¿Por qué se ha recomendado esta operación?

El Dr. Kieran Hirpara, cirujano ortopédico en el Mater Private Hospital Rockhampton, ofrece la artroplastia total de tobillo a pacientes cuidadosamente seleccionados que padecen artritis grave del tobillo. En la mayoría de las personas que padecen este problema crónico, iniciamos con tratamientos no quirúrgicos, como modificaciones en las actividades, fisioterapia y uso de férulas; solo consideramos la cirugía cuando estos tratamientos no logran mejoría suficiente.

La artroplastia total de tobillo sustituye las superficies desgastadas de la articulación por un implante artificial. Es una de las dos intervenciones posibles para la artritis avanzada del tobillo; la otra es la artrodesis, en la que los huesos se fijan para que crezcan juntos. Le explicaremos ambas opciones. La satisfacción de los pacientes con la artroplastia de tobillo suele superar el 90 %; el objetivo de la operación es aliviar el dolor y restaurar la movilidad, para que pueda caminar y mantenerse activo con mayor comodidad.

Antes de la operación

Una vez planificada la cirugía, organizamos las pruebas de imagen necesarias para la preparación: radiografías de carga de su tobillo; a veces también una resonancia magnética o una ecografía para examinar detalladamente la articulación, el cartílago y los tejidos blandos circundantes. En los días previos a la operación, deberá ayunar durante siete horas. Pedimos que sea un período de siete horas en lugar de uno más corto para poder adelantar la intervención si el programa quirúrgico lo permite. Algunos medicamentos deben suspenderse antes de la cirugía; su cirujano le indicará cuáles y cuándo debe dejar de tomarlos. Lleve una lista escrita de todos los fármacos que consume, vístase con ropa holgada y cómoda, y organice que alguien lo lleve a casa después de la operación. Si padece otras enfermedades, es posible que necesite análisis de sangre o una consulta con el anestesista.

El día de la intervención

Ese día, acudirá a la unidad de admisiones quirúrgicas del hospital, donde se le registrará y preparará para la cirugía. Conocerá al anestesista, quien se encargará de administrarle la anestesia. Esta operación se realiza bajo anestesia general. En ocasiones, se añade un bloqueo nervioso regional para aliviar el dolor postoperatorio; el anestesista hablará con usted al respecto ese mismo día. Posteriormente, será conducido al quirófano, donde se llevará a cabo la intervención.

Despertará en la sala de recuperación, donde las enfermeras lo vigilarán mientras la anestesia va desapareciendo. Una vez que su estado sea estable, será trasladado a la planta de hospitalización o podrá volver a casa, según el tipo de procedimiento y cómo evolucione su recuperación.

¿En qué consiste la operación?

El reemplazo total del tobillo consiste en sustituir las superficies desgastadas de la articulación por piezas artificiales, generalmente de metal y plástico. El cirujano retira el hueso y el cartílago dañados de los extremos de los huesos que forman el tobillo, y luego los modela para que se adapten a las nuevas superficies articulares. Las piezas nuevas se fijan en su lugar para que puedan moverse suavemente una contra la otra, tal como lo haría un tobillo sano.

La operación se realiza mediante una incisión en la parte frontal del tobillo. Algunos diseños también emplean un pequeño vástago que se inserta en el hueso principal de la parte inferior de la pierna para mantener el implante estable. Asimismo, el cirujano podría alargar un tendón tenso en la parte posterior del tobillo durante la misma intervención, si ello contribuye a una mejor flexión posterior del tobillo.

Una vez colocado y comprobado el implante, se cierran los bordes de la incisión con puntos de sutura y se cubre con un apósito. Deberá mantener ese apósito durante unos 10 días, tal como se describe en la sección de recuperación.

El tipo exacto de implante utilizado varía, y los diseños siguen mejorando a medida que los cirujanos adquieren más conocimientos sobre qué formas y materiales resultan más adecuados. Antes de la operación, su cirujano le explicará el plan de tratamiento para su tobillo, incluyendo si son necesarios pasos adicionales según los resultados de sus estudios de imagen .

Después de la operación

Despertará en la sala de recuperación, donde las enfermeras lo vigilarán mientras el efecto de la anestesia desaparece. Una vez que se haya estabilizado, será trasladado a la habitación o podrá irse a casa. Su equipo médico le indicará si podrá regresar a casa el mismo día o si deberá permanecer una noche en el hospital. Se le administrará analgésico antes de que la anestesia deje de hacer efecto; si el dolor no disminuye, informe a su enfermera. El tobillo se vendará con vendajes, que dejaremos puestos durante unos 10 días; por favor, no los retire antes de ese plazo a menos que se lo indiquemos. Los cambiaremos o los quitaremos cuando vengamos a verlo. Poco después de la operación, se le ayudará a ponerse de pie y a dar unos pasos, utilizando muletas o un andador para apoyo. Durante las primeras 24 horas después de llegar a casa, debería haber alguien con usted.

