Lý do phẫu thuật này được đề xuất

Bác sĩ Kieran Hirpara, bác sĩ phẫu thuật chi trên tại Bệnh viện tư nhân Mater Rockhampton, sẽ xác định phương pháp điều trị phù hợp với chấn thương cụ thể của bạn. Thông thường, bệnh nhân được bác sĩ đa khoa giới thiệu đến phòng khám chúng tôi; nếu vật lý trị liệu viên khuyên bạn nên đến gặp chúng tôi, bạn vẫn cần có giấy giới thiệu từ bác sĩ đa khoa để được hưởng mức hoàn trả từ Medicare. Trong buổi khám, chúng tôi sẽ hỏi bệnh sử, khám khuỷu tay và chỉ định chụp hình ảnh nếu cần thiết. Nếu các mảnh xương chỉ di lệch rất ít, thường có thể điều trị mà không cần phẫu thuật, bằng cách sử dụng nẹp cố định và chụp X-quang kiểm tra để đảm bảo chúng không bị dịch chuyển. Đây thường là phương án đầu tiên chúng tôi đề xuất. Phẫu thuật được đề xuất khi các mảnh xương bị di lệch nhiều đến mức không thể tự liền lại ở vị trí thích hợp. Mục đích của ca phẫu thuật là giữ các mảnh xương ở đúng vị trí bình thường cho đến khi chúng liền lại, nhờ đó bề mặt khớp khuỷu vẫn nhẵn. Nếu bề mặt khớp thô ráp, theo thời gian có thể dẫn đến viêm khớp do mòn. Việc cố định xương chắc chắn còn giúp bạn sớm vận động khuỷu tay, từ đó giảm nguy cơ bị cứng khớp lâu dài. Hầu hết bệnh nhân sau phẫu thuật vẫn giữ lại các dụng cụ cố định này; chỉ có 3% trường hợp gặp tình trạng dụng cụ bị di lệch sau đó.

Trước khi phẫu thuật

Khi kế hoạch phẫu thuật đã được xác định, chúng tôi sẽ cung cấp cho bạn những hướng dẫn cụ thể cần tuân theo. Bạn cần ngừng ăn và uống 7 giờ trước khi phẫu thuật. Chúng tôi yêu cầu thời gian 7 giờ thay vì 6 giờ như thông thường để có thể sắp xếp lịch phẫu thuật sớm hơn nếu danh sách mổ diễn ra sớm hơn dự kiến. Một số loại thuốc có thể ảnh hưởng đến ca phẫu thuật; vì vậy hãy mang theo danh sách ghi rõ tất cả các loại thuốc bạn đang dùng để chúng tôi hướng dẫn bạn nên ngưng dùng thuốc nào. Hãy sắp xếp người đưa đón bạn về nhà sau phẫu thuật. Hãy mặc quần áo rộng rãi, thoải mái, có tay áo dễ dàng kéo qua khuỷu tay. Vào ngày phẫu thuật

Bạn sẽ đến khoa tiếp nhận bệnh nhân phẫu thuật của bệnh viện, nơi bạn sẽ được làm thủ tục nhập viện và chuẩn bị cho ca mổ. Sau đó, bạn sẽ gặp bác sĩ gây mê – người chịu trách nhiệm về việc gây mê và giảm đau cho bạn. Ca phẫu thuật này được thực hiện dưới gây mê toàn thân. Đôi khi người ta còn áp dụng phương pháp chặn dây thần kinh vùng để giảm đau sau phẫu thuật; bác sĩ gây mê sẽ trao đổi với bạn về vấn đề này vào ngày hôm đó. Tiếp theo, bạn sẽ được đưa vào phòng mổ để tiến hành ca phẫu thuật. Sau đó, bạn sẽ tỉnh dậy tại khu hồi sức, nơi các điều dưỡng sẽ theo dõi tình trạng sức khỏe của bạn cho đến khi tác dụng của thuốc mê hết. Khi tình trạng sức khỏe ổn định, bạn sẽ được chuyển sang phòng bệnh hoặc về nhà, tùy thuộc vào loại phẫu thuật và diễn biến hồi phục của bạn.

Quy trình phẫu thuật

Phẫu thuật này được gọi là nắn chỉnh mở và cố định nội tại. “Mở” có nghĩa là bác sĩ phẫu thuật tiếp cận vùng gãy xương thông qua một đường rạch ở mặt sau khuỷu tay, ngay trên mỏm khuỷu. “Nắn chỉnh” là việc đưa các mảnh xương gãy trở lại vị trí bình thường; còn “cố định nội tại” là việc giữ chúng ở đó bằng các dụng cụ kim loại cho đến khi xương liền lại.

