi bạn dùng để ấn. Phần mô này được chia thành nhiều ngăn nhỏ bởi các vách sợi cứng. Khi nhiễm trùng khiến các ngăn này chứa đầy mủ, áp lực bên trong sẽ tăng lên nhanh chóng. Chính áp lực này gây ra cảm giác đau dữ dội và nhức nhối.

Thông thường, bạn sẽ nhận thấy đầu ngón tay bị sưng, đỏ và đau trước tiên. Toàn bộ phần mô mềm có thể trở nên căng cứng và đau khi chạm vào. Tình trạng này thường khởi phát sau một chấn thương nhỏ như bị gai gỗ đâm, bị kim đâm hoặc sau khi lấy máu xét nghiệm. Nếu cơn đau dữ dội kéo dài 12 giờ hoặc lâu hơn, thường đã hình thành ổ áp xe (một túi chứa mủ).

Cơn đau thường diễn ra liên tục chứ không thưa thớt. Việc ấn vào đầu ngón tay, cầm bút, gõ máy tính hoặc xoay chìa khóa đều gây đau đớn. Bất kỳ tác động nào làm tăng áp lực lên vùng sưng cũng khiến tình trạng tệ hơn. Nhiều người gặp khó khăn trong việc nhặt đồng xu, cài cúc áo hoặc cầm cốc bằng tay bị ảnh hưởng.

Nếu không được điều trị, nhiễm trùng có thể lan rộng. Nó có thể lan quanh móng tay hoặc đi lên phía lòng bàn tay dọc theo bao gân – cấu trúc giúp ngón tay có thể gập lại. Điều này khiến cả ngón tay bị sưng và đau khi cử động.

Một tình trạng có triệu chứng tương tự là mụn nước do virus herpes ở đầu ngón tay. Tình trạng này gây ra các mụn nước đau đớn trên một ngón tay, thường xuất hiện vài ngày sau chấn thương nhẹ. Khác với nhiễm trùng đầu ngón tay, các mụn nước này chứa dịch trong suốt chứ không phải mủ. Điều này rất quan trọng vì mụn nước do virus không nên được chích vỡ hay dẫn lưu.

Thông thường, vi khuẩn xâm nhập qua những vết rách trên da, ví dụ như vết găm mảnh gỗ hoặc vết kim đâm. Loại vi khuẩn gây bệnh phổ biến nhất là Staphylococcus aureus – một vi khuẩn thường gặp trên da. Do các ngăn này kín hoàn toàn, nhiễm trùng khó lan rộng sang các vùng lân cận. Thay vào đó, áp lực tích tụ bên trong có thể làm giảm hoặc ngừng cung cấp máu cho mảnh xương nhỏ ở đầu ngón tay.

Nếu không được điều trị, mủ sẽ tìm cách thoát ra ngoài. Nó có thể lan ra vùng dưới móng, đi dọc theo đường hầm gân hướng về phía lòng bàn tay, hoặc thâm nhập vào chính xương. Tình trạng nhiễm trùng xương được gọi là viêm tủy xương; nhiễm trùng khớp gần đó gọi là viêm khớp nhiễm trùng. Trường hợp mủ tích tụ ở mức độ nông hơn thì có thể làm rách da. Những điều có thể xảy ra

Nếu được phát hiện sớm, tình trạng nhiễm trùng ở đầu ngón tay thường hồi phục tốt. Mủ được dẫn lưu ra ngoài, áp lực trong mô mềm giảm xuống và cơn đau cũng nhanh chóng thuyên giảm. Hầu hết bệnh nhân đều có thể sử dụng ngón tay một cách bình thường sau khi lành bệnh.

Tiên lượng phụ thuộc rất nhiều vào thời điểm điều trị. Việc nhận biết và điều trị sớm các nhiễm trùng ở tay là yếu tố then chốt để đạt kết quả tốt nhất. Nếu điều trị bị trì hoãn, nhiễm trùng có thể lan sang xương hoặc theo các ống gân, dẫn đến tình trạng cứng khớp hoặc mất chức năng vận động vĩnh viễn ở tay. Trong những trường hợp nặng, việc chẩn đoán muộn có thể dẫn đến cắt cụt chi hoặc tử vong. Vì vậy chúng tôi luôn khuyến cáo không nên chần chừ hay chờ xem tình hình sẽ ra sao.

