Gãy xương sên là tình trạng gãy một xương nhỏ nằm sâu bên trong khớp mắt cá chân. Xương sên nằm giữa xương chày và xương gót, đóng vai trò nối liền hai xương này lại với nhau. Vì vị trí nằm giữa hai xương nên cơn đau thường xuất hiện sâu bên trong khớp mắt cá chân và vùng bàn chân sau, chứ không tập trung ở một điểm cụ thể nào mà bạn có thể chỉ ra được.

Cơn đau trở nên dữ dội hơn khi bạn chịu trọng lượng lên chân. Việc đi bộ, đứng thẳng, bước lên cầu thang hoặc xoay mắt cá chân thường khiến cơn đau tăng lên. Việc nghỉ ngơi và nâng chân lên cao thường giúp giảm đau. Khớp mắt cá chân và vùng bàn chân sau thường bị sưng; tình trạng sưng này có thể khiến giày trở nên chật chội. Nhiều người cảm thấy mắt cá chân cứng lại vào buổi sáng hoặc sau khi ngồi yên một lúc.

Các hoạt động hàng ngày đòi hỏi mắt cá chân vừa ổn định vừa linh hoạt sẽ trở nên khó khăn. Bạn có thể gặp khó khăn khi đi bộ đến hộp thư, đứng nấu ăn ở bàn bếp hoặc lái xe. Việc đẩy xe đẩy hàng hoặc đứng dậy từ sàn nhà cũng trở nên khó khăn vì chân không thể chịu lực đều được.

Những vết gãy này thường dễ bị bỏ sót. Chụp X-quang thông thường chỉ phát hiện được khoảng một nửa các tổn thương sụn ở mắt cá chân, ngay cả khi kết hợp với khám lâm sàng. Chụp CT cho thấy chi tiết về vị trí gãy xương nhưng không thể hiện được tình trạng sụn. Chụp MRI lại có thể hiển thị cả sụn lẫn xương bên dưới; hình ảnh này rất tương đồng với những gì bác sĩ phẫu thuật quan sát được khi soi khớp bằng máy nội soi.

Nếu sau khi bị vặn mắt cá chân, ngã từ trên cao hoặc gặp tai nạn xe cộ mà bạn cảm thấy đau sâu bên trong khớp mắt cá chân, đừng cố gắng chịu đựng. Hãy đi khám để được đánh giá đúng mức.

Chuyện gì đang xảy ra thực sự

Xương sên là một xương nhỏ nằm trong không gian hẹp giữa xương chày phía trên và xương gót phía dưới. Xương này không có cơ nào bám vào và phần lớn bề mặt được phủ bởi lớp sụn mịn. Hãy hình dung nó như một mảnh phấn cứng bị kẹp giữa hai vật thể: khi bàn chân bị ép lên trên và mắt cá chân bị nén chặt, xương sên có thể bị nứt giữa hai xương đang đè lên nó.

Các dạng gãy xương khác nhau xuất hiện do các lực tác động khác nhau. Việc tiếp đất mạnh khi bàn chân gập lên trên và xoay vào trong có thể làm vỡ ra một mảnh nhỏ ở phía ngoài xương sên. Một vặn xoắn mạnh hoặc việc bàn chân bị đẩy quá xa theo một hướng nào đó có thể khiến các mảnh nhỏ tách ra ở phía sau xương sên. Khi mắt cá chân bị nén mạnh theo chiều thẳng đứng, thân chính của xương sên có thể bị nứt; nếu đồng thời xương mắt cá trong cũng bị gãy, vết nứt có thể lan qua cả hai xương.

Vấn đề nghiêm trọng của các dạng gãy này nằm ở những tổn thương chúng gây ra. Xương sên tham gia tạo thành hai khớp hoạt động; bất kỳ mảnh xương nào bị vỡ ra đều có thể trôi tự do bên trong khớp, thậm chí bị lật ngược lại vào chỗ lõm vốn là nguồn gốc của nó. Những mảnh xương lỏng lẻo cùng bề mặt khớp không đều gây ra hiện tượng cọ xát thay vì trượt nhẹ, khiến mắt cá chân vẫn cứng và đau nhức ngay cả sau khi chấn thương đã qua. Một vết nứt nhỏ ở phía ngoài xương sên là dạng chấn thương thường gặp ở người chơi trượt ván tuyết; nếu không được phát hiện hoặc điều trị chậm trễ, khớp phía dưới mắt cá chân có thể bị tổn thương nghiêm trọng. Việc đưa các mảnh xương trở lại vị trí ban đầu và khôi phục bề mặt khớp cho bằng phẳng sẽ giúp mắt cá chân có cơ hội vận động bình thường mà không đau đớn.

Những gì chúng tôi có thể làm

Ngay cả khi điều trị tốt, những biến chứng này vẫn thường gặp; một số ít bệnh nhân bị chấn thương nặng phải chịu đựng tình trạng đau đớn và cứng khớp kéo dài, ảnh hưởng đến sinh hoạt hàng ngày. Những vết gãy nhỏ ở mép ngoài hoặc phía sau xương sên thường dễ bị bỏ sót; nếu phát hiện muộn hoặc điều trị không đầy đủ, chúng có thể dẫn đến suy giảm chức năng lâu dài của khớp dưới mắt cá chân.