Bác sĩ phẫu thuật sẽ chọn loại dụng cụ phù hợp với kiểu gãy xương của bạn, vì không có kỹ thuật nào có thể áp dụng cho mọi trường hợp gãy. Với những vết gãy đơn giản chỉ gồm hai mảnh, người ta có thể dùng vòng dây kim loại để ép hai mảnh xương lại với nhau khi bạn vận động, hoặc dùng ốc vít đặt vào giữa thân xương. Đối với những vết gãy có nhiều mảnh hoặc ở vùng xương mềm hơn, người ta thường dùng tấm kim loại được uốn theo hình dạng mặt sau khuỷu tay và cố định bằng các ốc vít nhỏ. Một số vết gãy có thể được giữ lại bằng các mũi khâu chắc chắn gắn vào xương thay vì dùng kim loại. Dù sử dụng phương pháp nào, mục đích vẫn là tạo sự cố định vững chắc để bạn có thể bắt đầu vận động khuỷu tay sớm.

Sau khi các mảnh xương đã nằm đúng vị trí và dụng cụ cố định được gắn chắc chắn, bác sĩ phẫu thuật sẽ kiểm tra xem bề mặt khớp khuỷu có bằng phẳng và khớp có di chuyển tự do hay không. Các lớp mô sẽ được khâu lại với nhau, da được khâu kín bằng chỉ khâu rồi phủ gạc lên trên.

Sau khi phẫu thuật

Khi tỉnh dậy, cánh tay của bạn sẽ được đặt trong chiếc đai treo đơn giản cho thoải mái. Các điều dưỡng sẽ thường xuyên kiểm tra tình trạng bạn và cho dùng thuốc giảm đau nếu cần thiết. Trong 24 giờ đầu sau khi về nhà, nên có người ở bên cạnh bạn. Đội ngũ y tế sẽ thông báo cho bạn biết là bạn có thể về nhà ngay hay phải ở lại bệnh viện qua đêm. Bạn có thể di chuyển khi cảm thấy vững vàng; chiếc đai treo sẽ được tháo ra khi bạn tắm rửa hoặc thực hiện các bài tập mà đội ngũ y tế hướng dẫn. Quá trình hồi phục

Việc vận động bàn tay và các ngón tay ngay từ ngày đầu rất hữu ích; đồng thời nên kê tay lên gối khi ngồi hoặc ngủ. Ban đầu, hãy uống thuốc giảm đau theo chỉ định đều đặn thay vì chờ cho cơn đau trở nên dữ dội hơn.

Trong những ngày đầu, bạn sẽ đeo một loại đai đỡ tay đơn giản để tăng cảm giác thoải mái. Bạn có thể tháo đai ra khi tắm rửa hoặc thực hiện các bài tập vật lý trị liệu. Việc trị liệu bàn tay sau phẫu thuật sẽ do Ruby Doolan tại Extend Rehabilitation đảm nhận. Ruby sẽ chỉ cho bạn các bài tập giúp khuỷu tay tiếp tục cử động trong lúc xương đang liền lại; nếu cần thiết, cô ấy sẽ làm nẹp hỗ trợ. Bạn nên vận động bàn tay và các ngón tay ngay từ đầu; các động tác nhẹ nhàng ở khuỷu tay mới được thêm vào khi tình trạng gãy xương ổn định hơn. Ban đầu, bạn cần lên kế hoạch cụ thể cho các hoạt động hàng ngày: sẽ cần người giúp đỡ trong việc ăn uống, mặc quần áo và mang vác đồ cho đến khi cánh tay ổn định trở lại.

Khi tình trạng sưng giảm và khả năng vận động được cải thiện, bạn sẽ thấy các bài tập dễ thực hiện hơn. Khi bác sĩ phẫu thuật xác nhận rằng xương đã liền tốt, bạn sẽ tháo hẳn đai đỡ và bắt đầu sử dụng cánh tay cho các công việc nhẹ nhàng trong nhà. Việc nâng vật nặng, tham gia thể thao hay lái xe chỉ được thực hiện sau đó, khi bác sĩ phẫu thuật và chuyên viên trị liệu đều xác nhận rằng khuỷu tay đã đủ khỏe mạnh. Mỗi người có tốc độ hồi phục khác nhau; vì vậy lộ trình phục hồi của bạn có thể khác đi. Bác sĩ phẫu thuật và chuyên viên trị liệu sẽ hướng dẫn bạn trong từng giai đoạn.