Ngay cả khi được chăm sóc tốt, các nhiễm trùng ở tay vẫn có nguy cơ gây biến chứng, và những biến chứng này thường rất khó kiểm soát. Nếu nhiễm trùng lan đến xương, cần kết hợp cả phẫu thuật và kháng sinh để loại bỏ hoàn toàn. Trong một số ít ca nghiêm trọng, việc cắt bỏ ngón tay bị ảnh hưởng sớm có thể là biện pháp an toàn nhất để ngăn bệnh lan rộng.

Có một yếu tố sinh học đáng mừng: khi phần lớn xương đầu ngón tay phải được cắt bỏ nhưng phần gốc vẫn còn nguyên, phần xương còn lại thường có khả năng tái tạo đáng kể. Đầu ngón tay có thể phục hồi nhiều hơn mức người ta thường nghĩ.

Nguyên tắc cấp cứu cũng áp dụng tương tự với các dạng nhiễm trùng có triệu chứng tương tự. Nhiễm trùng ở ống gân cần được chẩn đoán sớm và điều trị kịp thời để tránh mất chức năng vận động vĩnh viễn của tay; nếu để lâu, tình trạng này còn có thể làm gián đoạn nguồn cung máu đến ngón tay. Còn nếu bạn mắc chứng mụn nước do virus herpes như đã mô tả trước đó, việc chích rạch vùng tổn thương lại là điều không nên làm, vì điều đó có thể tạo điều kiện cho vi khuẩn xâm nhập thêm vào vùng bị nhiễm virus.

Tóm lại: Nếu được điều trị kịp thời, hầu hết các ca nhiễm trùng ở đầu ngón tay đều khỏi và ngón tay vẫn hoạt động bình thường. Ngược lại, nếu không được can thiệp, áp lực sẽ tiếp tục tăng lên và nhiễm trùng sẽ lan rộng. Sự khác biệt giữa hai hướng đi này nằm ở tốc độ bạn tìm đến sự chăm sóc y tế.

Khi nào nên đi khám bác sĩ

Nhiễm trùng ngón tay là một dạng nhiễm khuẩn; các dấu hiệu cảnh báo liên quan đến tốc độ phát triển của bệnh, chứ không phải việc nghỉ ngơi và chờ đợi. Hãy đến phòng cấp cứu nếu cơn đau dữ dội và nhói, hoặc nếu toàn bộ ngón tay bị sưng, đỏ và đau khi cử động. Điều này có thể là dấu hiệu mủ đã lan vào ống gân, và cần được đánh giá ngay trong ngày. Nếu đầu ngón tay vẫn đau kéo dài 12 giờ trở lên, hãy yêu cầu được khám gấp, vì lúc đó thường đã hình thành áp xe. Quan niệm thông thường về nhiễm trùng là kháng sinh sẽ tiêu diệt vi khuẩn và phẫu thuật chỉ đóng vai trò là biện pháp dẫn lưu mủ. Tuy nhiên, đối với trường hợp nhiễm trùng ngón tay không biến chứng thì hoàn toàn ngược lại.

Một nghiên cứu trên 46 ngón tay được điều trị bằng phương pháp cắt bỏ tổn thương mà không sử dụng bất kỳ loại kháng sinh nào sau phẫu thuật cho thấy 45 ngón tay đều lành lại. Trường hợp duy nhất không lành là do việc cắt bỏ chưa triệt để, và tình trạng này đã được khắc phục bằng phẫu thuật [1]. Cùng nghiên cứu này cũng ghi nhận kết quả khảo sát của Hiệp hội Phẫu thuật Bàn tay Pháp: 66% các bác sĩ phẫu thuật bàn tay không kê đơn kháng sinh sau khi dẫn lưu mủ trong các ca nhiễm trùng ngón tay không biến chứng; 63% cũng không dùng kháng sinh sau khi điều trị viêm quanh móng [1].

Điều kiện áp dụng rất quan trọng: những kết quả trên chỉ đúng với các ca nhiễm trùng ngón tay không biến chứng ở những bệnh nhân không có yếu tố nguy cơ nào khác. Trong trường hợp có viêm xương, viêm bạch mạch lan rộng, tổn thương bao gân gập, bệnh đái tháo đường hoặc suy giảm miễn dịch, thì phương pháp điều trị cần phải thay đổi hoàn toàn.