Recuperación

Durante los primeros días, lo importante es el descanso y reducir la hinchazón. El tobillo le dolerá y estará hinchado; la piel podría presentar hematomas. Mantener el pie elevado por encima del nivel del corazón, aplicar compresas de hielo envueltas en una toalla y tomar los analgésicos según las indicaciones ayudan a controlar estos síntomas. La hinchazón suele alcanzar su punto máximo en los primeros días y luego disminuye gradualmente; sin embargo, puede aparecer y desaparecer durante un tiempo, especialmente después de estar de pie.

Poco después de la operación, se le ayudará a ponerse de pie y dar algunos pasos, utilizando muletas o un andador. Su fisioterapeuta le guiará en ejercicios para mantener en movimiento el resto de la pierna y, una vez que el cirujano considere que la cicatrización avanza adecuadamente, para recuperar la movilidad y fortalecer el tobillo. Mientras el tobillo se recupera, deberá usar una bota o soporte similar; asimismo, se le enseñará cómo apoyar el peso en el pie cuando sea el momento adecuado. En casa, mantenga el pie elevado al sentarse, camine distancias cortas según las indicaciones y evite permanecer de pie durante largos periodos. Dormir boca arriba con el pie elevado suele ser la postura más cómoda.

La recuperación varía de una persona a otra. Algunas personas recuperan la estabilidad al caminar antes que otras; la hinchazón también puede persistir más tiempo de lo esperado sin que ello indique ningún problema. Su cronograma de recuperación podría diferir del de otras personas; su cirujano y fisioterapeuta le guiarán en cada etapa. Una vez que la hinchazón disminuya y el tobillo gane movilidad, caminar y realizar actividades cotidianas le resultará cada vez más fácil.

Qué puede salir mal

La mayoría de los pacientes evolucionan bien, pero en ocasiones pueden surgir problemas. Su cirujano y el equipo lo vigilarán de cerca para detectar cualquier anomalía a tiempo.

A veces, el hueso que rodea la nueva articulación se desgasta lentamente. Puede notar un dolor sordo cerca del implante, o sentir que el tobillo es menos estable que antes. Esto se detecta mediante radiografías durante los controles; por eso, asista a todas las citas de seguimiento aunque el tobillo parezca encontrarse bien. Si se detecta este desgaste a tiempo, a menudo se puede tratar sin necesidad de volver a realizar el reemplazo completo.

Las infecciones alrededor de una articulación nueva son poco frecuentes pero graves. Esté atento a un dolor profundo y palpitante que no ceda con analgésicos comunes, a enrojecimiento que se extienda desde la herida, o a secreción de líquido desde el corte. También podría sentir fiebre y escalofríos. Comuníquenos de inmediato cualquiera de estos síntomas, o acuda a urgencias si se siente mal.

Un coágulo sanguíneo puede provocar hinchazón y sensibilidad repentinas en la pantorrilla, a veces acompañadas de calor o sensación de pesadez en la pierna. Si el coágulo llega a los pulmones, puede causar dificultad para respirar o dolor torácico. Estos signos requieren atención urgente; por ello, acuda a urgencias o solicite ayuda en lugar de esperar.

El hueso situado en el borde del tobillo puede fracturarse durante la colocación de las nuevas piezas. Generalmente esto se detecta y trata durante la operación, pero puede implicar un período más largo de uso de bota o escayola después. Si poco después de la cirugía siente un dolor agudo y nuevo en los bultos óseos a ambos lados del tobillo, infórmenos.

La herida en la parte frontal del tobillo puede tardar en cicatrizar. Preste atención a bordes separados, aumento del enrojecimiento o supuración. Mencione esto en su control o llame a la clínica, pues algunas heridas requieren cuidados adicionales por parte de nuestro equipo.

En ocasiones se forma hueso extra cerca de la nueva articulación, lo que genera una sensación de pinzamiento en la parte frontal del tobillo, especialmente al flexionar el pie hacia arriba. Si este malestar persiste, una pequeña intervención quirúrgica mínimamente invasiva puede solucionarlo.

Si el reemplazo se desgasta o falla años después, aún existen opciones: otro reemplazo o la fusión del tobillo, en la cual los huesos se unen para formar una sola estructura.

¿Cuándo deben llamarnos?

La mayoría de los problemas aparecen en las primeras etapas, y una intervención rápida facilita su tratamiento. Llámenos si presenta fiebre, enrojecimiento creciente o secreción en la herida, o dolor que empeora en lugar de mejorar. Acuda a urgencias si experimenta hinchazón o sensibilidad repentina en la pantorrilla, dificultad para respirar o dolor torácico, ya que estos pueden ser signos de un coágulo sanguíneo. Acuda a urgencias también si pierde la sensibilidad en el pie o no puede moverlo. Si no está seguro de si algún síntoma es normal, llame a la clínica y le ayudaremos a decidir.