Những biến chứng có thể xảy ra

Hầu hết bệnh nhân đều hồi phục tốt, nhưng đôi khi vẫn có thể gặp phải các vấn đề. Bác sĩ phẫu thuật và đội ngũ y tế sẽ theo dõi sát sao để phát hiện sớm bất kỳ bất thường nào.

Kim loại dùng để cố định xương đôi khi có thể dịch chuyển khỏi vị trí ban đầu. Bạn có thể cảm thấy một cơn đau nhói mới xuất hiện tại mỏm khuỷu, một cục gồ dưới da mà trước đó không có, hoặc vùng da nơi có vật liệu cấy ghép bị đau rát, trầy xước. Nếu gặp các triệu chứng này, hãy gọi cho phòng khám ngay chứ đừng chờ đến lần khám tiếp theo.

Các dây kim loại hoặc vật liệu cấy ghép cũng có thể gây áp lực lên da hoặc xuyên qua da. Điều này gây đau đớn, kích ứng da và vết thương có thể bị hở ra tại chỗ đó. Hãy thông báo ngay cho chúng tôi nếu bạn nhận thấy tình trạng này, vì có thể cần phải loại bỏ vật liệu cấy ghép ra.

Nhiễm trùng là một rủi ro trong mọi ca phẫu thuật. Hãy chú ý nếu có cơn đau sâu, đau theo nhịp đập, không giảm khi dùng thuốc giảm đau thông thường; vùng da quanh vết mổ bị đỏ lan rộng; dịch tiết ra từ vết thương; hoặc sốt. Nếu gặp bất kỳ dấu hiệu nào, hãy liên hệ với phòng khám ngay trong ngày hoặc đến phòng cấp cứu nếu không thể liên lạc được với chúng tôi.

Nhiễm trùng cũng có thể làm chậm quá trình liền xương. Nếu quá trình hồi phục diễn ra chậm hơn nhiều so với dự kiến, hãy trao đổi với bác sĩ trong lần tái khám tiếp theo.

Dây thần kinh chạy phía sau mỏm khuỷu có thể bị kích thích, gây ra cảm giác ngứa ran, kiến bò hoặc tê dọc mặt trong cẳng tay, lan xuống ngón út và ngón áp út. Hãy báo lại cho bác sĩ nếu có những triệu chứng này, hoặc gọi cho phòng khám nếu chúng xuất hiện đột ngột.

Một số người nhận thấy khả năng duỗi thẳng khuỷu tay bị giảm đi một chút, ngay cả khi vết gãy đã lành tốt. Một số khác theo thời gian có thể bị viêm khớp do hao mòn, gây cảm giác đau nhức, tiếng click hoặc cảm giác lạo xạo tại khớp. Cả hai tình trạng này đều nên được đề cập trong lần tái khám để bác sĩ theo dõi.

Bony Anatomy

  • The elbow is a trocho-ginglymoid joint consisting of medial and lateral articulations that provide bony stability [3].

  • A functional arc for forearm rotation is 100° [1].
  • The angulation of the distal humeral articulation accounts for the change from a valgus carrying angle to a more varus position as the elbow is flexed [3].

Investigations

Physical Examination

  • The physical examination of the elbow is directed by the history and the location of the patient's pain in the anterior, posterior, medial, or lateral aspect of the elbow [1].
  • Pathologic entities associated with these discrete compartments aid the examiner in detecting pathologic conditions [1].
  • The soft tissue surrounding the elbow should be examined for previous skin incisions, grafts, eschar, or infection [9].
  • Active and passive flexion, extension, supination, and pronation should be evaluated using a goniometer for accurate measurement [9].
  • The contralateral elbow should be examined for comparison during range of motion assessment [9].
  • If the elbow has less than 90° to 100° of flexion, the posterior bundle of the medial collateral ligament is contracted and must be released to restore flexion [9].
  • Pain should be assessed during the mid-arc or at the terminal ends of motion [9].
  • Mid-arc range of motion pain is more common with intrinsic disease and may not improve with contracture release alone [9].
  • The ulnar nerve is of utmost importance during examination because of its anatomic proximity to the elbow [9].
  • The posterior bundle of the medial collateral ligament forms the floor of the cubital tunnel along the course of the ulnar nerve [9].
  • Electromyography and nerve conduction velocity studies should be performed if there is any question about neurologic dysfunction [9].
  • An assessment for ulnar nerve subluxation should be performed [9].
  • Subluxation of the ulnar nerve is a relative contraindication for an arthroscopic procedure secondary to possible iatrogenic nerve injury [9].
  • The presence of a prior ulnar nerve transposition should be verified if there is a history of prior surgical procedures [9].