Nếu phải dùng kháng sinh, các loại thường được chọn lại thường không phù hợp

Giả định thứ hai cần bác bỏ là việc cho rằng các loại kháng sinh tiêu chuẩn dùng đầu tiên sẽ có hiệu quả.

Một nghiên cứu kéo dài 10 năm trên 815 ca nhiễm trùng vùng tay ở khu vực thành thị cho thấy dù tỷ lệ mắc MRSA nói chung đã giảm, loại vi khuẩn này vẫn là tác nhân phổ biến nhất; đồng thời tình trạng kháng thuốc clindamycin và levofloxacin liên tục gia tăng trong suốt thập kỷ đó. Kết luận của các tác giả trong nghiên cứu này khá thẳng thắn: phác đồ điều trị dựa trên kinh nghiệm cho các ca nhiễm trùng vùng tay nên tránh dùng penicillin, các thuốc beta-lactam, clindamycin và levofloxacin [2].

Đó là những loại thuốc thường được dùng một cách tự phát. Đây cũng là lý do vì sao các ca nhiễm trùng vùng tay được “điều trị bằng kháng sinh” nhưng vẫn không khỏi lại là chuyện thường gặp chứ không phải điều bất ngờ; đồng thời lý do vì sao phát hiện nêu trên – rằng các ca nhiễm trùng vùng tay đã được dẫn lưu đúng cách thì không cần dùng kháng sinh – không hề mâu thuẫn như tưởng ban đầu.

Tại sao vùng mô mềm đầu ngón tay khác biệt so với các vùng khác trên cơ thể

Vùng mô mềm đầu ngón tay không đơn thuần là một túi chứa mô mỡ. Nó được chia thành nhiều ngăn nhỏ kín nhau nhờ các vách sợi chạy từ da xuống đến màng xương của đốt ngón tay cuối.

Điều này dẫn đến hai hậu quả. Mủ tích tụ trong một ngăn như vậy không có chỗ để lan ra, khiến áp lực tăng nhanh; đó là lý do tại sao bệnh áp xe đầu ngón tay gây đau đớn nhiều hơn nhiều so với kích thước của vết thương, và cơn đau thường có tính chất nhịp nhàng, dữ dội hơn vào ban đêm. Áp lực này lại tác động trực tiếp lên xương và các mạch máu nhỏ nuôi dưỡng xương; vì vậy nếu không được điều trị, áp xe này có thể dẫn đến viêm tủy xương đốt ngón tay cuối.

Điều này cũng giải thích cho nguyên nhân phổ biến nhất dẫn đến thất bại trong điều trị. Một vết mổ chỉ mở da ra mà không phá vỡ các vách sợi sẽ giúp dẫn lưu mủ từ một ngăn, nhưng các ngăn còn lại vẫn kín. Vết thương dường như đã được điều trị nhưng lại không lành. Việc giảm áp lực hoàn toàn, chứ không phải kích thước vết mổ, mới là yếu tố quyết định liệu bệnh có khỏi hay không.

Một phần nguyên nhân gây tổn thương là do hệ miễn dịch của chính bạn

Quan điểm mới đây đã làm thay đổi cách hiểu về mục đích của ca phẫu thuật. Phần lớn sự phá hủy mô trong các trường hợp nhiễm trùng tay không phải do vi khuẩn trực tiếp gây ra, mà là do các tế bào bạch cầu trung tính mà cơ thể bạn tự sản xuất ra để chống lại vi khuẩn; các enzyme có tác dụng tiêu diệt vi khuẩn này cũng đồng thời phá hủy các mô xung quanh [3]. Vì vậy, việc dẫn lưu và rửa sạch vết thương không chỉ giúp loại bỏ mủ và giảm áp lực; mà còn pha loãng các chất trung gian gây viêm – nguyên nhân chính gây tổn thương mô.

Đây là sự thay đổi về trọng tâm lý luận chứ không phải thay đổi trong thực hành lâm sàng; tuy nhiên đây là lời giải thích thỏa đáng cho hiện tượng các bác sĩ thường quan sát thấy: tình trạng bệnh cải thiện nhanh chóng và rõ rệt hơn sau khi giảm áp lực vết thương, so với mức độ cải thiện có thể đạt được chỉ bằng cách loại bỏ một lượng mủ nhỏ.

Anatomical Structure of the Digital Pulp
