Tại sao bạn được kê đơn melatonin

Giấc ngủ là một trong những khó khăn lớn nhất trong vài tuần đầu sau phẫu thuật vai. Khi nằm xuống, vùng vai lại càng đau hơn; bạn không thể nằm ở tư thế quen thuộc, và chiếc đai cố định vai khiến việc ngủ trở nên khó khăn. Việc giấc ngủ bị gián đoạn khiến cơn đau trở nên tồi tệ hơn vào ngày hôm sau. Melatonin là gì?

Những gì các bằng chứng khoa học cho thấy

Nghiên cứu về vai. Năm 2024, Viện Chỉnh hình Rothman tại Philadelphia đã công bố một nghiên cứu ngẫu nhiên gồm 80 bệnh nhân được phẫu thuật sửa chữa gân cơ vùng vai qua phương pháp nội soi. Một nửa số bệnh nhân uống 5 mg melatonin mỗi tối, một giờ trước khi đi ngủ, trong sáu tuần, bắt đầu từ ngày phẫu thuật; nửa còn lại tuân theo các hướng dẫn vệ sinh giấc ngủ thông thường. Nhóm dùng melatonin có chất lượng giấc ngủ cải thiện rõ rệt sau sáu tuần; họ đánh giá chất lượng giấc ngủ cao hơn trong suốt sáu tháng tiếp theo; điểm số trên bảng câu hỏi đánh giá chức năng vai ở tháng thứ tư và thứ sáu cũng cao hơn; đồng thời họ dùng ít thuốc giảm đau opioid hơn vào tháng thứ tư. Không có tác dụng phụ nào được ghi nhận ở cả hai nhóm.

Đây chỉ là một nghiên cứu duy nhất, thực hiện tại một trung tâm, và các bệnh nhân biết rõ mình thuộc nhóm nào; vì vậy kết quả này nên được xem là tích cực chứ chưa thể coi là chắc chắn. Tuy nhiên, đây là bằng chứng tốt nhất hiện có cho đúng tình huống mà bạn đang gặp phải.

Các loại phẫu thuật khác. Melatonin đã được thử nghiệm sau nhiều loại phẫu thuật khác nhau. Khi tổng hợp kết quả từ mười nghiên cứu với 725 bệnh nhân, người ta nhận thấy melatonin giúp cải thiện nhẹ chất lượng giấc ngủ sau phẫu thuật. Sau phẫu thuật thay khớp hông hoặc khớp gối, nhiều nghiên cứu có nhóm đối chứng giả dược cho thấy melatonin hầu như không mang lại lợi ích gì ngoài vài đêm đầu sau mổ.   • Tại sao phải kê đơn: Tại Úc, melatonin là thuốc kê đơn đối với hầu hết người trưởng thành. Dạng thuốc không cần kê đơn duy nhất là viên nén giải phóng chậm 2 mg do hiệu dược sĩ phân phối, chỉ được phép dùng cho những người trên 55 tuổi bị mất ngủ. Vì liều lượng và mục đích sử dụng sau phẫu thuật khác nhau, chúng tôi mới kê đơn để bạn nhận được dạng thuốc phù hợp.

Tác dụng phụ và lưu ý

Hầu hết mọi người đều dung nạp tốt melatonin. Các phản ứng phụ thường gặp là cảm giác mệt mỏi, uể oải vào buổi sáng; gặp nhiều giấc mơ sống động; đau đầu hoặc buồn nôn nhẹ. Hiện tượng mệt mỏi buổi sáng thường giảm bớt nếu uống thuốc sớm hơn một chút vào buổi tối hoặc giảm liều dùng. Melatonin không gây nghiện và không gây ra các triệu chứng cai nghiện khi ngừng sử dụng.

Trong vài tuần đầu sau phẫu thuật vai, bạn sẽ không được lái xe dù sao đi nữa; khi đã được phép lái xe, hãy chú ý đến tình trạng buồn ngủ vào buổi sáng trong vài ngày đầu quay lại vô lăng. Tránh uống rượu vào những đêm dùng melatonin.