Imaging

  • Plain radiographs remain the hallmark and the best screening test for elbow evaluation [1].
  • AP, lateral, and oblique radiographs are standard for elbow imaging [9].
  • Serial radiography is used as follow-up when heterotopic ossification is present [9].
  • Primary bony landmarks identified on radiographs include the ulnohumeral joint, coronoid process, radial head, capitellum, radiocapitellar joint, olecranon tip, coronoid/olecranon fossae, and trochlear ridge [9].
  • CT is helpful when assessing for malunion architecture and the location and pattern of osteophytes and/or loose bodies [9].
  • Three-dimensional CT is used to check for heterotopic ossification [9].
  • CT is not necessary when the stiffness is entirely soft-tissue related [9].
  • CT is beneficial if any joint incongruity or abnormal bony anatomy is present [9].
  • MRI can be used to evaluate ligaments and tendons, but it is rarely indicated for elbow stiffness [9].
  • CT with two-dimensional reconstruction and three-dimensional surface rendering best visualizes the pathology of valgus extension overload syndrome [11].
  • MRI may be most helpful in evaluating associated injuries including partial or complete tears of the medial collateral ligament in valgus extension overload syndrome [11].
  • Radiographic evaluations are essential when diagnosing an osteochondritis dissecans lesion of the elbow [12].
  • Important aspects of osteochondritis dissecans lesions may be better seen with MRI [12].
  • Standard AP and lateral radiographs typically show osteophyte formation at the coronoid process, coronoid fossa, radial fossa, radial head, olecranon tip, and olecranon fossa in elbow osteoarthritis [13].

Creative Commons Attribution-NonCommercial 4.0 International Public License

By exercising the Licensed Rights (defined below), You accept and agree to be bound by the terms and conditions of this Creative Commons Attribution-NonCommercial 4.0 International Public License ("Public License"). Section 1 -- Definitions.

e. Exceptions and Limitations means fair use, fair dealing, and/or any other exception or limitation to Copyright and Similar Rights that applies to Your use of the Licensed Material.

f. Licensed Material means the artistic or literary work, database, or other material to which the Licensor applied this Public License.

g. Licensed Rights means the rights granted to You subject to the terms and conditions of this Public License, which are limited to all Copyright and Similar Rights that apply to Your use of the Licensed Material and that the Licensor has authority to license.

h. Licensor means the individual(s) or entity(ies) granting rights under this Public License.

i. NonCommercial means not primarily intended for or directed towards commercial advantage or monetary compensation. For purposes of this Public License, the exchange of the Licensed Material for other material subject to Copyright and Similar Rights by digital file-sharing or similar means is NonCommercial provided there is no payment of monetary compensation in connection with the exchange.

j. k. Sui Generis Database Rights means rights other than copyright resulting from Directive 96/9/EC of the European Parliament and of the Council of 11 March 1996 on the legal protection of databases, as amended and/or succeeded, as well as other essentially equivalent rights anywhere in the world.

l. You means the individual or entity exercising the Licensed Rights under this Public License. Your has a corresponding meaning.

Section 2 -- Scope.

a. License grant.

1. Subject to the terms and conditions of this Public License, the Licensor hereby grants You a worldwide, royalty-free, non-sublicensable, non-exclusive, irrevocable license to exercise the Licensed Rights in the Licensed Material to:

a. reproduce and Share the Licensed Material, in whole or in part, for NonCommercial purposes only; and

b. produce, reproduce, and Share Adapted Material for NonCommercial purposes only.

2. Exceptions and Limitations. For the avoidance of doubt, where Exceptions and Limitations apply to Your use, this Public License does not apply, and You do not need to comply with its terms and conditions.

3. Term. The term of this Public License is specified in Section 6(a).

4. Media and formats; technical modifications allowed. The Licensor authorizes You to exercise the Licensed Rights in all media and formats whether now known or hereafter created, and to make technical modifications necessary to do so. The Licensor waives and/or agrees not to assert any right or authority to forbid You from making technical modifications necessary to exercise the Licensed Rights, including technical modifications necessary to circumvent Effective Technological Measures. For purposes of this Public License, simply making modifications authorized by this Section 2(a) (4) never produces Adapted Material.

5. Downstream recipients.

a. Offer from the Licensor -- Licensed Material. Every recipient of the Licensed Material automatically receives an offer from the Licensor to exercise the Licensed Rights under the terms and conditions of this Public License.

b. No downstream restrictions. You may not offer or impose any additional or different terms or conditions on, or apply any Effective Technological Measures to, the Licensed Material if doing so restricts exercise of the Licensed Rights by any recipient of the Licensed Material.

6. No endorsement. Nothing in this Public License constitutes or may be construed as permission to assert or imply that You are, or that Your use of the Licensed Material is, connected with, or sponsored, endorsed, or granted official status by, the Licensor or others designated to receive attribution as provided in Section 3(a)(1)(A)(i).

b. Other rights.

1. 2. Patent and trademark rights are not licensed under this Public License.

To the extent possible, the Licensor waives any right to collect royalties from You for the exercise of the Licensed Rights, whether directly or through a collecting society under any voluntary or waivable statutory or compulsory licensing scheme. In all other cases the Licensor expressly reserves any right to collect such royalties, including when the Licensed Material is used other than for NonCommercial purposes.

Section 3 -- License Conditions.

Your exercise of the Licensed Rights is expressly made subject to the following conditions.

a. Attribution.

1. If You Share the Licensed Material (including in modified form), You must:

a. retain the following if it is supplied by the Licensor with the Licensed Material:

i. identification of the creator(s) of the Licensed Material and any others designated to receive attribution, in any reasonable manner requested by the Licensor (including by pseudonym if designated);

ii. a copyright notice;

iii. a notice that refers to this Public License;

iv. a notice that refers to the disclaimer of warranties;

v. a URI or hyperlink to the Licensed Material to the extent reasonably practicable;

b. indicate if You modified the Licensed Material and retain an indication of any previous modifications; and

c. indicate the Licensed Material is licensed under this Public License, and include the text of, or the URI or hyperlink to, this Public License.

2. You may satisfy the conditions in Section 3(a)(1) in any reasonable manner based on the medium, means, and context in which You Share the Licensed Material. Section 4 -- Sui Generis Database Rights.

Where the Licensed Rights include Sui Generis Database Rights that apply to Your use of the Licensed Material:

a. for the avoidance of doubt, Section 2(a)(1) grants You the right to extract, reuse, reproduce, and Share all or a substantial portion of the contents of the database for NonCommercial purposes only;

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.

Section 5 -- Disclaimer of Warranties and Limitation of Liability.

a. UNLESS OTHERWISE SEPARATELY UNDERTAKEN BY THE LICENSOR, TO THE EXTENT POSSIBLE, THE LICENSOR OFFERS THE LICENSED MATERIAL AS-IS AND AS-AVAILABLE, AND MAKES NO REPRESENTATIONS OR WARRANTIES OF ANY KIND CONCERNING THE LICENSED MATERIAL, WHETHER EXPRESS, IMPLIED, STATUTORY, OR OTHER. Section 6 -- Term and Termination.

a. This Public License applies for the term of the Copyright and Similar Rights licensed here. However, if You fail to comply with this Public License, then Your rights under this Public License terminate automatically.

b. Where Your right to use the Licensed Material has terminated under Section 6(a), it reinstates:

1. automatically as of the date the violation is cured, provided it is cured within 30 days of Your discovery of the violation; or

2. upon express reinstatement by the Licensor.

For the avoidance of doubt, this Section 6(b) does not affect any right the Licensor may have to seek remedies for Your violations of this Public License.

c. For the avoidance of doubt, the Licensor may also offer the Licensed Material under separate terms or conditions or stop distributing the Licensed Material at any time; however, doing so will not terminate this Public License.

d. Sections 1, 5, 6, 7, and 8 survive termination of this Public License.

Section 7 -- Other Terms and Conditions.

a. The Licensor shall not be bound by any additional or different terms or conditions communicated by You unless expressly agreed.

b. Any arrangements, understandings, or agreements regarding the Licensed Material not stated herein are separate from and independent of the terms and conditions of this Public License.

Section 8 -- Interpretation.

a. For the avoidance of doubt, this Public License does not, and shall not be interpreted to, reduce, limit, restrict, or impose conditions on any use of the Licensed Material that could lawfully be made without permission under this Public License.

b. To the extent possible, if any provision of this Public License is deemed unenforceable, it shall be automatically reformed to the minimum extent necessary to make it enforceable. If the provision cannot be reformed, it shall be severed from this Public License without affecting the enforceability of the remaining terms and conditions.

c. No term or condition of this Public License will be waived and no failure to comply consented to unless expressly agreed to by the Licensor.

d. Nothing in this Public License constitutes or may be interpreted as a limitation upon, or waiver of, any privileges and immunities that apply to the Licensor or You, including from the legal processes of any jurisdiction or authority.
